Inline Skating and Prevention of The Related Injuries (인라인 스케이트와 부상의 예방)

  • The incidence of in-line skating injuries has increased with the rapid growth in the sport's popularity. The purpose of this article is to describing the type of the inline skating and document basic skills, protective equipment use, identify contributing or precipitating factors associated with injuries. Loss of control and inexperience were factors contributing to the injuries. Also protective equipment among skaters was underused. In conclusion, the safety education program for inline skating, optimal design and efficacy of protective equipment should identify with further study.

The Management of Chronic Wound with Modified Portable Negative Pressure Therapy (만성 창상의 치료에서 이동식 음압요법의 신고안)

  • Purpose: V.A.C. is a new concept which is closed dressing with negative pressure to promote wound healing. It has been widely used as the treatment of chronic and acute wounds such as pressure sores, burns, stasis ulcers, and other complicated wounds. However It has disadvantages such as high cost and the need of specific equipment. In this article, we described new method to overcome these disadvantages. Methods: We made newly innovated equipment with 50 cc syringe and spring to create negative pressure. From May 2006 to May 2007, we applied it to two patients with chronic wound. Results: The treatment period was 5 weeks for one case and 3 weeks for the other case. Both patients were healed completely without admission and wound healing was accelerated. During follow-ups, there were no complications. The mean cost for single dressing was 9,590 won. Conclusion: Modified portable negative therapy using newly innovated equipment could accelerate wound healing better than conventional dressing. It lowers the number of dressings, saves cost of treatment, and enables treatment as outpatient basis.

  • With the continuous incorporation of new technologies and advancements in surgical technique, microsurgical procedures around the world have generated a higher success rate and innovative procedures are now possible. In this setting, limitations regarding accessibility and acquisition of medical resources and equipment for these types of operations may be difficult in developing countries. We believe the dexterities of surgeons go beyond the surgical technique, meaning that we are able to use everyday materials to re-create affordable solutions that can be used during surgery in a safe way. This manuscript presents our experience with different surgical instruments and gadgets, developed out of necessity, to improve microsurgical interventions in developing countries.

Lymphaticovenular Anastomosis: Superficial Venous Anatomical Approach

  • Background Lymphaticovenular anastomosis (LVA) is an effective, functional treatment for limb lymphedema. This study reports an alternative surgical approach to lymphedema treatment without the use of indocyanine green mapping. Methods A retrospective analysis was performed on 29 consecutive lymphedema patients who underwent LVAs from January 2015 to December 2020, whereby incisions were made along the anatomy of the superficial venous systems in both upper and lower extremities around the joint areas. The evaluation included qualitative assessments and quantitative volumetric analyses. Result The mean number of anastomoses was 3.07, and the operative time was 159.55 minutes. Symptom improvement was recorded in 86.21% of the patients, with a mean volume reduction of 32.39%. The lymphangitis episodes decreased from 55.17% before surgery to 13.79% after surgery, and the median number of lymphangitis episodes per year decreased from 1 before surgery to 0 after surgery. Conclusions The superficial venous anatomical approach is an easy way to start a lymphedema practice using LVA without other advanced surgical equipment. With this reliable technique, microsurgeons can perform LVA procedures and achieve good results.

Requirements for Cerebrovascular Surgery in Comprehensive Stroke Centers in South Korea

