Fibromyalgia: practical considerations for oral health care providers

  • Fibromyalgia is a syndrome characterized by chronic pain in the skeletal system accompanied by stiffness, sleep disturbance, fatigue, and psychiatric problems, such as anxiety and depression. Fibromyalgia commonly affects orofacial health, presenting with a variety of oral manifestations, including temporomandibular disorder, xerostomia, glossodynia, and dysgeusia. Therefore, oral healthcare providers need to be aware of this clinical entity to effectively manage oral symptoms and provide proper oral self-care modification and education on the nature of fibromyalgia. This review focuses on the epidemiology, pathophysiology, clinical manifestation, diagnosis, orofacial concerns, and treatment of fibromyalgia.

Could Crepitus Be an Indication for Early Temporomandibular Joint Osteoarthritis?

  • Purpose: To determine whether crepitus may be a clinical indication for early temporomandibular joint (TMJ) osteoarthritis (OA) and to investigate the correlation between crepitus and the occurrence of TMJ OA with respect to factors, such as patient sex, age, chewing habits, and diagnosis. Methods: This is retrospective analysis of clinical data for 162 TMJs. The criteria for a joint to be included in this study was a minimum of two cone-beam computed tomography (CBCT) scans performed with no OA observed during the initial scan. The Diagnostic Criteria for Temporomandibular Disorders was used for OA diagnosis. Crepitus was recorded when it was objectively palpated during the follow-up period. Correlations between various patient factors and progression to TMJ OA were calculated using the Pearson's chi-square test. A linear-by-linear association was used to analyze trends of OA progression with increasing age. Results: Among the 162 joints, 101 progressed to OA and 61 did not. In the joints where crepitus had been present before OA was confirmed at next or last CBCT, OA progressed at a high rate, and especially higher in female and older patients (p<0.01). Patients in the pain-related disorder group with crepitus were observed to have higher rates of OA progression compared to patients in the intra-articular disorder group (p<0.01). Conclusions: If a patient experiences pain in the TMJs and crepitus, close monitoring through regular CBCT scans is necessary even if there is no evidence of radiologically confirmed OA after the first CBCT.

The Correlations between Temporomandibular Joint Symptoms and Magnetic Resonance Imaging Findings in German Patients

  • Purpose: The purpose of this study was to evaluate the correlation between the clinical symptoms of temporomandibular disorder and findings in the magnetic resonance imaging (MRI). Materials and Methods: Clinical data and MRI images were collected from a total of 240 German patients. Clinical symptoms were briefed as joint clicking, crepitus and pain. MRI findings were further defined according to the condyle position, condyle degeneration, disc positon, disc degeneration and the presence of osteophyte/sclerosis/synovitis. Hypermobility was separately recorded. Correlation analysis between parameters was performed. Result: Joint clicking had a positive correlation with unilateral disc degeneration, osteophyte, sclerosis and synovitis. Crepitus had a significant correlation with bilateral osteophyte. Pain was not correlated with any MRI findings except hypermobility. Conclusion: Selective correlations between the MRI findings and clinical symptoms were elucidated. The results of this study imply that condyle-disc deformities could be advanced without pain, and that joint clicking and crepitus could be clinical symptoms of condyle-disc degeneration.

Organized hematoma of temporomandibular joint

  • Organized hematoma is a pseudo-tumorous lesion mostly occurs at sinonasal cavity and often confused with malignant neoplasm. The initiation of this lesion is blood accumulation, probably due to trauma, and this hematoma develops into organized hematoma as it encapsulated with fibrous band and neo-vascularized. Since it is uninformed at temporomandibular joint (TMJ) region, imaging diagnosis might be challenging. Also, delayed detection of mass involving TMJ is not uncommon due to confusion with joint disorder. Thus, this report introduced the rare pathology, organized hematoma on TMJ with advanced imaging features. Also, diagnostic point for early detection was described for the TMJ tumors and pseudo-tumors considering complexity of surgical intervention in this region.

