Follow-up Evaluation of the Effect of Stellate Ganglion Block Using Thermography -A case report- (체열검사를 이용한 성상신경절 차단 효과 추적 -증례 보고-)

  • Among the sympathetic blocks used to treat various symptoms and diseases, including sympathetic dysfunction in pain clinics, a stellate ganglion block (SGB) is one of the easiest to apply. However, it is difficult to evaluate the effects of SGB due to the subjective nature of patient-reported data. A 26-year-old female, who presented with symptoms of coldness and sweating on both hands, received SGB 25 times on each side over a 2-month period. The effects of SGB were followed up using a cold stress test with thermography. Although the symptoms appeared again after 6 months, the recovery rate of the palmar temperature after the cold stress test improved significantly and was maintained for 18 months.

A Case Study about Soeumin Mang-yang Syndrome with Redness of Urine and Urination Difficulty (소편적삽증(小便赤澁證)을 동반한 소음인(少陰人) 망양증(亡陽證) 환자(患者) 치험(治驗) 1례(例))

  • 1. Objectives The primary purpose of this case study is to report that a Soeumin Mang-yang Syndrome with redness of urine and urination difficulty was treated with 'Soeumin Seungyangikgibuja-tang(升陽益氣附子湯)(SIBT)' and then his symptoms, laboratory test(urine) and urine color improved. 2. Methods The patient has the symptoms including redness of urine, discomfort after urinating, general body sweating, constipation and thirst. We diagnosed him as Soeumin Mang-yang Syndrome. So we treated him with 'SIBT(升陽益氣附子湯)'. 3. Results and Conclusions After the constitutional treatment with SIBT(升陽益氣附子湯) on Soeumin Mang-yang Syndrome patient, his symptoms and laboratory test(urine) improved and urine color was clear. 'SBIT(升陽益氣附子湯)' is considered to be effective in Soeumin Mang-yang Syndrome. The change of urine color can be good evidence for the symtoms of Mang-yang Syndrome is improved.

  • Purpose: This study was to report the effect of Traditional Korean Medicine to Rheumatoid arthritis after parturition. Methods: A woman who complained of arthralgia, edema, sweating and chilling after parturition was enrolled in this study. We treated her by oriental treatments (herb medicine, acupuncture and moxibustion) to improve her symptoms. We checked her by symptoms and serologic test. Results: After treatments, the clinical symptoms and serologic signs improved. Conclusions: This case study shows that TKM treatment is effective for rheumatoid arthritis after parturition.

  • In Oriental medicine, the subjective process of diagnosis often makes it difficult to explain the indications objectively, in spite of its excellent therapeutic value. The authors' objective Was to present more objective indications of Chongpesagan-tang(淸肺瀉肝湯), a prescription predominantly used to treat stroke in Oriental Medicine, We analyzed clinical symptoms of 105 patients who were hospitalized in the Department of Circulatory Internal Medicine, College of Oriental Medicine. Kyung Hee University, Seoul, who were given Chongpesagan-tang and improved without side effects, The symptoms analyzed included general symptoms which are usually checked in the process of Oriental Medicine Diagnosis: the so called Four Diagnosis(四診). We rearranged and selected symptoms which were predominantly found among other symptoms. We found some symptoms of which the patients frequently complained: sweating 84%, red tongue 74%, white tongue coat 65%, constipation 35%, tight pulse 21%, headache 20%, thirst 20%, chest discomport 17%, etc, We believe that these suggest an objective indication of Chongpesagantang.

The Experiences of Alternative Therapy with the Menopausal Symptom in middle-aged Women (중년여성의 갱년기 증상완화를 위한 대체요법 경험)