  • Objective : Cerebrovascular disease (CVD) was the third most common cause of death in South Korea in 2014. Evidence from abroad suggests that comprehensive stroke centers play an important role in improving the mortality rate of stroke. However, surgical treatment for CVD is currently slightly neglected by national policy, and there is still regional imbalance in this regard. For this reason, we conducted a survey on the necessity of, and the requirements for, establishing regional comprehensive cerebrovascular surgery centers (CCVSCs). Methods : This investigation was performed using the questionnaire survey method. The questionnaire was consisted with two sections. The first concerned the respondent's opinion regarding the current status of demand and the regional imbalance of cerebrovascular surgery in South Korea. The second section asked about the requirements for establishing regional CCVSCs. We sent the questionnaire to 100 board members of the Korean Society of Cerebrovascular Surgeons. Results : Most experts agreed that cerebrovascular surgery patients were concentrated in large hospitals in the capital area, and 83.6% of respondents agreed that it was necessary to alleviate the regional imbalance of cerebrovascular surgery. With regards to personnel, over 90% of respondents answered that at least two neuro-vascular surgeons and two neuro-interventionists are necessary to establish a CCVSC. Regarding facilities, almost all respondents stated that each CCVSC would require a neuro-intensive care unit and hybrid operating room. The survey asked the respondents about 13 specific neurovascular surgical procedures and whether they were necessary for a regional CCVSC. In the questions about the necessity of cerebrovascular surgical equipment, all seven pieces of equipment were considered essential by all respondents. A further five pieces of equipment were considered necessary on site: computed tomographic angiography, magnetic resonance angiography, conventional angiography, surgical microscope, and surgical navigation. Our results may provide a basis for future policy regarding treatment of cerebrovascular disease, including surgery. Conclusion : Raising the comprehensiveness of treatment at a regional level would lower the national disease burden. Policies should be drafted regarding comprehensive treatment including surgery for cerebrovascular disease, and related support plans should be implemented.

  • Surgery equipment in operating room is very important at treatment procedure of patients. In this circuit of large equipment, a sudden change of current make big problem such as short circuit. when the current is converted suddenly, the current converter becomes in saturate and it caused by the induction curve of the inductor. in this case, the rates of the primary and secondary winding are broken and it becomes a open circuit. in this paper, we will look around the current transform of the primary and secondary winding when current converter becomes in saturate.

  • High tibial osteotomy (HTO) is a widely accepted treatment for unicompartmental osteoarthritis of the knee and other lower extremity deformities, particularly for young and active patients. However, it is generally recognized as a technically demanding procedure. Thus the limitations of conventional surgical methods have been reported. Currently, the use of computer-aided surgery (CAS) system for successful surgery was increased. However it has been reported many problems such as expensive equipment, infection or fracture caused by inserting half-pin and delaying surgery. Therefore we propose a novel method which can be tracked by using tracking-pin inserted in arbitrary locations of the femur and tibia, and fluoroscopic images obtained from G-arm of antero-posterior and lateral-medial.

The Effect of Water-Filtered Infrared-A (wIRA) on Body Core and Body Surface Temperatures in Anesthetized Rabbits Maintained with Isoflurane

  • The purpose of this study was to evaluate body temperature changes in rabbits anesthetized using water-filtered infrared-A (wIRA). Ten rabbits were used for this study. For the experimental group (wIRA group; wG, n = 5), the experimental equipment was used and irradiated using wIRA. The control group (CG, n = 5) did not have any warming device. There were no significant differences in heart rate, respiration rate, and end tidal CO2 (EtCO2) between wG and CG. After 80 min, the core body temperature of wG rabbits was significantly higher than that of CG rabbits. The surface body temperature was significantly higher while receiving wIRA support at all time points after 5 min. In conclusion, in rabbits under inhalation anesthesia, the surface body temperature was better maintained than the core body temperature when using wIRA.

  • The pneumomediastinum after a dental treatment occurs rarely and shows almost good prognosis, however it is potentially life-threatening complication. Here we report a case of pneumomediastinum, occurred by air bubbles originated from dental high speed equipment, via head and neck fascial space with literatures review.

Study of Efficiency Test Evaluations Method for Imaging Device Based Laser Equipment (영상장치 기반 정밀치료용 레이저 수술기의 성능 평가 방법 개발)

  • Medical laser equipment using optical energy is used to surgery and treat diseases by destroying and removing tissue. Domestic laser equipment has been used steadily in the skin and cosmetics sectors and has been changed to radiate high-power energy in a wide range to shorten patient treatment time. However, side effects such as burns and damage of normal tissues occurred. To solve this problem, techniques for detecting lesions using an imaging device and selectively radiating the laser have been developed. In this study, we proposed an evaluation method to evaluate the safety and performance of target detection accuracy, laser irradiation accuracy and motion protection device technology derived from product analysis and investigation. Finally, the validity of the evaluation method was evaluated by evaluating the imaging device based laser equipment as the proposed evaluation method.
