The purpose of this study was to compare muscle activity in the lower extremity during walking wearing jogging and roller shoes. Twelve male middle school students (age: 15.0 yrs, height 173.7 cm, weight 587.7 N) who have no known musculoskeletal disorders were recruited as the subjects. Seven pairs of surface electrodes (QEMG8, Laxtha Korea, gain = 1,000, input impedance >$1012{\Omega}$, CMMR >100 dB) were attached to the right-hand side of the body to monitor the rectus femoris (RF), vastus medialis (VM), vastus lateralis (VL), biceps femoris (BF), tibialis anterior (TA), and medial (GM) and lateral gastrocnemius (GL) while subjects walked wearing roller and jogging shoes in random order at a speed of 1.1 m/s. An event sync unit with a bright LED light was used to synchronize the video and EMG recordings. EMG data were filtered using a 10 Hz to 350 Hz Butterworth band-passdigital filter and further normalized to the respective maximum voluntary isometric contraction EMG levels. For each trial being analyzed, five critical instants and four phases were identified from the recording. Averaged IEMG and peak IEMG were determined for each trial. For each dependent variable, paired t-test was performed to test if significant difference existed between shoe conditions (p<.05). The VM, TA, BF, and GM activities during the initial double limb stance and the initial single limb stance reduced significantly when going from jogging shoe to roller shoe condition. The decrease in EMG levels in those muscles indicated that the subjects locked the ankle and knee joints in an awkward fashion to compensate for the imbalance. Muscle activity in the GM for the roller shoe condition was significantly greater than the corresponding value for the jogging shoe condition during the terminal double limb stance and the terminal single limb stance. Because the subjects tried to keep their upper body weight in front of the hip to prevent falling backward, the GM activity for the roller shoe condition increased. It seems that there are differences in muscle activity between roller shoe and jogging shoe conditions. The differences in EMG pattern may be caused primarily by the altered position of ankle, knee, and center of mass throughout the walking cycle. Future studies should examine joint kinematics during walking with roller shoes.
The course of the sural nerve in the calf has been well documented, but there is a general lack of information concerning the distal course of the nerve. The purpose of this study was to describe the distal course of the sural nerve and its surgical implications. Seven fresh amputated specimens were dissected to show the anatomy of the sural nerve in the foot and ankle. At the level of about 10cm proximal to the plantar surface, the sural nerve coursed anteriorly and inferiorly away from the Achilles tendon. 2 to 4 lateral calcaneal branches arose. The first branch of the lateral calcaneal branches coursed along the lateral border of the Achilles tendon, and it arose at 8cm proximal to the plantar surface in 2 specimens, 12cm proximal to the plantar surface in 4 specimens, and at 12cm proximal to the plantar surface in one specimen. The main nerve trunk continued distally plantar to the peroneal tendons and divided into two terminal branches and crossed peroneus longus tendon at the level of the inferior border of the calcaneo-cuboid joint, at about 3cm(range, $2.5\sim3.0$)cm from the plantar surface. In conclusion, a longitudinal incision lateral to the Achilles tendon would cross the path of the sural nerve at about 10cm proximal to the plantar surface. When the first branch of them arise more than 10cm above the plantar surface, a logitudinal incision lateral to the Achilles tendon may be made without damage. The other lateral calcaneal branches will be cut when we make transverse incision paralled to the plantar surface. The terminal branch also may be in danger by the same transverse incision.
This study investigated gait characteristics, kinematics, and kinetics in the lower extremities between two different shoe conditions (high heeled shoes (7 cm), and high heeled shoes with a total contact insert (TCI)) after lower extremity muscle fatigue. Although TCI shave been applied in high heeled shoes to increase comfort and to decrease foot pressure, no study has attempted to identify the effects of TCI in fatigue conditions. The purpose of this study was to determine the effects of walking in high heeled shoes with TCI after lower extremity muscle fatigue was induced. This study was carried out in a motion analysis laboratory at Hanseo University. A volunteer sample of 14 healthy female subjects participated. All in fatigue conditions, the subjects were divided into two groups. The muscle fatigue was induced by 40 voluntary dorsi- and plantar-flexion exercises and 40 heel-rise exercises of the dominant foot. Surface electromyography was used to confirm the localized muscle fatigue using power spectral analysis of three muscles (tibialis anterior, gastrocnemius medialis and lateralis). The results were as follows: (1) In muscle fatigue conditions, the use of TCI decreased the peak flexion angle of the hip joint significantly in the early stance phase (p<.05) and increased the peak hip flexion moment in the terminal stance phase (p<.05). (2) In muscle fatigue conditions, the application of TCI also increased peak hip power generation in the early stance phase and peak hip power absorption in the terminal stance phase (p<.05). (3) In muscle fatigue conditions, the use of TCI reduced the impact force significantly and increased the secondary peak vertical GRF. These findings suggest that the TCI may provide beneficial effects when muscle fatigue occurs for a high heeled shoe gait. Future research employing the patient population and various types of TCI materials are required to clarify the effects of TCI.
In this study, 26 normal subjects were studied to compare the biomechanical Analysis of Lower Limbs on Speed of Nordic Walking. The biomechanical variables were determined by performing three-dimensional gait analysis, and the measurements items were spatial and temporal parameters; vertical ground reaction force; and moments of the hip, knee, and ankle joints. The purpose of this study based on the speed of Nordic Walking to the vertical ground reaction force and joint moments of each were analyzed. Nordic Walking with poles while being whether this weight is reduced to load, not the improvement of muscle activity by identify Nordic walking is to allow efficient. The results of the analysis were follows. The spatial parameters of step length, stride length significantly increased with increase in velocity(p<0.001). The temporal parameters of step time, stride time, the duration of double support use, and the duration of single support use also significantly decreased with increase in velocity(p<0.001), but cadence significantly increased(p<0.01). Analysis of the changes in ground reaction force revealed that vertical ground reaction force significantly increased at the initial contact and the terminal stance and decreased at the mid stance with increase in velocity(p<0.001). Moments of the hip and knee joints significantly in creased with increase in velocity whereas that of the ankle joint did not. Gait analysis revealed that weight-bearing decreased and moments of the hip and knee joints increased with increase in velocity(p<0.01). The results of this study may help people perform Nordic walking efficiently and Nordic walking can be used in the gait training of people with an abnormal gait.

