  • Tooth eruption disturbance is seen commonly in mixed dentition or early permanent dentition. During this period of time, children experience the growth and development of craniofacial skeleton and dentition, so the impaction and eruption disturbance of permanent teeth make many problems in oral and maxillofacial areas. Eruption guidance of impacted tooth is not simple because many factors related to impaction are considered. Several factors such as etiology, tooth development stage, location of impacted teeth or eruption pathway, patient cooperation, etc. need to be taken into consideration in deciding the management of the impacted tooth. In the present study, it was assessed the etiology and impaction status of impacted tooth through various cases, and try to explain the treatment method to guide eruption by surgical exposure.

  • For decades, the standard technique for tracheostomy was the open, surgical technique. However, during the past 20 years, the use of percutaneous dilatational tracheostomy has been increased and shown to be a feasible and safe procedure in critically ill patients. The purpose of this report is to review the percutaneous dilatational tracheostomy technique, describe the role of bronchoscopy as guidance for the procedure, and identify the available evidences comparing percutaneous dilatational tracheostomy to surgical tracheostomy.

Short-Wave Infrared Fluorescence-Guided Surgery Using Indocyanine Green in a Dog with a Cutaneous Mast Cell Tumor

  • A 6-year-old spayed, female golden retriever dog was presented with a skin mass on the dorsal region of the right carpus. The cytology result of the region revealed characteristics of mast cell tumors (MCTs). Short wave-infrared fluorescence-guided surgery using Indocyanine green (ICG) was performed to determine the surgical margin of the tumor. ICG was injected intravenously 24 hours before the surgery and the patient was hospitalized and carefully monitored. During the surgery, ICG fluorescence-based surgery was performed to identify the tumor and the surgical margin. The tumor was visible, and the skin mass was resected using NIR device for the guidance of the surgical margin of the tumor. Once the resection was complete, the surgical site was again inspected with SWIR fluorescence imaging to identify residual tumor cells. The resected tumor, using ICG navigation, was classified as low-grade cutaneous MCT and the margin was complete on the histopathological result. We report herein a case of resection of a cutaneous MCT in a dog using SWIR fluorescence imaging ICG which can be potentially used for the identification of tumors and evaluation of the surgical margin for complete resection.

Guidance of Nerve Stimulator and Ultrasound for Transforaminal Epidural Steroid Injection in Lumbosacral Radicular Pain : A Single Institution Experience in Vietnam

  • Objective : This study aimed to evaluate the clinical feasibility of the combination of ultrasound and nerve stimulator guidance in transforaminal epidural steroid injections (TESIs) to manage lumbosacral chronic radicular pain. Methods : Using the combination of nerve stimulator and ultrasound guidance, TESIs were performed in 125 segments of 78 patients who presented with chronic lumbar radicular pain. Demographic characteristics and surgical outcomes were recorded on admission, pre-procedural and post-procedural for 1-week, 1-month, 3-month, and 6-month follow-ups. The result was measured using the Numeric rating scale (NRS) and Oswestry disability index (ODI). Results : Patients who received TESIs showed significant improvements on two evaluation tools (NRS, ODI), compared to that before procedure (p<0.001). No significant complications were observed for 6 months' follow-up. Conclusion : The result suggests that a combination of ultrasound and nerve stimulator guidance in transforaminal epidural injections is safe, reliable and effective for short-term management of lumbar disc herniation. It is a promising technique and has shown good results in providing intermediate pain relief.

