The aim of the present study was to examine the effects of high and low frequency repetitive transcranial magnetic stimulation on motor cortical excitability and the balance function in subacute stroke patients. Twenty-four subjects were randomly assigned to either the high frequency (HF) rTMS group, or the low frequency (LF) rTMS group, with 12 subjects each. All subjects received routine physical therapy. In addition, both groups performed a total of 20 sessions of rTMS for 20 minutes, once a day, 5 times per week, for a 4-week period. In the HF rTMS group, 10 Hz rTMS was applied daily to the hotspot of the lesional hemisphere; and in the LF rTMS group, 1 Hz rTMS was applied daily to the hotspot of the nonlesional hemisphere. Motor cortex excitability was determined by motor evoked potentials, and the balance function was evaluated by use of the Balance Index (BI) and the Berg Balance Scale (BBS), before and after the intervention. The change rate in the value of each variable differed significantly between the two groups (p<0.05). Furthermore, significant differences were observed between all post-test variables of the two groups (p<0.05). In the HF rTMS, significant differences were found in all the pre- and post-test variables (p<0.05). On the other hand, in the LF rTMS, significant difference was observed only between the pre- and post-test results of BI and BBS (p<0.05). The findings demonstrate that HF rTMS can be more helpful in improving the motor cortical excitability and balance function of patients with subacute stroke treatment than LF rTMS, and that it may be used as a practical adjunct to routine rehabilitation.
본 연구는 TMS 분석기의 성능 상태를 판정하기 위한 지능형 진단기 설계 방법을 제안하였다. NOx와 SOx를 계측하는 TMS 분석기의 입, 출력 신호와 설비의 유지보수 경험을 이용하여 분석기의 동작 상태를 진단하는 Neruo-Fuzzy 진단 모델을 설계하는 방법을 제안하고, 실험 데이터로 시뮬레이션을 통해, 그 타당성을 입증하였다.
Transcranial magnetic stimulation (TMS) is a safe and noninvasive tool for investigating the cortical excitability of the human brain and the neurophysiological functions of GABAergic, glutamatergic, and cholinergic neural circuits. Neurophysiological biomarkers based on TMS parameters can provide information on the pathophysiology of dementia, and be used to diagnose Alzheimer's disease and differentiate different types of dementia. This review introduces the basic principles of TMS, TMS devices and stimulating paradigms, several neurophysiological measurements, and the clinical implications of TMS for Alzheimer's disease.
인삼의 주요 polyacetylens 화합물인 panaxynol, panaxydol 및 panaxytriol의 정량을 위한 GC 분석조건을 확립하였다. 이 때, GC column과 검출기는 SPB-1 fused silica capillary(0.25 mm i, d. $\times$30m, Supelco)와 불꽃이온화검출기(FID)를 사용하였으며 column oven의 온도는 $250^{\circ}C$ 등온이었다. Panaxynol, panaxydol 및 panaxytriol peaks의 retention time은 TMS 유도체화 전의 경우에는 각각 4.2, 5.1 및 6.9분이었으며 TMS유도체화 후에는 각각 4.5, 5.4 및 7.4 분이었다. TMS 유도체화 전의 panaxynol, panaxydol 및 panaxytriol의 정량가능 최소농도는 각각 20, 50 및 100ppm 이었던 데에 비하여, TMS 유도체화 후에는 세 화합물의 정량가능 최소농도가 모두 5ppm의 낮은 수준을 나타내었다. 한편, 홍삼 중의 panaxynol, panaxydol 및 panaxytriol 함량 분석값은 각각 724, 721 과 71$\mu\textrm{g}$/g이었다.
수평형 반응로에서 $TMS[Si(CH_3)_4]-H_2$ 와 H$_2$가스를 이용하여 SiC 초미분말 합성시 최적 공정 조건을 알아보기 위하여 수치모사방법을 이용하였다. 이론적인 해석 결과, 반응온도가 증가함에 따라 TMS의 전환률은 증가하였지만, 수소유량이 증가함에 따라서는 TMS 전환률이 감소하였다. 또한 반응온도가 높을수록 기상의 충돌확률이 증가하여 최종 생성된 SiC 입자농도는 감소하였지만, 수소유량과 TMS 농도가 증가하는 경우에서는 생성된 입자농도가 증가하였다. 한편 입자크기는 반응온도와 초기 TMS 농도가 증가함에 따라 증가하였지만 수소유량이 증가하는 결향을 나타내었다. 이러한 반응온도, 수소유량 및 TMS 농도에 따른 입자크기 변화는 실제 실한 결과와 이론적으로 고찰한 결과가 일치하였다.
