3차 종합병원 안과병동의 전화 상담 중 전화문의 내용 및 간호중재에 대한 조사 연구 (Telephone Inquiries to the Ophthalmology Nursing Unit and Nursing Interventions Through Telephone Consultations)

  • Purpose: This study examines the patient problems and nursing interventions during telephone consultations by nurses in an ophthalmology nursing unit. Methods: The data were collected during telephone consultations. A total of 13 nurses consulted 170 patients between September 29 and November 30, 2008 at a tertiary teaching hospital. Problems were raised by the patients and nursing interventions were provided to the patients by the nurses. The SPSS program was used to analyze the data. Results: There were 228 telephone inquiries by 170 patients. The problems were categorized into four groups based on a literature review and validated by 14 nurses: physical symptoms, medications, administrative matters, and follow-up care. Interventions provided by the nurses were categorized into six groups: instructing patients; providing information; providing background knowledge; assuring the patients; referring to the patients to other ancillary departments; and delivering doctor's orders. Conclusions: Nurse interventions for a specific patient problem varied depending on the nurse providing the consultation. The participating nurses expressed the need for a common protocol for telephone consultations. Thus, it would be beneficial to develop a guideline for telephone consultations to minimize practice variations among nurses.

RELP 방식을 이용한 디지털 음성 응답기 (A Digital Audio Respose System Based on the RELP Algorithm)

  • 본 논문에서는 디지탈 자동 음성 응답장치의 개발에 관하여 전반적인 사항을 기술하였다. 개발된 디지탈 음성 응답 장치는 전화국에서 가입자가 전화번호를 문의하였을 때 자동 응답할 구 있도록 특별히 구성된 시스템이다. 본 시스템의 구현 algorithm으로는 pitch predictive loop(PPL)을 가지는 RELP(residual excited linear pediction)방식을 사용하였는데 system memory는 비교적 적은 반면 음질은 아주 좋은 것이 개발된 자동 응답기의 큰 장점이라 하겠다. Hardware는 bit-slice microprocessor를 사용한 음성 합성기와 controller 및 I/O로 이루어져 있는데 이들은 실시간 신호처리와 시스템의 적응성 및 신뢰성을 고려하여 설계하였다.

서울시 주민생활지원을 위한 공공시설의 현황 및 복합화 특성에 관한 조사연구 (Research on the Status Inquiry and Complex Characters of Public Facilities for the Community Life Style in Seoul)

  • The aim of this research is to understand the characteristics and status of existing public facilities, in order to propose the necessary basic materials for the planning and development of improved public facilities to assist communities. Objects of this research are the public facilities provided by the Government throughout the 25gu in Seoul. The methods of this research were carried out through the home page of each facility, telephone inquiries and gujung backseo. Investigations were carried out 3 times between the periods of January-June and September in 2009. Results of the research are as follows - 1) Through the investigations of classifications and distribution status of 5 public facilities over 25gu, quantity supply problems were analyzed. 2) Building distributions of 5 public facilities were classified by types and analyzed. 3) A number of compound facilities were classified and analyzed to determine the complexities of the 5 public facilities. 4) Complex types and characteristics of public facilities in 25gu in Seoul were analyzed. Based on the results of the research, the following conclusions are drawn: 1) Public facilities throughout the 25gu in Seoul have common characteristics and functions and they are spread out evenly in each gu. This means that public facilities are standardized in Seoul. Characteristic public facilities suitable to each gu should be planned accordingly. 2) Public facilities in Seoul show a single form of one facility. Therefore, initiative plans for the complexity of facilities are required. 3) Complex public facilities in Seoul need to be the center of community life in each gu. It is necessary to take into account the diverse life styles and classified needs of residents for complex facilities. Based on the research, complex types such as public administrative+cultural facilities, culture+welfare facilities, medical+welfare facilities can be presented. These complex types should not be classified according to any particular use, but they should be commonly used by diverse generations and every social class. The inquiries of users of public facilities and the programs of each public facility are not included in this research. Thus, the reseaarch limits the proposals for complex public facilities. In the future, the characteristics of each gu in Seoul should be studied and understood, so that appropriate public facilities and their complex plans can be researched continuously and necessarily.