갓세르 신경절의 고주파 열응고술 -증례 보고- (Radiofrequency Thermocoagulation of the Gasserian Ganglion -A case report-)

  • Destruction of the gasserian ganglion can be carried out by creating a radiofrequency lesion under biplanar fluoroscopic guidance. This procedure is reserved for patients who have failed various interventions for intractable trigeminal neuralgia including retro-gasserian injection of glycerol and whose physical status otherwise precludes more invasive neuro-surgical treatments such as microvascular decompression. Radiofrequency thermocoagulation of the gasserian ganglion provides a safe method of achieving long-standing relief from trigeminal neuralgia with low risk. This technique is currently emerging worldwide as the surgical treatment of choice for trigeminal neuralgia. Recently we performed a successful radiofrequency gasserian ganglionotomy, without any complication, under fluoroscopic guidance. The procedure was successful and complete pain relief was achieved for a patient who already had treatments of various interventions including microvascular decompression but never experienced pain relief.

Free Hand Pedicle Screw Placement in the Thoracic Spine without Any Radiographic Guidance : Technical Note, a Cadaveric Study

  • Thoracic pedicle screw fixation techniques are still controversial for thoracic deformities because of possible complications including neurologic deficit. Methods to aid the surgeon in appropriate screw placement have included the use of intraoperative fluoroscopy and/or radiography as well as image-guided techniques. We describe our technique for free hand pedicle screw placement in the thoracic spine without any radiographic guidance and present the results of pedicle screw placement analyzed by computed tomographic scan in two human cadavers. This free hand technique of thoracic pedicle screw placement performed in a step-wise, consistent, and compulsive manner is an accurate, reliable, and safe method of insertion to treat a variety of spinal disorders, including spinal deformity.

  • Transoral microsurgery has come a long way as a go-to surgical intervention technique for head and neck cancers. This minimally invasive procedure had gained acceptance through comparative clinical studies against radical neck surgical procedures, radiotherapy, and chemotherapy. Laser technology has vastly improved the oncological outcomes of this procedure and brought about an appreciation of transoral laser surgery (TLM) as a mainstay for re-sectioning malignant tumors along the throat. As an established procedure, TLM has undergone several upgrades regarding the different energy devices used for cutting, ablation, and hemostasis. Continued advances in automation have eventually led to surgical robotics, resulting in the emergence of transoral robotic surgery (TORS) as a viable advanced alternative for TLM. Similarly, expansions and enhancements (image-based guidance, fluorescence spectroscopy, and advanced robotic system) have also been investigated as potential upgrades for TORS. This paper reviews a selection of publications on the significant technological advancements to TLM and TORS over the past five years.

Applicability of Radioguided Occult Lesion Localization for Non-Palpable Benign Breast Lesions, Comparison with Wire Localization, a Clinical Trial

  • Background: This study was designed to compare radioguided versus routine wire localization of nonpalpable non-malignant breast lesions in terms of efficacy for complete excision, ease of use, time saving, and cosmetic outcome. Materials and Methods: Patients with non-palpable breast masses and non-malignant core biopsy results who were candidates for complete surgical lumpectomy were enrolled and randomly assigned to radioguided or wire localization groups. Radiologic, surgical, and pathologic data were collected and analyzed to determine the difficulty and duration of each procedure, ease of use, accuracy, and cosmetic outcomes. Results: This prospective randomized study included 60 patients, randomly divided into wire guided localization (WGL) or radioguided occult lesion localization (ROLL) groups. The mean duration of localization under ultrasound guidance was shorter in the ROLL group (14.4 min) than in the WGL group (16.5 min) (p<0.001). The ROLL method was significantly easier for radiologists (p=0.0001). The mean duration of the surgical procedure was 22.6 min (${\pm}10.3min$) for ROLL and 23.6 min (${\pm}9.6min$) for WGL (p=0.6), a non-significant difference. Radiography of the surgical specimens showed 100% lesion excision with clear margins, as proved by pathologic examination, with both techniques. The surgical specimens were slightly heavier in the ROLL group, but the difference was not significant (p=0.06). Conclusions: The ROLL technique provides effective, fast, and simple localization and excision of non-palpable non-malignant breast lesions.

