하악골 골절선상에 위치한 치아의 예후에 관한 연구 (THE PROGNOSIS OF THE TEETH IN THE MANDIBULAR FRACTURE LINES)

  • Objective : The purpose of this study is to evaluate the vitality of the teeth in and adjacent to the mandibular fracture line according to variable conditions of fracture and to establish the protocol of treatment of fracture line teeth. Materials and Methods : The vitality of 97 teeth in fracture line and 104 teeth adjacent to fracture line of 52 patients were invested preoperatively. Of these, 66 teeth in fracture line and 72 teeth adjacent to fracture line were monitored at least 6 months after operation. An electric pulp tester was used to measure pulpal response. The relationships between the vitality of teeth in variable time(preoperation, immediate post-operation; within 1 week after operation, and 6 months after operation) and variable conditions of fracture(horizontal, vertical gap of fracture line, the number of fracture line)were evaluated statistically. Result : The vitality of fracture line teeth in the 6 months after operation statistically differed by the vertical gap of fracture line and the number of fracture line. The vitality of fracture line adjacent teeth in the immediate post-operation only statistically differed by the vertical gap of fracture line. There were statistically differences between preoperative EPT value and vitality of fracture line teeth on 6 months after operation. There were 5 cases of complications including periapical and periodontal abscess. Of these, only one tooth was extracted and the others were well treated with endodontic treatment and subgingival curettage. Conclusion : It is recommended to retain teeth and to monitor the vitality of teeth in and adjacent to fracture line, unless there is an absolute indication for extraction.

중증치주염이 치수조직에 미치는 영향 (The effect of advanced periodontitis on the dental pulp tissue)

  • In order to examine the effects of advanced periodontitis on the dental pulps, 38 extracted human teeth were examined histologically. The 38 teeth had a positive or negative state in the electric pulp test(EPT). In addition, almost of the 38 teeth had a deep pocket and severe mobility, and floating state. A medical and dental history was elicited. The extracted teeth fixed in 10% neutral formalin solution. The general tissue processing method was followed. The tissue block including the teeth was prepared for optical microscopy using hematoxillin-eosin staining. Among the 38 periodontally involved teeth, the dental pulps were respectively intact in 12(31%), and a pulp stone(or linear calcifications) was found in 18 teeeth(47%). In addition, 17 teeth(44%) had pulps exhibiting inflammatory reactions with varying intensities, such as hyperemia, pulp abscess, pulp necrosis. Among the 38 periodontally involved teeth, 37 teeth tested a positive to the EPT, and 7 teeth tested negative. The EPT positive 37 teeth had various histological features such as 7 normal pulp(18%), 17 pulp stone(44%), 1 hyperemia (2%), 9 pulpitis(23%), 5 root resorption(13%), 3 pulp abscess(7%), and 3 pulp necrosis(7%), In conclusion, it is suggested that in the EPT positive teeth, advanced periodontally involved teeth can cause inflammation of the dental pulp.

Comparison of shear bond strengths of different types of denture teeth to different denture base resins

  • PURPOSE. To determine the shear bond strengths of different denture base resins to different types of prefabricated teeth (acrylic, nanohybrid composite, and cross-linked) and denture teeth produced by computer-aided design/computer-aided manufacturing (CAD/CAM) technology. MATERIALS AND METHODS. Prefabricated teeth and CAD/CAM (milled) denture teeth were divided into 10 groups and bonded to different denture base materials. Groups 1-3 comprised of different types of prefabricated teeth and cold-polymerized denture base resin; groups 4-6 comprised of different types of prefabricated teeth and heat-polymerized denture base resin; groups 7-9 comprised of different types of prefabricated teeth and CAD/CAM (milled) denture base resin; and group 10 comprised of milled denture teeth produced by CAD/CAM technology and CAD/CAM (milled) denture base resin. A universal testing machine was used to evaluate the shear bond strength for all specimens. One-way ANOVA and Tukey post-hoc test were used for analyzing the data (α=.05). RESULTS. The shear bond strengths of different groups ranged from 3.37 ± 2.14 MPa to 18.10 ± 2.68 MPa. Statistical analysis showed significant differences among the tested groups (P<.0001). Among different polymerization methods, the lowest values were determined in cold-polymerized resin.There was no significant difference between the shear bond strength values of heat-polymerized and CAD/CAM (milled) denture base resins. CONCLUSION. Different combinations of materials for removable denture base and denture teeth can affect their bond strength. Cold-polymerized resin should be avoided for attaching prefabricated teeth to a denture base. CAD/CAM (milled) and heat-polymerized denture base resins bonded to different types of prefabricated teeth show similar shear bond strength values.

