The Clinical Study on 1 Case of Hyperthyroid Patient (갑상선기능항진증 환자 1례에 관한 증례보고)

  • Objective : This study was to investigate diagnosis and treatment based on an overall analysis of signs and symptoms on a hyperthyroid patient. Methods : The observation of the clinical progress was carried out by conducting diagnosis and treatment based on an overall analysis of signs and symptoms with the patient diagnosed hyperthyroidism. Results : Treatments such as the invigoration of qi (益氣) nourishing m (養陰) was given because the patient showed the qi deficiency (氣虛) and m deficiency (陰虛). Conclusion : Diagnosis and treatment based on an overall analysis of signs and symptoms of a hyperthyroid patient had moderate effects on the improvement of the patients condition.

  • Objectives: In order to study the effect of the oriental medical therapy on the patients who visited Neuropsychiatry and oriental medical differentiation of symptoms and signs with ABR (Autonomic Bioelectric Response recorder)-2000, these clinical studies were performed. Methods: Sixty six patients with headache were classified into three groups; the first is tension headache, the second is migraine headache, the last is the others, each group was investigated of a various characteristic, the effect of the oriental medical therapy and stress was measured by ABR-2000. Result: 1. According to the statics, the tention headache's rate was higher than migraine; on the whole woman's rate was higher than man's. however in the migraine the rate of sex was equal. 2. According to the cause facter, oversensitiveness, overexertion, dyspepsia, etc. were numorous in the order. 3. According to the oriental medical differentiation of symptoms and signs, the rate of stagnation of the liver-qi' s fire-transmission(肝鬱化火) and deficiency of qi(氣虛), these two types were highest. result of analyzing ABR-2000 is that stagnation of the liver-qi's fire-transmission(肝鬱化火) is the highest stress. 4. According to treatment rate, 80.3 percent of patients satisfied of treatment.

Yew Poisoning in 17 Dairy Cattle (젖소 17두의 주목나무 독성 중독)

  • We found seventeen dairy cattle with the abnormal signs in cardiovascular and gastrointestinal systems after feeding of the yew foilage. Among them three cattle were dead due to yew poisoning. Among the remaining fourteen dairy cattle, four cattle showed similar symptoms as did dead cattle. Although the remaining ten dairy cattle did not show any abnormal signs, we conducted a therapy of forced magnesium sulfate infusion because the yew consumption might have been occurred in all cattle. As a result of the therapy, we could not found further signs of yew poisoning. The performance of the therapy and the treatment procedures adapted by the therapy could be concluded as follows: The cardioselective toxicity and inhibition of peristaltic activity by the taxine in yew foilage might be involved in the symptoms of acute poisoning as anorexia, dullness, muscle tremor, dyspnea, and sudden death. We also performed the dose response relationship of taxine to the range of clinical symptoms and examined recovery performances. Through the autopsy of the cattle, we could confirm the presence of yew foilages that might have caused the poisoning in the gastrointestinal tract. However, we could not identify further abnormalities in other organs. In this case report, we demonstrated that practice of the forced magnesium sulfate infusion in yew poisoning was helpful for the attenuating the taxine poisoning by blocking the further proceeding of the toxic effect.

Study of Relationship between the Chapter of Channels in Miraculous Pivot of Emperor′s Classic of Internal Medicine and Yangmyung disease in Sanghanron (『영추ㆍ경맥편』과 『상한론』의 양명병에 대한 상관성 연구)