Metastatic cancer is the most common tumor of the skeleton. The prevalence of pathologic fracture may increase as patient survival is prolonged by improved cancer therapy. With recent advances in orthopaedic procedure and medical management of terminal cancer patients, it is generally agreed that aggressive treatment should be undertaken for patient with pathologic fracture secondary to metastatic disease, and a team approach should be utilized. The authors have reviewed twenty cases of pathologic fracture of the long bone due to metastatic tumor treated in the Department of Orthopedic Surgery, Yonsei University College of Medicine, from April 1989 to April 1994 and the following results were obtained. 1. The mean age at surgery was 58.4 years (ranged from 24years to 86years) and among 20 cases, 10 cases were male and the others were female. 2. The most frequent site of pathologic fracture in long bone is femur(15 cases, 75%), and followed by humerus(4 cases, 20%), tibia(1 case). 3. The frequently encountered primary tumors that metastases to long bone are those of the lung(7 cases, 35%), breast(4 cases, 20%), and prostate(2 cases, 10%). 4. The operative procedure was performed by resection of the tumor mass extensively, and we used polymethylmetacrylate for filling the dead space after resection, in all cases. 5. The mean survival period after operation is 9.2 months(ranged from 1 month to 4 years and 9 month). 6. The results of postoperative pain relief status were graded as fair to excellent in 17 cases(85%).
Purpose: Prior to closure of the epiphysis of the distal phalanx, fracture usually occurs through the growth plate, Salter-Harris type I or II, or through the juxtaepiphyseal region 1 to 2 mm distal to the growth plate. The terminal tendon of extensor inserts into the epiphysis only, while insertion site of the flexor digitorum profundus spans both the epiphysis and metaphysis. Because of the difference between these tendon insertions, this injury mimics a mallet deformity. But, this type of injury does not involve a tear or avulsion of the extensor, unlike mallet finger of adults. Seymour was the first to describe this type of injury in children and called after his name, Seymour's fracture. This fracture is prone to infection or remain the residual deformity unless adequate treatment. Methods: We report a case of Seymour's fracture. A 9-year-old boy presented a laceration of the nail matrix, with the nail lies degloved from the nail fold on the right middle finger gotten from an impact against a door. An X-ray examination showed the fracture line lying 1 mm distal to the growth plate. The injury was treated with debridement and the fracture was reduced by applying hyperextension force. Under the C-arm, a single 0.7 mm K-wire was used to immobilize the distal interphalangeal joint. Intravenous antibiotics were applied for 5 days after surgery. Results: The K-wire was removed in the 3rd week. No infection or significant deformity was found until follow-up of 12 months. Conclusions: Seymour's fracture may be at first classically mallet deformity by its appearance. But it is anatomically different and more problematic injury. If it isn't corrected at the time of injury, derangement of the extensor mechanism, and growth deformity of the distal phalanx may occur. The fracture site should be debrided, removed of any interposed soft tissue, and the patient should be given appropriate antibiotics. Reduction should be maintained by K-wire fixation. We experienced no infection or premature epiphyseal closure.

