  • As a result of surveying the process for sewing, the problems involving the outlook of sewn apparels were error off the standard dimensions and uneven sizes, which verifies the need for urgent technological guidance and development. On the other hand, the majority of the businesses were operating their sewing machines at the speed of 2,000 rpm or higher. Such a conventional working method may not well reflect the properties of the stretch fabrics, causing poorer quality products. Moreover, most of the businesses were using the ball-point needles which should be replaced with appropriate ones for stretch fabric. The types of feed dog which were used most were Drop feed, Union feed, Confound feed, Differential feed in their oder. Anyway, the automatized dog should urgently replace these inefficient ones for stretch fabric. Meanwhile, the types of presser foot which were used most were Plain foot, Teflon foot, Roller foot, Ring foot in their order, which also necessitates their replacement which Teflon foot. 2. As a result of surveying the process for finishing and inspection, while the causes scores less than 3 points, those due to pressers scored more than 3 points. This results suggests that a technological guidance is needed for the pressing work. Lastly, the causes of defect due to poor outlook attributable to such notions as button or zipper and those due to the uneven sizes scored more than 3 points, which suggests that the entire of needle works should be reformed through scientific research and technological guidance.

A Clinical Evaluatuin on Open Heart Surgery of Congenital and Acquired Heart Disease (선천성 및 후천성 심질환의 개심술)

  • The present study reports 41 cases of congenital and acquired heart diseases, who received open heart surgery under extracorporeal circulation [ECC] by Sarns Heart-Lung-Machine [HLM] at the Department of Thoracic and Cardiovascular Surgery, Hanyang University Hospital during the` period between July 1975 and February 1979. The priming of pump oxygenator was carried out by the hemodilution method using Hartman`s solution, whole blood, and fresh human plasma. The rate of hemodilution was in the average of 50.8 ml/kg. ECC was performed at the average perfusion flow rate of 85.0 ml/kg/min [2.43 L./ kg/2] and at moderate hypothermia. In the total cardiopulmonary bypass, arterial pressure ranged between 55 mmHg and 90 mmHg, but generally maintaining over 70 mmHg. Patient age ranged between 2 and 54 year old, in congenital heart diseases, between 2 and 28, in acquired heart diseases, between 17 and 54 Sex ratio of male to female was 20:21. The cases include a case of pulmonary valvular stenosis, 4 cases of atrial septal defect, 9 cases of ventricular septal defect, 9 cases of tetralogy of Fallot, 5 cases of pentalogy of Fallot, 3 cases of atypical multiple anomalies 7 cases of mitral stenosis or insufficiency, a case of myxoma in left atrium, and a case of ruptured aneurysm of Valsalva`s sinus. The surgical managements were 16 valvulotomy for pulmonary valvular stenosis, 2 Teflon patch graft closure and 5 simple suture closure of atrial septal defect, 16 Teflon patch graft closure and 5 simple suture closure of ventricular septal defect, 12 pericardial patch graft for infundibular stenosis of right ventricle, one anastomosis between left superior vena cava and right atrium, 2 open mitral commissurotomy, 5 mitral valve replacement using Starr-Edward`s ball valve, porcine xenograft by Hancock, by Carpentier-Edward, or Angell-Shiley, one removal of left atrial myxoma, and a repair of ruptured aneurysm of Valsalva`s sinus. Four [9.7%] out 41 cases expired postoperatively and the rest of 37 cases survived with satisfactory results. The causes of death were one coronary embolism in tetralogy of Fallot, 2 postoperative lower cardiac output in atypical multiple anomalies, and one right heart failure in large: ventricular septal defect with pulmonary hypertension.

A Study of Surface Discharge Characteristics for Dew-Point of Dry-Air and Materials or Shapes of Solid Insulator in Quasi-Uniform Field (준평등전계에서의 Dry-Air 노점과 고체절연물 재질 및 형상에 따른 연면방전 특성 연구)

  • This study investigates the surface discharge characteristics of solid insulators by varying their materials, their shapes, and the dew-point of dry-air. The methodology of this study is that a quasi-uniform field is first applied to a test chamber. Then, the chamber is filled with dry-air as an insulation gas which pressure is varied from 1 to 6atm while applying an AC voltage to the chamber. The used solid insulators are teflon, polycarbonate, and bakelite. As the dew-point is lower and the pressure of dry-air is higher, the flashover voltage of all solid insulators increases more. When each characteristic of the solid insulators is compared under the same gas pressure, the flashover voltage of teflon is the highest. Then, the flashover voltage of polycarbonate is higher than that of bakelite. Moreover, it is observed that the flashover voltage increases as the diameter and the thickness of each solid insulator become larger and thicker, respectively. However, the thickness of the solid insulators is more critical for increasing the flashover voltage than their diameter.