매복된 하악 제1대구치의 맹출 유도 (ERUPTION GUIDANCE OF IMPACTED MANDIBULAR FIRST MOLAR)

  • 영구치의 매복은 다양한 빈도로 보고되며, 인구 집단의 약 5.6~18.8%에서 발생한다. 그 중에서 하악 제2대구치의 맹출장애 빈도는 약 0.06%, 하악 제1대구치는 약 0.01%로 매우 드물게 나타난다. 영구 대구치는 기능적으로 저작을 위한 중요한 교합적 지지를 제공하며 안면의 조화로운 성장발육에 중요한 역할을 담당한다. 영구 대구치의 매복시 발생되는 문제점으로는 구치부의 개방교합과 대합치의 정출, 인접치의 치근흡수와 기울어짐, 낭종의 형성, 심할 경우 하안면고경의 감소 등 여러 가지 문제점이 유발될 수 있다. 매복치의 치료 방법으로는 우선적으로 외과적노출술을 생각해 볼 수 있으며, 외과적노출술 후에 추가적인 인위적 아탈구로 자발적맹출을 기대해 볼 수도 있다. 자발적맹출이 관찰되지 않으면 교정적 강제 견인이나 외과적재위치술을 고려해 볼 수 있으며, 최악의 경우에는 발치 후 보철치료를 준비해야 할 것이다. 맹출장애를 보이는 치아의 조기진단과 치료는 중요한 의미를 갖는데, 늦은 시기에 발견될 경우 자발적 맹출력이 감소되어 치료의 성공률도 감소하고, 치료기간도 증가하며 임상적으로 다양한 합병증을 야기하기 때문이다. 따라서, 하악 제1대구치의 맹출장애를 조기에 발견할 경우 우선적으로 맹출유도를 고려해 볼 필요가 있다. 다음 두 증례는 매복된 하악 제1대구치에 대하여 외과적 노출술을 시행 후 자발적 맹출을 기대하였으나 상반된 결과를 얻었기에 보고하는 바이다.

맹출장애를 보이는 상악 전치의 맹출유도 (THE ERUPTION GUIDANCE OF IMPACTED MAXILLARY ANTERIOR TEETH)

  • 상악 전치의 맹출장애는 임상적으로 비교적 흔한 치열발육 이상으로 환자측에서 인지하기 쉬우며 심미적, 사회 심리적으로 중요한 문제가 될 수 있다. 맹출장애의 치료방법으로는 주기적 관찰, 공간 확보, 치은 절제술을 포함한 단순 노출, 교정적 견인, 외과적 노출과 교정적 견인의 병행, 발거 후 자가치아이식 등 여러 방법이 있다. 이 중 적절한 치료방법을 선택하기 위해서는 우선 환자의 내과적, 치과적 병력을 고려해야 하고 치아의 위치, 상태, 치근 형성정토 및 만곡여부, 맹출을 위한 공간 확보 여부, 주위 치조골 및 치주조직의 상태, 견인 방법, 이동에 소요되는 기간 등을 고려해야 한다. 저자는 상악 전치부의 맹출장애를 보이는 두 증례를 치험한 바, 첫 번째 증례는 상악 우측 중절치의 이소맹출과 우측 견치의 맹출지연을 주소로 내원한 환자로서, 견치를 노출시킨 후 견인을 시행하였으며 현재 우측 중절치를 배열중이다. 두 번째 증례는 상악 좌측 중절치의 맹출지연을 주소로 내원한 환자로서, 상악 좌측 중절치를 발거 한 후 자가치아이식을 시행하였다. 위 증례를 통해서 치아의 맹출력을 기대할 수 없거나 맹출방향이 비정상적인 경우에는 외과적 노출과 견인을 동반한 방법을 우선 적용할 수 있으나, 치아의 위치 이상이 심각하거나 치근의 위치가 과도하게 이탈된 경우에는 자가치아이식을 포함한 외과적 술식이 맹출유도에 도움이 된다는 사실을 확인하였다.
