유치 발거 원인에 대한 후향적 연구 (Reasons for primary teeth extraction in children : Retrospective study)

  • The purpose of this study was to investigate the principal reasons for primary teeth extractions and the tooth type extracted in children. 1159 patients were selected in this study. Dental records and radiographs were reviewed and age, gender, medical history, type of tooth extracted and the reasons for extraction were collected. The data were statistically analyzed using Chi-square test. Total 2078 primary teeth were extracted. Central incisors(34.1%) were most frequently extracted. Extractions due to physiological mobility(77.5%) were the most frequent followed by caries(13.8%), orthodontic(3.9%), trauma(1.7%). Reason for the extraction was different according to age (p = 0.000), but there was no difference according to gender (p = 0.109). While extractions due to physiological mobility predominated overall, reason for extraction was different according to the tooth type. There was no previous treatment in 54.6% of primary teeth extracted due to caries. Excluded physiological mobility, caries are the most common reason for extraction of primary teeth. The importance of preventive care should be emphasized in order to preserve primary teeth and improve children's oral health.

매복지치 발거수술에 관한 임상적 연구 (CLINICAL STUDY ON EXTRACTION OF THE IMPACTED LOWER THIRD MOLAR)

  • The author has studied on extraction of the impacted lower third molar in terms of incision A designed by author, Extraction of those teeth is one of the critical problems involving all dentists and oral surgeons in connection with their professions. 128 patients aged 20-25 with the impacted lower third molar were divided into two groups extracted those teeth under incision A and B shown at the fig.1. And degree of swelling, pain complained, and clinical wound haling on 1,3 and 7 days after extraction of the teeth were observed and analyzed to evaluate which incision is better to extract those teeth. The results were summarized like follows: 1. Degree of swelling after extraction was less observed in extraction undertaken under incision A rather than that in incision B. 2. Patients extracted under incision A less complained pain than those extracted under incision B. 3. Clinical wound healing was better the patients extracted under incision A than those extracted under incision B.

치과의원 내원환자의 영구치 발거원인에 관한 조사연구 (Reasons for Extraction of Permanent Teeth in Private Dental Clinics)

  • 부산 시내에 소재하는 1곳의 치과의원에 영구치 발치를 위해 내원한 1,119명의 환자의 1,878개 치아를 대상으로, 발거원인 비중 및 발거연령을 조사한 본 연구에서 얻은 결론은 다음과 같다. 1. 발거된 영구치아의 발거원인으로는 치아우식증 25.9%, 치주병 56.5%로 나타났다. 2. 30대 전에는 치아우식증에 의한 발거가 치주병에 의한 발거보다 많았으며, 30대 이후부터는 치주질환에 의한 발거가 치아우식증에 의한 발거보다 많았다. 3. 영구치의 발거빈도를 비교하면, 남자가 여자보다, 상악이 하악보다 발거빈도가 높게 나타났다. 4. 가장 많이 발거하는 치아는 제3대구치로 주로 지치주위염 및 맹출 장애로 발거되었다. 5. 치아 중에서 하악 견치가 가장 오래 생존하였으며, 제3대구치를 제외하고 하악 제1대구치가 가장 먼저 발거하였다. 이런 결과는 구강건강의 최대 파괴현상인 치아의 발거원인과 발거연령을 분석하여 구강보건 문제를 해결하는 데 기초자료로 도움이 되리라 사료된다.

Evaluation of Bleeding After Denal Extraction in Patients Taking Single Antiplatelet Treatment

  • The purpose of this study was to evaluate the effect of single antiplatelet treatment on delayed bleeding, in patients undergoing dental extraction. A total of 154 teeth were extracted in 94 patients with ongoing antiplatelet treatment, who were taking single antiplatelet of either aspirin or clopidogrel. All patients underwent simple dental extraction; local hemostasis was performed with gauze-biting, suturing, and/or application of a local hemostatic agent. Delayed bleeding was recorded in 5 teeth out of a total of 154 teeth after extraction, the incidence of postoperative delayed bleeding being 3.2%. The bleeding was controlled by the patients themselves, through application of pressure with additional gauze-biting. No one visited the doctor or emergency room for hemostasis. These results confirmed that patients taking single antiplatelet drugs may have teeth extracted safely without interruption of the antiplatelet treatment.

영구치발거의 원인에 관한 임상 통계학적 연구 (A CLINICAL-STATISTICAL STUDY ON THE EXTRACTION OF PERMANENT TEETH)

  • This study is proposed to analyze statistically the causes of extraction of permanent teeth among outpatients who visited our dental department of Sung-Sim Hospital, Chung-Ang University from Jan. 29th 1979 to Nov. 30th 1980. the results are as follows. 1. The group of patients aged 25-34 had more number of extracted teeth than any other age group. 2. It was found that woman's (0.13) was more than man's(0.9) in average number of extracted teeth per patient. 3. In analyzing the causes of extraction, it was identified as caries 37.5%, periodontal diseases 27.2%, wisdom teeth 17.6%, for orthodontic treatment and impacted supernumerary teeth and others 8.0% trauma and fracture 4.2%. 4. The primary cause of extraction was found to be changed from caries to periodontal diseases according to the increase of age. 5. The number of teeth that were extracted by trauma was found that man's (38) was more than twice as much as woman's (17) and the group of patients aged 35-44 had the most number of all age groups.

