The Etiology & Treatment of Unilateral Vocal Cord Paralysis : A 10-Year Review of 210 Patients (편측성대마비의 원인과 치료 : 10년간 경험의 분석)

  • It is well known that unilateral vocal cord paralysis is a sign of a disease and not a disease entity. In an attempt to evalute incidence of unilateral vocal cord paralysis related to varoius causes and to analyze our treatment results, the records of 210 patients seen at the Department of Otorhinolaryngology, Yongdong Severance Hospital during the 10-year period from March 1985 to March 1995 were reviewed. Fifty-three patients(25.2%) of the 210 patients with unilateral vocal cord paralysis had surgery-related causes, and intubations including tracheostomy was the most common etilogy among them. One hundred and fifty seven cases(74.8%) was not related to surgery and, among these most common cause was idiopathic. Spontaneous recovery took place in 17(8%) patients. Phonosurgery including type Ⅰ thyroplasty(n=16) and arytenoid adduction(n=10) and combination of them(n=3) yielded good results except intrafold teflon injection(n=4). These results of our series were compared to other series reported over the past decades.

  • Surgical treatment options of symptomatic unilateral vocal fold paralysis are Teflon injection, type Ⅰ thyroplasty, and arytenoid adduction. Arytenoid adduction is preferable to type Ⅰ thyroplasty for correcting the level different that may be present between two vocal folds and the large glottal chink However there is no known therapeutic modality effective to correct the large posterior glottal chink of the vocal fold with relatively normal mobility. Recently we have experienced a case of severe large posterior glottal chink of the vocal 1314s with relatively normal mobility after thyroid lobectomy, successfully treated with type Ⅰ thyroplasty combined with arytenoid adduction.

  • The purpose of this study was to propose a new dental color system through color evaluation of generally used dental composite and porcelain. In this study, Vita Omega 900 dental porcelain and 10 kinds of composite resins were used. A disk specimens(15mm diameter, 4mm thickness) of each material/shade combination were made. Resin compasite was condensed into Teflon mold and pressed between glass plates to flatten the surface. Pressed compasite was polymerized using an Optilux 501(Demetron, USA) for 200 seconds of each side.(중략)

Objectives The purpose of this study was to compare in vivo the biocompatibility of new calcium phosphate-based root canal sealers(CAPSEAL I, CAPSEAL II) with another type of commercially available calcium phosphate sealer (Apatite Root Sealer type I, Apatite Root Sealer type II) and zinc oxide-eugenol-based sealer (Pulp Canal Sealer EWT) after implantaion in rat subcutaneous tissue. II. Materials and Methods 64 Sprague-Dawley rats were used. There were five groups of three animals each for experimental period of 1, 2, 4, and 12 weeks. The teflon tubes, 5mm in length with an inner diameter of 1.5mm, were washed with ethanol and distilled weter and autoclaved.(omitted)

  • Specimens were cured by using a 1 mm (thickness) ${\times}$5 mm (diameter) teflon mold, and were immersed in artificial saliva and in 75% ethanol for 1, 7, 14, 21 and 28 days in order to quantify and to identify toxic components and to determine any degra dation byproducts of Bis-GMA that might be released from five commercially available resin-based dental sealants. In artificial saliva, the only released component was triethylene glycol dimethacylate (TEGDMA). In 75% ethanol, TEGDMA, 2,2-bis[4-(2-hydroxy-3-methacryloyloxypropoxy) phenyl]propane (Bis-GMA) and urethane dimethacrylate (UDMA) were released highly at the initial stage, indicating that the amount of component released is not linearly correlated with the immersion time. The amount of released TEGDMA was found to be much higher in 75% ethanol than in artificial saliva. Importantly, bisphenol-A (BPA) was detected from all the uncured sealants tested, suggesting that all the sealants tested contain BPA as a contaminant.

Synthesis of New Biodegradable Crosslinked Polyesters for Biomedical Applcations and Their In-Vitro Degradation

  • Two kinds of new aliphatic diols were synthesized by the ring-opening reaction of lactide and glycolide with 1,4-butanediol, a difunctional initiator, in the presence of stannous octoate. The resulting aliphatic diols were melt-polymerized with D-tartaric acid at 150 ℃ to produce new crosslinkable polyesters. They were reacted with hexamethylene diisocyanate in THF at 65 ℃ in a teflon mold for 24 h to prepare sequentially ordered crosslinked polyesters (BD/LT/GL/D-tartarate). Degradation of the prepared yellow crosslinked films was carried out in a buffer solution in order to examine the effect of time, pH, temperature and crosslinking degree on their degradation rate and mechanism. The rate of degradation increased with an increase in pH and temperature, but it decreased with increasing degree of crosslinkage incorporated into the crosslinked polyesters. We also found that the crosslinked polymers were converted into the acidic compounds such as lactic, glycolic, and D-tartaric acids during the degradation.

