우울증 치료의 중의학 최신 임상연구 동향-무작위 대조군 임상연구와 체계적 문헌 고찰을 중심으로- (Review of Recent Clinical Trials for Depression in Traditional Chinese Medicine-Based on Randomized Controlled Trials and Systematic Reviews-)

  • The purpose of this study was to review the research on treating depression in Traditional Chinese Medicine(TCM) over the last 5 years to set the foundation for further studies. We searched for and analyzed articles about depression in CNKI(China National Knowledge Infrastructure) from January 2010 to December 2014. The results were as follows : The most frequently used diagnostic criteria was CCMD-3(The Chinese Classification of Mental Disorders-3), and the most used evaluation criteria was HAMD(Hamilton Depression Rating Scale). Prescription of decoction of medical herbs was most frequently used as a therapeutic method. Acupuncture, traditional Chinese psychotherapy, and music therapy were also used for some studies. The most frequent TCM Syndrome Differentiation Type was stagnation of liver-QI. For decoction of Chinese herbs, Soyo-san(Xiaoyao-san) and Sihosogan-san(Chaihushugan-san) were most often prescribed, and Bupleuri Radix, Paeoniae Radix Alba, Curcumae Radix, Poria cocos wolf, Angelicae Gigantis Radix, Atractylodis Rhizoma Alba were mainly used for medical herbs. BaekHoi(GV20) and Yindang(Ex-HN3) were often used as acupuncture sites. Post-Stroke Depression was the most common case of intercurrent or secondary depression. According to the Jadad Quality Assessment Scale, the quality of the reports was not high as most of the reports had a score of 3 or below. Most systematic reviews on depression were conducted by Chinese researchers. The problem with Clinical research on depression, according to those reviews, was that there were no standardized criteria for the diagnosis and treatment and the trials were usually not randomized nor controlled. We found out there are various clinical methods for treating depression in TCM, and hope that this research could provide the preliminary data for designing and conducting clinical trials for depression.

혀 운동(tongue exercise)이 연하장애를 가진 뇌졸중 환자에게 미치는 효과 : 체계적 고찰 (Effect of Tongue Exercise on Stroke Patients With Dysphagia : A Systematic Review)

  • 목적 : 본 연구는 연하장애를 가진 뇌졸중 환자에게 적용된 혀 운동을 체계적으로 분석한 논문이다. 혀 운동에 대한 전반적인 효과를 검증하고, 현재 이루어지고 있는 혀 운동에 대한 경향을 파악하는데 그 목적을 가지고 근거를 제공하고자 한다. 연구방법 : PRISMA(Preferred Reporting Items for Systematic Reviews and Meta-Analysis) 체크리스트 및 흐름도를 이용하여 체계적 고찰을 시행하였다. 데이터베이스는 PubMed, MEDLINE, CINAHL, RISS, e-article 을 통해 문헌 검색을 진행하였다. 본 연구에 사용된 문헌의 수는 총 6편이었고, 문헌의 질을 평가하기 위하여 PEDro scale(Physiotherapy Evidence Database scale)을 사용하였다. 결과 : 분석된 논문 6편에서 3가지의 중재 방법이 포함되었다. 혀 운동의 종류로는 혀 근력훈련 3편, 혀 근력 및 정확도 훈련 2편으로 모두 아이오와 구강 수행 도구를 통한 중재를 시행하였고, 혀 신장훈련을 적용한 논문이 1편이었다. 조사된 문헌에서 시행한 중재 별 치료 효과는 모두 효과가 있는 것으로 확인되었다. 하지만 대부분 표본수가 적어 일반화 시키기는 어려울 것으로 생각되며, 실험군과 대조군 비교 시 몇몇 그룹 간에 유의한 차이가 발견되지 않은 결과들이 있는 만큼 전통적 연하재활치료도 구강기 연하장애를 가지고 있는 환자들에게 적절한 치료 방법으로 판단된다. 결론 : 본 연구는 작업치료사들이 연하장애를 가진 뇌졸중 환자들에게 혀 운동을 적용함에 있어 효율적인 연하재활치료를 제공하는데 도움을 줄 수 있을 것이며, 혀 운동의 효과를 알아보기 위해 더 많은 연구축적이 이루어져야 할 것으로 생각된다.

