텐던 구동 시스템의 마찰 모델 파라미터 추정 (Parameter Estimation of a Friction Model for a Tendon-sheath Mechanism)

  • Mechanical systems using tendon-driven actuators have been widely used for bionic robot arms because not only the tendon based actuating system enables the design of robot arm to be very efficient, but also the system is very similar to the mechanism of the human body's operation. The tendon-driven actuator, however, has a drawback caused by the friction force of the sheath. Controlling the system without considering the friction force between the sheath and the tendon could result in a failure to achieve the desired dynamic behaviors. In this study, a mathematical model was introduced to determine the friction force that is changed according to the geometrical pathway of the tendon-sheath, and the model parameters for the friction model were estimated by analyzing the data obtained from dedicated tests designed for evaluating the friction forces. Based on the results, it is possible to appropriately predict the friction force by using the information on the pathway of the tendon.

무지 신전건에 발생한 섬유종: 증례 보고 (Fibroma of the Extensor Digitorum Longus and Extensor Digitorum Brevis Conjoined Tendon Sheath: A Case Report)

  • Fibroma of the tendon sheath (FTS) was initially described in 1936 by Geschickter and Copeland as a benign firmed soft tissue tumor that is rare and less common than another soft tissue tumors, especially giant cell tumors (GCT) of the tendon sheath. The common distinct feature is a slow-growing least painful rare entity arising from the tendon or tendon sheath. FTS is detected mostly in the fingers, hands and wrists but less commonly in the foot. Very few cases of FTS have been described arising from a flexor tendon of the foot. This article describes a 51-year-old patient with FTS that developed in the extensor tendon of the foot, which is the only known FTS to form in this area. Heterogeneous low signal intensity in both the T1- and T2-weighted images was observed in magnetic resonance imaging. The lesion was excised completely by open surgery. Histologically, it showed randomly arranged, fibroblast-like spindle cells in dense fibrous tissue and had insufficient hemosiderin-laden macrophages that are typical for GCT.

  • The giant cell tumor of tendon sheath is very rarely present inside the knee joint. The authors report a case of intraarticular giant cell tumor of tendon sheath arising from posterior cruciate ligament which was successfully excised arthroscopically using posterior trans-septal portal at the time of arthroscopic reconstruction of anterior cruciate ligament.

Arthroscopic Treatment of Synovial Chondromatosis of the Shoulder Joint with Mini-open Procedure for the Lesions of Biceps Tendon Sheath

  • Synovial chondromatosis is an uncommon condition, and the involvement of the shoulder joint is rare. A 15 year old female patient presented to author's institution for right shoulder pain. We checked the plain radiographs and MRI. And they showed that a diagnosis of synovial chondromatosis in the shoulder, and they also demonstrated that the disease involved the bicipital tendon sheath as well as glenohumeral joint. We removed all loose bodies with total synovectomy by arthroscopic procedure, and a mini-open procedure for the lesions of biceps tendon sheath. Arthroscopic treatment affords excellent visualization of the shoulder joint with less morbidity. However, with current arthroscopic techniques, it is difficult to manage the synovial chondromatosis of biceps tendon in bicipital groove. The authors suggest that the complete elimination of synovial chondromatosis involving shoulder requires a mini-open procedure for the lesions of biceps tendon sheath in addition to the arthroscopic resection of the affected synovium and loose body removal in the glenohumeral joint.

족부의 중위지골에 천공을 일으킨 건막 거대 세포종 - 1례 보고 - (Giant cell tumor of the tendon sheath penetrating the middle phalanx of the foot)

  • We experienced 1 case of the giant cell tumor of the tendon sheath penetrating the middle phalanx of the third toe. Patient had painless huge mass all around the third phalanx of the foot which made a penetrating hole through the middle phalanx of the third toe in plain radiography. The period from the onset of the symptom until operation was about several years. The mass extended to the base of the distal phalanx and compressed digital nerves and vessels. Patient presented with a chief complaint of the pain on the plantar aspect of the toe and limitation of the motion of the distal interphalangeal joint. Giant cell tumor of the tendon sheath was cured by total en block resection.

