The Relationship between Coronary Risk Factors and Coronary Calcium Score Detected by Computed Tomography Coronary Angiography in Korean Middle Aged Men (다중절편 방사선단층촬영 결과에 기반한 한국 중년 남성에서의 관상동맥 석회화와 심혈관질환 위험인자와의 연관성)

  • Objectives: The objective of this study was to investigate the relationship between coronary artery calcification and risk factors of cardiovascular disease with multidetector computed tomography. Methods: Data were collected from 5,899 males between 30 and 59 years old by interview, survey, physical examination, blood test, and multidetector computed tomography in the university hospital from January 2010 to December 2011. We confirmed the coronary artery calcium scores of subjects by multidetector computed tomography and identified risk factors of cardiovascular disorders. We investigated the relationship between coronary artery calcification and risk factors of cardiovascular disorders. Results: Mean calcium score of the coronary arteries in 5,899 participants was 8.20, and 773 participants (13.1%) exhibited coronary artery calcification. The presence of coronary artery calcification was correlated to risk factors of cardiovascular disease (age, blood pressure, triglyceride, HDL cholesterol, LDL cholesterol, glucose, Apolipoprotein A-1, Apolipoprotein B, body mass index, waist circumference) and risk assessment tools of cardiovascular disorders. Significant predicted factors of coronary artery calcification had different patterns in each age group (30-39, 40-49, 50-59 years old). Conclusions: We confirmed the relationship between coronary artery calcification and either typical risk factors of cardiovascular disease or risk assessment tools of cardiovascular disease. In addition, we also observed that the pattern of these factors varied according to age. Therefore, age-related variation needs to be considered in management strategies to prevent cardiovascular disease.

Biological Monitoring of Paint Handling Workers exposed to PAHs using Urinary 1-Hydroxypyrene (다핵방향족탄화수소류에 노출된 페인트 취급 근로자에서 요 중 1- Hydroxypyrene을 이용한 생물학적 모니터링)

  • To investigate the exposure effect of polynuclear aromatic hydrocarbons (PAHs), we measured airborne total PAHs as an external dose, urinary 1-hydroxypyrene (1-OHP) as an internal dose of PAHs exposure, and analyzed the relationship between urinary 1-OHP concentration and PAHs exposure. The study population contained 44 workers in steel-pipe coating and paint manufacture industries. The airborne PAHs was obtained during survey day, and urine were sampled at the end of shift. Personal information on age, body weight, height, eniployment duration, smoking habit, and alcohol consumption was obtained by a structured questionnaire. Airborne PAHs were analyzed by the gas chromatograph with mass selective detector. Urinary 1-OHP levels were analyzed by the high performance liquid chromatograph with ultraviolet wavelength detector. For statistical estimation, t-test, ${\chi}^2$-test, analysis of variance, correlation analysis, arid regression analysis were executed by SPSS/PC (Windows version 10). The mean of environmental total PAHs was $87.8{\pm}7.81{\mu}g/m^3$. The mean concentration ($526.5{\pm}2.85{\mu}g/m^3$) of workers in steel-pipe coating industries using coal tar enamel was the higher than that ($17.5{\pm}3.36{\mu}g/m^3$) of workers in paint manufacture industries using coal tar paint. The mean of urinary 1-OHP concentration ($51.63{\pm}3.144{\mu}\;mol/mol$ creatinine) of workers in steel-pipe coating industries was the higher than that ($2.33{\pm}4.709{\mu}\;mol/mol$ creatinine) of workers in paint manufacture industries. The mean of urinary 1-OHP concentration of smokers was the higher than that of non-smokers. There was significant correlation between the urinary concentration of 1-OHP and the environmental concentration of PAHs (r=O.S48, p<0.001), pyrene(r=0.859, p<0.001), and urinary cotinine (r=0.324, p<0.05). The regression equation between the urinary concentration of 1-OHP in ${\mu}g/g$ creatinine($C_{1-OHP}$) and airborne concentration of PAHs (or pyrene) in ${\mu}g/m^3$ ($C_{PAHs}$ or Cpyrene) is: Log ($C_{1-OHP}$)=-0.650+0.889×Log($C_{PAHs}$), where $R^2=0.694$ and n=38 for p<0.001.Log ($C_{1-OHP}$)=1.087+0.707${\times}$Log(Cpyrene), where $R^2=0.713$ and n=38 for p<0.001. From the results of stepwise multiple regression analysis about 1-OHP, significant independents were total PAHs and urinary cotinine (adjusted $R^2=0.743$, p<0.001). In this study, there were significant correlation between the urinary concentration of 1-OHP and the airborne concentration of PAHs. The urinary 1-OHP was effective index as a biomarker of airborne PAHs in workplace. But it was influenced by non-occupational PAHs source, smoking.

