소양인체질병증 임상진료지침: 소양상풍병 (Clinical Practice Guideline for Soyangin Disease of Sasang Constitutional Medicine: Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology)

  • Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Soyangin Disease of Sasang Constitutional Medicine(SCM): Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology. This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods First, it was performed that search and collection of literature related SCM such as "Dongeuisusebowon", Textbook of SCM, Clinical Guidebook of SCM and Fundamental research to standardize diagnosis of Sasang Constitutional Medicine. And journal search related to clinical trial or Human complementary medicine of SCM was performed domestic and overseas. Finally, 1 articles were selected and included in CPG for Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology of Spleen Cold-based Exterior Cold (Bisuhan-pyohan) disease in Soyangin Disease. Results & Conclusions The CPG of Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology in Soyangin Disease include classification, definition and standard symptoms of each pattern. Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology is classified into mild and moderate pattern by severity. Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology mild pattern is classified into initial pattern and advanced pattern. Lesser-Yang Wind-Injury (Soyang-sangpung) symptomatology moderate pattern is classified into Chest-binding (Gyeolhyoong) pattern and Chest-binding (Gyeolhyoong) advanced pattern.

Results & Conclusions CPG of Chest-Heat congested(Hyunggyeok-yeol) symptomatology in Soyangin disease includes classification, definition and standard symptoms of each pattern. Chest-Heat congested(Hyunggyeok-yeol) symptomatology is classified into mild and moderate pattern by severity. Chest-Heat(Hyunggyeok-yeol) symptomatology Mild pattern is classified into Chest-Heat congested(Hyunggyeok-yeol) initial pattern and Chest-Heat congested(Hyunggyeok-yeol) advanced pattern. And Chest-Heat congested (Hyunggyeok-yeol) moderate pattern is classified into Clear Yang Failure of Stomach(Weguck-cheongyang Bulsagnseung) pattern (Upper wasting-thirst(Sangso) pattern), Clear Yang Failure of Large Intestine (Daejang-cheongyang Bulsangseung) pattern (Middle wasting-thirst (Jungso) pattern).

  • Objectives This research was carried out to establish the clinical practice guideline(CPG) for Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology of Soyangin disease. Methods Dongeuisusebowon(sinchuk edition) and several kinds of literatures including journal articles concerning this symptomatology of Soyangin disease were collected and classified. Sasang constitutional specialists' conference was held to make an agreement on the conflicting issues as well. Consensus was drawn as a result of the conference. Results & Conclusions 3 papers were selected as an inclusion and exclusion criteria for the relevant articles to Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology of Soyangin disease. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology consists of two aspects : Yin-Deficit Diurnal-Heat (Eumheo-oyeol) severe pattern and Yin-Deficit Diurnal-Heat (Eumheo-oyeol) critical pattern. In Yin-Deficit Diurnal-Heat (Eumheo-oyeol) severe pattern contains 1 disease, namely, Clear Yang Depletion of Large Intestine (Daejang-cheongyang Moson) pattern (Lower wasting-thirst (Haso) pattern). In Yin-Deficit Diurnal-Heat (Eumheo-oyeol) critical pattern contains 2 diseases, Yin-Deficit Diurnal-Heat (Eumheo-oyeol) pattern and Yin-Deficit Diurnal-Heat (Eumheo-oyeol) advanced pattern. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology has several kinds of symptoms like dry mouth, disliking to drink much water, diurnal body fever, coldness on the back and nausea as well as body fever, chest discomfort, constipation or dry stool as a common symptoms of Interior Heat disease. Clear Yang Depletion of Large Intestine (Daejang-cheongyang Moson) pattern (Lower wasting-thirst (Haso) pattern) has above mentioned symptoms and much urine/turbid urine, thin thigh and knee joints and twinge of joint pain over the body. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) pattern has symptoms like indigestion and epigastric discomfort, abdominal pain and vomiting in addition. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) advanced pattern has symptoms like hematemesis as well.

소음인(少陰人) 태음병(太陰病) 심하비증(心下痞證)의 병증약리(病證藥理) 고찰 (A Study on the Pathology and Therapeutics of Epigastric Discomfort in the Soeumin Taeeum Symptomatology)