  • The primary purpose of this study was to describe psychological aspects of menopausal women who have experiences on alternative therapies, such as pomegranate tree fluid, life pack, evening primerose, zacro syrup, zacro pill, multi-vitamin, oriental medicine instead of estrogen. In order to have insights on the participants behaviors, this qualitative research was conducted. The participants in this study were six women who have been taking at least one of the alternative therapies to relieve the menopausal symptom. Data had been collected from November 2002 to May 2003. The interview questions were about motivations to seek the alternative therapy, menopausal symptoms, feelings or positive effects after the treatment. The interviews were recorded and transcribed into written documents. The method described by Giorgi was utilized for data analysis. The study results were as follows : 1. The participants sought the alternative therapy based on their own information or by friends' recommendations. The motivations of taking the therapy were to live younger and healthier by reliving menopausal symptoms including somatic symptoms, such as irregular menstruation, flushing sense, cold sweating, insomnia, dull headache, vagina dryness, cystitis after sexual intercourse, joint pain, nervousness, depression, skin dryness, and suppression of activity. 2. The participants expressed that they became more active and felt better with disappearing menopausal symptoms after about 2 to 3 weeks treatment. 3. The participants did not want to accept the fact that they had menopause, which caused avoidance of having medical diagnosis and medical treatment. They also did not want to let their spouses know about their symptoms. 4. The major reason of taking the alternative therapy was to avoid the side effects of estrogen therapy in spite of high costs of the alternative way. 5. The participants suggested that menopausal women needed to be educated about physical and psychological symptoms of menopause. They also needed to be informed about how to manage it. In conclusion, accurate information about foods and drugs utilized for the alternative therapy was needed. Medically proved methods to recognize positive effects and side effects were also necessary. These goals could be achieved by continuous medical observations and further research.

A Case of Insulinoma (Insulinoma 1례 보고)

  • Insulinoma is the most frequent endocrine tumor of the pancreas and the first of the endocrine-secreting tumor of the gut to be recognized by Nicholls in 1902. Recurrent episodes of hypoglycemia is the main cause of the symptoms and signs which were sweating, pallor, dizziness, habitual change, convulsion and coma. In 1935, Whipple and Frants were described so-called "Whipple's triad" : the patient's symptoms occur with fasting or exercise; at the time of symptoms, the serum glucose in 50mg/dl or less; and the symptoms are relieved by the administration of glucose. While these criteria were timely, they proved to be rather nonspecific and may be found in other conditions that result in fasting hypoglycemia. We experienced a 44-year-old female patient who had repeated attacks of convulsion, unconsciousness and coma for 3 years. Although she has been treated with anticonvulsant, the symptoms and signs were not disappeared. At the time of administration, she was a full coma state due to hypoglycemia and was dramatically reversed by intravenous administration of the glucose solution. The preoperative test such as provocative test, abdominal CT and celiac angiography revealed insulinoma and after enucleation the pathologic diagnosis was the same. We like to report a adult female patient with insulinoma and the review of literatures briefly.

The Clinical Study on Childhood Common Cold in Four Seasons (계절별(季節別) 증상(症狀)을 중심으로 한 소아감모(小兒感冒)의 임상적 고찰)

  • Objectives: This study was performed to investigate the character of childhood common cold according to four seasons. Methods: We made an investigation into medical record of 432 children under fifteen years of age who visited pediatrics or emergency room suffering from common cold. We analyzed the principal and secondary symptoms of the children classifying four seasons by $SPSS^{\circledR}$ 12.0 for windows. Results: Male to female ratio were 1.42:1, the average of age was 3.86 years old. Distribution of principal symptoms was that nasal discharge was 231(56.1%), cough was 233(54.1%), nasal obstruction was 162(39.3%), secretion and sputum were 126(30.6%), fever was 121(29.4%), and headache was 13(3.2%). Out of these fever occurred in high frequency during spring and summer, nasal obstruction occurred in high frequency during spring and autumn. Distribution of secondary symptoms was that throat pain was 93(22.6%), otalgia was 39(9.5%), vomiting was 35(8.5%), abdominal pain was 30(7.3%), anorexia was 23(5.6%), sweating was 21(5.1%), diarrhea was 19(4.6%), and startle was 11(2.7%). Out of these otalgia occurred in high frequency during spring and summer, diarrhea occurred in high frequency during autumn and winter. Conclusions: A few symptoms can be the character according to four seasons.