  • Chang Chung-ching(張仲景) in the Later Han(Eastern Han) Dynasty of Chinese history wrote the treatise on Diseases Caused by Cold Factors(傷寒論; Shang Han Lun) on the basis of the fundamental theory of Emperor's Classic of Internal Medicine(黃帝內經; ECIM) after collecting medical treatment experiences until the Han Dynasty. It had great significance that Shang Han Lun was the origin of treating six-channels(六經) and there showed the peculiar guidelines on the diagnosis and treatment of oriental medicine to divide diseases into six-channels. The oriental medical doctors who had studied Shang Han Run thought highly of meridians and until now it was generally known that the chapter of heat in the Plain Questions of ECIM(黃帝內經, 素問ㆍ熱論) was the basis of Shang Han Run. The chapter of heat in the Plain Questions of ECIM was the first text in which the basic theory on six-channels according to the types of illness was introduced. In my point of view, the theory of treating six-channels had close relation to the Chapter of Channels in Miraculous Pivot of ECIM(黃帝內經, 靈樞ㆍ經脈篇) as well as the chapter of heat in the Plain Questions of ECIM. Therefore I took a look at the origin of treating six-channels in Shang Han Lun and illuminated again the meaning to compare the parts of in Shang Han Lun with the Chapter of Channels in Miraculous Pivot of ECIM. Conclusion: The, symptoms divided into six-channels in the chapter of channels in ECIM gave the fundamental basis of diagnosis and treatment basesd on overall analysis of signs and symptoms(辨證論治) an illness in the Zangfu(臟腑) in respect of meridians. Viewed in the light of diagnosis and treatment basesd on overall analysis of signs and symptoms(辨證論治), the symptoms of YangMing-channel(陽明經) in the Chapter of Channels in Miraculous Pivot of ECIM were, for the most part, accord with those of YangMing-disease in Shang Han Lun. Furthermore, the symptoms in Shang Han Lun were explained definitely and in detail. Therefore the theory of Shang Han Lun has been developed on the basis of ECIM with the changes of the times. YangMing-disease in Shang Han Lun implied medical cases in stomach meridian(胃經) and large intestine meridian(大腸經). Therefore Shang Han Lun was the foundation of treatment basesd on overall analysis of signs and symptoms(辨證論治) in respect of meridian as well as the text in which the steps of infectious diseases(外感病) of human bodies were explained.

Study of Deficiency of Qi Pattern Identification Diagnosis Criteria in Stroke (중풍환자의 기허변증 진단 기준에 관한 연구)

  • To report Sensitivity and specificity about utility as diagnosis criteria for deficiency of Qi in stroke. Korean medicine doctor surveyed deficiency of Qi of the symptoms for the Stroke case report form in stroke patients within 1 month of onset. We analyzed 643 patients have diagnosed stroke, neurological deficit continued over twenty-four hours and within one month of onset, except traumatic cerebrovascular attack (EDH, SDH) using the result by medical specialist and residents diagnosed differentiation and written CRF(Case Report Forms) which based on 'Korean Standard Differentiation of the Symptoms and Signs II' in twenty multi centers. The sensitivity of "more 1/5 in major sings and 1/5 in helpful signs", "more 1/5 in major signs and 2/5 in helpful signs", "more 2/5 in major signs and 1/5 in helpful signs", "more 2/5 in major signs and 2/5 in helpful signs""more 3/5 in major signs and 1/5 in helpful signs""more 3/5 in major signs and 2/5 in helpful signs" are respectively 83%, 50%, 72%, 46%, 47%, 32%. The specificity are respectively 28%, 59%, 55%, 74%, 80%, 89%. The sensitivity(72%) and specificity(55%) of "more 2/5 in major signs and 1/5 in helpful signs" that to be implanted. Although this values are not high, after values of sensitivity and specificity should be more than current value, and then we should be able to suggest as objective diagnosing criteria.

  • Objective : Stroke is the most common disabling neurological disease of adult life. And some part of the clinical symptoms and signs are correlated to the post stroke complication. This study was to analyze the therapeutic effect of the East-West collaborative treatment, especially on the motor power and the clinical symptoms and signs improvement of ischemic stroke patients. Method : For 2 weeks as possible, we observed the patients with acute ischemic stroke receiving East-West collaborative treatment. We assessed the patients using Motricity Index (MI) and the changes of clinical symptoms and signs three times at seven-day intervals. At the end of the study, we compared the results. Result : Motricity Index of patients changed from 71.55 to 76.03 (upper limb) and 73.05 to 79.42 (lower limb), respectively. Some items of clinical symptoms and signs showed the statistically significant result. Especially, the urination and defecation symptoms improved obviously. Conclusion : The East-West collaborative treatment has an effect on motor power and clinical symptoms and signs improvement of acute ischemic stroke patients.

Oral Cancer Awareness and its Determinants among a Selected Malaysian Population

  • Objective: To assess oral cancer awareness, its associated factors and related sources of information among a selected group of Malaysians. Methods: A cross-sectional survey was conducted on all Malaysian ethnic groups aged ${\geq}15$ years old at eight strategically chosen shopping malls within a two week time period. Data were analysed using chi-square tests and multiple logistic regression. Significance level was set at ${\alpha}$ <0.05. Results: Most (84.2%) respondents had heard of oral cancer. Smoking was the most (92.4%) recognized high risk habit. Similar levels of awareness were seen for unhealed ulcers (57.3%) and red/white patches (58.0%) as signs of oral cancer. Age, gender, ethnicity, marital status, education, occupation and income were significantly associated with oral cancer awareness (p<0.05). Conclusions: There was a general lack of awareness regarding the risk habits, early signs and symptoms, and the benefits of detecting this disease at an early stage. Mass media and health campaigns were the main sources of information about oral cancer. In our Malaysian population, gender and age were significantly associated with the awareness of early signs and symptoms and prevention of oral cancer, respectively.

