암환자를 돌보는 간호사의 임종간호수행에 영향을 미치는 요인 (Predictors of Terminal Care Performance of Clinical Nurses for Cancer Patients)

  • Purpose: The purpose of this study was to identify the influences of death perception, terminal care attitude on clinical nurses' terminal care performance for cancer patients. Methods: Data were collected through self-reported questionnaires filled by 526 nurses at a General Hospital in Seoul. Data were analyzed using a multiple regression analysis. Results: Death perception showed a positive correlation with terminal care attitude (r = .45, p < .001), while there was no correlation with terminal care performance. Additionally, terminal care attitude had a positive correlation with terminal care performance (r = .18, p < .001). The explanatory power of nurses' death perception and terminal care attitude toward terminal care performance was 14%. Conclusions: The study results imply that nurses' death perception and terminal care attitude are significant variables affecting terminal care performance.

Effects of Compassion Satisfaction, Compassion Fatigue, Burnout on the Attitude towards Terminal Care among Care Providers in Long- term Care Facilities

  • The purpose of this study was to investigate the effects of compassion satisfaction, compassion fatigue, burnout on the attitude towards terminal care among care providers in long-term care facilities. In order to achieve the purpose of the study, data was collected from 126 care providers in long-term care facilities in Chungbuk and Kangwon province. Data were collected with self-administrated questionnaires and analyzed by hierarchical multiple regression. Findings of this study is as follows. Firstly, attitude towards terminal care was positively affected by compassion satisfaction. Secondly, the attitude towards terminal care was negatively affected by compassion fatigue. Thirdly, the attitude towards terminal care was negatively affected by burnout. In conclusion, these findings provide strong empirical evidence for the importance of compassion fatigue, compassion satisfaction and burnout in explaining attitude towards terminal care among care providers in long-term care facilities. Therefore, strategies to decrease compassion fatigue and burnout and improve compassion satisfaction are needed to improve the attitude towards terminal care among care providers in long-term care facilities.

임종간호 태도에 영향을 미치는 융합적인 요인분석 (Convergence Analysis of the Factors Influencing Terminal Care Attitude)

  • 본 연구는 임종간호 태도에 영향을 미치는 요인들을 파악하기 위하여 시도하였다. 연구 대상은 종합병원에 근무하고 있는 간호사 190명이였으며 측정 도구는 죽음에 대한 인식, 죽음불안, 임종간호 스트레스, 죽음에 대한 태도, 소진, 임종간호 태도에 관한 문항으로 구성되었다. 죽음에 대한 인식, 죽음불안, 임종간호 스트레스, 죽음에 대한 태도, 소진, 임종간호 태도정도는 기술통계로 분석하였고 제 변수들 간의 상관관계는 Pearson correlation coefficients, 임종간호 태도에 영향을 미치는 요인은 다중 선형 회귀분석(Multiple linear regression)을 적용하였다. 임종간호 태도는 죽음에 대한 인식, 임종간호 스트레스와 유의한 정적상관성이 있는 것으로 나타났으며 죽음 불안, 죽음에 대한 태도, 소진과는 유의한 부적 상관성이 있는 것으로 나타났다. 죽음에 대한 인식, 죽음 불안, 임종 간호 스트레스, 죽음에 대한 태도 등이 유의미한 영향을 주는 것으로 나타나 임종간호 태도의 32.7%를 설명하였다. 본 연구 결과를 토대로 실제적인 임종간호 교육프로그램 개발에 활용할 수 있을 것이다.

