Novel Appreciation for Taeeum-byung through Revision of Shanghan-lun Clause 273 (상한론(傷寒論) 273조(條)의 교감(校勘)을 통한 태음병(太陰病)에 대한 새로운 해석)

  • Generally, clause 273 of Shanghan-lun was called as a principle of Taeeum-byung and was regarded that it contains essential substances of Taeeum-byung. In this study, I compared the text of diverse woodblock-printed versions, the opinions of many annotators, and other clauses of Shanghan-lun which related with clause 273 in pathologic view points. As results, "the symptoms become worsen by purgative medicine" of remained version is more reasonable rather than "diarrhea become worsen" of Song version, in the side of pathologic interpretation of Taeeum-byung and to understand the reason of mistreatment. In addition, the symptoms of "an intermittent and autonomous abdominal cramp" and "feeling hard on sub-chest region" should be recognized as transformed symptoms by mistreatment of purgative medicine. Thus, diarrhea cannot be accepted as a peculiar symptom of Taeeum-byung, but should be regarded as a possible symptom transformed from Taeeum-byung or its preceding diseases by purgative medicine.

The type of disease and symptom according to Sasang constitution of children (소아(小兒)의 사상체질별(四象體質別) 질병(疾病) 및 증상(症狀) 유형(類型)의 차이)

  • We proceeded the judgement of Sasang constitution of 79 children(the age 101 between 15) who have visited Dongguk Bundang Oriental Medical Hospital during 3 months from August 2002 to October 2002 by Questionnaire of Sasang Constitution Classification and conducted questionnaire survey about the type of disease and symptom occurred frequently. The conclusion is following. 1. The distribution of Sasang constitution: Soeum group(少陰人) 32, Taeeuml group(太陰人) 29, Soyang group(少陽人) 18. 2. The distribution of systemic disease occurred frequently : in digestive system, Soeum (46.9%) & Soyang(44.4%) group more than Taeeum group(34.5%) and in genitourinary system and others, Soeum(28.1%) more than Taeeum group(17.2%). 3. The distribution of symptom occurred frequently : in rhinitis and sinusitis, Taeeum(60.0%) & Soeum(60.0%) more than Soyang(45.5%), in abdominal pain, Soeum(58.8%) & Soyang(62.5%) more than Taeeum(36.4%), and in diarrhea, Taeeum(45.5%) more than Soeum(123.5%) & Soyang group(25.0%). Headache & vexation(心煩) are almost found in Soeum group with abdominal pain and excessive sweating is only found in Taeeum group. 4. The efficacy of herbal medication combined with Cervix Cornu Pranum (鹿茸加味劑) : after the medication, change of condition have showed in Taeeum group at the most high rate(31.0%) and showed in Soeum(15.6%) and Soyang(11.1%) group. 5. The distribution of allergic disease: allergic rhinitis and atopic dermatitis took up the majority of all allergic diseases. In morbidity of allergic rhinitis, Taeeum(62.1%) & Soyang(55.6%) more than Soeum(37.5%). In morbidity of other allergic diseases and degree of severity, it has no difference among three Sasang constitution groups.

  • Objectives This study aimed to provide a basis for treatment of taeeum person by analysing published case reports and case series in Journal of Sasang Constitutional Medicine.Methods We reviewed case reports and case series of taeeum person in the Journal of Sasang Constitutional Medicine from 2006 to 2015.Results Of the forty papers, reporting 182 cases of taeeum person. Thirty-five papers reported 6 types of Sasang Constitutional Medicine pattern. A total of 10 types of interventions were described, of which herbal medicine (100%) was most frequently used. A total of 30 types of outcome measeure were used in papers, for which symptom change (79.2%) was most frequently used. A total of 8 types of diagnosis criteria were used in 38 papers.Conclusions A sufficient number of clinical studies are warranted to provide a clinical evidence for treatment of taeeum person.

