Oral manifestations and their correlation to baseline CD4 count of HIV/AIDS patients in Ghana

  • Objectives: Acquired immunodeficiency syndrome (AIDS) is a disease of the human immune system caused by the human immunodeficiency virus (HIV). People with AIDS are much more vulnerable to infections, including opportunistic infections and tumors, than people with a healthy immune system. The objective of this study was to correlate oral lesions associated with HIV/AIDS and immunosuppression levels by measuring clusters of differentiation 4 (CD4) cell counts among patients living in the middle western regions of Ghana. Materials and Methods: A total of 120 patients who visited the HIV clinic at the Komfo Anokye Teaching Hospital and the Regional Hospital Sunyani of Ghana were consecutively enrolled in this prospective and cross-sectional study. Referred patients' baseline CD4 counts were obtained from medical records and each patient received an initial physician assessment. Intraoral diagnoses were based on the classification and diagnostic criteria of the EEC Clearinghouse, 1993. After the initial assessment, extra- and intraoral tissues from each enrolled patient were examined. Data analyses were carried out using simple proportions, frequencies and chi-square tests of significance. Results: Our study included 120 patients, and was comprised of 42 (35.0%) males and 78 (65.0%) females, ranging in age from 21 to 67 years with sex-specific mean ages of 39.31 years (males) and 39.28 years (females). Patient CD4 count values ranged from 3 to 985 cells/mL with a mean baseline CD4 count of 291.29 cells/mL for males and 325.92 cells/mL for females. The mean baseline CD4 count for the entire sample was 313.80 cells/mL. Of the 120 patients we examined, 99 (82.5%) were observed to have at least one HIV-associated intraoral lesion while 21 (17.5%) had no intraoral lesions. Oral candidiasis, periodontitis, melanotic hyperpigmentation, gingivitis and xerostomia were the most common oral lesions. Conclusion: From a total of nine oral lesions, six lesions that included oral candidiasis, periodontitis, melanotic hyperpigmentation, gingivitis, xerostomia and oral hairy leukoplakia were significantly correlated with declining CD4 counts.

Detection of Y Mosaicism in Blood and Gonad of Patients with Gonadal Dysgenesis (성선 이형성 환자 혈액 및 성선 조직에서 Y 염색체 모자이시즘의 진단)

  • Objective: The presence of Y chromosome in patients with gonadal dysgenesis is related to the risk of gonadoblastoma. Since the patients with abnormal sexual differentiation may have cryptic Y mosaicism, it is important to detect the presence of Y material in these patients. But sometimes it is difficult to detect Y material only with karyotyping. This study was performed to evaluate the usefulness of the SRY gene screening in blood and gonad by using PCR in detecting the presence of Y material and possible tissue mosaicism in patients with gonadal dysgenesis as Turner syndrome and 46,XY pure gonadal dysgenesis (PGD, Swyer syndrome). Method: In 26 patients with gonadal dysgenesis, we screened for Y material by using PCR for SRY gene in peripheral leukocytes and in gonadal tissues of some patients. They were 22 cases of Turner syndrome (7 45,XO, 2 46,Xi(Xq), 3 45,XO/46,XX, 5 45,XO/46,Xi(Xq), 1 45, XO/46,XY, 1 45,XO/46,Xi(Yq), 1 45,XO/47,XYY, 1 46,XX,del(X)(q24) and 1 46,X,+mar) and 4 cases of 46,XY pure gonadal dysgenesis. PCR for SRY gene in the gonadal tissue was performed in 5 Turner syndrome and 2 PGD to determine the cryptic Y mosaicism between blood and gonad. Results: By using PCR analysis for SRY, Y chromosome material was detected in the blood of 4 of 22 Turner syndrome patients (45,XO/46,Xi(Xq), 45,XO/46,Xi(Yq), 45,XO/46,XY, and 45, XO/47,XYY), 3 of 4 46,XY pure gonadal dysgenesis. Discrepancy between karyotyping and blood PCR for SRY was noted in 1 Turner syndrome (45,XO/46,Xi(Xq)) and 1 PGD. Laparoscopic gonadectomy was performed in Y containing or SRY positive cases. In addition, PCR analysis for SRY in the gonads of 5 Turner syndrome and 2 PGD showed discrepancy between blood and gonad or between both gonads in 3 Turner syndrome (45,XO/46,Xi(Xq), 45,XO/46,Xi(Y q), 45,XO/46,XY) and 2 PGD patients. Conclusion: In gonadal dysgenesis, PCR analysis for SRY gene is useful to detect the cryptic Y mosaicism that is sometimes undetected by karyotyping. And since there may be tissue mosaicism, it is necessary to evaluate Y mosaicism in various tissues even in the case without Y chromosome on karyotyping.