Relationship Between Neurologic Soft Signs and Some Clinical Variables in Patients with Schizophrenia (정신분열증환자의 연성 신경학적 증상과 임상변인과의 관련성)

  • This study was performed to know the relationship between neurologic soft signs (NSS) and clinical variables such as psychopathology. history of illness, and premorbid social adjustment in patients with schizophrenia. The authors evaluated NSS in 31 patients with schizophrenia using the structured tool for measuring neurologic abnormalities, Neurological Evaluation Scale- Korean Version(NES-K). Relationships between NSS and clinical variables such as duration of illness, intensity of precipitating stressors, duration of outpatient treatment, schooling, peer relationship, total duration of unemployment, total days of psychiatric admission, age, total days of being medicated, age at the first psychiatric admission, frequency of admissions, content of treatment, social adjustment, and severity of symptoms were analyzed. Differences between paranoid and non-paranoid schizophrenics were examined. In addition, Differences between patients with schizophrenia who have predominant positive symptoms and who have predominant negative symptoms were examined too. Total scores of NES-K were correlated with lower schooling (${\gamma}$=0.44, p<0.01). Scores of motor coordination subcategory were correlated with poor peer relationship(${\gamma}$=0.67, p<0.001). Other clinical variables were not correlated with any scores of NES-K. Paranoid and non-paranoid schizophrenics were not different in scores of NES-K. Also positive and negative schizophrenics were not different in scores of NES-K. Most clinical variables except schooling and peer relationship were not related with NSS. This results indicated that the meaning of these signs was not fully be understood. Introduction of the new classification concepts such as deficit or non-deficit syndrome will be helpful to elucidate the meaning of NSS in patients with schizophrenia.

An analysis of the causes of prehospital delays in patients with suspected acute stroke (급성 뇌졸중 의심 환자의 병원 전 지연 원인 분석)

  • Purpose: Stroke is a time-sensitive disease that could have reduced complications and mortality with timely diagnosis and treatment. This study aimed to analyze the causes of delay in detecting the clinical signs and symptoms of stroke. Methods: This retrospective observational study analyzed the emergency medical services reports of suspected stroke patients with positive predictive values on the Cincinnati Prehospital Stroke Scale. The study was conducted in Daejeon, Republic of Korea from January 1, 2016 through December 31, 2017. Results: Prolonged prehospital time was associated with high blood pressure, history of cerebrovascular disease, and incidences during daily activities, and sleep. High blood pressure and complications from a previous stroke strongly associated with the prolonged stroke-detection phase (p<.05). Total prehospital time was shortened when patients had evident stroke symptoms, such as decreased level of consciousness, dysarthria, and hemiplegia (p<.05). There was no significant difference in gender or age as a factor that delayed the total prehospital time of the suspected stroke patients. Conclusion: Many patients did not recognize the early clinical symptoms and signs of a stroke. Furthermore, risk factors, such as high blood pressure and history of stroke, prolonged the total prehospital time. Therefore, we need targeted interventions that educate about warning symptoms of stroke, along with emphasis on the importance of emergency calls to substantially reduce the prehospital delays.

Glomus Tumors of the Fingers -A Report of 66 Cases - (수지의 사구체 종양 - 66예 증례 분석 -)

  • Purpose: We reviewed 66 cases of the glomus tumors in the hands. The purpose of this study was to review the characteristic signs and symptoms of glomus tumors and the surgical outcomes. We made hypothesis that (1) our guideline for surgical excision are reliable and (2) our surgical technique is efficient and less harmful to the nail root. Materials and Methods: The clinical diagnosis was made with occasional radiological studies and surgical excision was performed according to our indications. Patients with electrical shock-like pain by touching (the essential symptom) and at least 2 of 4 clinical signs and symptoms (cold hypersensitivity, paroxysmal pain that radiated proximally, blue discoloration, nail deformity for dorsal tumor or a palpable nodule for pulp tumor) underwent surgical excision. Results: The duration of symptoms of the patients was about 5.3 years. No difference of prevalence was found in both hands, but the thumb and the middle finger were more affected than the other fingers. The mean follow up period was 2.4 years and none of the patients complained the same characteristic pain at the last visit. Conclusion: To the best of our knowledge, this report is the largest case series about glomus tumors in the finger tip. The clinical signs and symptoms are important to make a diagnosis of glomus tumors, and surgical excision is the treatment of choice.