제주도 암발생 양상에 관한 가설 제기 및 규명 (Explaining Cancer Incidence in the Jejudo Population)

  • Objectives : Using the population-based cancer registry in Jejudo, we found that Jejudo had lower incidence in stomach cancer than other regions in Korea. The aim of this study was to evaluate reasons for this difference. Methods : Citrus is the leading agricultural production in Jejudo, suggesting that lower cancer incidence in Jejudo could be explained by citrus fruit intake. We evaluated this hypothesis with quantitative systematic review(QSR). Results : Stomach cancer incidence was significantly lower, with a summary odds ratio(SOR) after QSR of 0.72 [95% CI=0.64-0.81]. In addition, the SOR of pancreatic cancer tended to be lower at 0.83 [95% CI=0.70-0.98]. The SOR of prostate cancer was slightly higher at 1.03 [0.89-1.19]. Conclusions : Quantitative systematic reviews for the effect of citrus fruit intake on cancer occurrence suggested that lower cancer incidence in Jejudo could be explained by intake of citrus fruits.

The Occurrence Climatology of Equatorial Plasma Bubbles: A Review

  • Electron density irregularities in the equatorial ionosphere at night are understood in terms of plasma bubbles, which are produced by the transport of low-density plasma from the bottomside of the F region to the topside. Equatorial plasma bubbles (EPBs) have been detected by various techniques on the ground and from space. One of the distinguishing characteristics of EPBs identified from long-term observations is the systematic seasonal and longitudinal variation of the EPB activity. Several hypotheses have been developed to explain the systematic EPB behavior, and now we have good knowledge about the key factors that determine the behavior. However, gaps in our understanding of the EPB climatology still remain primarily because we do not yet have the capability to observe seed perturbations and their growth simultaneously and globally. This paper reviews the occurrence climatology of EPBs identified from observations and the current understanding of its driving mechanisms.

A systemic review of literature in clinical practice research for nursing students

  • This study is an attempted content analysis study to analyze recent studies on clinical practice of nursing college students to identify problems with clinical practice and provide evidence for desirable clinical practice. For data collection, a total of 14 papers published in academic journals between 2017 and 2021 were selected. The analysis method was performed according to the systematic review reporting guidelines presented by Cochrane Collaboration and the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) group. As a result of the analysis, two themes, Four sub-themes, and seventeen concepts were derived, and the clinical practice study of nursing college students showed the necessity of standardized clinical practice protocol regulations for nursing college students and respect for patient human rights. Based on the above research results, it is proposed to prepare a nursing and clinical practice protocol that can clearly present the role of nursing college students during clinical practice.

  • Objective: The purpose of this systematic review and meta-analysis was to investigate potential effects of HRT (hormone replacement therapy) on motor functions in postmenopausal women. Method: In this meta-analysis, 19 studies that examined changes in motor functions between postmenopausal women with and without HRT intervention were qualified. We additionally conducted moderator variable analyses including: (1) motor function type, (2) hormone type, and (3) duration of HRT intervention. Results: The random effects model showed no significant overall effects (SMD = 0.199; SE = 0.115; 95% CI = -0.026~0.425; Z = 1.730; p = 0.084; I2 = 93.258%). Additional three moderator variable analyses revealed no significant effect sizes indicating that specific HRT protocols did not improve different motor functions in postmenopausal women. Conclusion: These meta-analytic findings suggest that HRT had no positive effects on motor functions in postmenopausal women.

Panax ginseng: An Overview of the Clinical Evidence

  • This overview is focused on Panax ginseng, one of the most commonly used herbal remedies. The currently available data regarding its clinical efficacy is somewhat confusing, despite the fact that many clinical trials and systematic reviews have been published. The risks of Panax ginseng seem to be only minor. Numerous mechanisms of action have been described. Future studies of this fascinating herbal remedy seem warranted but they should overcome the methodological limitations of the previous research.