Oral health status and treatment need of institutionalized elderly patients (노인요양시설에 있는 노인환자의 구강실태 및 치료수요도)

  • Statement of problems: In the area of dental care, the institutionalized elderly have placed the most vulnerable state, and we cannot find their subjective need of dental treatment because of the physical and mental disabilities, But we have no basic investigation of their oral health conditions. Purpose: The aims of the current study were to investigate the oral health status of institutionalized elderly patients who are in the least benefited side of dental service, and to analyze their dental treatment needs. Material and methods: The survey of the oral status was carried out on 758 institutionalized elderly, and 212 elderly who was more than 65 years old from D dental office, and it was based on the Guidelines of Oral Health Research of year 2000 in Republic of Korea. Results and conclusion: The DMFT index of the institutionalized elderly appeared higher than that of the same ages in control group, and it increased with age. The number of residual teeth of the institutionalized elderly appeared lower than that of the same ages in control group, and it decreased with age (P < .05). The number of fixed partial denture in institutionalized elderly was lower than that of the same ages in control group (P < .05). The percentage wearing removable partial denture was not significant between the elderly in institutions and the control group, and was not different according to age between the two groups. The percentage of institutionalized elderly wearing complete denture appeared lower than that of the same ages in control group, and it increased with age. The percentage of institutionalized elderly needing complete denture was higher than that of control group, and the percentage of elderly needing complete denture on the maxilla was higher than that of the mandible. 16.35% of the institutionalized elderly was living without denture in spite of their fully edentulous state. The need for complete denture increased rapidly with age. The number of valued teeth and dental prostheses in shortened dental arch concept and number of occluding pairs of teeth of institutionalized elderly were lower than that of the control group (P < .05). In institutionalized elderly, the number of residual teeth, the number of fixed partial dentures, and the percentage wearing removable partial dentures were higher in the mandible, and the percentage wearing complete dentures was higher in the maxilla (P < .05). The rate of institutionalized elderly needing prosthodontic treatment appeared to be 67.82%, where the number of occluding pairs of teeth was less than 10. When it is difficult to evaluate the subjective need of dental treatment as with the institutionalized elderly, estimation using the number of occluding pairs of teeth can be a useful indicator that can project treatment needs. For the oral health care of institutionalized elderly, it is essential to increase the awareness of nurses and caregivers who take care of them, about the importance of the oral health. Since the average life span and number of residual teeth are increasing gradually, the welfare policy should be changed to implementing regular dental examinations, preservative treatment forms and oral health control of dentulous patients where the traveling-treatment system and visit system are supplemented. And principles should be set that the present denture project of edentulous patients should be done by specialists who'll also be responsible for postmanagement. Through this research of institutionalized elderly, the oral health status which is worsened by aging could be confirmed. And the interest and positive participation of dental society on the elderly should come first in order to solve the rising treatment needs of the elderly patients.