Separation of Light Rare-Earth Elements Using Gas-Pressurized Extraction Chromatography

  • A new method for chemical separation of light rare-earth elements (LREEs) using gas-pressurized extraction chromatography (GPEC) is described. GPEC is a microscale column chromatography system that features a constant flow of solvents, which is created by pressurized nitrogen gas. The separation column with a Teflon tubing was packed with LN resin. The proposed GPEC method facilitates production of lesser chemical wastes and faster separation owing to the use of low solvent volume compared to traditional column chromatography. We evaluated the separation of Ba, La, Ce, and Nd using various elution solvents. The column reproducibility of the proposed GPEC system ranged from 2.4% to 4.9% with RSDs of recoveries, and the column-to-column reproducibility ranged from 3.1% to 6.3% with RSDs of recoveries. The proposed technique is robust, and it can be useful for the fast separation of LREEs.

  • The Schottky barrier diode has been used as an element of the mixer inspite of its conversion loss. In this paper the use of a GaAs MESFET is shown as a device of mixer, and the conversion gain is obtained. Also, input matching circuits aredesigned by s-parameter and fabricated on a dielectric teflon epoxy fiber glass substrate. According to the results, the conversion gain is 9 dB at the signal frequency of 4 GHz and the intermediate frequency of 1.217GHz.

The Clinical Analysis of Sulcus Vocalis (성대구증에 관한 임상적 고찰)

  • The major advancement in phonosurgery due to recent development of laryngomicrosurgery enabled more accurate diagnosis and treatment of patient with voice disorders. Among large proportion of voice disordered patients, prominent linear furrow running parallel along the free edge of vocal cord extending from the vocal process to anterior commissure can be seen as well as incomplete closure during phonation. These cases were illustrated and coined as sulcus vocalis by Salvi in 1901, since then other similar paper was reported in Europe and Japan, but has not been reported in Korea. The exact etiology and therapeutic methods of sulcus vocalis has not been elaborated. At Department of Otolaryngology of Yonsei University College of Medicine a series of voice analysis were performed among those 35 patients with sulcus vocalis visited to Vocal Dynamics Laboratory from May, 1981 to March, 1982. Following is the result of clinical statistical investgation and therapeutic modality. 1) The incidance of sulcus vocalis among 290 patients with voice disorder visited to Vocal Dynamics Laboratory was approximately 12%(35 cases). 2) Onset of this voice disorder was most frequent among patient under 10 year-old groups; 19 cases (54%) followed by second decade, third decade groups in decreasing frequency respectably. 3) The etiology of sulcus vocalis was mostly unknown. The sequelae after measle (4 cases) and severe upper respiratory infection (3 cases) and congenital deformity (2 cases) were the possible causes of sulcus vocalis. 4) These patients were involved bilaterally in 25 cases (71%), left side only in 8 cases (23%) and right side only in 2 cases (6%). 5) Almost all patients complained hoarseness and 7 patients were suffering from chronic laryngitis. 6) In aerodynamic analysis, Maximal Phonation Time was decreased in 20 cases (57%), Phonation Quotient was increased in 22 cases (63%) and Mean Air Flow Rate was increased in 23 cases (66%). 7) Among them, 33 cases were analyzed with stroboscopy. The findings were as follows; incomplete glottic closure during phonation in 31 cases (93%), regular vocal cord movement in whole cases, asymmetric cord movement in 4 cases (12%), decreased amplitude in 5 cases (21%) and small mucosal wave in 24 cases (73%). 8) Intracordal Teflon injection in 5 cases and Sulcusectomy in 1 cases were performed as therapeutic management, however, the therapeutic results were not effective except one case with Teflon injection.

Changes in Quality of UV Sterilized Takju during Storage by Honeycomb Type-UV Sterilizer (허니컴방식 UV 살균기를 이용한 살균 탁주의 저장 중 품질변화)

  • A cylindrical ultraviolet (UV) sterilization system was developed for decreasing microorganisms in takju. The takju was passed through 110 strips of honeycomb-type teflon tubing with 9 UV lamps (1,395 W) set between the teflon tubes. Thus, during passage, the takju was effectively exposed to the UV rays without loss. In terms of the overall quality aspects of the takju, the optimum sterilization condition was set for 4 min at 2 L/min. A 2-3 log cycle decrease in viable cell numbers of total bacteria and fungi was observed at this operating condition. Quality changes in the UV-sterilized takju were examined via UV irradiation of samples followed by storage at 30oC for 8 days. To evaluate the quality changes, pH, amino nitrogen content, acidity, reducing sugar content, and viable cell numbers of total bacteria and fungi were measured. Increases in pH, acidity, and amino nitrogen content were observed in both the takju control and UV sterilized takju with increasing storage time. However, reducing sugar content was decreased in both samples. The L, a, and b values of the control takju and UV sterilized takju showed similar trends over the storage period. Viable cell numbers of fungi did not change in the control or UV sterilized takju during storage, showing approximately $10^8\;CFU/mL$ and $10^4-10^5\;CFU/mL$, respectively. In addition, viable cell numbers of total bacteria remained lower in the UV sterilized takju over 4 days compared to the non-sterilized takju.
