A Case of Panic Disorder of Soeumin Concurrently Treated with Psychotherapy Based on Sasang Constitutional Medicine (사상의학적으로 접근한 정신요법을 병행한 소음인 공황장애 치험 1례)

  • Objectives The purpose of this case study was to report that a patient with Panic Disorder concurrently treated with psychotherapy based on Sasang Constitutional Medicine was able to discontinue the drugs. Methods The patient was diagnosed as Soeumin and consulted according to her Sasang constitution while being administered with herbal medicines. The psychotherapy applied in the consultation was to reduce excessive expectations towards her family members or situations. Results and Conclusions Anxiety of the patient was alleviated and the patient was able to discontinue the drugs during the treatment period. And no panic attack or adverse effect of the discontinuation of the drugs was observed during the rest of the time of 7 months and 4 months later after the termination of the treatment.

Antidiabetic Effect of So-Dang-Hwan in Streptozotocin-induced Diabetic Rats (소당환이 Streptozotocin으로 유발된 흰쥐의 당뇨에 미치는 영향)

  • Objectives : So-Dang-Hwan (SDH) is used as a traditional treatment of diabetes in oriental clinics in Korea. This study aimed to evaluate antidiabetic effect of SDH in streptozotocin (STZ)-induced diabetic rats. Methods : Diabetes was induced by i.p. injection of STZ (45 mg/kg) to Sprague-Dawley rats. Experimental animals (eight per group), were treated by oral administration of SDH (60 mg/kg body weight) and glibenclimide (1 mg/kg), a known antidiabetic drug for comparison, during 5 weeks. To veridy the effect of SDH, the levels of glucose, triglyceride, insulin, BUN and creatinine were measured in sera from experimental diabetic rats, and an oral glucose tolerance test (OGTT) was also performed. Results : SDH prevented body weight loss in diabetic rats. SDH exhibited at termination, a significant reduction in blood glucose levels in STZ-induced diabetic rats. SDH significantly reduced serum creatinine levels toward the normal levels. The OGTT results showed a significant improvement in glucose tolerance in rats treated with SDH. Conclusions : These data indicate that SDH treatment may improve glocose homeostasis in STZ-induced diabetes.

  • Tetrahydrocurcumin (THC) is a major metabolite of curcumin, which is obtained from Curcuma longa. THC has various benefits and overcomes the bioavailability issue of curcumin. To establish it as a pharmacologically active molecule, its safety profile has to be determined. Thus, the present study aimed to determine the preclinical safety profile of THC in a 90-day subchronic and reproductive/developmental toxicity study in Wistar rats. THC at oral doses of 100, 200, and 400 mg/kg was administered daily for 90 days. Rats in the recovery group were kept for 14 days after treatment termination. The animals were observed for treatment-related morbidity, mortality, and changes in clinical signs, clinical pathology, and histopathology. In the reproductive/developmental toxicity study, THC at 100, 200, and 400 mg/kg was administered orally to rats and the reproductive/developmental parameters in adult male and female rats and pups were observed. THC at up to 400 mg/kg/day of did not have any significant effect on all parameters in male and female rats in both toxicity studies. Thus, 400 mg/kg/day can be considered as the no-observed-adverse-effect-level of THC in rats.

The Effect of Animal Menure on the Soil Characters and Productivity of Grassland (가축분이 초지의 토양과 생산성에 미치는 영향)

  • This experiment was conducted to investigate the effect of animal stable manure on the dry matter yield, botanical composition, crude protein content rates, in vitro digestibility and soil characters with treatments, which were divided into cattle manure, swine manure and poultly excreta of 375 and 750kg/lOa, respectively. The results obtained were summarized as follows: 1. A comparison made on the grass yield for one year period following the termination of the experiment did not indicate any signification in the yield of dry matter difference between chemical fertilizer treatment and 750kg/10a of organic manure. 2. Botanical composition was not shown an increasing of weed or bare land in organic manure of 375 and 750kg/lOa application during the whole period of growth. 3. Crude protein content and in vitro digestibility were shown an increasing tendency according to the increase of organic manure application. 4. Crude protein yield in organic manure of 750kg/10a was not a large difference compared with chemical fertilizer treatment. 5. Soil composition of organic manure had higher than the soil composition of chemical fertilizer treatment, which was shown an increasing tendency according to the increase of organic manure. 6. Therefore, It was suggested that the effect of organic manure on dry matter yield, botanical composition was similar to chemical fertilizer treatment and soil composition of organic manure had higher organic matter, available phosphate, potassium and C.E.C than the soil composition of chemical fertilizer treatment.