  • A case of chest wall malignant peripheral nerve sheath tumor(MPNST) was reported in the U.S.A by Mark and coworkers6)(1991), but none in korea. MPNST accounts for approximately 10% of all soft tissue sarcoma, mostly in patients between 20 and 50 years of age. MPNST arises in association with a major nerve trunk, including the sciatic nerve, brachial plexus, and sacral plexus and the most common anatomical site is the proximal portion of the upper and lower extremity and trunk. Surgical treatment is local excision of mass in low grade sarcoma but unblock resection is necessary in high grade sarcoma. We experienced multiple huge low grade MPNST on left chest wall of a 50 years old man. The tumor and invalved chest wall were removed, and the chest wall defect(15$\times$8 cm) was reconstructed with Teflon. Postoperative course was unevenful.

Formation of Fine Pitch Solder Bumps on Polytetrafluoroethylene Printed Circuit Board using Dry Film Photoresist (Dry Film Photoresist를 이용한 테프론 PCB 위 미세 피치 솔더 범프 형성)

  • We have demonstrated the applicability of dry film photoresist (DFR) in photolithography process for fine pitch solder bumping on the polytetrafluoroethylene (PTFE/Teflon ) printed circuit board (PCB). The copper lines were formed with 100$\mu\textrm{m}$ width and 18$\mu\textrm{m}$ thickness on the PTFE test board, and varying the gaps between two copper lines in a range of 100-200$\mu\textrm{m}$. The DFRs of 15$\mu\textrm{m}$ thickness were laminated by hot roll laminator, by varying laminating temperature from $100{\circ}C$ to 15$0^{\circ}C$ and laminating speed from 0.28-0.98cm/s. We have found the optimum process of DFR lamination on PTFE PCB and accomplished the formation of indium solder bumps. The optimum lamination condition was temperature of $150^{\circ}C$ and speed of about 0.63cm/s. And the smallest size of indium solder bump was diameter of 50$\mu\textrm{m}$ with pitch of 100$\mu\textrm{m}$.

Quantification of Total Mercury in Antarctic Surface Snow using ICP-SF-MS: Spatial Variation from the Coast to Dome Fuji

  • The total mercury concentration ($Hg_T$) of surface snow samples collected along a ~1500 km transect in east Queen Maud Land was determined using inductively coupled plasma sector field mass spectrometry to address the behavior of Hg on the Antarctic Plateau. Due to the volatile nature of mercury, measures were taken against Hg loss from standard solutions by choosing appropriate container material and stabilizing agents. Glass bottles with Teflon-lined caps were superior to Teflon and polyethylene containers in protecting against Hg loss, but addition of gold chloride ($AuCl_3$) or bromine chloride (BrCl) was necessary to ensure preservation of Hg. As Hg loss was also observed in snowmelt samples, our analysis may underestimate the actual amount of HgT in the snow. Even so, the measured HgT was still very low (< 0.4-10.8 pg $g^{-1}$, n = 44) without a signal of depositional enhancement accompanying photo-oxidation of atmospheric elemental mercury in austral midsummer. Moreover, the dynamic variation along the traverse implies spatial and temporal heterogeneity in its source processes.

  • We have recently experienced a case of decannulation difficulty resulted from head and thoracic injury. The patient was 21-year-old male who undergone craniectomy and tracheotomy at other hospital about 7 months ago prior to admission On admission, there was swelling in glottic and subglottic region in indirect laryngoscopy and bronchoscopy with fixation of vocal cords in paramedian position. We tried to reestablish an adequate air way with bougination using Jackson esophageal bougie but there was no effect with it. So we performed vertical incision through cricoid cartilage and tracheal rings and insertion of Teflon tube in stenotic lesion for 9 months and removed it. At present time, the patient has been satisfactory corking training course.