  • "The Golden Mirror of Medicine(醫宗金鑑)" was compiled by the medical officers of the Cheong(淸) government headed by Ogyeom(吳謙: 1736-1795) in 1742, and was adopted as a textbook by the Institute of Imperial Physicians(太醫院) in 1749. This book provides a good summary of academic contents and clinical experiences from before the Cheong(淸) dynasty, and serves as a convenient and practical guide book. "The Notes of Treatise on Cold-Induced Diseases(傷寒論注)" is one part of "The Golden Mirror of Medicine(醫宗金鑑)", and this is placed at the beginning of the book, indicating its importance. The chapter on taeeum-disease[太陰病篇], which is the third part of "The Notes of the Treatise on Cold-Induced Diseases(傷寒論注)", has not yet been translated into Korean. Therefore, in this study, the characteristics of Ogyeom's(吳謙) notes are inspected through a comparative study of the chapter of taeeum-disease[太陰病篇] based on translation and the notes of famous scholars. The texts first provide an outline of taeeum-disease[太陰病], which is followed by diarrhea, vomiting and therapeutic methods of syndrome involving both the exterior and interior[表裏兼證], as well as abdominal distension and pain. The prognoses are then explained in succession. The eight texts that have been shown in the chapter of taeeum-disease[太陰病篇] of original text were relocated and the seven texts that existed in the chapters of taeyang(太陽), yangmyeong(陽明) and gwol-eum(厥陰) were moved to this chapter. Furthermore, Ogyeom(吳謙) moved the cold-dysphagia[寒格] text from a chapter of gwol-eum-disease[厥陰病] to a chapter of taeeum-disease[太陰病] and explained vomiting due to pathogenic cold. The origins of taeeum-disease[太陰病] are purported to occur through the yang-channel[陽經] to the eum-channel[陰經], and taeeum-disease[太陰病] was reported to include both interior-deficiency-cold-syndrome[裏虛寒證] and interior-excess-heat-syndrome[裏實熱證]. In the case of diarrhea-more-severe-symptoms[自利益甚], he thought it induced by faultpurgation[誤下], and in indication for decoction of cinnamon with peony[桂枝加芍藥湯] and decoction of cinnamon with rhubarb[桂枝加大黃湯], he thought it included the exterior syndrome of taeyang-disease[太陽表證], and rhubarb was used in purgation of taeeum-excess[太陰實].

Impact of the Sasang Constitutional Type on the High-Sensitivity C-Reactive Protein, a Predictor of Cardiovascular Disease (사상체질(四象體質)과 심혈관질환 예측인자인 High-sensitivity C-reactive protein의 상관성에 관한 연구)

  • 1. Objectives The purpose of this study was to investigate the high-sensitivity C-reactive protein (hsCRP) related characteristics found in each Sasang Constitutional type. 2. Methods This cross-sectional epidemiological study was conducted using data from 2,842 men and women. After typing each participant into one of the four Sasang constitutional types, they were analyzed on various cardiovascular disease-related variables. 3. Results 1) The predictors of cardiovascular disease (metabolic syndrome, high risk FRS group, high risk hsCRP group) was found to show the highest prevalence in the Taeeum type. 2) In the group with metabolic syndrome, the mean hsCRP concentration in the Taeeum and Soyang types were higher than in the Soeum type. In the group without metabolic syndrome, the mean hsCRP concentration in the Taeeum type were higher than in the Soyang and Soeum types. 3) In the FRS low risk group, the mean hsCRP concentration were higher in order of Taeeum type > Soyang type > Soeum type. In the FRS high risk group, the mean hsCRP concentrations were not significantly different among the Sasang constitutional types. 4. Conclusions The results of this study suggest that the Taeeum constitutional type is a risk factor for high hsCRP and cardiovascular disease. These findings suggest that the prevention of cardiovascular disease is more important in the Taeeum type compared to other constitutional types. Moreover, preventive measures are warranted even in the Taeeum-type persons with low clinical cardiovascular risk.