A New Finding-peripheral Vision Phobia: A Possible Subtype of Social Phobia

  • Background and objectives : Social phobia is a nervous anxiety characterized by one or more constant fear in social activity or specific situation which causes serious damage to individual's social functions leading to chronic functional impairment when it is untreated. The objective of this study is to report a new finding of a possible subtype of social phobia according to a case survey and study of sufferers found via online support group. Methods : A single case review using survey and interview on patient was employed. Following a no-treatment baseline period for examination and interview, biweekly acupuncture treatment with interview for in-depth examination was delivered for 6months. Also, for in-depth investigation of peripheral vision phobia, survey questionnaire was done by the members of a Korean support group on on-line social group forum. Result : A new type of social phobia which has not yet been reported nor mentioned in any journals, namely called 'peripheral vision phobia' has been identified. Biweekly acupuncture treatment reached a comfort state of mental status on one case of patient with social phobia. Limitation : Due to the shift of environment on patient's part, the termination of the school session at where the phobia occur the most, the reoccurrence of the phobia is yet to be examined. Also, the generalization of the new type of phobia as a subtype only by reviewing one case alone with survey interview of on-line social group has limitation. Conclusion : The result of the case review suggests that 'peripheral vision phobia' is strictly concentrated on malfunctioning of peripheral vision without functional or organic hindrance of pupils or of any parts of eyes including eye muscles due to peripheral vision phobia. PVP is similar to social phobia and it may be a subtype of social phobia, however, since etiology, diagnosis, medical treatment methods are under researched, investigation is yet to be done.

Experimental and Clinical Study on Hyperlipidemia of Tongbiyeum (고지혈증에 대한 통비음의 실험 및 임상적 고찰)

  • This study was carried out to investigate the clinical effects of Tongbiyeum(TBY) ₂on hyperlipidemia. The hyperlipidemia of rats was induced by feeding high cholesterol diet for 4 weeks. We checked serum lipids and body weight weekly for 4 weeks. After the termination of treatment, we measured liver weight and observed histopathological change. We compared result of TBY group with control group. Thereafter, we made a study of 37 hyperlipidemic patients who visited us, the oriental medical center Daejeon and cheongju of Daejeon University, from November, 2001 to April, 2002. After administering TBY for 1 month, we followed up their total cholesterol, triglyceride and HDL-Cholesterol levels. The results were as follows; TBY treatment inhibited the liver weight gain induced by high cholesterol diet as compared with control group. TBY treatment inhibited lipid droplet accumulation and apoptotic change in liver as compared with control group. TBY treatment significantly inhibited the increasing of serum triglyceride and total cholesterol levels induced by high cholesterol diet as compared with control group but not affect HDL-cholesterol level. After administering TBY, the serum total cholesterol, triglyceride and LDL-cholesterol levels of hyperlipidemic patients were decreased significantly. After administering TBY, the serum HDL-Cholesterol level of hyperlipidemic patients had no significant changes. According to above mentioned results, we can infer that TBY has hypolipidemic effect to be applicable to artheriosclerosis.

A Comparative Coagulopathic Study for Treatment of Vasospasm by Using Low- and High-molecular Weight Hydroxyethyl Starches

  • Objective : High-molecular-weight hydroxyethyl starch (HES) compromises blood coagulation more than does low-molecular-weight HES. We compared the effects of low- and high-molecular-weight HES for the treatment of vasospasm and investigated the dose relationship with each other. Methods : Retrospectively, in a series of consecutive 102 patients with subarachnoid hemorrhage (SAH), 35 patients developed clinical symptoms of vasospasm of these fourteen patients were treated with low-molecular weight HES for volume expansion while the other 21 received high-molecular-weight HES as continuous intravenous infusion. Prothrombin time (PT), partial thromboplastin time (PIT), fibrinogen level, and platelet count were all measured prior to initiation, during treatment and after termination of therapy for symptomatic vasospasm. The total dose of HES ranged from 5 L to 14 L and median infusion duration was 10 days. Results : A more pronounced PTT prolongation was observed in high-molecular-weight HES group compared with low-molecular-weight HES group. No other coagulation parameters were altered. Dosage (=duration) shows a positive correlation with PTT. Clinically, significant bleeding episodes were noted in four patients who received high-molecular-weight HES. Conclusion : Coagulopathy was developed in direct proportion to molecular weight of starch and dosages. We propose the extreme caution in the administration of HES solution for the vasospasm treatment.

