Gait Analysis and Functional Outcomes Following Pan Tarsal Arthrodesis of Tarsal Joint in a Toy Poodle

  • A 12-year-old castrated Toy Poodle was referred with a continuous non-weight bearing lameness of right hind limb due to a traffic accident 9 years ago. Physical examination and radiographs revealed partial loss of right calcaneal bone, loss of Achilles tendon and disused muscular atrophy. Arthrodesis was performed to preserve the function of the right hind limb. Pan tarsal arthrodesis plate was applied medially with 2.7 mm cortical screws and 2.0 mm cortical screws. During fixation, cancellous bone chip was transplanted into the arthrodesis site. A bone union was confirmed 9 weeks after surgery. Furthermore, on the gait analysis, the weight-bearing of right hind limb was restored to 70% of the opposite hind limb. Arthrodesis can be used to treat more difficult and serious problems affecting the joints. In particular, arthrodesis is indicated in cases such as comminuted intraarticular fractures and irreparable injury of the calcaneal tendon apparatus in the hock joint. In this case, the pan tarsal arthrodesis provided stability to the hock joint and improved the gait by restoring severe chronic damage. In conclusion, we successfully treated a challenging disability of hock joint using pan tarsal arthrodesis to restore the legs that were non-weight bearing due to chronic injury by traffic accident and objectively ascertained the increased weight bearing by gait analysis.

횡족근 관절의 단일 및 이중 유합술의 비교 (Comparison of Single and Double Fusions in Transverse Tarsal Joint)

  • Purpose: The purpose of this study was to compare the clinical and radiological results of single and double fusions in the transverse tarsal joint. Materials and Methods: Between December 2000 and April 2009, 16 patients (16 feet) who had been treated by fusion of transverse tarsal joint were included in this study. In 8 patients, only talonavicular joint was fused and in the other 8 patients, both talonavicular and calcaneocuboid joints were fused simultaneously. We have measured talo-first metatarsal angle, calcaneal pitch angle, talonavicular coverage angle and presence of adjacent joint arthritis for radiological assessment at both preoperative and last visit. Furthermore, we have evaluated Visual Analogue Scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS) score and patient's satisfaction. Results: In a single fusion group, VAS was improved from $6.4{\pm}1.4$ to $0.8{\pm}1.0$ (p=0.0011) and AOFAS score was improved from $63.8{\pm}6.2$ to $89.4{\pm}9.8$ (p=0.0012). In a double fusion group, VAS was improved from $8.0{\pm}0.75$ to $2.0{\pm}1.8$ (p=0.0011) and AOFAS score was improved from $60.5{\pm}11.2$ to $89.5{\pm}6.0$ (p=0.0012). In the difference of talo-first metatarsal angle between two groups, a single fused group was more improved than a double fused group (p=0.04). Conclusion: Both single and double fusions are useful and attractive treatment for the transverse tarsal joint arthritis. Furthermore, a single fusion has advantages of less invasiveness and preserving some degree of hindfoot motion and could be an effective alternative to a double fusion if patient meets appropriate criteria through careful preoperative evaluation.

중족근 관절의 탈구 및 골절: 증례 보고 (Fracture and Dislocation of the Midtarsal Joint: A Case Report)

  • The midtarsal joint is composed of the talonavicular and calcaneocuboid joints. It is also known as the Chopart joint. Midtarsal joint fracture and dislocation are relatively rare and frequently missed or misdiagnosed. A proper understanding about the anatomy of the midtarsal joint is an essential part in comprehending the mechanism of injury and rationale for treatment. Anatomical reduction of midtarsal joint with correction of the column in length and shape are important; however, it is technically challenging and may require open procedure. Herein, we described a case of initial open reduction and internal fixation for midtarsal joint fracture and dislocation with a brief literature review.

  • The mare had lameness and asymmetry edematous on its tarsal and metatarsal joints at the initial physical examination. The pain was elicited with a palpation along the metatarsal articulations. No significant abnormalities were detected in the screening test. However, thermographic images revealed a significant increase in the surface temperature at the joint of the hindlimb when compared to the reference range. At necropsy, an irregularity of the surface and excessive synovial fluid were observed on the right tarsal joint. No bacterial growth was shown in the cultures of synovial fluid. Staphylococcus aureus was detected in the subcutaneous discharge. Taken together, the thermography images were very useful in localizing the area of injury and were an effective diagnostic methodology for assessing lameness.

  • Ankle arthrodesis has been used frequently for end-stage ankle arthritis that does not respond to conservative treatment. On the other hand, there are concerns regarding the degenerative changes to the adjacent joint, such as the subtalar or talonavicular joint, due to the altered biomechanics after the loss of ankle motion. Because the arthrodesis for these midtarsal joints may overload stress on another contiguous joint, a salvage procedure should be considered rather than joint sacrificing. This paper reports a case of talonavicular arthritis after malunited ankle arthrodesis that was treated with interpositional arthroplasty using the tibialis anterior tendon.