턱관절음양균형요법에 의한 운동틱과 뚜렛장애 치료 보고 (Case Reports on Two Motor Tic Disorders and a Tourette's Disorder Managed by Yin-yang Balancing Therapy of the Temporomandibular Joint)

  • It was observed that the effectiveness of Yin-yang balancing therapy of the tempromandibular joint (YBT) or functional cerebrospinal therapy (FCS) in three cases: an acute and a chronic motor tic disorder and a Tourrette's disorder (TD). These three cases were mainly managed with cervical balancing appliance for the Yin-yang Balancing on tempromandibular joint (TMJ) and pelvic balance therapy. They were treated concurrently with acupuncture, cupping and herb-medicine. Clinical outcome measurement was based on subjective measures with visual analogue scale (VAS), Yale Global Tic Severity Scale (YGTTS) and clinical observations for 235, 279 and 273 days respectively. The patient showed positive changes after the treatment and this effect maintained over the follow-up period. Although it is not clear whether the effect is sustained afterwards or not, a positive effect on the motor tic disorders and TD was observed. And so, furthermore strict clinical and structural researches for verification on YBT is expected.

Reliability of Quantifying Maximal Mouth Opening and Lateral Mandibular Shift in Individuals With and Without Temporomandibular Disorder Using Three-dimensional Ultrasound-based Motion Analysis

  • Background: Although magnetic resonance imaging is accurate, it is expensive to measure the movement of temporomandibular joint. The three-dimensional (3D) motion analysis system is an inexpensive measurement tool. Objects: This study examined the reliability of quantifying the mouth opening and lateral mandibular shift and differences between individuals with and without temporomandibular disorder (TMD) using the hygienic method of surface markers on the skin with 3D ultrasound-based motion analysis. Methods: This study included 24 subjects (12 with and 12 without TMD). Temporomandibular joint motion during mouth opening was recorded using two surface markers with 3D ultrasound-based motion analysis. An intraclass correlation coefficient [ICC (3,k)] was used to confirm the intrarater reliability of quantifying kinematic temporomandibular joint motion, and an independent t-test was used to evaluate differences in maximal mouth opening and lateral mandibular shift between the two groups. Results: Assessment of mouth opening and lateral mandibular shift showed excellent test-retest reliability with low standard error of measurement. The lateral mandibular shift and opening-lateral mandibular shift ratio were significantly increased in the TMD group during maximum mouth opening (p<.05). However, no significant difference in maximal mouth opening was observed between the groups with and without TMD (p>.05). Conclusion: This hygienic and simple surface marker method can be used to quantify the mouth opening and lateral mandibular shift at the end-range of mouth opening. The TMD group showed an increased lateral mandibular shift movement at the end-range of mouth opening. The lateral mandibular shift movement can be regarded as a symptom in the diagnosis and treatment of TMD.

턱관절장애 유무에 따른 깨물근, 목빗근의 두께 및 근긴장도, 최대 입벌림 범위의 비교 및 상관성 연구 (Comparison and Correlation on Muscle Thickness and Muscle Tone of Masseter Muscle and Sternocleidomastoid Muscle, Maximum Jaw Opening in Subjects With and Without Temporomandibular Joint Disorder)

  • Purpose : Temporomandibular joint disorder (TMJD) is often accompanied by pain and limited range of motion of the jaw joint, which affect patients' quality of life and result in hypertrophy or hyperactivity of the muscles around the jaw joint. In this study, we compared the muscle thickness and tone of the masseter and sternocleidomastoid (SCM) muscles and the jaw range of motion in individuals with and without TMJD. Correlation comparison was performed on the results of the TMJD group. Methods : This study included 40 patients; 20 patients were assigned to an experimental group (TMJD group) and 20 to a control group (non-TMJD group). Ultrasonography, myotonometry, and measurements performed with digital Vernier calipers were used to determine the changes in muscle thickness, muscle tone, and maximum jaw opening, respectively. The independent t-test was used for intergroup comparison of data, and Pearson correlation coefficients were used to compare correlations in the TMJD group results. Results : We observed a significant intergroup difference in the masseter and SCM thickness during the relaxed and clenched phases (p<.05). A significant intergroup difference was also observed in maximum jaw opening (p<.05). With regard to muscle tone, we observed a significant intergroup difference in frequency (p=.011) and stiffness (p=.011) of the masseter, as well as in the frequency (p=.009) and stiffness (p=.026) of the SCM. We observed a moderate negative correlation (r=-.524) between maximum jaw opening and the frequency of the masseter. Additionally, we observed a moderately negative correlation between jaw opening and muscle stiffness (r=-.321). Conclusion : Planning exercise programs to treat patients with TMJD who present with pain should focus on efforts to reduce muscle thickness and achieve muscle relaxation (to reduce muscle tension) for improved jaw range of motion.