Comparison study on Constitution Classification between QSCCII and Clinical Research (사상체질분류검사지(QSCCII)를 이용한 체질감결방법에 대한 비교 연구)

  • Objectives: One of the most important and difficult things in SaSang Constitution Medicine is to diagnose and classify constitution. So there have been a lot of methods and tools to diagnose and classify it. We have attempted to show the reliability and clinical application of the QSCC Ⅱ (Questionnaire for the Sasang Constitution Classification Ⅱ), comparing with our own clinical research. Methods: We surveyed information from QSCC Ⅱ and compared QSCC Ⅱ' s reliability with clinical research of 54 patients' constitution(men 24, women 30). Results: Taeeum-in is the most commonly reported constitution by clinical research, and the next is Soeum-in. Soyang-in is the most commonly reported constitution by QSCC Ⅱ and the next is Taeeum-in. In Taeeum-in and Soeum-in' s items of QSCC Ⅱ, these matched those of the clinical research. Soyang-in' s items of QSCC Ⅱ did not match. Conclusions: The efficiency of QSCC Ⅱ is higher than clinical research in Taeeum-in, Soeum-in. But lower than clinical research, Soyang-in and Taeyang-in have no reliability in QSCC Ⅱ. So Soyang-in and Taeyang-in require alternative complementary studies in the future.

A Study on the Pathology and Therapeutics of Epigastric Discomfort in the Soeumin Taeeum Symptomatology (소음인(少陰人) 태음병(太陰病) 심하비증(心下痞證)의 병증약리(病證藥理) 고찰)

  • 1. Objective: Gastric Stuffiness (心下痞證) in the Soeumin Taeeum symptomatology (少陰人 太陰病) in Sasang Constitutional Medicine shows an interesting evolutionary process. Originally found in the texts of Sanghanlun (傷寒論), the perspectives regarding the symptoms of Gastric Stuffiness were expanded by many medical scholars and Lee Jema. To comprehend the Jema's concept of Taeeum symptomatology and utilize his prescription clinically, it deserves to study on the Gastric Stuffiness in Donguisuseowon (東醫壽世保元). 2. Methods: Many texts including Sanghanlun (傷寒論) and Dongeuibogam (東醫寶鑑) were reviewed in order to track the evolutionary process concerning the symptoms of Gastric Stuffiness. Also, Lee Jema's works including the different versions of Donguisuseowon (東醫壽世保元) were reviewed to examine the development of Lee's concepts regarding the Gastric Stuffiness in the Soeumin Taeeum symptomatology. 3. Results and Conclusions: 1) Most of the previous concepts on Gastric Stuffiness were generally similar to those found in Sanghanlun, especially in that they addressed Yang deficiency of the middle triple energizeer as the central pathogenic process. 2) Lee Jema's views on the epigastric discomfort in the Soeumin Taeeum symptomatology evoloved over time and culminated in the finalized pathology and therapeutics appearing in his last work, the Shinchuk version of Donguisuseowon (東醫壽世保元). 3) 4 kinds of prescription which invented by Lee Jema to treat the Gastric Stuffiness in the Soeumin Taeeum symptomatology can be matched to the prescription of Sanghanlun respectively. However through evolutionary process of Jema's prescriptions, the indication of them expanded: not only to treat Gastric Stuffiness but to diverse symptoms of Soeumin.

A Study of the Influence of Daesihog-tang on Taeeum-in's Patholoy and Prescription (대시호탕(大柴胡湯)이 태음인(太陰人) 병증용약(病證用藥)에 끼친 영향에 대한 고찰)