종합병원간호사의 DNR(Do-not- resuscitate)에 대한 윤리적 태도와 임종간호수행에 관한 연구 (A Study on Ethical Attitude to DNR and Terminal Care Performance of Hospital Nurse)

  • Purpose. The purposed of this study was done to DNR attitude and terminal care performance among hospital nurses. Methods. The participants were 207 nurses working in hospital nurses in D and G cities. Data on DNR attitude and terminal care performance were collected via questionnaire between April 2015 and July 2015. Data analysis was done with SPSS 12.0 program and included one-way ANOVA, independent t-test, and Pearson correlation. Result. DNR attitude of participants in this study were shown to have high levels (DNR attitude: M=32.64/60, SD=6.14). Terminal care performance of participants in this study were shown to have poor levels (physical M=20.72/32, SD=3.77, psychological M=20.26/32, SD=3.85, spiritual M=9.62/24, SD=3.65). The attitude of the hospital nurse was significantly different according to the marital status. The terminal care performance was significantly different according to experience of terminal care. The DNR attitude by nurses was positively correlation to physioloical terminal care performance(r=.137, p<.049) but the relationship between the psychological terminal care performance( r=.016, p=.815) and spiritual terminal care performance showed no correlation(r=-.099, p=.157). Conclusion. The results of this study indicate that it is necessary to increase DNR attitude and to encourage terminal care performance among hospital nurses.

간호대학생의 죽음에 대한 태도와 회복탄력성이 임종간호에 대한 태도에 미치는 영향 (Influence of Attitude to Death and Resilience on Terminal Care Attitude among Korean Nursing students)

  • Purpose: This study aimed to explore the influences of attitude to death and resilience on terminal care attitude among Korean nursing students. Methods: Participants were 230 nursing undergraduates from two nursing schools in Korea. The students responded to a self-report questionnaire that included demographics, attitude to death, resilience, and terminal care attitude. Results: The majority of the participants who had undertaken a clinical practicum had experienced the death of a patient during their clinical placements but had not yet received any support from their instructors or professionals, but also academic training on patients' death or terminal care. The mean score of terminal care of the students who had death-related education was significantly higher than among those who had not. Regression analysis indicated that attitude to death, grade, and resilience were the most significant predictors of terminal care attitude. These explained 30.3% of their terminal care attitude. Conclusion: Death-related education is needed throughout the curriculum including not only death but also resilience to develop emotional competences. In this way, nursing undergraduates will be better prepared to cope positively and constructively with the suffering and death they encounter, and thus may minimize the distress they experience in the patients' dying process. It may also create a significant positive increase in their terminal care attitude.

  • Purpose: This study examined the effects of nurses' attitude toward death and their perception of hospice and palliative care on their terminal care stress in long-term care hospitals (LCHs). Methods: Participants included 127 nurses from 6 Incheon LCHs. Data were collected between July and August, 2020. Self-report questionnaires were administered to collect data on their general characteristics, terminal care stress, attitude toward death, and perception of hospice and palliative care. Data analysis included descriptive statistics, independent t-test, one-way ANOVA, Pearson's correlation coefficient, and stepwise multiple linear regression using the SPSS 23.0 statistical program. Results: Nurses' terminal care stress was affected by their attitude toward death (𝛽=.30, p<.001) and perception of hospice and palliative care (𝛽=.28, p=.002) with an explanatory power of 21.6%. Conclusion: Terminal care stress was significantly associated with their attitude toward death and perception of hospice and palliative care. Therefore, educating nurses in LCHs about death and hospice and palliative care is essential to manage their terminal care stress effectively.

  • Purpose: This study aimed to identify the factors affecting the terminal care stress of nurses in intensive care units in terms of their death perception, attitude toward terminal care, and mental health. Methods: This descriptive study collected data from 118 nurses in intensive care units in one tertiary referral hospital and three general hospitals. The instruments used in the study were the Terminal Care Stress Assessment Tool, the View of Life and Death Scale, the Frommelt Attitudes toward Nursing Care of the Dying Scale (FATCOD), and the Mental Health Assessment Tool. The data were analyzed by t-test, ANOVA, Pearson's correlation coefficient, and multiple regression. Results: There was a significant positive correlation between terminal care stress and death perception (r=.31, p<.001). The factors significantly influencing the terminal care stress of the participants included gender (β=.33, p<.001), religion (β=.24, p=.004), and death perception (β=.35, p<.001), and the overall explanatory power was 23.1% (F=12.73, p<.001). Conclusion: To decrease terminal care stress among nurses, establishing the death perception of nurses based on value clarification about death may be necessary. Furthermore, this study suggests an intervention study examining the effect of an education program on terminal care stress among ICU nurses.