인터넷 게시판 질문 분류를 위한 인터랙티브 접근방법에 관한 연구 (An Interactive Approach to Categorize Questions on the Internet BBSs)

  • 전통적인 고객지원방법에서는 콜 센터와 서비스 센터가 고객의 질문과 요구 사항을 접수하고 응대하는 기능을 담당해왔다. 최근 인터넷의 급속한 확산에 따라 전화, 우편, 방문 등의 전통적인 고객과의 의사소통수단이 전자우편과 인터넷 게시판과 같은 웹기반의 고객지원시스템으로 전환되고 있다. 인터넷 게시판은 기본적으로 고객의 질문에 관리자가 응답하는 시스템이므로 고객이 응답을 받는데 시간이 걸리는 제약이 있다. 이러한 시간적 제약을 해결하기 위하여 고객이 인터넷을 통하여 고객지원시스템에 접속하여 미리 구축된 지식 데이터베이스로부터 원격에서 질문에 대한 응답을 받을 수 있도록 공통적인 질문과 응답을 FAQ와 같은 형태를 제공한다. 그리고, 인터넷 게시판에 다양한 내용과 형태의 질문이 혼재되어 사용됨으로써 응답과 관리상의 어려움이 많다. 따라서 질문들을 체계적으로 분류하여 FAQ를 만들고, 인터넷 게시판의 관리작업을 지원하기 위한 도구의 필요성이 대두되고 있다. 본 연구에서는 키워드와 키워드들간의 친밀도를 이용하여 벡터형태로 표현한 질문들간의 유사 도를 계산하여 질문들을 클러스터링 하는 방법을 제안한다. 제안한 방법은 기본적으로 자동으로 질문들을 분류하지만, 내용이 애매모호한 질문의 경우 사용자가 상호작용을 통하여 사용자의 판단을 받아들일 수 있도록 개발되었다. 그리고, 제안한 방법의 성능을 평가하기 위하여 프로토타입 시스템을 개발하고 제한된 상황하에서 실험을 수행하였다.

2015년 12월 22일 발생한 익산지진의 단층면해와 진도 분석 (An Analysis of the Fault Plane Solution and Intensity on the Iksan Earthquake of 22 December 2015)

  • 2015년 12월 22일 전북 익산시 북쪽 지역에서 발생한 익산지진($M_L=3.85$)의 본진 및 2개 여진에 대해 FOCMEC (FOCal MEChanism determination) 프로그램을 이용하여 단층면해를 구하였다. 본진의 단층면해는 북동-남서 또는 북서-남동 방향의 단층면을 가지는 역단층성 주향이동단층운동의 특성을 보이며, 이는 남한 내륙지진의 단층운동 특성과 거의 유사하다. 익산지진 전후 발생한 미소지진 이벤트를 검출하고자 2015년 12월 15일~2016년 01월 22일 진앙으로부터 반경 100 km 이내에 위치한 13개 관측소에 기록된 연속 지진파형 자료를 PQLII 프로그램(PASSCAL, 2017)으로 분석하고, 19개 지진의 진원지를 새로 결정하였다. 미소지진의 진앙 분포는 특정 단층 혹은 구조선에 집중되는 현상은 나타나지 않았으나, 익산지진과 3개의 여진은 한곳에 집중 분포한다. 익산지진의 진도 분포는 전화 문의 및 피해 접수 자료를 수집하여 구하였으며, 또한, 각 관측소에 기록된 PGA (Peak Ground Acceleration)를 이용하여 계기진도를 도출하였다. 그 결과 익산지진의 MMI 진도등급은 최대 V로 평가된다.

