A Clinical Case Report of Taeeumin Patient Diagnosed with Systemic Contact Dermatitis and Facial Flushing (전신성 접촉피부염과 안면홍조를 호소하는 태음인 환자 치험 1례)

  • Objectives This study is to report a significant improvement in a Taeeumin patient with systemic contact dermatitis after eating urushiol chicken by herbal medicine treatment. Methods The patient had complaints of erythema, swelling, pruritus, scaly skin, and facial flushing. We treated the patient with an herbal medicine 'Galgeunhaegi-tang' for three months. We evaluated the treatment outcome of systemic contact dermatitis using the Three Item Severity(TIS) score every visit and facial flushing by Image color summarizer at the first and last visit. Results After the treatment, the severity of the patient's skin complaints lessened from moderate to mild. The Image color summarizer showed a minor decrease in the normalized red color level and a significant increase in the brightness level and facial color percentage. Conclusions The patient diagnosed with systemic contact dermatitis, treated with Galgeunhaegi-tang for three months, showed a significant improvement in skin complaints with brighter and even facial color.

Design and Implementation of EMS for Real-Time Power Generation Control of Wind Farm Based on Wake Effect Optimization (후류 영향 최적화 기반 실시간 풍력발전단지 발전 제어용 EMS의 설계 및 구현)

  • This paper aimed to design and implement an EMS for real-time power generation control based on wake effect optimization of wind farm, and then to test it in commercial operating wind farm. For real-time control, we proposed the wake band-based optimization and setting the wake effect distance limit, and when the wake effect distance limit was set to 7D in the actual wind farm layout, the calculation time was improved by about 93.94%. In addition, we designed and implemented the script-based EMS for flexible operation logic management in preparation for unexpected issues during testing, and it was installed and tested on a wind farm in commercial operation. However, three issues arose during the testing process. These are the communication interface problem of meteorological tower, the problem of an abnormal wake effect, and the problem of wind turbine yaw control. These issues were solved by modifying the operation logic using EMS's script editor, and the test was successfully completed in the wind farm in commercial operation.

  • Stereolithography is a process used to rapidly produce polymer components directly form a computer representation of the part. There are several considerations to be made for the efficient use of the process. Especially, the build-up orientation of part critically affects the part accuracy, total build time and the volume of support structures. The purpose of tis study is to determine the optimal build-up part orientation for the SLA process with improving part accuracy, and minimizing total build time and the volume of support structures. The forst factor is related to the area of surfaces whioch have staircase protrusions after solidification, the second factor is related to the total number of layers, and the third factor is related to the area of the surfaces which need to be supported with support structures. An algorithm is developed to calculate the staircase area, quantifying the process errors by the volume of materials supposed to be removed or added to the part, and the optimal layer thickness for the SLA system which can handle the variable layer thickness. So the optima l part orientation is determined based on the user's selections of primary criter- ion and the optimal thickness of layers is calculated at any part orientations.

Unraveling the Role of Cytochrome P450 as a Key Regulator Lantipeptide Production in Streptomyces globisporus

  • The aim of this study was to investigate the regulation of lantipeptide production in Streptomyces globisporus SP6C4, which produces the novel antifungal lantipeptides conprimycin and grisin, and to identify the role of cytochrome P450 (P450) in tis regulation. To investigate the regulation of lantipeptide production, we created gene deletion mutants, including ΔP450, ΔtsrD, ΔlanM, ΔP450ΔtsrD, and ΔP450ΔlanM. These mutants were characterized in terms of their morphology, sporulation, attachment, and antifungal activity against Fusarium oxysporum. The gene deletion mutants showed distinct characteristics compared to the wild-type strain. Among them, the ΔP450ΔlanM double mutant exhibited a recovery of antifungal activity against F. oxysporum, indicating that P450 plays a significant role in regulating lantipeptide production in S. globisporus SP6C4. Our findings highlight the significant role of P450 in the regulation of lantipeptide production and morphological processes in S. globisporus. The results suggest a potential link between P450-mediated metabolic pathways and the regulation of growth and secondary metabolism in SP6C4, thereby highlighting P450 as a putative target for the development of new antifungal agents.

