MPDS 환자의 임상적 양상 (A Clinical Study on the MPDS Patients)

  • The author has studied on the clinical features and symptomatology of 97 MPDS patients, who visited the Dept. of Dentistry, Kyungpook National University, from January to September in 1982. The results were as follow : In age and sex distribution of patients in this population, the third decades appeared to have the highest prevalence and the male to female ratio was almost 1 to 2.23. As to occupation, students and housewives were revealed to have the highest incidence of MPDS. The chief complaints of patients were pains, TMJ noises, and limitation of mouth opening in order of frequency, and the most prevalent site of symptom was that of preauricular area. In symptom sequence, the majority of initial symptoms were TMJ noise and pain but limitation of mandibular movement was progressively developed, regardless of nature of early symptoms. Chronic unfavorable oral habits were found to be the most possible predisposing factors in this study. The average maximum interincisal distance was $38.13\pm10.00mm$ in males and $30.73\pm8.75mm$ in females, and a deviation of mandible during mouth opening was observed in 60.8% of patients. In TMJ signs, tenderness of the TMJ to palpation was found in 60.8% of patients and TMJ noise was audible in 50.5% of patients with a stethoscope. The muscles of masticatory system were palpated according to usual methods, and a significant indidence of muscular tenderness was present, with the lateral pteygoid muscles being most frequently involved, followed by masseter, medical pterygoid and temporalis muscles.

Graft Transmission and Cytopathology of Pear Black Necrotic Leaf Spot (PBNLS) Disease

  • Graft transmission and cytopathological studies of a severe pear disease, pear black necrotic leafspot(PBNLS), were carried out to determine the causal agent of the disease. No evidence was found that a fungal or bacterial pathogen could be the causal agent of the disease. Attempts to transmit the agent by sap-inoculation to other plants including herbaceous hosts failed. How-ever, the pathogen was readily graft-transmitted from symptomatic diseased pears to healthy pears. Graft transmission of the pathogen was also demonstrated by using an indicator plant, PS-95, developed in the laboratory through various grafting methods. Ultrastructural study of the disease revealed the consistent presence of flexuous rod-shaped virus-like particles (VLP) in the symptomatic leaves of both Niitaka cultivar and indicator pear, PS-95. The particles, approximately 12 nm in diameter with undetermined length, occurred in the cytoplasm of mesophyll parenchyma cells. Cells with VLPs also contained fibril-containing vesicles, which are common in cells infected with plant viruses with ssRNA genome. The vesicles were formed at the tonoplast. Based on the symptomatology, the presence of fibril-containing vesicles, and graft-transmissibility, it is believed that the VLPs that occurred on symptomatic leaves of black necrotic leafspot of pear are viral in nature, possibly those of a capillovirus.

Occurrence and Epidemics of Bacterial Canker of Kiwifruit in Korea

  • Bacterial canker is the largest limiting factor in the cultivation and production of kiwifruit worldwide. Typical symptoms comprise necrotic spots on leaves, canker and dieback on canes and trunks, twig wilting, and blossom necrosis. Pseudomonas syringae pv. actinidiae (Psa), which is the causal agent of kiwifruit bacterial canker, is divided into four biovars based on multilocus sequence analysis of different genes, additional PCR testing of pathogenic genes (argKtox cluster, cfl, and various effector genes), and biochemical and physiological characterization. Bacterial canker caused by Psa biovar 2 designated Psa2 was detected for the first time on the green-fleshed kiwifruit cultivar Hayward in 1988 and the yellow-fleshed kiwifruit cultivar Hort16A in 2006 in Korea. Psa biovar 3 designated Psa3, responsible for the current global pandemics of kiwifruit bacterial canker, began to appear in Korea in 2011 and caused tremendous economic losses by destroying many vines or orchards of yellow-fleshed kiwifruit cultivars in one or several growing seasons. Bacterial canker epidemics caused by both Psa2 and Psa3 are prevalent in Korea in recent years. In this review, we summarize the symptomatology, etiology, disease cycle, diagnosis, and epidemiology of kiwifruit bacterial canker in Korea.