  • 1. Objective: Gastric Stuffiness (心下痞證) in the Soeumin Taeeum symptomatology (少陰人 太陰病) in Sasang Constitutional Medicine shows an interesting evolutionary process. Originally found in the texts of Sanghanlun (傷寒論), the perspectives regarding the symptoms of Gastric Stuffiness were expanded by many medical scholars and Lee Jema. To comprehend the Jema's concept of Taeeum symptomatology and utilize his prescription clinically, it deserves to study on the Gastric Stuffiness in Donguisuseowon (東醫壽世保元). 2. Methods: Many texts including Sanghanlun (傷寒論) and Dongeuibogam (東醫寶鑑) were reviewed in order to track the evolutionary process concerning the symptoms of Gastric Stuffiness. Also, Lee Jema's works including the different versions of Donguisuseowon (東醫壽世保元) were reviewed to examine the development of Lee's concepts regarding the Gastric Stuffiness in the Soeumin Taeeum symptomatology. 3. Results and Conclusions: 1) Most of the previous concepts on Gastric Stuffiness were generally similar to those found in Sanghanlun, especially in that they addressed Yang deficiency of the middle triple energizeer as the central pathogenic process. 2) Lee Jema's views on the epigastric discomfort in the Soeumin Taeeum symptomatology evoloved over time and culminated in the finalized pathology and therapeutics appearing in his last work, the Shinchuk version of Donguisuseowon (東醫壽世保元). 3) 4 kinds of prescription which invented by Lee Jema to treat the Gastric Stuffiness in the Soeumin Taeeum symptomatology can be matched to the prescription of Sanghanlun respectively. However through evolutionary process of Jema's prescriptions, the indication of them expanded: not only to treat Gastric Stuffiness but to diverse symptoms of Soeumin.

  • Objectives We surveyed clinical status of Sasang Constitutional Medicine (SCM) for preparation of adaptation of Clinical Practice Guidelines for Sasang Constitutional Symptomatology. Methods We emailed Google survey form to Doctors of Korean Medicine registered the Association of Korean Medicine on 10/13/2020 and 10/21/2020 and closed survey 10/23/2020. We got a data of 654 cases, removed a case of error, analyzed data of 620 responses by frequency analysis Results and Conclusions Out of 620 respondents, 499 have answered that they refer to or make use of SCM. SCM is mainly applied in the field of treatments using Herbal Medicine. Doctors of Korean Medicine participated in survey of SCM, they think Diseases of digestive system are the most effectively treated by the methods of SCM. Sasang Constitutional Symptomatology most frequently observed are Soeumin Congestive Hyperpsychotic symptomatology, Soyangin Chest-Heat congested Symptomatology, Taeeumin Liver Heat-based Interior Heat disease. The diseases that are mainly applied by SCM are 'Diseases of digestive system' for Soeumin, 'Diseases of digestive system' for Soyangin, 'Diseases of respiratory system' for Taeeumin and 'not applicable' for Taeyangin. The important diagnostic methods of SCM are Inspection-listening and smelling examination-Inquiry-Palpation and survey. In the majority of cases, Prescription of Herbal medicine is used by adding or subtracting some herbal medicine from the original prescription suggested in the oriental medical classics and Saam acupuncture method is the most frequently used for the acupuncture therapy. Diet therapy is the most frequently used for the method of prevention and care.

친지에 의한 클라이언트의 우울증상 및 사회기능 측정의 유효성 연구 (Effectiveness of the Assessment of the Depressive Symptomatology and Social Functioning as Rated by the Client's Significant Other)

  • 다양한 형태의 심리사회적 개입이 우울증을 겪는 클라이언트에게 가져오는 변화는, 대략, 우울증상의 완화와 클라이언트 개인이 수행하던 사회기능의 증진이다. 우울증에 대한 기존 임상적 개입의 성과 연구에서 이러한 변화의 측정자로서 클라이언트에게 의미있는 타자인 가족이나 친척 혹은 친구(친지)의 역할은 간과되어 왔다. 본 연구는 우울 클라이언트의 일상생활의 다양한 측면을 자세히 관찰할 수 있는 친지의 위치에 초점을 두어 친지의 클라이언트에 대한 측정의 유효성을 고찰하였다. 미국에서 수행된 한 우울증 치료를 위한 심리사회적 개입연구의 자료를 사용하여 클라이언트의 친지가 치료개입 전 후 우울증상과 사회기능을 어떻게 보며, 그 측정이 치료 종결 후 6개월 및 1년이 경과된 시점(추적 기간)에서 클라이언트가 보이는 우울증상과 사회기능을 얼마나 설명하는가를 탐구하였다. 세 가지 주요 발견은 다음과 같다. 첫째, 일반적으로 치료 전 보다는 치료 후에 친지가 측정한 우울증상과 사회기능이 추적기간 중 클라이언트의 증상과 사회기능을 더 강하게 설명하였다. 둘째, 특히 친지가 측정한 사회기능의 네 척도 중 클라이언트의 실제적인 수행에 대한 측정과 친지가 기대하는 수행 정도의 측정이 추적기간 중 우울증상 및 사회기능을 유의하게 설명하였다. 셋째, 친지의 치료 전 측정 중 사회기능 측정은 증상 측정이 추적기간 중 사회기능의 변량을 설명하는 것 이상의 설명력을 추가하였다. 치료 후 측정에서는 친지의 증상 측정이 사회기능 측정이 추적기간 중 증상 및 사회기능의 변량을 설명하는 것 이상으로 설명력을 추가하였다. 이러한 발견은 정신건강전문가와 연구자들이 친지를 클라이언트의 치료에 참여하여 도움을 줄 수 있는 협력자로 여겨야 한다는 주장을 뒷받침할 수 있다. 연구 결과와 관련한 논의 및 함의를 고찰하였고 본 연구의 한계점 및 후속연구를 위한 제언을 하였다.