Comparison of Health Risks according to the Obesity Types Based Upon BMI and Waist Circumference in Korean Adults: The 1998-2005 Korean National Health and Nutrition Examination Surveys (한국 성인에서 체질량지수와 허리둘레를 기준으로 분류한 비만 유형에 따른 특성 비교: 1998-2005 국민건강영양조사)

  • This study was performed to examine the health risks according to the obesity types based upon body mass index (BMI) and waist circumference (WC) cutoffs. The subjects were 8,966 adults aged 40-79 years from the 1998, 2001 and 2005 Korean National Health and Nutrition Examination Surveys. The subjects were classified into 4 types of obesity groups based on BMI and WC: BMIWC (BMI${\geq}$ 25 kg/$m^2$ and WC${\geq}$ 90 cm for males and ${\geq}$ 85 cm for females), WC (BMI < 25 kg/$m^2$ and ${\geq}$ 90 cm for males and ${\geq}$ 85 cm for females), BMI (BMI ${\geq}$ 25 kg/$m^2$ and < 90 cm for males and < 85 cm for females), normal (BMI < 25 kg/$m^2$ and < 90 cm for males and < 85 cm for females) groups. More than half (n = 5,103) of the subjects fell into the normal group and BMIWC group accounts for 58%, followed by BMI group of 27% and WC group of 16%. Mean ages of subjects were highest in WC group and lowest in BMI group. Blood levels of total cholesterol, triglyceride, LDL-cholesterol and glucose, and blood pressures were higher and HDL-cholesterol was lower in 3 types of obesity groups (BMIWC, WC, BMI) than in the normal group. Among the 3 obesity groups, energy and fat intake was significantly higher in BMIWC than in normal group in males, however, no differences were found among the 4 groups in females. The three obesity groups had greater odds ratios for hypertension, diabetes mellitus, hypercholesterolemia, hyper-LDL-cholesterolemia, hypo-HDL-cholesterolemia, hypertriglyceridemia, and metabolic syndrome compared to the normal group. Among 3 obesity groups, BMIWC group had the highest odds ratios for hypertension, diabetes mellitus, and metabolic syndrome. Especially BMIWC and WC groups showed the highest odds ratios for diabetes mellitus and metabolic syndrome, suggesting the potential role of abdominal obesity in diabetes mellitus and metabolic syndrome. Our data indicate that different types of obesity may show different degrees of health risks. The appropriate selection and use of obesity indexes may be effective for management of obesity as well as obesityrelated diseases.

Reproductive Factor and Food Intake Pattern Influencing on the Breast Cancer Risk in Daegu${\cdot}$Gyungbuk Area, Korea (대구${\cdot}$경북지역 유방암 위험에 영향 미치는 생식적 요인 및 식품섭취 패턴)

  • The present study examined the reproductive factors and food intake pattern which influence on the breast cancer risk in Daegu${\cdot}$Gyungbuk area. The case subjects were 103 patients newly diagnosed as breast cancer at Kyungpook National University Hospital. The control subjects were 159 healthy women selected by frequency matching of age and menopausal status in the same community. The survey was conducted by individual interviews using questionnaires which include general and reproductive characteristics, dietary habits, and food frequency. The odds ratios were calculated by using unconditional logistic regression after adjusting for confounding variables. The mean age of the subjects was 50 yrs, and mean body mass index was significantly high in the patient group than in the control. The patient group had a significantly higher familial history of breast cancer and a significantly shorter breast-feeding period compared to the control group. It has been found no significant relationship between extrinsic hormone use such as oral contraceptives or estrogen replacement therapy and breast cancer risk. In regard of food habits, a high preference for the cooking method of steaming rather than frying, panbroiling or roasting was significantly associated with lower relative risk of breast cancer. The higher intake frequencies of fruits for all subjects, and seaweeds only for postmenopausal subjects were related with a significantly lower relative risk of breast cancer. The results of the study suggest that the possible risk factors for the breast cancer occurrence include high BMI, family history, less breast feeding experience, preference for the cooking method of frying, and less consumption of fruits and seaweeds. This study provides an useful data for nutrition education to prevent breast cancer for the residents in Daegu${\cdot}$Gyungbuk area.