전신열감을 호소하는 소양인 여환 치험 3례 (A Clinical Case Report of Soyangin Female Patients with Flushing)

  • Objectives The aim of this study was to report improvements on the symptoms of Flushing for three soyangin female patients after using sasang constitutional medicines. Methods The patients were diagnosed with Soyangin Symptomatology and treated with Soyangin sasang constitutional medicines. The patient's subjective symptoms of flushing and discomfort in sleeping were observed using the Visual Analogue Scale(VAS) respectively during the treatment period. And the patient's subjective symptoms of the pain were observed using Numerical Rating Scale(NRS). The period of the patient's defecation interval were also observed. Results and Concluions The symptoms of flushing, discomfort in sleeping and the pain improved remarkedly in all respects. In conclusion, this study shows that Sasang Constitutional Medicine can be an effective treatment for Soyangin patient with Flushing.

과민성 방광을 동반한 쇼그렌 증후군 소양인 환자 치험 1례 (A Clinical Case Report of Soyangin Patient Diagnosed as Sjögren's Syndrome with Irritable Bladder)

  • Introduction This study aims to report a significant improvement of Sjögren's syndrome patient with irritable bladder, diagnosed as 'Soyangin chest-binding symptomatology' based on Sasang medicine. Methods The patient had treatment with 'Dojeokganggi-tang' and western medications. We evaluated the treatment outcome of Sjögren's syndrome symptoms by using a numerical score of 0 to 10, based on the patient's subjective discomfort of dry eye and dry mouth, and irritable bladder symptoms by recording the number of night urination every month. Results Sjögren's syndrome symptoms were significantly improved, and the number of night urination was decreased. After 41 weeks of treatment, the patient didn't have dry mouth any more and had only a little dry eye. The number of night urination was decreased from 3-4 times to once a night. Discussion The patient diagnosed with Sjögren's syndrome, treated with Dojeokganggi-tang and western medications for 41 weeks, showed a significant improvement in dry eye, dry mouth, and night urination compared to the first visit.

사상체질병증 진단도구 개발 연구 (A Study on the Development of Diagnostic Tools for Sasang Constitutional Patterns)

  • Objectives The purpose of this study was to develop diagnostic tools for Sasang Constitutional patterns based on the middle classification of the Sasang Constitutional patterns. Methods Diagnosis and assessment indicators of Soeumin, Soyangin, Taeeumin, and Taeyangin patterns were extracted based on the 『Clinical Practice Guideline for Sasang Constitutional Medicine Symptomatology』 and 『Donguisusebowon』. An online survey was conducted on the 'importance of diagnosis and assessment indicators' by the Sasang Constitutional Medicine expert group. Results Based on the expert consultation results, the importance weight for each diagnosis and assessment indicators symptom was calculated, and the importance was ranked to develop diagnostic tools for Soeumin, Soyangin, Taeeumin, and Taeyangin patterns. Diagnostic tool consisted of 58 questions for Soeumin patterns, 68 questions for Soyangin patterns, 81 questions for Taeeumin patterns, and 42 questions for Taeyangin patterns. The final total score was calculated by reflecting each response score and the weight of each question. Conclusions The developed 'Diagnostic Tools for Sasang Constitutional patterns' can be used to make an effective and objective diagnosis in the clinical site. In the future, if the reliability and validity of these diagnostic tools are tested through clinical study, it will be possible to improve clinical applicability and contribute to standardization of diagnosis.