A Systematic Review and Meta-Analysis of Randomized Controlled Trials on Chuna Manual Therapy for Cervicogenic Headache

  • Objectives: We conducted this study to evaluate the efficacy of Chuna Manual Therapy (CMT) for treatment of cervicogenic headache (CeH) through systematic review and Meta-analysis of randomized controlled trials (RCTs) as a preceding research to further research the effective of Chuna Manual Therapy for patients who suffered from CeH. Methods: We conducted a systematic review and meta-analysis by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. We searched the studies from MEDLINE, Elsevier-EMBASE, The Cochrane Library, CAJD, KISS, KMBase, Korean Traditional Knowledge Portal, NDSL, and OASIS. The studies selected only in randomized controlled trials. We selected the chosen studies by the selection and the exclusion criteria, and evaluated the quality of the selected studies using the Jadad score and the Cochran ROB tool. We used the Visual Analogue Scale score (VAS) and Clinical total Effective Rate (CER) for the results and analyzed the results of the included studies using RevMan 5.3 software provided by the Cochran library. Results: We included 20 RCTs, including 1,673 subjects, in the systematic review and meta-analysis. After the intervention, the CMT group showed better results than the pharmacotherapy group, the physiotherapy group, and the combined treatment group. The CMT group showed a good effect on the CER and the VAS but showed a significant heterogeneity compared to the pharmacotherapy group. Conclusions: The CMT as monotherapy might have benefits on Cervicogenic Headache patient. Further well-designed studies need to be conducted.

A Systematic Review and Meta-Analysis of Randomized Controlled Trials on Acupuncture for Improving Post-Operative Cognitive Dysfunction (POCD) and Chemotherapy-Related Cognitive Impairment (CRCI)

  • Objectives: This study was conducted to evaluate the efficacy of Korean Medicine Therapy (KMT) for the treatment of Chemotherapy-induced Cognitive Impairment (CICI) through systematic review and meta-analysis of randomized controlled trials (RCTs) as proceeding research to further research the efficacy of KMT for CICI patients. Methods: We conducted a systematic review and meta-analysis according to the Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines (PRISMA). The RCTs on the efficacy of KMT for treatment of CICI were searched by structured search strategies in MEDLINE, EMBASE, Cochrane library, CAJD, KISS, NDSL, KoreaMed, and OASIS. The searched RCTs were screened by inclusion and exclusion criteria. We evaluated the quality of the method in the included studies using the Jadad score and Cochran ROB tool. The efficacy outcomes were the Visual Analogue Scale (VAS) and the Clinical total Effective Rate (CER) of CICI. They were analyzed using mean difference for continuous variable or Relative Risk (RR) for Dichotomous variable in the random-effect model. The RevMan 5.3 program was used for meta-analysis. Results: We included 19 RCTs, including 653 participants, in the systematic review and meta-analysis. There were favorable results for the KMT group after the intervention compared with the pharmacotherapy group, physiotherapy group, and combined treatment group. KMT group showed improvement using CER and VAS compared with exercises, but their heterogeneities were slightly significant. KMT was more effective compared to the Rehabilitation program in CER and the subgroup analysis results showed that KMT had a significant difference compared to other therapies in VAS, to Medication therapy in CER. Conclusions: KMT presented reasonable evidence on improving the Clinical total Effective Rate and Visual Analogue Scale in CICI patients. However, further evaluation in future research is required.

Allied Health Professionals and Work-Related Musculoskeletal Disorders: A Systematic Review

  • Work-related musculoskeletal injuries and disorders (WMSD) are a significant issue in the health care sector. Allied Health professionals (AHP) in this sector are exposed to physical and psychosocial factors associated with increased risk of developing a WMSD. Clarification of relevant hazard and risk factors for AHP is needed to improve understanding and inform WMSD risk management. A systematic analysis of the literature was undertaken to determine prevalence and risk factors for WMSD in AHP. Databases of Ovid MEDLINE, CINAHL (EBSCO), EMBASE and the Cochrane Database of Systematic Reviews were reviewed. This quality of articles was low. Outcome measures were varied, with prevalence rates of WMSD reported from 28% to 96% over a one-year time period. The lower back was the most commonly affected body part. Relevant factors identified with the development of WMSD included inexperience in the role and area of employment. Future research needs to focus on undertaking high quality prospective studies to determine the factors associated with WMSD development in AHP.
