태음인(太陰人) 병증(病證) 분류(分類)에 관한 연구(硏究) (A study on the schematic organization of the sub-classification system of the Taeeumin symptomatology)

  • 1. Objectives: We aimed to propose a sub-classification system for the Taeeumin symptomatology by examining the Taeeumin pathology and symptomatology descriptions appearing in "Donguisusebowon". 2. Methods: The Gabo Edition and the Sinchuk Edition (the upgraded and revised edition) of "Donguisusebowon" were reviewed and examined for relevant information on the Taeeum pathology and symptomatology. 3. Results and Conclusions: 1) In the Taeeumin symptomatology, the Exterior disease develops from the basic pathology of Esophagus-Cold and the Interior disease from that of Liver-Heat, eventually progressing to damage of the expirational and dispersive energy of the Lung Sector, the Prime Core Organ or the excessively small organ of the Taeeum constitutional type. The resulting pathology can be broadly defined as the "Lung-Dryness symptomatology". 2) The case reports introduced in the Exterior disease section, including the Zhang Zhongjing Mahuang-tang treatment, Prolonged-affliction disease treatment, and Exterior disease Pestilential disease treatment, share several points in common. They all arise from the pathology of "weakness in the Lung sector and deficiency in the Exterior sector", and they can all be assigned to the same symptomatological division that presents with systemic heat and cold intolerance; this symptomatology can be defined as the "Esophagus-Cold symptomatology", the milder subdivision of the exterior symptomatology. 3) The body of text appearing in the last part of the Interior disease section commonly referred to as the "Taeeumin Conspectus" is in fact not a conspectus when its contents are actually examined. Instead, it can be understood from its pathological and symptomatological descriptions that the passage is explaining the more severe subdivision of the exterior symptomatology that has progressed from Esophagus-Cold to a pathology characterized by damaged expirational and dipersive energy of the Lung Sector. 4) The relocation of the "dry-related pathology" indicates a change in perspective regarding the "Dry-related symptomatology", which caused the rearrangement of the Interior disease into divisions of Liver-Heat symptomatology that is characterized by fulminant heat pathology and Dry-Heat symptomatology that is also accompanied by Lung-Dryness. 5) The Interior disease Yin-Blood Consumptive symptomatology should be included in the Dry-Heat symptomatology in the pathological scheme. 6) Based on the above, the subdivisions of the Taeeumin symptomatology should be arranged as "Esophagus-Cold symptomatology" and "Lung-Dry-Cold symptomatology" in the Exterior disease and "Liver-Heat symptomatology" and "Dry-Heat symptomatology" in the Interior disease.

  • Objectives This research was proposed to present clinical practice guideline (CPG) for Taeyangin Disease of Sasang Constitutional Medicine (SCM). This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods This guideline was performed that search and collection of literature related SCM such as "Dongeuisusebowon", textbook of SCM, clinical guidebook of SCM and fundamental research to standardize diagnosis of Sasang Constitutional Medicine. Journal search related clinical trial or human complementary medicine of SCM was performed domestic and overseas. Finally, 1 article was selected and included in CPG for Taeyangin disease. Results & Conclusions The CPG of Taeyangin disease include classification, definition and standard symptoms of each pattern. Taeyangin disease is classified into exterior-origin lower back (EOLB) disease and interior-origin small intestine (IOSI) disease by region of symptom. EOLB can be replaced with Oegam-yocheok and IOSI can be replaced with Naechok-sojang that is Korean pronuncation. EOLB disease is classified into lower back favorable symptomatology (LBFS) and lower back unfavorable symptomatology (LBUS). Lower back is to say Yocheok, so LBFS can be called Yocheok favorable symptomatology and LBUS can be called Yocheok unfavorable symptomatology. LBUS is to say paraparesis symptomatology or Haeyeok, that is Korean pronunciation, symptomatology. IOSI disease is classified into small intestine favorable symptomatology (SIFS) and small intestine unfavorable symptomatology (SIUS). Small intestine is to say Sojang, so SIFS can be called Sojang favorable symptomatology and SIUS can be called Sojang unfavorable symptomatology. SIUS is to say regurgitation symptomatology or Yeolgeok, that is Korean pronunciation, symptomatology.

Greater Yin symptomatology is classified into mild and moderate pattern by severity. Greater Yin Symptomatology Mild pattern is classified into Greater Yin Symptomatology accompanied abdominal pain and bowel irritability and Greater Yin pattern accompanied Epigastric stuffiness and fullness. And Greater Yin Symptomatology moderate pattern is classified into Greater Yin pattern accompanied Jaundice, Greater Yin pattern accompanied Edema and Greater Yin pattern by Yin toxin.