Computed tomographic features of Scottish Fold osteochondrodysplasia

  • An 18-month-old, Scottish Fold with severe lameness and pain in the fore- and hind-limbs was diagnosed as Scottish Fold osteochondrodysplasia based on radiography showing mis-shapened phalanges, bone formation around the tarsal and metatarsal joints, joint space narrowing, and ankylosis. Although computed tomography (CT) is not considered a primary method for examining Scottish Fold osteochondrodysplasia, CT was undertaken in this case to investigate the cause of pain and reluctant ambulation in the forelimbs. CT showed immaturely ossified bone proliferation in the carpal bones. CT-based multiplanar reconstruction can reveal the anatomic relationship between the bone lesion and the joint in various directions.

  • Chopart joint fracture and dislocation are rare injuries compared with other joint injuries with various clinical manifestations. Moreover, there is a lack of knowledge of the radiological findings of the joints, and thus, the extent of joint ligament damage may be underestimated, leading to improper treatment. This paper reports three cases of Chopart joint injury and seeks to reconsider the importance of Chopart joint evaluation and treatment.

유관 나사와 분쇄 골이식을 사용한 거골하 관절 유합술 (Subtalar Arthrodesis using Cannulated Screws and Morselized Bone Graft)

  • Purpose: To analyze the clinical and radiological outcome of subtalar arthrodesis using cannulated screws and morselized bone graft. Materials and Methods: Twenty one patients with follow-up of more than 1 year after subtalar arthrodesis were included in this study. Mean age was 40.8 years, and mean follow-up duration was 38 months. Underlying diseases were 19 cases of posttraumatic arthritis (18 calcaneal fractures and 1 talar fracture) and 2 cases of tarsal coalition. Clinically AOFAS ankle-hindfoot score, operation time, complication and satisfaction of patients were analyzed. Radiologically time to union, arthritis of surrounding joints, preoperative and postoperative talar declination angle were analyzed. Results: AOFAS ankle-hindfoot score was improved from preoperative 33 points to postooperative 79 points. Eighteen patients (86%) were satisfied with the results. Mean operation time was 91 minutes. All cases were fixed with 1-2 cannulated screws and morselized bone graft. Mean time to radiologic union was 12.1 weeks. There was 1 case of delayed union. There was no significant perioperative changes in talar declination angles. Conclusion: Subtalar arthrodesis using cannulated screws and morselized bone graft seems to be relatively simple and effective treatment method for subtalar arthritis.

Concurrent Validity of a Universal Goniometer and a Double Meter Inclinometer for Passive Range of Motion in Beagle Dogs

  • The purpose of this study was to evaluate the concurrent validity of the double meter inclinometer (DMI) for passive joint range of motion (ROM) in beagle dogs and to compare these results to a universal plastic goniometer (UPG). Fifteen beagle dogs were recruited for this study. Joint ROM was evaluated twice with each device to calculate the intraobserver reliability. The intraclass correlation coefficient (ICC) values of the UPG were good to excellent (> 0.75) for all joint ROM tests. Similar results were obtained with the DMI. The ICC values of the DMI were good to excellent (> 0.75) except in extension of the tarsal joint (ICC = 0.69). The majority of the ICC results between each device were poor (< 0.50) with the exception of six joints. Our findings suggest that the inclinometer can be used for passive joint ROM in veterinary medicine. However, caution should be taken when comparing measured values of passive joint ROM obtained utilizing both the DMI and UPG.

관절염 이환자돈과 건강한 모돈으로부터 분리한 Streptococci의 균종 및 항균제 내성 (Prevalence of Streptococci in arthritic piglets and healthy sows with reference to antibiotic resistance of isolates)

  • In order to establish the extent of Streptococcal arthritis piglets, isolation of Streptococci from arthritic lesions of 34 piglets were undertaken from November 1987 to October 1988 in Korea. Also determined were isolation frequency of Streptococci in nasal cavity of 250 healthy sows and antibiotic susceptibilities of the isolates. Streptococci were isolated from 52.9% of 34 arthritic piglets and 20 strains isolated belonged to 4 S suis type I, 8 S suis type II, 2 Lancefield group C and 6 group E. From 28.8% of 250 healthy sows, 72 strains of Streptococci were isolated and these consisted of 9 S suis type I, 51 S suis type II and 12 group C. Streptococcal arthritis seemed to occur prominently in piglets aged 2 to 4 weeks and in male than female. No significant difference were recognized in tarsal and carpal joints as affecting site. All of 92 isolates were sensitive to ampicillin and penicillin, and all strains of S suis type I and group E Streptococcus were also sensitive to chloramphenicol and cephalothin. To cephalothin all strains of group C Streptococcus were sensitive. The 1. 7 to 100% of 92 isolates were resistant with different prevalence to colistin, erythromycin, kanamycin, tetracycline, gentamicin, chloramphenicol and cephalothin. The 92.5% of these resistant Streptococci were multiply drug-resistant strains. The drug resistant patterns most frequently encountered were Tc Cl Em Km Gm(16.3%) in quintuple pattern, Tc Cl Em Km(16.3%) in quadruple pattern, Tc Cl Em(10.9%) in triple pattern and Cl Em(14.1%) in double pattern.