Lipid accumulation mediated by adiponectin in C2C12 myogenesis

  • Plasma concentrations of adiponectin have been shown to be decreased in patients with obesity, cardiovascular diseases, hypertension and metabolic syndrome. Recent studies have found that adiponectin reduces lipid accumulation in macrophage foam cells which may impact the development of atherosclerosis. However, it remains unclear whether adiponectin is involved in the process of lipid accumulation during myogenesis. Using C2C12 myoblasts, we investigated the effect of adiponectin on intramyocellular lipid accumulation during myogenesis. The results showed that intracellular lipid accumulation is significantly decreased during C2C12 differentiation, apparently due to increased fatty acid oxidation and decreased fatty acid synthesis during this process. C2C12 cells transiently transfected with adiponectin gene showed reduced lipid accumulation as compared to controls. Further experiments demonstrated that adiponectin can suppress lipid accumulation by increasing fatty acid oxidation during C2C12 myogenesis.

  • Objectives : The purpose of this study is to investigate the method of setting up logic and estimate Fang Youzhi's work through Shanghanluntiaobian. Methods : Wang Shuhe's old edition Shanghanlun and Fang Youzhi's Shanghanluntiaobian were compared and several essential provisions were analyzed and addressed the positive and negative aspects of Fang Youzhi's revising old edition Shanghanlun mixed bamboo tablets. Results : Fang Youzhi's changing Bianmaifa("辨脈法") and Pingmaifa("平脈法") and placement Bianchishiyemaizhengbingzhi chapter after Liujingbingmaizhengbingzhi for discriminating convulsion dampness summer heat stroke from febrile diseases is appropriate. Fang Youzhi's deletion Shanghanli("傷寒例") chapter that has a guiding characteristics shows his editorial policy. Fang Youzhi's addition Bianwenbingfengwenzabingmaizhengbingzhi chapter is for discriminating febrile disease from epidemic febrile disease wind warmth syndrome miscellaneous disease and it indicates Shanghanlun is a book of treatment after differentiation of syndromes containing epidemic febrile disease and miscellaneous disease. Through Fang Youzhi's revising method I can see several positive aspects. First he tried to refine the logic of the content of Shanghanlun and accord with an actual situation. Second he shows Shanghanlun is a book of treatment after differentiation of syndromes containing epidemic febrile disease and miscellaneous disease and tried to see Shanghanlun clearly. A criticism of the Preserving Old Edition Shanghanlun group could interrupt flexibility and creative thinking reading Shanghanlun and a criticism to the Sangangdingli Principles of the several exegetists is a productive criticism because medicine treats human being so it cannot be completed by a logic. Conclusions : All of these processes are Fang Youzhi's endeavour for strengthening the system and logicality of the old edition Shanghanlun and it is meaningful to show a method to read Shanghanlun to beginners.