검진 기관에서의 선별 유방촬영술 시행에 따른 연령 제한의 필요성에 대한 연구 (A Study on the Necessity of an Age Limitation in Screening Mammography)

  • National Cancer Screening Project and Korean Society of Breast Imaging recommend that breast cancer screening should be performed on those aged 40 and above. Nevertheless, this recommendation is usually ignored by a number of medical institutions. The purpose of this study is to emphasize the necessity of an age limitation in screening mammography. Ten institutions were randomly selected and telephone inquiries about patients' age limitation and internal guidelines were set up. The 3,214 women, who underwent screening mammography through 'GE Senography 2000D' in each hospital, were classified into five groups according to age(from 20s to 40s, at intervals of 5). And then, collected data was analyzed by a radiologist in accordance with ACR-BIRADS(American College of Radiology Breast Imaging Reporting and Data System), through which breast parenchymal density and the results of analysis were categorized in order to predict the sensitivity of mammography. Information about craniocaudal-view mammograms was automatically produced by use of GE Senography 2000D, and the average glandular dose was retrospectively analyzed through the program 'Excel 2007.' Two institutions did not set the age limitation. Other seven institutions internally allowed those who wanted to receive mammography regardless of age. Approximately 99% of those aged 20 to 29 were judged as having the dense breast. In those aged 35 to 39, breast parenchymal density tended to be lower, but the fatty breast to increase. In the case of 'category-zero' that does not need additional tests, the rate of 'heterogeneously dense' and 'extremely dense' reached to 83.1% and 15.1% respectively. Regarding dense breasts, there was no sufficient information for image reading. The glandular dose, applied to 3,214, was 1.47mGy on the average. In those aged 20 to 24 who are sensitive to radiation, the average glandular dose indicated 1.59mGy. Those aged 35 and above showed the lowest value, 1.43mGy. In those aged 35 to 39, the breast tended to change from denseness to fattiness. The average glandular dose was lowest in those aged 35 and above, which suggests that screening mammography should be periodically performed on those aged 35 and above in order that breast cancer may be early detected. On the other hand, in those aged less than 35, it is difficult to analyze mammograms due to the high density of breast parenchyma, and also retakes become frequent. In particular, subjects may be exposed to excessive doses. Accordingly, it should be substituted by breast self-examination or clinical breast examination. In case of need, it is advisable to perform ultrasonography.

기자의 통화내역 조회와 취재원 보호 간의 갈등: 통신비밀보호법 제13조 제2항 논의를 중심으로 (A Study on the Conflict Between the Call for Journalists' Phone Records and the Shield Law: Focusing on the Review of Paragraph 2, Article 13 of the Act of Protection of the Secrecy of Correspondence)

  • 헌법은 통신의 자유와 비밀을 기본권으로 보장하고 있다. 주거의 자유와 사생활의 자유를 기본권으로 규정하는 한편, 별도로 통신의 자유를 보장하고 있는 것은 그것이 개인과 개인간의 비공개적인 의사소통의 성격을 가지고 있기 때문이라고 된다. 유 무선을 통해 취재가 빈번하게 이뤄지는 언론인들에 대한 통신비밀의 침해, 구체적으로 취재기자들에 대한 통화내역을 조회하고 이를 토대로 취재원들을 추궁하는 것은 통신의 자유와 비밀 보장이라는 기본권 침해일 뿐만 아니라 언론자유와 국민의 알권리에 대한 중대하고도 심각한 위협이라고 할 수 있다. 이 연구는 취재기자들에 대한 통화내역조회의 합법적 근거가 되고 있는 통신비밀 보호법 제13조 제2항의 규정이 영장주의와 명확성의 원칙은 위반함으로써 위헌의 가능성이 매우 크다는 점을 지적하면서 통비법상 동조의 개정방향을 제시하였다. 통화내역 조회의 절차에 있어서 검사의 청구에 외해 법관이 발부한 영장을 제시하도록 하고, 당사자에게 반드시 통보하도록 조치하며, 국가안보에 위해가 되는 통화내역 조회의 대상과 범위, 기준은 명확히 규정해야 한다고 보았다. 더불어 언론인들도 취재원을 보호하고, 취재원을 다양한 방식으로 활용하는 새로운 취재 기법의 개발과 적용을 위해 배가의 노력을 할 필요가 있음도 지적하였다.