두통, 현훈을 동반한 섬유근통 태음인 환자 치험 1례 (A Clinical Case Report of Taeeumin Patient Diagnosed as Fibromyalgia with Headache and Dizziness)

  • Objectives This study aimed to report significant improvement of fibromyalgia patient who was diagnosed with 'Taeeumin esophagus-cold lung-dry symptomatology' based on Sasang Constitutional Medicine. Methods The patient was treated with herbal medicine. To evaluate the results of this treatment, the patient assessed pain of Fibromyalgia by using the Numeral Rating Scale (NRS) and American College of Rheumatology Preliminary Diagnostic Criteria (ACR, 2010). Results After 64 days of treatment, the NRS of pain reduced from 6 to 2. There was no difference before and after treatment with Widespread Pain Index 7, but Symptom Severity scale score decreased from 8 to 4. Headache, dizziness, and chilling symptoms were also improved. Conclusions The patient showed a significant improvement of Fibromyalgia and accessory symptoms without any side effects, who was treated with herbal medicine.

  • Extra-enteric infections by Blastocystis spp. have rarely been documented. Here, we report a case of extra-enteric blastocystosis in a patient with minimal cervicitis symptoms. A 47-year-old Hispanic female patient was attended in a primary health centre in Michoacan state, Mexico, for her routine gynaecological medical examination. As only symptom, she referred to a slight vaginal itching. The presence of several vacuolar-stages of Blastocystis spp. were identified by Papanicolaou staining; molecular identification was attempted by culture-PCR sequencing of a region of 18S gene from cervical and faecal samples obtained 2 months after cytological examination, even when patient declared that she tried self-medicating with vaginal ovules. Blastocystis ST1 was identified only in the faecal sample. The presence of Blastocystis spp. in the cervix of a patient with scarce symptomatology, demonstrates the extraordinary flexibility of this microorganism to adapt to new environments and niches.

비정형파킨슨증후군 태음인의 어지럼증과 보행장애 치험례 (A Case-Study of Taeeumin Patient with Atypical Parkinsonian Disorders Having Dizziness and Gait Disturbance)

  • A 52-year-old female diagnosed with Atypical Parkinsonian Disorders (APD) about one year ago suffered from dizziness and gait disturbance. The patient was identified as Taeeumin esophagus-cold lung-dry symptomatology pattern and was treated with Jowisengcheong-tang. The patient's symptoms of dizziness and night sweats were assessed using Global Assessment Scale (GAS). And, the Unified Multiple System Atrophy Rating Scale (UMSARS) and the Unified Parkinson's Disease Rating Scale (MDS-UPDRS) were used to assess the overall function of the patient. The patient's symptoms of dizziness and night sweats were significantly improved, and UMSARS and MDS-UPDRS scores were decreased. This case showed that Sasang constitutional medicine treatment can improve APD symptoms and improve the quality of life of the patient.

태음인(太陰人) 마황정천탕(麻黃定喘湯)의 기원, 변천과정 및 구성 원리 (Reconsideration on the Origins, Changes, and Constructive Principle of Mahwangjeongchun-tang)

  • Objectives This paper was aimed to review the preceding research on the origins, changes and constructive principle of Mahwangjeongchun-tang. Method Based on Jema Lee's works such as Dongyisusebowon and Dongmuyoogo, we analyzed contents related to Mahwangjeongchun-tang and its modification. Moreover, we reviewed physiological and pathological pecularities of Taeeumin symptomatology and applied the theory to induce the principle of the modified Mahwangjeongchun-tang in Sasang medicine. Results and Conclusion 1) The origins of Mahwangjeongchun-tang might be Mahwang-tang, Jungchun-tang and Saengmaek-san. 2) Mahwangjeongchun-tang had been changed in composition when it moved from Manbyonghoichun to Dongyisusebowon. 3) Mahwangjeongchun-tang consists of a) Platycodi Radix (桔梗), Liriopis Tuber(麥門冬) and Scutellariae Radix(黃芩) which directly or indirectly enhance lung function, b) Ephedra Sinica(麻黃) which affects circulation of lung companion, c) Prunus Armeniaca(杏仁) and Ginkgo Biloba(白果) which affect wiwan's upward circulatory force, d) Radish Seed (蘿葍子) which affects small intestine's digesting effect, e) Mori Cortex Radicis(桑白皮) and Farfarae Flos(款冬花) which affect cough and asthma.