  • The results of analyzing "Shanghanlun-Shanghan examples" of Song Dynasty Text and Liuwansu's writings in the pursuit of comprehending his views written in "Fire-Heat" & "Warm disease" -are as follow. 1.He introduced Exterior-Interior Shanghan and Latent in the Winter & recurrent in the Spring Shanghan concept in the Febrile disease pathology as the link and the bifurcation point. His perception on Febrile disease encompassed Shanghan and Febrile disease on the same plane but actually discriminated in the treatment and medication between the two actually. 2.He suggested Febrile disease pathology in the concept of Shanghan and Febrile disease, which is coherent in the formation of interior heat as the result of the invasion of the lung by warm pathogen described in Warm disease Wei, Qi, Ying and Xue Fen Syndrome differentiation. He expanded the concept of esoteric portal 玄府 of "Suwon Tiaojinglun", to that of the omni-present portal & way of Qi's all directions movement, which is in accordance of invasion of upper energizer 上焦 by warm or hot pathogen via mouth and nose as used in Warm disease three-energizer differentiation. 3.He recognized both exterior-interior febrile disease in the context of both exterior-interior contact cause disease. He prescribed pungent-cool exterior releasing medicinal similar to that of exterior-releasing treatment adopted in warm disease. As lily-talc powder百合散 introduced in "Golden chamber synopsis" dispells heat by promoting excretion of urine and defecation, it is regarded as the original formula of later ikwonsan yiyuansan 益元散.

Urological Evaluation of Tethered Cord Syndrome

  • To describe how to perform urological evaluation in children with tethered cord syndrome (TCS). Although a common manifestation of TCS is the development of neurogenic bladder in developing children, neurosurgeons often face difficulty in detecting urological problems in patients with TCS. From a urological perspective, diagnosis of TCS in developing children is further complicated due to the differentiation between neurogenic bladder dysfunctions and transient bladder dysfunctions owing to developmental problems. Due to the paucity of evidence regarding evaluation prior to and after untethering, I have shown the purpose and tools for evaluation in my own practice. This may be tailored to the types of neurogenic bladder, developmental status, and risks for deterioration. While the urodynamic study (UDS) is the gold standard test for understanding bladder function, it is not a panacea in revealing the nature of bladder dysfunction. In addition, clinicians should consider the influence of developmental processes on bladder function. Before untethering, UDS should reveal synergic urethral movement, which indicates an intact sacral reflex and lack of TCS. Postoperatively, the measurement of post-void residual urine volume is a key factor for the evaluation of spontaneous voiders. In case of elevation, fecal impaction, which is common in spinal dysraphism, should be addressed. In patients with clean intermittent catheterization, the frequency-volume chart should be monitored to assess the storage function of the bladder. Toilet training is an important sign of maturation, and its achievement should be monitored. Signs of bladder deterioration should be acknowledged, and follow-up schedule should be tailored to prevent upper urinary tract damage and also to determine an adequate timing for intervention. Neurosurgeons should be aware of urological problems related to TCS as well as urologists. Cooperation and regular discussion between the two disciplines could enhance the quality of patient care. Accumulation of experience will improve follow-up strategies.

Study on Validity of Using Injinho-tang in Non Alcohoic Fatty Liver Disease (비알콜성 지방간의 약물개발 중 인진호탕(茵蔯蒿湯)의 타당성 연구)

  • Now a days, number of non alcoholic fatty liver patients are increasing more rapidly compare to past rate, and the average age of patients is getting younger, but there are no appropriate therapeutics in non alcoholic fatty liver disease. This study was aimed to analyze relationship between non alcoholic fatty liver disease and Injinho-tang. The papers were collected and analysed from domestic and international journals. The effects of Injinho-tang and constituent-herb were researched. Non-alcoholic fatty liver disease was induced complex causes of the metabolic syndrome. Medications that can be used in non-alcoholic fatty liver disease, it should be have many effects such as anti-hepatic fibrosis, hepatocyte protection, liver cancer inhibitory effect, inflammatory cytokine regulation, improving hyperlipidemia, weight control, decrease the toxicity of the drug, antioxidant. Injinho-tang (Artemisia capillaris Thunb, Gardenia fructus, Rhei rhizome) has been widely used in disease that causes jaundice and liver biliary disease. Drugs for standardization of Injinho-tang index components(6,7-Dimethylesculetin, geniposide, rhein) have been presented. And Injinho-tang has been proven reliability in the administration of single dose toxicity. Also clinical stability in the administration of four years was reported. Injinho-tang has been reported some effects which anti-hepatic fibrosis, hepatocyte protection, liver cancer inhibitor, inflammatory cytokine regulation, improving hyperlipidemia, weight control, decrease the toxicity of the drug, and antioxidant. Therefore, Injinho-tang can be used in Non alcoholic fatty liver disease without Syndrome Differentiation.