  • Objectives The purpose of this study is to understand the influence of Daesihog-tang on Taeeum-in's patholoy and prescription in "Donguisusebowon" of Sasang Constitutional Medicine. Methods The pathologic condition in which Daesihog-tang used and the relevant herbs and prescriptions in "Sanghanron", "Donguibogam", "Dongmuyoogo haedong", "Chobongwon", and "Donguisusebowon". Results and Conclusions 1. Daesiho-tang in "Sanghanron" was used to the symptom of Yangmyung disease merging with Soyang disease. In "Donguibogam", its indications were mainly used to the severe fever-symptom of Yangmyung disease and expanded to Onbyeong and so on. 2. The concept of Daesiho-tang in "Donguibogam" was changed into Hwanggeumdaehwang-tang in "Chobongwon", which can be used to treat Onbyeong with heat-dysenteric diarrhea, and severe fever of Taeeum-in. 3. The notion of combination between Daehwang and hwanggeum used in "Dongmuyoogo haedong" and "Chobongwon" was connected to Chungmongsukgondamhwan or Jogakhwanggeumdaehwang-tang in "Chobongwon". Especially, Jogakhwanggeumdaehwang-tang for Yeokkigammo and severe constipation was formed while the concept of Onbyeong in Hwanggeumdaehwang-tang' merged with the usage of Jogak and Daehwang or Yisunggugo-hwan. 4. Jogakhwanggeumdaehwang-tang influenced on the usage of Jogakdaehwang-tang for Yangdok and Onbyeong, while the usage of Chungmongsukgondamhwan was not found in "Donguisusebowon". 5. As a result, Daesiho-tang may be presented without usages for a specific patholoy in "Donguisusebowon" because it indirectly influenced on the concept of Yangdok and Onbyeong of Taeeum-in, and the formation of Jogakdaehwang-tang in "Donguisusebowon".

Analysis of the Relationship between Sasang Constitution and Measuring Items of the Head and Face (QSCC II 체질과 두면부 계측항목의 연관성 분석)

  • The objectives of study are to investigate the relationship between Sasang Constitution(四象醫學) and measuring items of the head and face and to develop useful diagnosis standard of Sasang Constitution. Subjects took QSCCII(Questionnaire for the Sasang Constitution Classification II) tests repeatedly at an interval of one year and were diagnosed as same constitutions. The 9 Items were measured by Martin-type anthropometric method in 112 subjects. Compared with measurements of 3 constitutions, the mean values in Taeeum-in(太陰人) showed highest and there were significant differences in 7 items between Taeeum-in(太陰人) and other constitutions. Especially, Measurements of Bitragus to Submandibular Arc Length(BSAL) item and Bitragus to Menton Arc Length(BMAL) item showed significant differences between Taeeum-in(太陰人) male and other constitutions male. It seems that BSAL and BMA items can differentiate Taeeum-in(太陰人) from other constitutions and these items have possibility to be a diagnosis standard of Sasang Constitution.

Approaching obesity through constitutional therapy: a series of 4 obesity cases who were classified as the Taeeum constitutional type (태음인(太陰人) 병증(病證) 분류(分類)에 근거한 비만환자 치험 4례)

  • 1. Objectives: We report a series of 4 obesity cases who responded well to the weight-loss therapy based on the Taeeum Sasang constitutional therapy. 2. Methods: In treating 4 obese patients who were constitutionally typed as the Taeeum type, Case 1 and Case 2 fell into the category of the Lung Dry-Cold symptomatology, Case 3 into the Dry-Heat symptomatology, and Case 4 into the Exterior-Interior Combined symptomatology. Each patient was given Sasang medication according to protocol: Taeeumjowi-tang (Taiyintiaowei-tang) to Case 1 and Case 2, Cheongpyesagan-tang (Qingfeixiegan-tang) to Case 3, and Seunggijowi-tang (Chengqitiaowei-tang) to Case 4. On each visit to the clinic (7-21 times in total), the patients were checked on their general condition and change in body weight. 3. Results and Conclusions: All patients showed improvements in their general condition and a satisfactory decrease in weight (Case 1 showed a decrease of 12.7 kg in 84 days, Case 2 a decrease of 8.9 kg in 39 days, Case 3 a decrease of 33 kg in 250 days, and Case 4 a decrease of 29.3 kg in 155 days). Treating obesity based on Taeeum constitutional therapy was found to be effective for weight loss and general improvement of health. The sustained effects of the therapy should be checked during the following visits. The obesity treatment protocol suggested in this investigation should be explored and verified in future studies.
