  • Although the lateral discoid meniscus of the knee has been observed frequently, the discoid medial meniscus has rarely been reported in the literature. A discoid medial meniscus with the horizontal tear and the peripheral tear of upper lip were observed. This tear pattern is not usual in the lateral discoid meniscus as well as in the normal meniscus. A plausible mechanism of the tear pattern compared with the normal shape and the discoid lateral meniscus was discussed in the following.

  • Objectives : The purpose on this study is to show the clinical effects of oriental medical treatments for labral tear. Methods : The patients were treated using acupuncture, pharmacopuncture, herbal medication, moxibustion, physical treatment and cupping treatment. And the effects for labrum tear have measured in VAS, ROM of shoulder joint. Results : VAS of shoulder pain went down to 2 in case 1, 4 in case 2. ROM of shoulder joint was improved. Conclusions : From the above results, It is demonstrated that oriental medical treatments is effective on labral tear.

Displaced Double-Layered Lateral Meniscus That Mimicked the Bucket-Handle Tear: a Case Report

  • Among the various types of congenital meniscal anomalies, the double-layered lateral meniscus is extremely rare. The double-layered meniscus consists of both the upper additional and the lower normal meniscus. As the upper additional meniscus is mobile, it can be easily displaced, while the lower lateral meniscus is usually normal in shape and volume. A 42-year-old woman suffering from pain and locking of her left knee underwent Magnetic resonance imaging (MRI) examination and an arthroscopic surgery. A rare meniscal abnormality was seen in her left knee, which presented as a double-layered lateral meniscus with displacement. It was remarkable that the upper additional meniscus was displaced over the intercondylar eminence of the tibia and it mimicked a bucket-handle tear. Even though it is rare, it is necessary to consider the possibility of displaced double-layered meniscus in the differential diagnosis of a bucket-handle tear. Here, we report the MRI and arthroscopic findings of a displaced double-layered lateral meniscus, which was similar to the bucket-handle tear.

Tear Extraction from Ultrasonic Images of Shoulder using Fuzzy Stretching and SOM Based Quantization (퍼지 스트레칭과 SOM 기반 양자화를 이용한 어깨 초음파 영상에서의 인대 손상 영역 추출)

  • 본 논문에서는 어깨 초음파 영상을 분석하여 인대 손상(Tear) 영역을 추출하는 방법을 제안한다. 제안된 방법은 초음파 영상에서 ROI(Region of Interest) 영역을 추출하고 추출된 ROI 영역에서 사다리꼴 형태의 소속 함수를 적용한 퍼지 스트레칭 기법을 이용하여 명암 대비를 높인다. 명암 대비가 조정된 ROI 영역에서 밝기 평균 이진화 기법을 적용하여 ROI 영역을 이진화한다. 이진화가 적용된 ROI 영역에서 워터쉐드 기법을 적용하여 연골과 힘줄의 후보 영역들을 추출한다. 추출된 연골과 힘줄의 후보 영역들 중에서 위에서 아래로 스캔하여 수평 너비가 가장 큰 영역에 해당하는 힘줄 영역의 상단 경계선을 추출한다. 그리고 아래에서 위로 스캔하여 수평 너비가 가장 큰 영역의 상단 경계에 스플라인 곡선을 적용하여 연골 영역의 상단 경계선을 추출한다. 힘줄 영역의 상단 경계선과 연골 영역의 상단 경계선 양 끝에 2차 함수 곡선을 적용하여 곡선 사이의 양자화할 영역을 추출한 후, SOM 기법을 적용하여 인대 손상 후보 영역을 양자화한다. 양자화된 인대 손상 후보 영역을 분석하여 어깨 힘줄의 손상 영역과 비손상 영역을 구분하고 인대 손상(Tear) 영역을 추출한다. 제안된 방법을 어깨 힘줄이 있는 초음파 영상을 대상으로 실험한 결과, 인대 손상(Tear) 영역이 비교적 정확히 추출되었다.

Arthroscopic Repair of Full Thickness Rotator Cuff Tear (회전근개 전층 파열에서 관절경 감시하의 봉합술)

  • Purpose: To evaluate the usefulness of arthroscopic repair that was related with full thickness rotator cuff tear and assess clinical result. Materials and Methods: Twenty-one cases of arthroscopically repaired full thickness tear of rotator cuffs were studied. Between October 1998 to July 2002 we have analysed 21 repairs of FTRCT the average age 54(42∼74) years old, mean follow-up was 24(12∼41) months We analyzed the results statistically by paired t-test. Results: Postoperative VAS of pain improved average 7.2 to 1.9, UCLA score improved 13.9 to 31.9, ADL improved 11.5 to 25.5 respectively(all, p<0.001). Eighty-seventh % of the patients showed excellent St good results at the final follow-up. The satisfied rate was 90.5%(19cases). Conclusions: Arthroscopic repair in full thickness rotator cuff tear is effective surgical methods.

