상악 견치의 매복은 소아치과 의사가 흔히 접할 수 있는 맹출장애이며, 이를 방치하였을 경우 인접치의 치근 흡수, 낭종 형성 등의 임상적인 문제점을 유발할 수 있다. 이에 대한 치료는 간단한 유치 발치에서부터 매복치아의 교정적 견인, 외과적 자가이식 등 다양한 방법을 고려할 수 있다. 이 중 자가 이식은 매복치아가 교정적 견인술을 시행하기 어려운 위치에 존재하거나 재위치 시키는데 실패했을 경우에 매복치아의 발거에 앞서 고려할 수 있으며, 그 예후는 치근의 완성도, 환자연령, 외과적 술식, 근관치료 시기, 치아고정 기간 등에 의해 결정된다. 본 두 증례들은 혼합치열기 말기에 상악 견치가 매복된 환자에서 그 매복 위치가 자발적인 맹출 유도나 교정적 견인 및 배열이 어렵다고 판단된 경우이며, 자가 이식 후 근관치료와 교정 치료를 시행하고 현재까지 성공적으로 유지되고 있어 보고하는 바이다.
상악 견치는 가장 긴 치근을 가지며 저작기능과 함께 하악의 측방운동을 유도하고 심미적으로도 치열궁의 구각부위에 위치하여 교합관계의 조화와 대칭을 유지하는 등의 중요한 기능을 수행한다. 상악 견치는 발육기간이 가장 길며 형성되어 맹출하기까지 복잡한 경로를 갖기 때문에 다른 치아보다 매복의 빈도가 높다. 이러한 매복치를 치료하기 위해 임상의는 관찰을 하거나 외과적 노출과 교정적 견인, 자가치아이식, 발치 등 다양한 치료방법을 고려할 수 있다. 외과적 노출과 교정치료는 견치를 교합선상에 배치할 수 있기 때문에 가장 이상적인 접근으로 생각할 수 있다. 본 증례 보고는 구개측으로 매복된 상악 견치를 외과적 노출과 교정적 치아이동을 통해 기능적, 심미적으로 양호한 결과를 얻었다.
Background and Objectives Tracheostomy is a relatively safe procedure, and the recent emergence of COVID-19 has raised the need to perform tracheostomy immediately in the bed of an intensive care unit (ICU) rather than an operating room. The purpose of this study was to determine the occurrence of complications related to surgical tracheotomy performed in the ICU by an ENT specialist. Materials and Method From March 2019 to January 2022, a total of 101 patients underwent tracheostomy in the ICU. Demographics and complications were classified according to postoperative period. Results Within 24 hours after the procedure, bleeding events were confirmed in 2 patients (2.0%) with mild bleeding. One case (1.0%) of ventricular fibrillation occurred shortly after the procedure. There were no complications from 24 hours to 1 week after procedure. After one week, 4 patients (4.5%) had a local infection, and 3 patients (3.4%) had a tube obstruction. During all follow-up periods, there were no serious side effects such as death, major vascular injury, pneumothroax. No complications were observed throughout the entire period in 6 COVID-19 patients. Conclusion The number of complications of surgical tracheotomy in the ICU performed by a specialist was lower than in previous studies, and there were no complications that delayed treatment or endangered life. The ENT training hospitals should provide sufficient training opportunities for residents to perform surgical tracheostomy and strive to minimize complications associated with the procedure and pre- and post-operative management under the detailed guidance and supervision of specialists.
The first patient of this case was 13 year and 8 month old girl who had the Angle's class I malocclusion with uneruption of upper left central incisor and left canine. The second patient was 12 year old boy who had the Angle's class I malocclusion with uneruption of right central incisor. After surgical exposure of impacted teeth of those patients and bonding of attachment on exposed crown, those teeth were orthodontically pulled with removable appliances. The results were obtained as follow. 1. The functionally proper occlusion was established by guidance of the impacted teeth into the normal position in the arch. 2. Establishment of esthetic and balanced arch form was achieved. 3. The possibility of restorative prosthetic treatment induced by extraction of impacted teeth was eliminated. 4. The psychologic relief of pubertal patient who was very sensitive to facial esthetics was gained.
It is a relatively rare clinical experience to see the impacted first molar. The first permanent molar is the key in normal occlusion. Abnormal eruption of the first permanent molars would inflict normal development of dental arches and give rise to abnormal root resorption of the adjacent teeth. Therefore, the abnormally erupted first permanent molars should be detected and treated early in order to obtain proper occlusal guidance in mixed dentition period. This report presents two cases on distally tilted and impacted mandibular first molars which were treated by surgical exposure and orthodontic traction using facial mask.