Protein Quality Evaluation of Cooked Hagfish (Eptatretus burgeri) Meats

  • The effect of cooking methods on in vivo and in vivo indices of the protein quality of hagfish meat were investigated. In vivo protein digestibilities of cooked meats (81.3~83.5 %) were not significant different (p<0.05) from those of van meat (82.9%), with the exception of steamed (11$0^{\circ}C$, 15 min) meat (86.3 %). Convection oven cooking (22$0^{\circ}C$, IS min) resulted in a higher trypsin indigestible substrate (TIS, 49.2 mg/g solid) compared with that of raw meat (38.9 mg/g solid). free amino acid content of raw meat was decreased after boiling (10$0^{\circ}C$, 10min). Both convection oven and microwave cooking (2,450 MHz, 3 min) decreased available lysine from 4.9g/16g N to 3.8~4.1g/16g N. In vivo apparent protein digestibilites (AD) of hagfish meat were similar fur raw (92.4%) and cooked meats, but were somewhat lower than ANRC (Animal Nutrition Research Council) casein (945%). The PERs (3.7~4.1) and NPRs (3.7~4.9) of cooked meats were significantly higher (p<0.05) than those of raw meat (PER 3.3, NPR 3.6 and ANRC casein (PER 2.5, NPR 2.6), despite their lower in vivo protein digestibilities. These results demonstrate that cooking at optimal conditions resulted in remarkably positive effects on in vivo and in vivo protein qualities of hagfish meats. Therefore, steamed hagfish meat is an excellent source of high quality protein from seafood products.

The Effect of Stabilizing Reversal and Rhythmic Stabilization in PNF on Walking and Balance in Patients with Stroke (PNF의 안정적 반전과 율동적 안정화 기법이 뇌졸중 환자의 보행 및 균형에 미치는 영향)

  • Purpose: The purpose of this study was to demonstrate the effects of proprioceptive neuromuscular facilitation (PNF) stability techniques on walking speed, trunk stability, and balance in stroke patients. Methods: Ten stroke patients volunteered to participate in the study, and each of subjects was randomly assigned to either the stability technique (ST) group (n=5) or to the treadmill (TM) group (n=5). Each therapeutic exercise program was provided for 30 minutes a day, 5 days per week for 4 weeks. The ST group performed a PNF pattern combined with stabilizing reversal and rhythmic stabilization of the PNF stability technique. Walking speed (measured using a 10-meter walking test), trunk stability (TIS), and balance (BBS, FRT) were evaluated before and after training. All data were analyzed using SPSS version 18.0. The significance level for statistical inspection was set at 0.05. Results: Both groups showed improvements on the 10-meter walking test, the trunk impairment scale, the Berg balance scale, and the functional reaching test. Conclusion: PNF stability techniques are effective for improving trunk stability, balance, and walking speed in stroke patients. For stroke patients, PNF stability techniques are very useful and effective, including in clinical practice.

  • Osteoarthritis is a noninflammatory degenerative disease affecting the articular surfaces and is accompanied by remodeling of the underlying bone. The sympotms of osteoarthritis of the temporomandibular joint are pain in the joint and muscles of mastication, limitation. Osteoarthritis is generally accepted that several factors can contribute to the trauma, aging process, internal derangement and MPDS. Radiographic features of the osteoarthritis are seen flattening of joint, sclerosis on flattened area and osteophyte or anterior lipping. In the past, osteoarthritis was considered to self-limiting disease. Currently, synovial chondromembrane is part of the process of osteoarthritis secondary to trauma. Synovial chondromatosis is an uncommon disease of cartilaginous transformation of synovial membrane with formation of loose bodies within the joint space. The pathogenesis is more an active metaplastic than a neoplastic process. The cause of synovial chondromatosis is unknown. Although trauma and inflammation have all been cited as possible factors in tis pathogenesis. The clinical sign and symptoms are unilateral swelling of the joint region, pain in the joint area and crepitus seem to be the most reliable signs and symptoms. Radiographic evidence of loose bodies may or may not be present. This is a case report of 66 year old female with synovial chondromatosis, that is advanced disease of the osteoarthrits. We treated patient with surgical excision of lose bodies, diskectomy and synovectomy. The defected articular fossa area was reconstructed with temporalis fascia flap. The result was satisfactory.