A Study on Development of Evaluation Indicator for Golf Course User's Preference (골프장 이용자 선호도 평가지표 개발)

  • This study was conducted to develop evaluation indicators to improve athletic performance and operational management of golf courses and the results of the research are as follows. Through theoretical research and a preliminary professional survey, 15 on-going evaluations of golf course composition and operational management and 55 sub-evaluation indices were rejected while 10 on-going evaluations and 52 sub-evaluation indicators were reconfigured as final for environmental-friendliness, level of member services, level of human service of game personnel, difficulties of course, management level of the course, fairness of operational management, accessibility and location characteristic, traditions and ambiance of the golf club, quality of course, and course layout. When analyzing the important decision factors in golf course user preference evaluation indicators, the following contributed in the order of higher to lower contributions: the management level of the course, excellence of the course, level of human services for personnel, course layout and environmental-friendliness. When identifying the path coefficient of golf course evaluation indicators, the curvature of a hole and the length of the course had a causal effect on the 'course layout' section. Tournament facilities and various shot values had a causal relationship with 'excellence of the course', in the order of higher to lower, and convenience of waiting and fair allocation of reservations for 'fairness of operational management'. The history of the golf course and its environmental characteristics, history and culture of the region have relatively higher causal effects on 'traditions of the golf club' and geographical conditions on 'accessibility and location characteristics', pesticide and fertilizer usage and water pollution on 'environmental-friendliness', and member benefit and kindness of employees on 'level of member services'. The kindness and expertise of the game personnel had a relatively higher causal effect on the 'level of human services of game personnel', the location of tenning area, and location of OB and hazards on 'difficulties of course', and rough conditions and obstacles management on 'management level of the course'. There is a need to complete a systematic evaluation index system for golf course user preferences through future studies for a more detailed assessment, as well as a process to verify these evaluation indicators by application to domestic and international golf courses.

Prevalence and Associated Factors of Snoring in School-Aged Children (학동기 아동에서 코골이의 유병률과 관련인자)

  • Purpose : We studied the prevalence of snoring and its association with diseases, obesity and environmental factors, as well as sleep disturbance arising from snoring, in school-aged children. Methods : The survey was performed by a special questionnaire on 1,707 children at elementary schools from 1 to 30 July 2001 in Gwangju City. The prevalence of snoring, associated factors, and sleep disturbance were evaluated. Results : 266(16.5%) of the children snored at least once a week. 73(4.3%) of the children snored almost every day. Regarding the associated factors of snoring : There was a statistically significant difference between snoring and nonsnoring groups due to gender, obesity, sinusitis, tonsillar hypertrophy, bronchial asthma and allergic rhinitis. Of them, male gender, tonsillar hypertrophy, and obesity were significant risk factors for development of snoring. Especially, the odds ratio increased to 1.048(CI, 1.004-1.659), 1.748(CI, 1.175-2.599), and 2.266(CI, 1.300-3.950) in children with an obesity index of 20-29%, 30-49%, ${\geq}50%$, respectively. There was a statistically significant association of snoring with decreased sleep duration, sleep talking, and drowsiness during the day as a result of sleep disturbance. Conclusion : 16.5% of school-aged children snored at least once a week. The association of snoring with tonsillar hypertrophy, obesity and gender as risk factors was demonstrated in this study.

The Prevalent Rates of Abnormal Serum Aminotransferase Levels and Total Cholesterol Levels among Adolescents with Obesity (청소년기 비만아에서 이상 혈청 간효소치 및 총콜레스테롤치의 유병율)