The saphenous vein has been the principal conduit for coronary bypass grafting from the beginning, circa 1970. This report briefly traces this history and concomitantly presents one surgeons experience and personal views on use of the vein graft. As such it is not exhaustive but meant to be practical with a modest number of references. The focus is that of providing guidance and perspective which may be at variance with that of others and recognizing that there may be many ways to accomplish the task at hand. Hopefully the surgeon in training/early career may find this instructive on the journey to surgical maturity.
유착된 유구치의 치료에 있어 조기 진단이 매우 중요하다. 유착으로 인하여 발생된 저위교합을 방치하는 경우 교합적인 문제가 생길 수 있으므로 정기검진을 통한 진단과 이에 따른 적절한 치료가 필요하다. 저위교합 된 유구치의 치료 방법으로 정기검진, 보존적 접근, 보철적 수복, 발치를 수반한 공간획득 등이 있다. 본 증례에서는 상악 제2유구치의 유착으로 인하여 제2소구치 치배의 변위를 보이는 두 명의 환아를 대상으로 이환치의 발거를 먼저 시행하고 고정성 혹은 가철성 교정장치를 이용하여 공간확장술을 함으로써 변위 되었던 제2소구치 치배의 정상 맹출을 유도하였으며 장기간에 걸쳐 부작용이 없었음을 보고하는 바이다.
Vocal fold injection (VFI) is widely accepted as a first line treatment in treating unilateral vocal fold paralysis and other vocal fold diseases. Although VFI is advantageous for its minimal invasiveness and efficiency, the invisibility of the needle tip remains an essential handicap in precise localization. Real-time light-guided vocal fold injection (RL-VFI) is a novel technique that was developed under the concept of performing simultaneous injection with precise placement of the needle tip under light guidance. RL-VFI has confirmed its possibility of technical implementation and the feasibility in injecting the needle from various directions through ex vivo animal studies. Further in vivo animal study has approved the safety and feasibility of the procedure when various transcutaneous approaches were applied. Currently, RL-VFI device is authorized for clinical use by the Ministry of Food and Drug Safety in South Korea and is clinically applied to patients with safe and favorable outcome. Several clinical studies are currently under process to approve the safety and the efficiency of RL-VFI. RL-VFI is expected to improve the complication rate and the functional outcome of voice. Furthermore, it will support laryngologists in overcoming the steep learning curve by its intuitive guidance.
Purpose: The purpose of this study was to determine the effect of preoperative guidance and operating room environment experience using virtual reality on increasing satisfaction with information and reducing anxiety in preoperative patients undergoing general anesthesia and local anesthesia. Methods: A non-equivalent control group quasi-experimental design was employed. The participants were 80 surgical patients from 4 wards (40 experimental group and 40 control group) of the general hospital located in Gyeonggi-do. Data collection was conducted from June to November 2023 after completing the control group survey in January 2023. Data were analyzed using Chi-square, t-test, and Mann-Whitney U test using SPSS 23.0 program. Results: Satisfaction with preoperative information was higher in the experimental group than that of the control group. Additionally, anxiety related to surgery in the experimental group was significantly lower than that of the control group. The preoperative state anxiety score in the experimental group was not significantly lower than that of the control group. Conclusions: These results suggest that providing patient education and information using virtual reality technology can not only alleviate patients' anxiety related to surgery, but also have the potential to be used as an effective intervention to improve positive patient experiences.
성공적인 상악전치부의 임플란트 식립은 많은 고려를 필요로 한다. 환자를 만족시키며 기능적이며 심미적인 보철수복을 위해서는, 치료에 앞서 정확한 진단이 선행되어야 하며 이는 구내스캔, 콘빔시티, 그리고 안면스캔을 중첩함으로 시도해 볼 수 있다. 구내스캔을 통해 얻어지는 환자의 교합양상과 콘빔시티에서 얻어지는 환자의 치조골의 양상 및 질, 그리고 안면스캔에서 얻어지는 환자의 특성과 구외평가를 어우르는 진단은 보다 성공적인 보철수복을 위한 발판이 될 수 있다. 다양한 정보의 중첩을 통해, 보철적으로 이상적인 치관의 위치를 우선순위에 두지만, 환자의 생물학적 한계를 포용하는 "탑다운"형식의 임플란트 진단은, 이를 고려한 서지컬 가이드를 3D 프린팅하고, 수술에 사용함으로써 구현되었다. 환자의 치조골 소실로 인한 임플란트 식립위치의 한계점은, 치아의 선각과 착시효과를 이용한 보철수복을 통하여 극복하려고 노력하였다. 이후, 임시수복물을 사용하고 환자가 가장 편하다고 느끼는 보철물을 장착한 뒤 디지털 교합장비(Arcus Digma II)를 이용하여 전방유도와 하악의 운동을 측정하여 이를 최종보철물에 적용하였다.