Clinical Study of Primary Chest Wall Tumors (원발성 흉벽종양의 임상적 고찰)

  • Between January 1979 and August 1996, resection of a primary chest wall tumor was done in 51 patients. The mean age of the patients was 36.1 years(2 to 69 years). A palpable mass was the most common symptom(32 patients, 62.7%). The tumor was malignant in 11 patients (21.6%) and benign in 40 patients(78.4%). The tumors in 32 patients(62.7%) had developed from the bony or the cartilaginous wall and in 19 patients(37.3%) from soft tissue. Thirty seven of the patients with benign tumors were treated by excision (three of the patients: wide resection and reconstruction) without recurrence or death, and they are currently free from disease. Most malignancies(8 patients) were treated by wide resection and chest wall reconstruction. Five of them are currently alive. The chest wall reconstruction with Marlex mesh, Prolene mesh, or Teflon felt was done in five of the patients with malignant tumors. There was no operative or hospital mortality among the total 51 patients.

Successful Correction of Atypical Coarctation of the Aorta -Report of 4 Cases- (선천성 대동맥 축착증 4례)

  • This is a report on four cases of successful surgical correction of coarctation of the aorta [COA] in Department of the Thoracic & Cardiovascular Surgery, Hanyang University Hospital. The first case was a postductal type of coarctation of the aorta associated with Patent ductus arteriosus [PDA], Persistent left superior vena cava [LSVC] and richly developed collateral circulation. Blood pressure was measured to be hypertensive at the arm, but hypotensive at the legs. The coarctation of the aorta was corrected with following procedure: Partial resection of the aortic wall with diaphragmatic structure lust above and below the coarctating line of the aorta, and then the defect of the aortic wall was closed by lateral aortographic suture. PDA was closed by ligation procedure. The second case a preductal type of coarctation of the aorta associated with PDA, LSVC, ventricular septal defect [VSD] and poorly developed collateral circulation. Normal blood pressure was measured at the arm, but hypotension was observed at the legs. Correction of coarctation of the aorta was performed under the establishment of tube bypass because of poor collateral circulation. After resection of coarctating short segment, end to end anastomosis was performed without any tension. PDA was closed by division procedure. Simple suture closure of VSD was performed by open heart surgery two weeks after correction of COA. The third case was a long segment COA without any other anomaly. Blood pressure was measured to be hypertensive at the arm, but hypotensive at the legs. Vascular prosthesis was performed using Teflon graft tube after resecting coarctating long segment [6.5 cm] of the aorta. The fourth case was a long segment COA associated with aortic insufficiency and richly developed collateral circulation. Normal blood pressure was measured at the arm, but hypotension was observed at the legs. Vascular prosthesis was performed using Teflon graft tube after resecting coarctating long segment [6.0 cm] of the aorta. Both blood pressure and peripheral pulse on the arm and the legs returned to normal postoperatively in all patients.

Surgical stent for dental implant using cone beam CT images (콘빔형 전산화단층영상을 이용한 치과임플란트 식립유도장치 개발)

  • Purpose : The purpose of this study is to develop a surgical stent for dental implant procedure that can be easily applied and affordable by using cone beam computerized tomography (CBCT). Materials and Methods : Aluminum, Teflon-PFA (perfluoroalkoxy), and acetal (polyoxymethylene plastic) were selected as materials for the surgical stent. Among these three materials, the appropriate material was chosen using the CBCT images. The surgical stent, which could be easily placed into an oral cavity, was designed with chosen material. CBCT images of the new surgical stent on mandible were obtained using Alphard-3030 dental CT system (Asahi Roentgen Co., Ltd., Kyoto, Japan). The point of insertion was prescribed on the surgical stent with the multiplanar reconstruction software of OnDemand3D (CyberMed Inc., Seoul, Korea). Guide holes were made at the point of insertion on the surgical stent using newly designed guide jig. CBCT scans was taken for the second time to verify the accuracy of the newly designed surgical stent. Results : Teflon-PFA showed radiologically excellent image characteristics for the surgical stent. High accuracy and reproducibility of implantation were confirmed with the surgical stent. Conclusion : The newly designed surgical stent can lead to the accurate implantation and achieve the clinically predictable result.
