영상치의학 분야에서의 원격판독 (Teleradiology in Oral and Maxillofacial Radiology)

  • Teleradiology is the electronic transmission of radiologic images from one location to another for the purpose of interpretation and/or consultation. Today, images can be easily transmitted to another location using the high-speed internet. In medical area, teleradiology has developed rapidly in recent 10 years due to the imbalance between the demand and availability of diagnostic services. However, teleradiology in dentistry is still inactive. I would like to overview the current teleradiology system, and discuss the necessity and the potentiality.

Clinical usefulness of teleradiology in general dental practice

  • Purpose: This study was performed to investigate the clinical usefulness of teleradiology in general dental practice. Materials and Methods: Two hundred and seventy five cases were submitted for inquiry to the case presentation board of the website of The Korean Academy of Oral and Maxillofacial Radiology for a 5 year periods. The diagnosis results of those cases were analyzed according to the disease classification, the correlation with the patient's chief complaint, the necessity of additional examinations or treatments, the image modalities, and the number of dentists inquiring. Results: Differential diagnoses of normal anatomic structures were the most frequently submitted cases, covering 15.6% of all cases. Among 275 cases, 164 cases required no additional treatments or examinations. Panoramic radiographs were the most frequently submitted images, accounting for 248 inquiries. The 275 cases were submitted by 96 dentists. Fifty-two dentists wrote one inquiry, and 44 inquired 2 or more times. The average inquiry number of the latter group was 5.0 cases. Conclusion: A teleradiology system in general dental practice could be helpful in the differential diagnosis of common lesions and reduce unnecessary costs.

Picture Archiving and Communication System(PACS) & Teleradiology

  • Picture archiving and communications systems(PACS) for digital image distribution, archiving and transmission, represent the modern hospital. PACS is the medical image management system within a hospital, whereas teleradiology system is the medical image communication system between remote sites. PACS and teleradiology systems usually consists of mainly four parts such as image acquisition part, image storage and database part, Image communication network part, and image display workstation part. Among these components comprised In the PACS, workstations are the point of contact between a PACS and the radiologist or referring physician. Therefore, the acceptance of PACS is highly dependent on workstation functionality, performance and easy of use compared with the conventional film-based radiology. This paper describes overall configurations and some requirements of PACS and teleradiology.

원격판독 의료기관에서 시행한 원격판독의 동료평가: 세부전공분야와 촬영장비에 따른 판독 불일치 정도와 불일치 판독의 임상적 의의 비교 (Peer Review of Teleradiology at a Teleradiology Clinic: Comparison of Unacceptable Diagnosis and Clinically Significant Discrepancy between Radiology Sections and Imaging Modalities)

  • 목적 동료평가를 시행하여 원격판독 불일치의 정도와 불일치 판독의 임상적 의의를 세부전공분야와 촬영장비별로 평가하고자 하였다. 대상과 방법 한 국내 원격판독 의료기관에서 2018년과 2019년에 시행한 동료평가를 대상으로 세부전공분야와 촬영장비별로 판독 불일치를 분석하였다. 동료평가 점수를 적격 진단과 부적격 진단으로 분류하였고, 판독 불일치는 임상적 의의에 따라 무의한 불일치와 유의한 불일치로 분류하였다. 카이제곱 검증으로 통계를 시행하였다. 결과 총 1312건의 동료평가 중 부적격 진단은 117건(8.9%)이고, 총 462건의 진단 불일치 중 유의한 불일치는 104건(21.6%)이었다. 세부전공분야에서 부적격 진단율은 근골격(21.4%)이 가장 높았고(각각 p < 0.05), 복부(7.3%)는 신경두경부(1.3%)에 비해 높았다(p < 0.05). 유의한 불일치는 흉부(32.7%)가 복부(17.1%)와 근골격(19.5%)에 비해 높았다(각각 p < 0.05). 촬영장비에서 부적격 진단은 MR (16.2%)이 일반촬영(7.8%)에 비해 높았고(p < 0.05), 유의한 불일치는 차이가 없었다. 결론 동료평가를 통해서 원격판독의 부적격 진단과 임상적 유의한 불일치의 정도, 세부전공분야와 촬영장비에 따른 차이를 알 수 있었다.

원격 진단 시스템에서 의료영상의 적절한 해상도 및 압축방법 결정에 관한 연구 (Determination of the adequate resolution and compression method in teleradiology)

  • This study was performed to determine the adequate resolution and compression method in teleradiology. A digital imaging system using Machintosh IT ci computer, 15' Sony high resolution RGB monitor, Umax Power look flatbed scanner with transparency unit and 12 panoramic radiographs were used. The results were as follows : 1. Relative detectability at the group scanned by 30ddpi, 600dpi and 1200dpi was same as those at the real panoramic radiographs. 2. Perceivable image quality degradation was found at the 25% of middle quality of JPEG compression. But those were not diagnostically significant. 3. Perceivable image quality degradation was found at the 100% of low quality of JPEG compression. And 8cases among them were diagnostically significant. On the basis of the above results, it is considered that the adequate resolution in scanning radiographs for teleradiology is 300dpi and compression method is the middle quality of JPEG compression.

  • 최근 코로나 19가 장기화하면서, 비대면서비스로 대체되고 있는 한편, 의료분야에도 서비스 패러다임이 변화되고 있다. 특히, 국내의 법 제도적으로 묶여 있는 원격 의료서비스의 적용이 가능하고 상급종합병원에서는 비대면 진료서비스를 도입하고 있다. 본 논문에서 제안하는 비대면 원격판독시스템은 모바일 의료영상진단기기를 기반으로 의료사각지대에 있는 환자들의 영상촬영과 이에 대한 판독 서비스를 제공하기 위한 시스템이다. 제안한 시스템은 의료환경에 적용하기 위해 환자의 개인정보를 보호하고, 원격으로 환자의 영상 데이터를 판독하기 위한 시스템과 그 처리 과정을 보인다. 그리고 끝으로 구축된 시스템의 수행 결과를 보인다.