임상간호사의 임종간호태도와 영적건강이 임종간호스트레스에 미치는 영향 (The Impact of Clinical Nurses' Terminal Care Attitude and Spiritual Health on Their Terminal Care Stress)

  • 목적: 본 연구는 임상간호사의 임종간호태도와 영적건강이 임종간호 스트레스에 영향을 미치는 요인을 파악하기 위한 조사연구이다. 방법: G 광역시 일 종합병원에 종사하는 근무하는 238명의 간호사를 대상으로 자가 보고식 설문지를 이용하여 자료를 수집하였다. 결과: 임상간호사의 임종간호스트레스에 영향을 미치는 요인은 임종간호태도, 영적건강, 결혼유무, 총 임상경력 순이었으며, 임종간호태도와 영적건강이 좋을수록, 기혼일수록, 총 임상경력이 높을수록 임종간호스트레스가 낮게 나타났다. 이들 변수들은 임종간호스트레스에 대하여 52.3%의 설명력을 나타냈다. 결론: 이상의 결과로 임상간호사의 임종간호스트레스의 가장 중요한 요인으로는 임종간호태도로 나타났다. 따라서 간호사의 임종간호스트레스를 효과적으로 대처하고 완화시키기 위해 임종간호태도와 영적건강을 증진시킬 수 있는 교육적 중재프로그램 개발이 필요하다.

호스피스 간호사의 생명윤리의식과 환자중심간호가 임종간호태도에 미치는 영향 (The Effect of Bioethics Awareness and Patient-Centered Care on Attitude of Terminal Care of Hospice Nurses)

  • 본 연구는 호스피스 간호사의 생명윤리의식과 환자중심간호가 임종간호태도에 미치는 영향을 파악하기 위해 수행된 서술적 조사연구이다. 대상자는 국내 병원의 호스피스 병동에서 근무하는 간호사 145명이다. 자료는 2018년 5월 1일부터 6월 30일까지 수집되었다. 자료의 분석은 SPSS 24.0 프로그램을 이용하여 서술적 통계, t-test, one way-ANOVA, Pearson 상관관계분석과 위계적 다중회귀분석을 실시하였다. 임종간호태도에 영향을 미치는 요소로는 호스피스 전문간호사 자격증(β=.15, p=.031), 생명윤리의식(β=-.24, p=.003)과 환자중심간호(β=.36, p<.001)가 나타났으며, 이 변수들은 임종간호태도를 39.6% 설명하였다. 본 연구의 결과를 토대로 호스피스간호사의 임종간호 향상을 위해서는 생명윤리의식을 포함한 환자중심간호 능력을 향상시키기 위한 프로그램이 제공되어야 한다.

간호사의 임종간호 태도에 미치는 영향요인 (Factors Influencing Nurses' Attitudes Toward Terminal Care)

  • Purpose : The purpose of this study was to identify the extent to which nurses' death perceptions, death anxiety, and stress associated with terminal care affects their attitudes toward terminal care. Method : A total of 214 nurses were asked questions about characteristics, death perceptions, death anxiety, stress associated with end-of-life care, and terminal care attitudes, using structured questionnaires. The data were analyzed using descriptive analysis, t-test, ANOVA, Scheffé's test, Pearson's correlation coefficients, and stepwise multiple regression. Results : Factors influencing terminal care attitudes in participants were positive meaning associated with death, respect for life, final education, end-of-life nursing education experiences, stress associated with end-of-life care, death anxiety, and position. These factors explained 38.7% of variance in participants' terminal care attitudes (F=20.18, p<.001). Conclusion : In order for nurses to have a positive attitude toward terminal care, it is necessary to raise positive awareness about death and respect for life, and it is necessary to develop various educational programs and strengthen their capacity through continuous education.