Cloning and Characterization of Replication Origins from Misgurnus mizolepis (미꾸라지로부터의 복제원점 클로닝 및 그 특성에 관한 연구)

  • The nuclear matrix was isolated from Misgumus mizolepis liver nuclei by low salt extraction and restriction enzyme treatment. The structure was digested with proteinase K. After centrifugation, matrix attachment regions (MARs) were obtained by RNase treatment and phenol-chloroform extraction. The result leads to the appearance of smeared bands in the range of about 0.3-15 kb. pURY19 vector was constructed by inserting 2.13 kb Eco47 III fragment of the yeast uracil 3 gene into the unique Ssp I site of pUC19 plasmid vector as a selection marker. This vector is unable to be maintained in Sacrharomyces cerevisiae by itself since it cannot replicate as an extrachromosomal element. Using this system, we attempted cloning the ARS (autonomously replicating sequence) from M. mizelepis to develop an efficient expression vector for the transgenic fish. pURY19N_{l-62}$ were constructed by inserting MARs in pURY19 plasmid vector and transformation of E. coli $DH5\alpha$. Replication origins (ARS) of M. mizolepis were isolated, which enabled the vector to replicate autonomously in S. cerevisiae. The cloned DNA fragments were sequenced by Sanger's dideoxy-chain termination method. All clones were AT-rich. $pURY19N_6$, one of the clones, expecially contained ARS consensus sequence, Topoisomerase II consensus, near A-box and T-box.

  • In this paper, The Supreme Court of Korea 2016. 1. 28. 2015Da9769 was reviewed. In the previous case, Korean Supreme Court 2009Da17417 for the element to requirement for permission of the withdrawal of life-sustaining treatments, the patient's consent for withdrawal of life-sustaining treatments was assumed a declaration of intention to terminate the contract. But the consent for withdrawal of life-sustaining treatments corresponds not to those. The consent for medical treatments is not the juristic acts but the real acts. If the presumptive intention about these withdrawal regards as the termination of medical contract, the contract must be up to the starting the civil proceedings. According to this case, although the partial cancellation of medical contract is admitted, on the other hand medical expenses obligation ist exempted only after the final decision. At the withdrawal of life-sustaining treatments the medical obligation ist exempted because of the inability to providing the medical payment, which confirmed by the final decision about the withdrawal of life-sustaining treatments. Therefore the judgement of this case ist appropriate in that sense, the medical obligation ist waived only after the final decision. However that legal basis lies not at the partial cancel but at the partial inability.

The Effect of Complementary Access to Milk Replacer to Piglets on the Activity of Brush Border Enzymes in the Piglet Small Intestine

  • The activity of brush border enzymes (sucrase, lactase and maltase) in the piglet small intestine was evaluated as well as piglet performance during the weaning period in the present study. There were two treatment groups: Piglets of six litters were fed dry feed plus milk replacer (Group M) and of six litters fed dry pelleted feed (Group C). One piglet from each litter was sacrificed on day 3 before weaning, and day 3, 10 and 17 postweaning, respectively. Providing milk replacer caused an increased piglet live weight at weaning (p<0.001) and until termination of the experiment (p<0.001). A slightly higher (p<0.16) level of protein was measured in the jejunum of group M piglets as compared with group C piglets. Before weaning the activity of lactase was high in the jejunum of group C piglets. The activity of lactase in the jejunum was lowered in the jejunum of group C piglets and in distal jejunum of group M piglets during the postweaning period as compared with pre-weaning period (p<0.05). Lowered activity of lactase in the distal jejunum of piglets was found at day 10 and 17 postweaning, respectively. No treatment differences were found in the activity of lactase in the piglet jejunum. No treatment differences were seen in the activity of maltase and sucrase in the piglet jejunum also. However, weaning caused a higher activity of sucrase in the distal jejunum of group M piglets as compared with pre-weaning period. In conclusion, providing milk replacer to piglets caused an improved growth performance. Feeding milk replacer did not influence the activity of lactase, maltase and sucrase in the jejunum of piglets. Weaning resulted in a markedly lowered activity of lactase, while no dramatic changes in the activity of maltase took place during the period around weaning.