Clinical and radiographic outcomes of regenerative endodontic treatment performed by endodontic postgraduate students: a retrospective study

  • Objectives: Regenerative endodontic treatment is a clinical procedure aimed at biologically regenerating damaged root canal tissue of immature permanent teeth. This study aimed to report the outcomes of regenerative endodontic treatment performed by endodontic postgraduate students. Materials and Methods: Clinical and radiographic data of 27 patients, aged 10-22 years, who underwent regenerative treatment of immature permanent teeth from 2015 to 2019 were followed up, wherein clinical and radiographic examinations were performed for each patient. Postoperative success rate and tooth survival were analyzed, and the postoperative radiographic root area changes were quantified. Results: A total of 23 patients attended the dental appointments, showing that all teeth survived and were asymptomatic. Specifically, 7 periapical pathosis cases were completely healed, 12 were incompletely healed, and 4 cases failed. Moreover, significant differences were found between discolored and non-discolored teeth, and between the presence or absence of periapical radiolucency. Additionally, 3 anterior teeth showed complete closure of the apical foramen, while the apical foramen width was reduced in 17 teeth and failed in 3 teeth. Root length was also found to have been increased in 7 anterior and 4 posterior teeth, and the average length ranged from 4.00-0.63 mm in the anterior teeth, 2.85-1.48 mm of the mesial root, and 2.73-2.16 mm of the molar teeth distal root. Furthermore, calcified tissue deposition was observed in 7 teeth. Conclusions: A favorable outcome of regenerative endodontic treatment of immature permanent teeth with necrotic pulp was achieved with a high survival rate.

Assessment of mechanical allodynia in healthy teeth adjacent and contralateral to endodontically diseased teeth: a clinical study

  • Objectives: The present study investigated the prevalence of mechanical allodynia (MA) in healthy teeth adjacent and contralateral to endodontically diseased teeth. Materials and Methods: This cross-sectional study included 114 patients with symptomatic irreversible pulpitis and apical periodontitis in permanent mandibular first molars who possessed healthy teeth adjacent and contralateral to the endodontically diseased tooth. The mechanical sensitivity of the teeth was determined by percussion testing. The presence or absence of pain on percussion in the teeth adjacent and contralateral to the endodontically diseased tooth and the tooth distal to the contralateral symmetrical tooth was recorded according to coding criteria. The prevalence of MA was computed as a percentage, and binary logistic regression analysis was done. The Fisher exact test and Mann-Whitney U test were used for binary and ordinal data. Results: Age and sex did not influence the prevalence of MA. An increased prevalence of MA was found in patients with higher levels of spontaneous pain (p < 0.001). The prevalence of allodynia was 57% in teeth adjacent to endodontically diseased teeth and 10.5% in teeth contralateral to endodontically diseased teeth. In addition, on the ipsilateral side, there were more painful sensations distal to the diseased tooth than mesially. Conclusions: Despite being disease-free, teeth adjacent and contralateral to endodontically diseased teeth exhibited pain on percussion. There was a direct association between the severity of the patient's pain and the presence of MA.

  • The purpose of this study is to examine the effects of pre-school children's past lactation types and food habits on dental health status. This study was carried out by analyzing the information obtained from oral inspection of 163 preschool children and questionnaires answered by their mothers. The results are as follows: 1. 80.4% of the preschool children have decayed and filled teeth. 2. The older they are, the more decayed and filled teeth they have 3. The earlier they started to have weaning foods, the more decayed and filled teeth they have. Preschool children who started weaning foods after 12 months of their birth have significantly more decayed and filled teeth. 4. Preschool children who had weaning foods by using milk-bottles have significantly more decayed and filled teeth than others. 5. Preschool children who had weaning foods regularly have significantly less decayed and filled teeth. 6. Preschool children who have frequently skipped breakfast have the least decayed and filled teeth. 7. Preschool children who would like to have sweet things have significantly more decayed and filled teeth than others.

X-선상에 의한 선천성 치아이상의 발생빈도에 관한 통계학적 연구. (THE STATISTICAL STUDY OF PREVALENCE OF CONGENITAL DENTAL ANOMALIES THROUTH DENTAL ROENTGENOGRAPHIC FINDINGS.)