Objectives : The aim of this study was identification of the anti-oxidative and anti-inflammatory effects of Talmyung-san (TMS) in mouse macrophage, RAW264.7 cells. Methods : To identify the anti-oxidative effect of TMS, scavenging activities of DPPH radical, nitric oxide and peroxynitrite were measured in vitro. In RAW264.7 cells, DCFH-DA assay was conducted to examine the inhibitory effect of TMS on ROS production in response to lipopolysaccharide. And the productions of nitric oxide (NO), $PGE_2$ and pro-inflammatory cytokines were measured. The levels of COX-2, iNOS, nuclear NF-${\kappa}B$ p65 expression and phosphorylation of $I{\kappa}B-{\alpha}$ in cytosol were detected by western blotting analyses. Results : TMS couldn't scavenged DPPH radical, but nitric oxide and peroxynitrite were decreased. TMS decreased intracellular ROS, NO, and IL-$1{\beta}$ production effectively. However, TMS inhibited $PGE_2$ levels only in high concentration ($300{\mu}g/m{\ell}$) and TMS failed to suppress the production of IL-6 and TNF-${\alpha}$. Results from immunoblot analyses revealed that TMS decreased activation of COX-2, iNOS, phosphorylation of $I{\kappa}B-{\alpha}$ and nuclear translocation of p65. Conclusions : TMS has anti-RNS and anti-inflammatory effects via NF-${\kappa}B$ pathway and more intensive studies will be required to evaluate therapeutic potential of TMS.
Repetitive transcranial magnetic stimulation (rTMS) modulates cortical excitability beyond the duration of the rTMS trains themselves. Depending on rTMS parameters, a lasting inhibition or facilitation of cortical excitability can be induced. Therefore, rTMS of high or low frequency over motor cortex may change certain aspects of motor learning performance and cortical activation. This study investigated the effect of high and low frequency subthreshold rTMS applied to the motor cortex on motor learning of sequential finger movements and brain activation using functional MRI (fMRI). Three healthy right-handed subjects (mean age 23.3) were enrolled. All subjects were trained with sequences of seven-digit rapid sequential finger movements, 30 minutes per day for 5 consecutive days using their left hand. 10 Hz (high frequency) and 1 Hz (low frequency) trains of rTMS with 80% of resting motor threshold and sham stimulation were applied for each subject during the period of motor learning. rTMS was delivered on the scalp over the right primary motor cortex using a figure-eight shaped coil and a Rapid(R) stimulator with two Booster Modules (Magstim Co. Ltd, UK). Functional MRI (fMRI) was performed on a 3T ISOL Forte scanner before and after training in all subjects (35 slices per one brain volume TR/TE = 3000/30 ms, Flip angle $60^{\circ}$, FOV 220 mm, $64{\times}64$ matrix, slice thickness 4 mm). Response time (RT) and target scores (TS) of sequential finger movements were monitored during the training period and fMRl scanning. All subjects showed decreased RT and increased TS which reflecting learning effects over the training session. The subject who received high frequency rTMS showed better performance in TS and RT than those of the subjects with low frequency or sham stimulation of rTMS. In fMRI, the subject who received high frequency rTMS showed increased activation of primary motor cortex, premotor, and medial cerebellar areas after the motor sequence learning after the training, but the subject with low frequency rTMS showed decreased activation in above areas. High frequency subthreshold rTMS on the motor cortex may facilitate the excitability of motor cortex and improve the performance of motor sequence learning in normal subject.
교수자들은 팀기반 프로젝트 학습을 통해 학생들이 협업능력과 실생활 문제해결력을 기르기를 기대한다. 하지만 프로젝트 학습에서 학생들은 단순분업방식으로 문제를 해결하려 할 뿐 아니라 학습전이가 잘 일어나지 않는 경향이 있다. 학생들이 협업의 시너지를 내고 서로의 지식을 이용하여 학습전이를 일어나게 하기 위해서, 이 연구에서는 교류기억체계(TMS) 기반 협력학습 모형을 개발하여 적용하고 그 효과를 검증하고자 한다. 교류기억체계 기반 협력학습 모형은 과학개념을 학습하면서 동료의 전문성을 파악하는 교류기억체계 개발 단계, 간단한 협력적 문제해결을 통해 신뢰를 기르는 교류기억체계 정교화 단계, 마지막으로 조직화된 지식처리를 하는 교류기억체계 적용의 3단계로 이루어져 있다. 첫 번째 단계에서 동료교수법을 통해 과학개념을 학습함과 동시에 그룹 구성원이 서로 어떤 지식을 잘 알고 있는지 전문성을 파악한다. 두 번째 단계에서 잘 정의된 실험 문제를 협력적으로 해결하며 근전이를 경험한다. 세 번째 단계에서 근전이 경험을 바탕으로 프로젝트 수행을 위한 원전이를 통해 빈약하게 정의된 문제를 해결한다. 이 모형을 기반으로 기하광학과 음파에 관한 15주 교육 프로그램을 만들어 대학생들에게 적용하였다. 이 중 프로젝트 1개를 적용한 5주간의 자료를 수집하고 분석하였다. 적용결과, 일반 프로젝트 학습을 적용한 그룹의 TMS변화는 유의하지 않았음에 비해, 교류기억체계 기반 협력학습모형을 적용한 그룹의 TMS는 단계적으로 향상되었고, 첫째 주와 마지막 주의 차이는 통계적으로 유의하였다. 또한 실험집단은 비교집단에 비해 프로젝트 수행에서 학습전이가 더 잘 일어난 것을 확인할 수 있었다. 학생들이 협력을 통해 시너지를 얻는 방법을 익히고, 학습한 내용을 문제해결에 잘 적용하도록 하는데 교류기억체계기반 협력학습 모형이 사용될 수 있을 것이다.