  • Objective The purpose of this study was to examine the correlation between the Cold-damage Six-meridian disease of Qibo (岐伯六經病證) and Sasang Constitutional Symptomatology (四象體質病證), presented in Discourse on the Origin of Eastern Medicine (醫源論) of Longevity and Life Preservation in Eastern Medicine (Donguisusebowon, 東醫壽世保元). Method The process of development from Cold-damage Six-meridian disease of Qibo (岐伯六經病證) to Six Meridians Physical Symptoms and Medicines (六經形證用藥) in the chapter Cold(寒門) of the Treasure Mirror of Eastern Medicine (Donguibogam, 東醫寶鑑) was investigated. And the correlation between Six Meridians Physical Symptoms and Medicines (六經形證用藥) and Sasang Constitutional Symptomatology (四象體質病證) was considered. Results and Conclusions 1. The Cold-damage Six-meridian disease of Qibo (岐伯六經病證) in the chapter Heat Treatise (熱論篇) of Basic Questions (素問) had evolved into Six Meridians Physical Symptoms and Medicines (六經形證用藥) in the chapter Cold(寒門) of the Treasure Mirror of Eastern Medicine (Donguibogam, 東醫寶鑑) through Book for Life Saving (Huorenshu, 活人書), a work of Zhu Gong (朱肱), Six Books on Cold Damage disease (Shanghanliushu, 傷寒六書), a work of Tao Hua (陶華) and Introduction to Medicine (YixueRumen, 醫學入門), a work of Li Chan (李梴). 2. The correlation between the Cold-damage Six-meridian disease of Qibo (岐伯六經病證) and Sasang Constitutional Symptomatology (四象體質病證) can be analyzed and understood through Six Meridians Physical Symptoms and Medicines (六經形證用藥) in the chapter Cold(寒門) of the Treasure Mirror of Eastern Medicine (Donguibogam, 東醫寶鑑). 3. Greater Yang meridian disease of Qibo (岐伯) is related to Soyangin early stage of Lesser-Yang Wind-Injury symptomatology and Soyangin early stage of Chest-Heat symptomatology, Yang Brightness meridian disease and Greater Yin meridian disease to Taeeumin Liver-Heat symptomatology, Lesser Yin meridian disease to Soyangin Chest-Heat symptomatology, Lesser Yang meridian disease to Soyangin early stage of Lesser-Yang Wind-Injury symptomatology and Reverting Yin meridian disease to Soeumin Reverting Yin symptomatology of Greater Yang disease.

CPG of Yang Depletion symptomatology in Soeumin Disease include classification, definition and standard symptoms of each pattern. Yang Depletion symptomatology is classified into severe and critical pattern by severity. Yang Depletion Symptomatology severe pattern is classified into initial phase pattern and intermediate phase pattern. And Yang Depletion Symptomatology critical pattern is classified into advanced phase pattern.

Congestive Hyperpsychotic symptomatology is classified into mild and severe pattern by severity. Congestive Hyperpsychotic symptomatology mild pattern is classified into initial pattern. Congestive Hyperpsychotic symptomatology severe pattern is classified into intermediate and advanced pattern and Greater Yang disease Reverting Yin pattern.

The CPG was developed by the national-wide experts committee consisting of SCM professors. Methods The CPG was developed by the national-wide experts committee considering of the society of Sasang Constitutional Medicine. It was performed by search and collection of literature related SCM, opinion of SCM experts and journal search and it was followed by CPG's guideline. Results & Conclusions The CPG of Yin-Depletion (Mangeum) Symptomatology in Soyangin Disease include classification, definition and standard symptoms of each pattern. Yin-Depletion (Mangeum) Symptomatology is classified into severe and critical pattern by severity. The severe pattern of Yin-Depletion (Mangeum) Symptomatology is classified into the pattern of Heat-related diarrhea accompanied by headache (Sinyeol-dutong Mangeum) and the advanced pattern of Heat-related diarrhea accompanied by headache (Sinyeol-dutong Mangeum). The critical pattern of Yin-Depletion (Mangeum) Symptomatology is classified into the pattern of Cold-related diarrhea accompanied by the abdominal pain (Sinhan-bocktong Mangeum) and the advanced pattern of Cold-related diarrhea accompanied by abdominal pain (Sinhan-bocktong Mangeum).