A Study on Relations among General Characteristics, Lifestyle Habits, and Menopausal Symptoms Measurement Indicators during Treatment for Hot Flush in Menopausal Women (안면홍조 증상으로 치료 중인 갱년기 여성에서 일반적 특성 및 생활 습관과 갱년기 측정 지표들간의 상관성 연구)

  • Objectives: To understand relations general characteristics, lifestyle habits including smoking, alcohol habit, exercise, eating habit and menopausal symptoms measurement indicators during treatment for hot flush in menopausal women. Methods: The participants were 159 women (45~60 yr) who were not currently on hormone therapy, and had reached hot flash scores of 10 or higher. The evaluating indexes of this trial are hot flush score, hot flush visual analogue scale(VAS), Hot flush consistence time, sweating visual analogue scale(VAS), Menopause Rating Scale(MRS), Menopause-specific quality of life questionnaire(MENQOL), Kupperman's Index. Results: 1. When the participants divided according to body mass index(BMI), overweight group showed a high score in all indicators. Therefore, obesity can aggravate the symptoms of menopause. 2. When the participants divided according to drinking habits, Hot flush consistence time of non-alcohol group showed a statistically significant difference. However, the sample size is uneven. Additional studies will be needed. 3. When the participants divided according to eating habits, Hot flush consistence time of regular group showed a statistically significant difference. However, the sample size uneven. Additional studies will be needed. 4. When the participants was divided depending on whether exercise, non-exercise group showed a high score in all indicators. Thus, exercise will be able to improve menopausal symptoms. Conclusions: Improvement of obesity and exercise will be able to improve menopausal symptoms.

Clinical Observation and Diagnosis in Leukemia (백혈병의 임상관찰 및 변증에 대한 연구)

  • Leukemia is a blood disease that occurs in the abnormal process of leukocyte maturation. Its main medical treatment is chemical therapy and bone medical transplant. But its treatments give rise to ill effects and sequela. At present, the cause of leukemia is not fully understood. But oriental concept of curing leukemia is the holistic thinking that emphasizes the unity, wholeness, and the relationship between the human organs, including western medical concept of blood cause. So it is estimated that holistic medical treatment in oriental medicine could present new medical way of curing leukemia. I read over the diary on struggles against leukemia, survey on the leukemia patients, and, medical charts in the oriental clinics that treated in the holistic way, and I interviewed the leukemia patients. With these materials, I classified the early main attacking symptoms according to the chief complaints of one hundred leukemia patients. With these results collected, I present new alternative treatment by oriental medical diagnosis. The chief early complaints that leukemia patients give are fatigue, enervation, cold, contusion, fever, high fever, sweating at sleep, myalgia, arthralgia, and dizziness, in order of main symptoms. Fatigue occurs mainly by spleen and stomach weakness and marrow shortage. So it is estimated that It is important to treat the spleen that is responsible for supplementing the marrow. Because the leukemia patients have anemia and hemorrhage, to treat the spleen is important for hematopoiesis and controlling blood. In case of cold, it penetrates into the body when the body is weak. So its treatment is to increase body's health. But the cause of fever is difficult to classify into outer cause and inner cause. But in case of children under 14 years old, fever is the main sypmptom. I think this is because children have the body with pure vital energy. Hemorrhage is thought to be the result of yin-lack and heat-miasma of spleen and stomach channels. Contusion occurs from the qi-weakness and the not-controlling-blood. Sweating at sleep is from the yin-weakness. It is found with all weak symptoms. Dizziness is from the yin-blood impairment. Weight-loss is from the marrow shortage. Myalgia and arthralgia is mainly from inner weakness, not outer maisma. Most leukemia patients have the idea that holistic treatment of leukemia could be of assistance and give help to the low immunity. So it is expected that holistic medical treatment could contribute to knowing the cause and treatment of leukemia, and give people reliability on oriental medical treatment, through the profound diagnosis of leukemia.

A Clinical Case Study of Vasovagal Syncope (미주신경성 실신 증례보고 1례)

  • Objectives: The purpose of this clinical case report is to describe the efficacy of Korean medicine in the treatment of vasovagal syncope. Methods: The patient was treated with acupuncture, pharmacopuncture, and herbal medicine. The symptoms (headache, palpitation, sweating, flushing, weakness, coldness) of the patient were observed. Results: After the treatment, the patient's symptoms improved, but more treatments were required. Conclusion: This study can add to understanding of the effect of Korean medicine on vasovagal syncope.