Test turnaround Time for Complete Blood Cell Count using Delta and Panic Value Checks and the Q-flag Limit

  • Test turnaround time (TAT) is the lead time from reception to reporting. In the complete blood cell count (CBC), 4 units of the XE-2100 (Sysmex Corp., Japan) processed around 80% of quantity, 1 unit of the LH-780 (Beckman-Coulter Incorp., USA) processed around 10% and 1 unit of ADVIA-2120 (Siemens AG, Munich, Germany) processed around 10%. We analyzed the change in the TAT for the CBC for over 7 years, from January of 2005 to December of 2011. The delta check made alterations of delta to WBC, hemoglobin, hematocrit, platelet and metamyelocyte, however, did not made them to band neutrophil, eosinophil, basophil and monocyte. The panic value check made alterations of panic value to hemoglobin, hematocrit, platelet and monocyte. In the criteria of currently slide review, LH-780 and ADVI-2120 analyzers prepared suspect flags of "Blast, Imm NE2, Immature granulocyte, Imm NE1, Left shift, Variant lymphocyte, Atypical lymphocyte, Platelet clumps and NRBC". The New slide review in the XE-2100 analyzer altered the preparations of a smear slide more than a "Platelet clumps flag(${\geq}200unit$), a single flag excluding the "Platelet clumps flag (${\geq}250unit$) and a multiple flag (${\geq}200unit$)". Also, below the 240 unit, medical technologists prepared manual slides selectively according to their evaluations. The automatic reporting rate was 33.4% without alterations, whereas it was 41.0% without alterations, and was thus improved by 7.6%. The slide review rate was 15.2% before using the Q-flag limit, whereas it was 12.1% for a reduce 3.1%. TAT was 45 minutes without the creation alterations of the delta and panic value checks, whereas it was 35 minutes after making alterations of the delta and panic value checks and thus was shortened by 10 minutes. We came to the conclusion that the establishment and operation of delta and panic value checks and slide review criteria suitable for laboratory environment can reduce unnecessary smear slides, re-checking, re-sampling, re-testing, telephone inquiries and concentrated workloads during specific times of the day.

뇌졸중 환자에 적용한 핫라인 전화상담 프로그램의 효과 (Effects of Telephone Hotline Counseling Program on Stroke Care)

  • Background: This study focuses on the establishment and operation of a stroke patient hotline program to help patients and their caregivers determine when acute neurological changes require emergency attention. Method: The stroke hotline was established at the Gyeonggi Regional Cerebrovascular Center, Seoul National University Bundang Hospital, in June 2016. Patients diagnosed with stroke during admission or in outpatient clinics were registered and provided with stroke education. Consulting nurses managed hotline calls and made decisions about outpatient schedules or emergency room referrals, consulting physicians when necessary. The study analyzed consultation records from June 2016 to December 2020, assessing consultation volumes and types. Outcomes and hotline satisfaction were also evaluated. Results: Over this period, 6,851 patients were registered, with 1,173 patients (18%) undergoing 3,356 hotline consultations. The average monthly consultation volume increased from 29.2 cases in 2016 to 92.3 cases in 2020. Common consultation types included stroke symptoms (22.3%), blood pressure/glucose inquiries (12.8%), and surgery/procedure questions (12.6%). Unexpected outpatient visits decreased from 103 cases before the hotline to 81 cases after. Among the 2,244 consultations between January 2019 and December 2020, 9.6% were recommended hospital visits, with two cases requiring intra-arterial thrombectomy. Patient satisfaction ratings of 9-10 points increased from 64% in 2019 to 69% in 2020. Conclusion: The stroke hotline program effectively reduced unexpected outpatient visits and achieved high patient satisfaction. Expanding the program could enhance the management of stroke-related neurological symptoms and minimize unnecessary healthcare resource utilization.