Ganglion of the Flexor Tendon Sheath between A1 Pulley and A2 Pulley

  • Few articles have been written about the flexor tendon sheath ganglion in the finger, especially, between A1 pulley and A2 pulley. We report on rare cases of flexor tendon sheath ganglion with one symptomatic and two asymptomatic. All masses were evaluated using real-time ultrasonography and well-defined anechoic cystic lesions with posterior enhancement were observed. A 17-year-old female had a small mass at the 4th metacarpophalageal joint of her right hand, with pain and triggering. The patient underwent simple excision and a ganglion measuring $1.0{\times}0.8$ cm in size was derived from Camper's chiasm, between A1 pulley and A2 pulley. In two asymptomatic cases, ganglia measuring less than 0.5 cm in size observed. Based on our experience, real-time ultrasonography would be an excellent diagnostic modality in determining the treatment method in flexor tendon sheath ganglia, and surgical excision is recommended in symptomatic, especially triggering patients.

건초에 생긴 거대세포종에 대한 임상적 연구 (The Clinical Results of Giant Cell Tumor in Tendon Sheath)

  • Giant cell tumor of tendon sheath is a slow-growing, unilateral and solitary lesion that is most commonly seen in the digits of the hand and knee, but occasionally occurs in the hips, ankles, toes and wrists. The lesion is asymptomatic or mildly painful, especially if it is diffuse and located in a major joint. The concepts concerning about the pathogenesis of these lesion have undergone constant revision, which include neoplastic process, inflammation and lipid metabolism. Authors analysized 20 patients with giant cell tumor of tendon sheath about the etiology, clinical findings, pathologic findings and treatment results. The results were as follows: 1. 13 cases were females (65%) and 7 cases were males (35%), and the range of age was from 9 years old to 60 years old. 2. The hand was most frequently involved site in which 14 cases (70%) were included, and the foot was involved in 5 cases (25%). 3. Solitary lesions were 15 cases and multiple lesions were 5 cases. 4. Radiographically, in 3 cases bony erosion was seen. 5. All cases were treated by surgical excision and presented no recurrence. In conclusion, the giant cell tumor of tendon sheath, which has been considered to be benign tumorous conditions appeared to be necessary for complete surgical excision to prevent recurrence.

  • 국소형 건막 거대세포종이 체중 부하에 관여하는 큰 관절에서 발생하는 경우는 매우 드문 일이며 슬관절의 경우 지금까지 4례만이 보고되고 있다. 슬관절에서 국소형 건막 세포종이 발생할 경우 관절통, 종창, 잠김, 불안정감 또는 신전제한과 같은 증상이 나타날 수 있지만 임상적으로 십자인대파열 혹은 반월상연골 파열이 있는 경우와 감별하기는 어렵다. 저자들은 전방십자인대 재건술 때 국소형 거대세포종 2례를 우연히 관찰하였다. 두 경우 전방십자인대에서 발생하였으며, 관절경하에서 절제한 후 병리학적으로 진단을 확인하였다. 종양의 완전절제후 좋은 결과를 얻었기에 문헌고찰과 함께 보고하고자 한다.

  • Tarsal tunnel syndrome caused by ganglion as space occupying lesion is unusual and known that excellent result can be expected from surgical treatment carried out soon after onset of the condition. The object of the current study is to report our experience of tarsal tunnel syndrome caused by ganglion of flexor digitorum longus tendon sheath treated with operative management in a 39 year-old man with a review of the literature.

  • Synovial chondromatosis is a benign lesion forming multiple round cartilagenous nodules or osseous loose bodies in joint cavity. Predilection sites are known as knee, hip and elbow joints. However, the involvement of ankle joint was rarely reported in the literature. Moreover, extraarticular chondromatosis in synovial sheath or bursa of extremities is extremely rare. We present a case of synovial chondromatosis of the left ankle joint and flexor hallucis longus tendon sheath.