Sleep-related Breathing Disorder and Its Relationship with Temporomandibular Disorders: Literature Review

  • In order to establish a relationship between sleep related breathing disorder (SBD) and temporomandibular disorders (TMDs), a literature review was performed. A researching was performed based on PubMed, including english languages. Any clinical study identified relationship between SBD and TMD were selected in this review. 13 studies were analyzed in this review. According to SBD diagnosis, articles were grouped as follows: polysomnographic diagnosis (n=7), clinical diagnosis (n=2) and survey/self-report (n=4). 12 articles established a positive relation between SBD and TMD and 1 did not. SBD would be associated with protrusion/retrusion degree, myofascial pain, muscular and joint pathology, and other orofacial findings. It was analyzed that the retrusion of the mandible had the significant influence on the severity of the SBD.

목 안정화 운동이 턱 관절 장애 환자의 라이프 케어를 위한 턱 관절 기능 및 압통역치에 미치는 효과 연구 (Effect of Cervical Stabilization Exercise on Temporomandibular Joint Function and Pain Threshold for Life Care of Patients with Temporomandibular Joint Disorder)

  • 본 연구의 목적은 상부 목 안정화 운동이 턱 관절 기능장애과 압통 역치에 미치는 효과를 알아보기 위하여 연구를 진행 하였다. 36명의 대상자를 상무목 안정화 운동 그룹과 대조군 두 그룹으로 설정하여 상부 목 안정화 운동은 압력센서를 활용하였으며, 연구를 총 4주간 주 3회 20분씩 진행하였다. 연구결과 개구범위에 있어서 상부 목 안정화군이 대조군보다 유의한 효과를 보였으며(p<.01, 95% CI: 8.074, 16.899), 우측 치우침에서 상부 목 안정화 운동군이 대조군보다 유의한 효과를 보였다(p<.01, 95% CI: .826, 3.243). 압통 역치의 변화에서는 관자근(p<.01, 95% CI: .251, 1.382)과 깨물근(p<.01, 95% CI: .462, 1.826) 모두 상부 목 안정화군이 유의한 향상을 보였다. 본 연구결과 턱 관절 기능장애 환자에게 상부 목안정화 운동이 턱 관절 장애에 효과적이었다. 턱 관절 장애를 경험하고 있는 환자들에게 더욱더 효과적인 중재방법을 제공할 수 있을 것이며, 나아가 턱 관절 장애로 인한 고통받고 있는 환자들에게 새로운 접근방법을 제시할 수 있을 것이다.

  • Objectives : This study was carried out to investigate the effects of Chuna & General Oriental Method on Temporomandibular Joint Disorder Patient(Functional Lateral Mandibular Displacement) with Many Clinical Symptoms. Methods : The therapies were Chuna, acupuncture treatment and herbal medicine. We estimated the effects of treatment by measuring the length from the end of frenulum labii superioris to the frenulum labii inferioris, Visual Analog Scale(VAS) and Patient Global Assessment(PGA) before and after treatment. Results : After treatment, we confirmed these improvements: the length from the end of frenulum labii superioris to the frenulum labii inferioris changed from 10-7mm to 0-2mm, the VAS was changed from 10 to 0-2 and many clinial symptoms improved. Conclusions : These results suggusted that Chuna & General Oriental Method effected for Temporomandibular joint disorders caused many clinical symptoms.