A rock slide in amount of 100 tons in weight happened at 7: 10 a.m., 4th October 1982, on the southwestern ridge of Mt. Seunghag, Busan City. The original rock mass of the rock slide is located 850m west from the Dangri Crushed Stone Quarry. The geology of the area consists of agglomerate, andesite, andesitic tuff, and shale hornfels of the Cretaceous Yucheon Group. The rock blocks were first shifted along the steep joint plane on an andesite outcrop at the site and then to the eastern foot of the mountain slope where some private houses are placed. The mountain slope is covered with thick superficial soil. A slided monolithic-block with 83 tons in weight from the rock slide met with an accident of striking against a house with a tremendous force, to which much damage was done as much as its half was destroyed. The rock-slided block pierced the board-floored room only posited at the center and by the bedroom of the house making a pass like a bullet hole, and hence cut a big pine tree with 24 centimeters in diameter at a distance of 26 meters down the house. However nobody was killed or injured in the stricken house, though seven family members were stayed therein at the very time of accident. They really met a rare opportunity in an unhappy disaster. Measurements of the rock slide were made in the course of the field survey. The monolithic mass was transported by way of saltation, rolling, and sliding to a distance of 300 meters down along the slope dipping 30$^{\circ}$ east. It took about 16 seconds front tile outcrop to the terminal. The acceleration value of the monolithic rock slide is 2.35m/sec$^2$.

In Korea mobile phone subscribers exceeded 1 million people in 1995, and ten million people in 1998. Prevalence of the mobile phone increased dramatically over 110% compared to the population. Almost every family members including children uses more than one mobile terminal. Mobile phone is obviously more necessary good than washer, vacuum cleaner and passenger car. Our daily life are conveniently positioned with mobile communication, but communication related costs including the price of the device and service charges become a big burden on the household. But the government does not seem to do the right role at the right time to prevent the market failure to maximize consumer welfare and social welfare through supporting fair competition. This principal-agent problem issues of mobile communication policies will be understood in perspective of public choice against joint use of the mobile communication network, number portability, separation of phone distribution and phone service, network neutrality, mobile phone subsidy since the early 1990s when mobile phone service was introduced in Korea.

The joint tactical information distribution system (JTIDS) is used as the communication terminal of Link-16 by the United States armed forces, north atlantic treaty organization (NATO), and other allied forces. A portion of Link-16 frequencies may be shortly remapped to other systems owing to the growing demand for frequencies, especially in civil aviation, which is witnessing a constant increase in air traffic. This will affect the performance of Link-16. Therefore, in this paper, we analyze the effect of frequency band reduction on the performance of Link-16 waveform under partial-band noise jamming with Nakagami fading, via simulation and numerical analysis. The multi-net and anti-jamming performance of Link-16 with frequency remapping is compared with that of conventional Link-16 systems. The results show that the performance of Link-16 waveform is degraded with the reduction in frequencies. Nonetheless, Link-16 retains its jam resistance, and it can support multiple users in the same time slots.

This study conducted joint research with the Navy logistics command ship technology research institute to resolve the occurrence of naval vessel's high-temperature warning and equipment shutdown caused by VRTU overheating during summer operation and the dispatch of troops to equatorial regions. The cooling effect was checked according to the installation of a thermoelectric device cooling system, and heat flow and heat transfer characteristics inside VRTU was analyzed using Computational Fluid Dynamics. In addition, the temperature distribution inside the engine room was assessed through interpretation, and the optimal installation location to prevent VRTU overheating was identified. As a result, the average volume temperature inside the VRTU decreased by approximately 10 ℃ with the installation of the cooling system, and the fan installed in the cooling system made the heat circulation smooth, enhancing the cooling effect. The inside of the engine room showed a high-temperature distribution at the top of the engine room, and the end of the HVAC duct diffuser showed the lowest temperature distribution.