Extraction of Impacted Supernumerary Teeth with Navigation System

  • Computer-aided navigation system is helpful in maxillofacial surgery with real time instrument positioning and clear anatomic identification. Generally, completely impacted tooth extraction surgery have e high risk by iatrogenic injury such as, adjacent tooth injury, normal anatomical structure injury. This case report describes performing extraction of impacted supernumerary teeth on anterior maxilla by using the navigation system in a 15 years old male patient.

Use of elevator instruments when luxating and extracting teeth in dentistry: clinical techniques

  • In dentistry, elevator instruments are used to luxate teeth, and this technique imparts forces to tooth particles that sever the periodontal ligament around tooth roots inside the socket and expand alveolar bone around tooth particles. These effects can result in extraction of the tooth particles or facilitate systematic forceps extraction of the tooth particles. This article presents basic oral surgery techniques for applying elevators to luxate teeth. Determination of the optimal luxation technique requires understanding of the functions of the straight elevator and the Cryer elevator, the concept of purchase points, how the design elements of elevator working ends and tips influence the functionality of an elevator, application of forces to tooth particles, sectioning teeth at furcations, and bone removal to facilitate luxation. The effectiveness of tooth particle luxation is influenced by elevator tip shape and size, the magnitude and the vectors of forces applied to the tooth particle by the tip, and sectioning and bone removal within the operating field. Controlled extraction procedures are facilitated by a dental operating microscope or the magnification of binocular surgical loupes telescopes, combined with co-axial illumination.

매복지치 발치 시 발생한 합병증 양상에 대한 후향적 연구 (Complications of impacted third molar extraction: retrospective study)

  • Introduction: Surgical extraction of third molar is one of the most frequently performed procedures in oral and maxillofacial surgery unit as the impacted wisdom teeth could cause various complications. Even though, however, extraction of the impacted wisdom teeth is an obligation for the reason of possible complication even for general practitioners, it has been avoided. Various factors concerning surgical extraction of impacted third molar are considered: general condition of patients, relationship with relative anatomies, aspects of impaction, surgeon's skill. Materials and Methods: The consideration and crossing analysis of these factors with 2,463 patients who visit Dankook University dental hospital to extract those impacted third molar. Results: 1. Gender doesn't affect. 2. Medical problems have more complications. 3. There are more complications in high difficulty index (DI) impacted teeth. 4. When inferior alveolar canal overlap mandibular third molar, complication rate is 26.92%. 5. The most common complication was swelling and pain. 6. There is no statistical difference between the incidence of complication and surgeon's experience. Conclusion: In regard to these results, it seems that clinical or radiological examination can predict potential complications of wisdom teeth, and it is helpful to bear in mind the fact.

Light-emitting diode assessment of dentinal defects: the role of presumed extraction forces

  • Objectives: The evaluation of iatrogenic dentinal defects in extracted teeth may be influenced by extraction forces and prolonged dry times. The purpose of this study was to compare the presence of dentinal defects in freshly extracted, periodontally compromised teeth with those in a group of teeth with uncontrolled extraction forces and storage time. Materials and Methods: The experimental group consisted of eighteen roots obtained from teeth extracted due to periodontal reasons with class II or III mobility. They were kept in saline and sectioned within 1 hour following extraction. The control group consisted of matched root types obtained from an anonymous tooth collection, consistent with previous dentinal defect studies. The slices were obtained at 3, 6, and 9 mm from the apex. The imaging process exposed all specimens to no more than 60 seconds of dry time. The ${\times}12.8$ magnification was used for the 9 mm slices and ${\times}19.2$ magnification for the 3 mm and 6 mm slices under light-emitting diode (LED) transillumination. The root canal spaces and periodontal tissues were masked to minimize extraneous factors that might influence the evaluators. Chi-square test was used for statistical analysis. Results: Dentinal defects were detected in 17% of the experimental group teeth, compared to 61% of control teeth (p = 0.015). Conclusions: LED transillumination assessment of freshly extracted roots with class II or III mobility showed smaller number of dentinal defects than roots with uncontrolled storage time and extraction forces. The use of freshly extracted roots with mobility should be considered for future dental defect assessment studies.

매복과잉치 및 영구치 동시 발거 후 임플란트의 즉시 식립 증례 (IMMEDIATE PLACEMENT OF IMPLANT FOLLOWING EXTRACTION OF IMPACTED SUPERNUMERARY TEETH AND PERMANENT TEETH : A CASE REPORT)

  • Supernumerary teeth are extra teeth or tooth-like structures which may have either erupted or unerupted in addition to the 20 deciduous teeth and the 32 permanent teeth. Clinicians sometimes confront unerupted supernumerary teeth where implants would be placed. Many clinicians consider immediate placement of implant into an extracted socket as an effective technique compared to the conventional method of waiting several months for bone healing, because of the advantages such as reduction of treatment period, preservation of alveolar bone and soft tissue, etc. We could hardly find the case of immediate placement of implant after extraction of impacted supernumerary teeth and permanent teeth together. Therefore we report the case of immediate implantation following extraction of impacted supernumerary teeth and poor periodontal conditioned permanent teeth.