  • Purpose : The purpose of this study was to look at prevalences of abnormal serum aminotransferase levels and abnormal serum total cholesterol levels among adolescents with obesity in Seoul area. Methods : Body mass index[$BMI(kg/m^2)$], serum aspartate aminotransferase(AST) alanine aminotransferase( ALT), and total cholesterol levels were measured in 26,876 adolescents(male : 13,287, female : 13,589) of first grade of high school in Seoul between the ages of 15 and 16 years, enrolled in a large health examination survey by Seoul School Health Center(SSHC) in 2001. Obesity was defined as BMI more than 95th percentile for age and sex and normal weight was defined as BMI between 25th percentile and 75th percentile for age and sex. Abnormal levels of AST, ALT and total cholesterol were defined as more than 95th percentile for age and sex. Results : The BMI at 95th percentile was 28.7 in males and 26.9 in females. The prevalence of abnormal AST levels was higher in obesity group than in normal weight group. In males, those were 23.7% and 3.7%, respectively(P<0.01, $x^2-value$ : 529.2). In females, those were 11.8% and 4.9 %, respectively(P<0.01, $x^2-value$ : 56.3). The prevalence of abnormal ALT levels was significantly higher in the obesity group compared with the normal weight group. In males, those were 33.9% and 1.7%, respectively(P<0.01, $x^2-value$ : 1,693.4). In females, those were 22.3% and 3.9%, repectively(P<0.01, $x^2-value$ : 397.6). The prevalence of abnormal total cholesterol levels was higher in the obesity group than in the normal weight group. In males, those were 16.8% and 3.5%, respectively(P<0.01, $x^2-value$ : 268.3). In females, those were 9.0% and 5.4%, respectively(P<0.01, $x^2-value$ : 14.2). Conclusion : This study revealed that the prevalences of abnormal AST, ALT and total cholesterol level were higher in the obese group than in the normal weight group.

Analysis of functional habitat groups and community stability of benthic macroinvertebrates before and after Yeongju Dam completion in Korea (영주댐 준공 전후 저서성대형무척추동물의 서식기능군 및 군집안정성 분석)

  • This study was conducted to investigate the environmental impact of the construction of the Yeongju Dam, located near the upper stream of the Nakdong River, on benthic macroinvertebrates. The investigation was conducted four times from 2015 to 2016 and the composition of the benthic macroinvertebrate fauna from the four surveyed sites was compared. After the dam construction, the number of benthic macroinvertebrate species decreased and the species composition ratio of the E.P.T taxa(Ephemeroptera, Plecoptera, and Trichoptera) decreased, which favored the previous environment and the large riverbed structure. However, the species composition ratio of the O.C.H taxa (Odonata, Coleoptera, and Hemiptera), which favored the reservoir environment, increased. For the pattern of changes in the functional habitat groups, the proportion of 'Clinger(CL)' taxa decreased and the 'Burrower(BU), Climber(CB), Sprawler(SP), Swimmer(SW)' taxa increased in all surveys after the dam construction. A cluster stability comparison showed that the 'Stability Group II' species of St.2 and St.3 showed a drastic decrease. The average Benthic Macroinvertebrate Index(BMI) of all the survey sites in 2016 was lower than that in 2015. This research confirmed the temporal and spatial changes in the habitat type and community structure of the benthic macroinvertebrates resulting from the dam construction.

Family Caregivers' Quality of Life, Depression and Anxiety according to Symptom Control in Hospice Patients (호스피스 환자의 증상조절에 따른 가족간병인의 삶의 질과 우울, 불안)

  • Purpose: It is well known that a terminal cancer condition affects not only patient themselves but their family members because the patients experience a variety of symptoms. This study was aimed to investigate modifiable factors that influence family caregivers' quality of life, depression, and anxiety. Methods: From January 2015 through May 2015, a survey was conducted with 61 family caregivers of hospice patients who were hospitalized in two university hospitals and one municipal hospital in Busan. The questionnaire was consisted of characteristics of family caregivers and patients, the Korean version of the Caregiver Quality of Life Index-Cancer (CQOLC-K), Beck's Depression Inventory II (BDI-II), Beck's Anxiety Inventory (BAI), and patient's symptom controlling scores rated by family caregivers. Results: Family caregivers' depression was associated with religion. Quality of life and depression of family caregivers were also influenced by monthly household income. Patient age was inversely related to family caregiver's quality of life ($r_s=-0.259$, P=0.043). Family caregivers' quality of life was associated with patient's anxiety (r=0.443, P=0.001). Family caregivers' depression was affected by patient's constipation (r=0.276, P=0.046), anxiety (r=0.508, P<0.001), and daytime drowsiness (r=0.377, P=0.005). And family caregivers' anxiety was influenced by patients' sleep disturbance (r=0.276, P=0.046), depression (r=0.297, P=0.031), and anxiety (r=0.357, P=0.009). Conclusion: According to our findings, family caregivers had higher quality of life and less depression and anxiety when symptoms in hospice patients were well controlled.