Hwa-Byung Treated by Using Ascending Kidney Water and Descending Heart Fire Pharmacopuncture: Three Case Studies

  • Objectives: We report the results of three case studies on the clinical efect of ascending kidney water and descending heart fre (AKDH) pharmacopuncture on patients with Hwa-Byung. Methods: Tis study involved three patients, all female, who had been admitted to a hospital of traditional Korean medicine and had been diagnosed as having Hwa-Byung by using the Hwa-Byung Structure Clinical Interview for DSM-IV. AKDH pharmacopuncture is administered by injecting pharmacopuncture at a specifc acupoint to change from a state of water-fre disharmony to one of water-fre harmony. For our three patients, Hwangyeonhaedoktang (黃連解毒湯) pharmacopuncture was injected at GB21 and GB20 on both sides; a total of 0.4 cc was injected, with 0.1 cc being injected at each point. Fel ursi, Bezora bovis and moschus (BUM) pharmacopuncture was injected at CV17, CV12, and CV6; a total of 0.15 cc was injected, with 0.05 cc being injected at each point. Treatment was done daily. Progress was evaluated using the Hwa-Byung diagnostic interview (HBDIS), beck depression inventory (BDI), and numeric rating scale (NRS) scores. Results: For the frst patient (case 1), after treatment, the HBDIS score was reduced from 44 to 30 points, the BDI score from 37 to 14, and the NRS score from 10 to 7. For the second patient (case 2), after treatment, the HBDIS score was reduced from 41 to 27 points, the BDI score from 13 to 7, and the NRS score from 10 to 5. For the third patient (case 3), after treatment, the HBDIS score was reduced from 42 to 28 points, the BDI score from 12 to 9, and the NRS score from 10 to 4. Conclusion: Ascending kidney water and descending heart fre pharmacopuncture treatment can be efective for improving ascending kidney water and descending heart fre energy and can be used to alleviate Hwa-Byung.

The Effects of Task Oriented Training with Suspension Device on Trunk Stability and Gross Motor Function of Children with Spastic Diplegia Cerebral Palsy (현수보조장치를 이용한 과제 지향적 훈련이 경직형 양하지 뇌성마비아동의 체간 안정성과 대동작기능에 미치는 영향)

  • PURPOSE: The purpose of this study was to examine the effect of using a suspension device for arm reaching activity on trunk stability and gross motor function of children with spastic diplegia cerebral palsy. METHODS: The subject in this study consisted of 11, GMFCS(Gross Motor Function Classification System) III~IV children with spastic diplegia cerebral palsy, all of whom agreed to participate in the study. All subjects were divided into two groups: the experimental group using a suspension device, and the control group using no suspension device. For each group, a thirty-minute intervention was done twice per week during 8 weeks. Before and after intervention, each test was measured using TIS(Trunk Impairment Scale), GMFM (Gross Motor Function Measure) and PRT(Pediatric Reaching Test) to change trunk stability, gross motor function and arm reaching activity. The data were analyzed with the Wilcoxon signed rank test. RESULT: All two groups had a meaningful increase in GMFM-Sit data measured before and after intervention. The experimental group had a significant increase from an average of 78.83 to an average of 84.83 in GMFM-Crawling. For both groups, there was a substantial increase in the change in sitting position and arm reaching. CONCLUSION: According to the results of this study, the arm reaching activity using suspension device had an effect on trunk stability and gross motor function and it changed arm reaching activity.

Effectiveness of radiotherapy for head and neck skin cancers: a single-institution study

  • Purpose: External beam radiotherapy (EBRT) is a useful option to treat head and neck skin cancer patients who are not indicated for surgery. In this study, we evaluated the treatment outcomes of EBRT in an Asian population. Materials and Methods: The records from 19 head and neck skin cancer patients (10 with squamous cell carcinoma and 9 with basal cell carcinoma) who were treated with definitive or adjuvant EBRT from 2009 to 2017 were retrospectively reviewed. The radiotherapy doses administered ranged from 50 to 66 Gy (median, 55 Gy) with 2.0-2.75 Gy per daily fraction (median, 2.5 Gy). The T stage at presentation was as follows: Tis (1 patient), T1 (11 patients), T2 (6 patients), and T3 (1 patient). None had regional lymph node disease or distant metastasis at presentation. The local failure-free survival (LFFS) rates, toxicity, and cosmetic results were analyzed. Results: The median age was 75.5 years (range, 52.6 to 92.5 years). The median follow-up duration from the completion of radiotherapy was 44.9 months (range, 5.8 to 82.6 months). One local failure occurred in a patient with a 2.1-cm posterior neck squamous cell carcinoma at 32.5 months after radiotherapy (1/19, 5.3%). The 3-year LFFS rate was 91.7%. No patients died from skin cancer during follow-up, and no grade 3 complications occurred. The cosmetic outcomes were excellent for 16 (84.2%) and good for 3 (15.8%) of the 19 patients. Conclusion: EBRT offers good local control and cosmetic outcomes in patients with head and neck skin cancer, with no grade 3 complications.