  • This study was undertaken in order to observe the prevalence of congenital dental anomalies especially that of oligodontia and supernumerary teeth in 1024 children at age from 4 to 14 years, through history taking, oral examinations, roentgenographic observations of subjects, and statistic analysis was made. The following results were obtained: 1. The prevalence of oligodontia in 1024 children was $6.45{\pm}.8$ percent with a total of 122 teeth absent in sixty-six subjects, excluding 3rd molars, and most of children (86.4%) who have congenitally missing teeth showed abscence of one or two teeth individually. 2. The teeth most frequently absent were the mandibular second premolar, maxillary second premolar, maxillary lateral incisor, mandibular central incisor, and mandibular lateral incisor in order. 3. There was no statistically significant difference found in tooth abscence in the following comparisons; .Male vs Female .Bilateral vs Unilateral .Maxilla vs Mandible .Right vs Left 4. The prevalence of supernumerary teeth in 1024 children was $4.79{\pm}.67$ percent with a total of 59 supernumerary teeth in forty-nine subjects, all of children who have supernumerary teeth showed one or two supernumerary teeth individually. 5. Statistically significant difference was found in supernumerary teeth in the comparison of male vs female, and they were more frequent in male.

순구개열자의 선천결손치와 과잉치의 발생빈도에 관한 연구 (A STUDY OF THE FREQUENCY OF CONGENITAL MISSING AND SUPERNUMERARY TEETH IN CLEFT LIP AND PALATE PATIENTS)

  • The purpose of this study was to evaluate the frequency of congenital missing teeth and supernumerary teeth in cleft patients. The subjects were divided into bilateral cleft lip and palate(BCLP), unilateral cleft lip and palate(UCLP) and cleft palate alone(CP alone) groups. 97 cleft patients(BCLP 15, UCLP 70, CP alone 12) between 6-20 years old were evaluated. Panorama film, Orthodontic chart and initial intraoral photogram were employed for this research. The obtained results were as follows. 1. The incidence of congenital missing teeth in total cleft samples was $57.7\%$, and the incidence of supernumerary teeth was $26.8\%$. Each incidence was higher than non-cleft. 2. The incidence of congenital missing teeth was the highest in BCLP and the lowest in CP alone. 3. The number of congenital missing teeth per perso was usually one, and the frequency was higher in the maxillary lateral incisors$(67.8\%)$, and maxillary second premolar$(14.9\%)$ than other teeth. 4. Most of tooth number anomalies in cleft patients were found in maxilla, especially adjacent region to the cleft site.

『어약원방(御藥院方)』 「인후구치문(咽喉口齒門)」 중 치아질환 처방에 관한 고찰 (A Review on Prescriptions for Mouth and Teeth in 「Throat, Mouth and Teeth」 of 『Yuyaoyuanfang』)

  • Objectives : "Yayaoyuanfang" is a prescription book, compiled by Xuguozhen in 1267. In "Yayaoyuanfang", there are 13 chapters including "Throat, Mouth and Teeth". Because there are few prescription books focused only on mouth and teeth disease in traditional medicine, "Throat, Mouth and Teeth" in "Yayaoyuanfang" is distinctive. Methods : We try to analyze prescriptions by form, effectiveness and report experiences in "Yayaoyuanfang". Results : There are 95 prescriptions with 77 powder, 5 paste, 5 liquid, 2 granule and 5 other forms. Effectiveness of those are divided into teeth, gum and mouth. Effectiveness for teeth are about pains, weakness, cavity, discoloration and so on. Effectiveness for gum are about pain, swelling, weakness, shrinking, etc. Effectiveness for mouth is about bad breath. Conclusions : "Throat, Mouth and Teeth" in "Yayaoyuanfang" has professional prscriptions for throat, mouth and teeth with several form and effectiveness.

영구전치(永久前齒)의 치근발육단계(齒根發育段階)에 따른 Electric Pulp Stimulator에 대(對)한 치수반응(齒髓反應) (PULP RESPONSES TO AN ELECTRIC PULP STIMULATOR IN THE DEVELOPING PERMANENT ANTERIOR DENTITION)

  • The author studied the response to the stimulation of an electric pulp stimulator of healthy 854 permanent anterior teeth in 122 children aged from six to eleven years old, during different stages of tooth development. The results were as followings: 1) In completely open apices, 13.1% (18 teeth) showed positive responses, and 86.9% (120 teeth) showed negative responses. In two-thirds open apices 33.1% (75 teeth) showed positive responses, and 66.9% (152 teeth) showed negative responses. In one-third open apices 57.8% (118 teeth) showed positive responses, and 42.2% (86 teeth) showed negative responses. In closed apices 80.0% (228 teeth) showed positive responses, and 20.0% (57 teeth) showed negative responses. 2) The number of positive responses increased in each upper and lower central incisors during the stages of root development, but not in upper lateral incisors. 3) There were no significant differences statistically in responses between the teeth of the right and left sides and the upper and the lower jaw, in the same stages of root development.