재가치매 환자의 미충족요구와 가족부양자의 돌봄경험 예측모형 (A Prediction Model for Unmet Needs of Elders with Dementia and Caregiving Experiences of Family Caregivers)

  • Purpose: The purposes of this study were to develop and test a prediction model for caregiving experiences including caregiving satisfaction and burden in dementia family caregivers. Methods: The stress process model and a two factor model were used as the conceptual frameworks. Secondary data analysis was done with 320 family caregivers who were selected from the Seoul Dementia Management Survey (2014) data set. In the hypothesis model, the exogenous variable was patient symptomatology which included cognitive impairment, behavioral problems, dependency in activity of daily living and in instrumental activity of daily living. Endogenous variables were caregiver's perception of dementia patient's unmet needs, caregiving satisfaction and caregiving burden. Data were analysed using SPSS/WINdows and AMOS program. Results: Caregiving burden was explained by patient symptomatology and caregiving satisfaction indicating significant direct effects and significant indirect effect from unmet needs. The proposed model explained 37.8% of the variance. Caregiving satisfaction was explained by patient symptomatology and unmet needs. Mediating effect of unmet needs was significant in the relationship between patient symptomatology and caregiving satisfaction. Conclusion: Results indicate that interventions focusing on relieving caregiving burden and enhancing caregiver satisfaction should be provided to caregivers with high levels of dementia patients' unmet needs and low level of caregiving satisfaction.

  • 1. Objectives The purpose of this case is to report the effects of Sibimijihwang-tang(十二味地黃湯) which is based on Sasang constitutional Medicine for Yin-Deficit Diurnal-Heat Symptomatology(陰虛午熱證) in Soyangin. 2. Methods We treated a Soyangin patients that had Fever Unknow Origin. We prescribed Sibimijihwang-tang(十二味地黃湯) for her physical symptoms. The improvement of her diseases was evaluated by her fever and other physical symptoms. 3. Results After the Sasang constitutional medication of Sibimijihwang-tang(十二味地黃湯) was given, the patient's fever and physical symptoms were improved. 4. Conclusions This case study show an efficient result of using Sibimijihwang-tang(十二味地黃湯) in the Yin-Deficit Diurnal-Heat Symptomatology(陰虛午熱證) of Soyangin.

  • Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Taeeumin and Taeyangin Disease of Sasang Constitutional Medicine(SCM): Diagnosis and Algorithm. This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods We searched the literature and articles related to Taeeumin and Taeyangin Symptomatology diagnosis and algorithm. For developing diagnosis and algorithm, we searched the classification, ordinary symptom, present symptom of the Taeeumin and Taeyangin Symptomatology Results and Conclusions We classified the Taeeumin Symptomatology by 4 steps: Exterior-Interior disease, favorable-unfavorable pattern, mild-moderate-severe-critical pattern, initial-intermediate-advanced pattern. And we classified the Taeyangin Symptomatology by 3 steps: Exterior-Interior disease, favorable-unfavorable pattern, mild-moderate-severe-critical pattern. At the unfavorable pattern, ordinary symptom is very important. So doctors are considered to need to focus on the symptom of unfavorable's ordinary symptoms.

소음인 태음증으로 진단한 소뇌성 운동실조 환자 1례 (A Case Study of Soeumin Greater Yin Symptomatology Patient Diagnosed as Cerebellar Ataxia)

  • Objectives The aim of this study was to examine significant improvement of gait ataxia, vertigo and tremor after treatment with Sipyimigwanjung-tang, Hyangsayangwi-tang in a Soeumin Greater Yin Symptomatology patient Diagnosed as Cerebellar ataxia. Methods The patient was diagnosed with Soeumin Greater Yin Symptomatology and treated with Soeumin's constitutional medications and acupuncture. The primary outcome measures for this study were the Unified Multiple System Atrophy Rating Scale(UMSARS) and Cerebellar function test to assess the overall function of patient. Secondary outcome assessment included Global Assessment Scale(GAS), change of patient's sleep, stool and digestion. Results The symptoms of gait ataxia, vertigo and tremor decreased from GAS 100 to GAS 0~40 after treatment, and the UMSARS score decreased in Part I, II. Conclusions This case showed that Sasang Constitutional medicine treatment can be effective treatment method for cerebellar ataxia. We consider that consistent treatment can contribute to improve the patient's quality of life.