  • Objectives This research was performed to establish the clinical practice guideline(CPG) for Lesser Yin Symptomatology of Soeumin disease. Methods Dongeui suse bowon(sinchuk edition), textbook for Sasang constitutional medicine, Clinical guidebook for Sasang constitutional medicine, and standardization reports on Sasang constitutional medicine and papers concerning symptomatology of Soeumin Disease, especially Lesser Yin Symptomatology was collected and classified. Additionally experts' conference was held to make agreement on the conflicting issues on a regular basis. Results & Conclusions There was no concerning paper on Lesser Yin Symptomatology. Experts' agreement was needed to establish the CPG. Lesser Yin pattern can be classified into 2 groups; Lesser Yin severe pattern and Lesser Yin critical pattern. There are Lesser Yin pattern accompanied abdominal pain and bowel irritability pattern and Lesser Yin pattern accompanied green tinged watery diarrhea pattern in Lesser Yin severe pattern. There are Visceral syncope pattern and Exuberant yin repelling yang pattern in Lesser Yin critical pattern. Lesser Yin symptomatology has several symptoms like abdominal pain and diarrhea, thirst, oral discomfort, chest discomfort, whole body pain, articular pain and coldness of hands and feet. Additionally there are abdominal pain and diarrhea in Lesser Yin symptomatology accompanied abdominal pain and bowel irritability pattern, there is green tinged watery diarrhea in Lesser Yin pattern accompanied green tinged watery diarrhea pattern and if this symptoms exacerbate, delirious speech and constipation can occur. There are restlessness and coldness on hands and feet in Visceral syncope pattern and severe restlessness and coldness on hands and feet and symptom which the patient cannot drink water in Exuberant yin repelling yang.

  • Objectives We aimed to analyze the meanings of ordinary symptoms and the developing aspects of present symptoms according to the ordinary symptoms based on the Soeumin pathology, and present the new methodology to make use of the ordinary symptoms in the clinical field. Methods The Soeumin symptomatology and pathology of the Sinchuk Edition (the upgraded and revised edition) of "Donguisusebowon" were reviewed and examined for relevant information on the ordinary symptoms. Results and Conclusions 1) In the Soeumin symptomatology, the representative symptoms, by which the aspects of whole physio-pathological conditions can be decided, were showed as ordinary symptoms, and especially the aggravated state of the deflection of Seong-Jeong are presented additionally in the ordinary symptoms of the unfavorable patterns of Exterior and Interior symptomatologies. 2) In the Soeumin symptomatology, the ordinary symptoms have the clinical meanings as 'pathological predisposition', and can be estimated as one symptomatology in the whole schemes of symptomatology. 3) In the Soeumin symptomatology, the ordinary symptoms can affect the present symptoms, and can be the principal factors in the pattern identifications of the present symptoms and the determinations of therapeutical prescriptions. 4) In the Soeumin symptomatology, the ordinary symptoms can develop to the present symptoms across the Interior or the Exterior symptomatologies within the categories of the favorable and unfavorable patterns, and this developing aspects can be explained from the Soeumin pathological perspective.

This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods First, collection and organization of literature related to SCM such as Donguisusebowon, Text book of SCM, Clinical Guidebook of SCM and Fundamental research to standardize diagnosis of Sasang Constitutional Medicine was performed. Secondly, journals related to clinical trial or Human complementary medicine of SCM were searched. Finally, 7 articles were selected and included in CPG for Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease. Results & Conclusions The CPG of Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease in Taeeumin Disease include classification, definition and standard symptoms of each pattern. Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease consists of two aspects : Esophagus-Cold (Wiwanhan) and Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology. Esophagus-Cold (Wiwanhan) symptomatology is classified into mild and moderate pattern by severity. Mild pattern of Esophagus-Cold (Wiwanhan) symptomatology is classified into Supraspinal Exterior (Baechu-pyo) initial and Wheezing-Dyspnea (Hyocheon) pattern. Moderate pattern of Esophagus-Cold (Wiwanhan) symptomatology is classified into Cold-reversal (Hanguel) and Cold-reversal (Hanguel) advanced pattern. And Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology is classified into severe and critical pattern by severity. Severe pattern of Esophagus-Cold Lung-Dry (Wiwanhan-paejo) is classified into Dry-Cold (Johan) pattern and Dry-Cold (Johan) advanced pattern. Critical pattern of Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology consists of Dry-Cold (Johan) intense pattern (Eumhyeol-mogal handa pattern).