  • mTOR, the mammalian target of rapamycin, is a conserved serine/threonine kinase which belongs to the phosphatidyl-linositol kinase-related kinase (PIKK) family. It has two complexes called mTORC1 and mTORC2. It is well established that mTOR plays important roles in cell growth, proliferation and differentiation. Over-activation of the mTOR pathway is considered to have a relationship with the development of many types of diseases, including polycystic ovary syndrome (PCOS) and ovarian cancer (OC). mTOR pathway inhibitors, such as rapamycin and its derivatives, can directly or indirectly treat or relieve the symptoms of patients suffering from PCOS or OC. Moreover, mTOR inhibitors in combination with other chemical-molecular agents may have extraordinary efficacy. This paper will discuss links between mTOR signaling and PCOS and OC, and explore the mechanisms of mTOR inhibitors in treating these two diseases, with conclusions regarding the most effective therapeutic approaches.

Multi-resistance strategy for viral diseases and in vitro short hairpin RNA verification method in pigs

  • Objective: Foot and mouth disease (FMD) and porcine reproductive and respiratory syndrome (PRRS) are major diseases that interrupt porcine production. Because they are viral diseases, vaccinations are of only limited effectiveness in preventing outbreaks. To establish an alternative multi-resistant strategy against FMD virus (FMDV) and PRRS virus (PRRSV), the present study introduced two genetic modification techniques to porcine cells. Methods: First, cluster of differentiation 163 (CD163), the PRRSV viral receptor, was edited with the clustered regularly interspaced short palindromic repeats-CRISPR-associated protein 9 technique. The CD163 gene sequences of edited cells and control cells differed. Second, short hairpin RNA (shRNAs) were integrated into the cells. The shRNAs, targeting the 3D gene of FMDV and the open reading frame 7 (ORF7) gene of PRRSV, were transferred into fibroblasts. We also developed an in vitro shRNA verification method with a target gene expression vector. Results: shRNA activity was confirmed in vitro with vectors that expressed the 3D and ORF7 genes in the cells. Cells containing shRNAs showed lower transcript levels than cells with only the expression vectors. The shRNAs were integrated into CD163-edited cells to combine the two techniques, and the viral genes were suppressed in these cells. Conclusion: We established a multi-resistant strategy against viral diseases and an in vitro shRNA verification method.

Different Level of Plasma Free Hemoglobin between Qi-deficiency and Fire Heat among Korean Stoke Subjects (한국인 중풍환자의 기허군 화열군의 plasma free hemoglobin의 비교)

  • The purpose of this study was to fine proteins, which have significantly different level in plasma between Qi-deficiency and Fire-heat group of Korean Oriental Stroke pattern identification (PI) among Korean stroke patients. Eighteen stroke patients with Qi-deficiency and forty nine patients with Fire-heat, which had critical syndrome of each PI, were participated in this study. Plasma protein pattern were analyzed by SELDI-TOF MS using Q10 strong anion exchange chip and Mass spectral data (m/z) statistically determined. The expression level of proteins, which were different between Qi-deficiency and Fire-heat in the results by SELDI-TOF MS, were confirmed by western blot. As a result of analyzing plasma protein by SELDI-TOF MS, six protein peaks were significantly higher in Fire-heat group than Qi-deficiency group. Two peaks among of them, M15003 and M15745, were respectively identified as hemoglobin alpha and beta in previous study. Expression level of plasma free hemoglobin of Fire-heat group was also confirmed higher in Fire-heat group than in Qi-deficiency group. These findings suggest that plasma free hemoglobin is a candidate for discriminating Qi-deficiency and Fire-heat group according to pattern identification (PI) of stroke.