Trimethoprim-Sulfamethoxazole for the Treatment of Tear Staining Syndrome in Dogs (개의 눈물착색증후군(Tear Staining Syndrome) 치료를 위한 Trimethoprim-Sulfamethoxazole)

  • 22 dogs (31 eyes) that had treated with trimethoprim-sulfamethoxazole for tear staining syndrome at Snoopy Pet Clinic from October 2000 to September 2002 were reviewed. Of the 22 dogs, 12 were female and 10 male. Their mean (${\pm}$ SD) age was 3.5 (${\pm}\;1.3$) years. The breeds of the dogs consisted of Maltese (8 dogs), Shih tzu (6 dogs), Poodle (5 dogs), Yorkshire terrier (2 dogs), and Mixed (1 dog). The dogs received 30 mg/kg trimethoprim-sulfamethoxazole perorally twice daily for two to six weeks. 26 (19 dogs) of the 31 eyes (22 dogs) recovered completely and did not show relapse at $26{\sim}30$ weeks after treatment. Any complications did not observed. Five eyes of three dogs were not cured. Two eyes (one dogs) of them had not response to medicament and three eyes (two dogs) recurrence but the clinical signs decreased. It was considered that the trimethoprim-sulfamethoxazole was effective for the treatment in dogs with tear staining syndrome.

Arthroscopic Decompression of Subacromial Impingement Syndrome (관절경적 감압술에 의한 견봉하 충돌증후군의 치료)

  • The authors performed the arthroscopic decompression and cuff debridement on 47 cases in 45 consecutive patients with either stage II or stage III impingement syndrome from July 1990 to January 1994. The summarized results are as follows. 1. 47 cases in 45 consecutive patients had arthroscopic decompression for the subacromial impingement syndrome. 2. There were 31 males and 14 females and the mean age was 40 years for men and 46years for women. 3. The follow up duration was from 3 years 9 months to 1 year (average 2 years 1month). 4. Among 47 cases, 19 cases were found to have no cuff tear (stageII) 13 cases partial thickness cuff tear (stage IIIa) 10 cases complete tear on cuff less than 3 cm long (stage IIIb) and 5 cases complete tear on cuff more than 3 em in length (stage IIIc). 5. The arthroscopic subacromial decompression and rotator cuff debridement was a good treatment method in stage II and stage IIIa and stage IIIb. in the case of stage IIIc rotator cuff tear, it was useful for pain relief and improvement of shoulder function.

Adrenal Insufficiency in Patients with Rotator Cuff Tear: Prevalence, Risk Factors and Influence on Outcome

  • Background: This study measured the rate of adrenal insufficiency among rotator cuff tear patients, and determined the roles of steroid treatment and herbal medicine as specific risk factors of adrenal insufficiency. Methods: We prospectively evaluated the 53 patients who agreed to participate in the study and underwent arthroscopic rotator cuff repair due to a chronic full-thickness tear from March 2012 to September 2013. The diagnosis of adrenal insufficieny was made by rapid adrenocortical hormone test before the operation. We collected information regarding their history of steroid and herbal medicine treatment within the last 12 months. Results: The rate of adrenal insufficiency among the rotator cuff tear patients in our study was 18.9% (10 of 53 patients). Steroid treatment (p=0.034), frequency of local steroid injection (p=0.008), and herbal medicine treatment (p=0.02) showed significant association with the risk of adrenal insufficiency. Conclusions: In this study, careful taking of a patient's medical history, such as the use of steroids and herbal medicine, is important when investigating adrenal function before surgery.

Report of Two Cases of Shoulder Pain Diagnosed through MRI as Partial Tear of Supraspinatus Tendon Treated by Oriental Medical Treatment (MRI상 극상근건 부분파열로 진단받은 견비통 환자의 치험 2례 보고)

  • Objectives: The purpose of this study is to report the improvement after oriental medical treatment of partial tear of supraspinatus tendon. Method: We treated 2 patients having shoulder pain due to partial tear of supraspinatus tendon with Oriental medical treatment, including Sa-am acupuncture DaeJang-Jeonggyeok, A-shi point(阿是穴) and herbal medicine. We checked visual analog scale (VAS) score and range of movement (ROM). Results: We treated shoulder pain. Thereafter ROM improved and V AS score dropped to the level of 2 to 4 points from 10 points at onset. Conclusion: This report shows Oriental medical treatment has effectiveness on partial tear of supraspinatus tendon. Follow-up study is needed.

  • Purpose: Cemental tear is an uncommon form of root fracture that can lead to rapid localized attachment loss. When it is exposed in the periodontal pocket, it should be removed to prevent accumulation of dental plaque and calculus. Material and Methods: 2 patients were diagnosed as a cemental tear and they were treated with conventional flap operation and subgingival curettage. Additional treatments such as bone graft or guided tissue regeneration were not performed. Result: Symptoms subsided after the treatment. Periodontal pocket has been reduced but no gain of clinical attachment was observed. Remnant of cemental fragment remained after curettage. However, periodontal pocket was stably maintained and there was no recurrence. Conclusion: Periodontal attachment loss associated with cemental tear can be successfully treated with conventional periodontal surgical and nonsurgical procedures.
