The Effect of Seat Incline Angle in Hemiplegic Patients' Standing up Training

  • Objective: This study analyzes the effect of angle conditions of rehabilitation equipment used for supporting hemiplegic patients on their rehabilitation training for standing action. The study was performed by adjusting the rear angle of seat inclination through a motion analysis. Background: Owing to a loss of muscle rigidity and degradation of muscle control ability, hemiplegic stroke patients suffer from asymmetrical posture, abnormal body balance, and degraded balance abilities due to poor weight-shifting capacity. The ability to shift and maintain one's weight is extremely essential for mobility, which plays an important role in our daily life. Thus, to improve patients' ability to maintain weight evenly and move normally, they need to undergo orthostatic and ambulatory training. Method: Using a motion analysis system, knee movements on both hemiplegic side and non-hemiplegic side were measured and analyzed in five angles ($0^{\circ}$, $10^{\circ}$, $30^{\circ}$, $50^{\circ}$, $70^{\circ}$) while supported by the sit-to-stand rehabilitation equipment. Results: The knee movements on both sides increased as the angle increased in angle support interval to support a hemiplegic patient's standing up position. In standing up interval, a hemiplegic patient's knee movement deviations on both sides decreased, and the movement differences between hemiplegic and non-hemiplegic legs also decreased as the angle increased. Conclusion: The results of this study showed that the rehabilitation effectiveness increases as the angle increases, leading to a balanced standing posture through the decrease of movement difference between hemiplegic and non-hemiplegic sides and an improved standing up ability through the increase of knee movement on both sides. However, angles higher than $50^{\circ}$ didn't provide a significant effect. Therefore, a support angle under $50^{\circ}$ was proposed in this study. Application: The results of this study are expected to be applicable to the design of sit-to-stand support equipment to improve the effectiveness of the rehabilitation process of hemiplegic patients.

A new dynamic construction procedure for deep weak rock tunnels considering pre-reinforcement and flexible primary support

  • The current theories on the interaction between surrounding rock and support in deep-buried tunnels do not consider the form of pre-reinforcement support or the flexibility of primary support, leading to a discrepancy between theoretical solutions and practical applications. To address this gap, a comprehensive mechanical model of the tunnel with pre-reinforced rock was established in this study. The equations for internal stress, displacement, and the radius of the plastic zone in the surrounding rock were derived. By understanding the interaction mechanism between flexible support and surrounding rock, the three-dimensional construction analysis solution of the tunnel could be corrected. The validity of the proposed model was verified through numerical simulations. The results indicate that the reduction of pre-deformation significantly influences the final support pressure. The pre-reinforcement support zone primarily inhibits pre-deformation, thereby reducing the support pressure. The support pressure mainly affects the accelerated and uniform movement stage of the surrounding rock. The generation of support pressure is linked to the deformation of the surrounding rock during the accelerated movement stage. Furthermore, the strength of the pre-reinforcement zone of the surrounding rock and the strength of the shotcrete have opposite effects on the support pressure. The parameters of the pre-reinforcement zones and support materials can be optimized to achieve a balance between surrounding rock deformation, support pressure, cost, and safety. Overall, this study provides valuable insights for predicting the deformation of surrounding rock and support pressure during the dynamic construction of deep-buried weak rock tunnels. These findings can guide engineers in improving the construction process, ensuring better safety and cost-effectiveness.

Diagnosing Parkinson's Disease Using Movement Signal Mapping by Neural Network and Classifier Modulation

  • Parkinson's disease is a growing and chronic movement disorder, and its diagnosis is difficult especially at the initial stages. In this paper, movement characteristics extracted by a computer using multilayer back propagation neural network mapping are converted to the symptoms of this disease. Then, modulation of three classifiers of C4.5, k-nearest neighbors, and support vector machine with majority voting are applied to support experts in diagnosing the disease. The purpose of this study is to choose appropriate characteristics and increase the accuracy of the diagnosis. Experiments were performed to demonstrate the improvement of Parkinson's disease diagnosis using this method.

The Effect of the Base of Support on Anticipatory Postural Adjustment and Postural Stability

  • Purpose: This study was to identify the anticipatory postural adjustment (APA) mechanism which is represented by the onset time of trunk muscles and the displacement of the center of pressure (COP) according to the different base of support (BOS) during upper extremity movement. Methods: Thirty healthy subjects (14 males, 16 females) participated in this study. The movement was performed for 10 trials during each of various BOS (shoulder - width double leg stance, narrow base double leg stance, tandem stance, non-dominant single leg stance) at the 1.2 Hz frequency. Electromyography was used to measure muscle onset time and biorescue was used to measure characteristics of the displacement of COP. Surface bipolar electrodes were applied over the right deltoid anterior, right latissimus dorsi, both rectus abdominis, both internal oblique and both erector spinae. The data were analyzed by repeated one-way ANOVA and Duncan's post hoc test. Results: The study has revealed following. There were significant differences with muscle onset time in each BOS (p<0.01). There were significant differences in characteristics of the COP in each BOS (p<0.01). Conclusion: The study found that the more narrowed the basis requires the more rapid anticipatory postural control in contralateral postural muscle when the upper extremity movement is performed.

견갑대 운동 기능장애에 대한 치료 접근 (Treatment approach for the movement dysfunction of the shoulder girdle)

  • Functional stability is dependent on integrated local and global muscle function. Movement dysfunction can present as a local and global problem, though both frequently occur together. To good understand how movement induces pain syndrome, the optimal actions and interaction of the multiple anatomic and functional systems involved in motion must be considered. Minor alterations in the precision of movement cause microtrauma and, if allowed to continue, will cause macrotrauma and pain. These alteration of the movement result in the development of compensatory movement and movement impairment. Muscle that become tight tend to pull the body segment to which they are attached, creating postural deviation. The antagonistic muscles may become weak and allow postural deviations due to lack of balanced support. Both hypertonic and inhibited muscles will cause an alteration of the distribution of pressure over the joint(s) that they cross and, thus, may not only result from muscle dysfunction, but produce joint dysfunction as well. Alteration of the shoulder posture and movement dysfunction may sometimes result in compression of neurovascular structures in the shoulder and arm. There is a clear link between reduced proprioceptive input, altered motor unit recruitment and the neurovascular compression. This report start with understanding of the impaired alignment, movement patterns and neuromuscular compression of the shoulder girdle by movement impairment to approach method of the movement dysfunction.

선택적 기능 동작 평가의 연구 동향 분석 (Investigation of Research Trends in the Selective Functional Movement Assessment (SFMA))

  • Purpose : Selective functional movement assessment(SFMA) is a movement-based assessment tool designed for clinicians to diagnose and treat pain and functional physical problems. This concept is used for assistance in occupations related to movement, including clinical medical personnel, such as physical therapists, physical education instructors, and athlete coaches. Although this concept is widely used by clinicians, research and scientific proofs are relatively insufficient. This study aimed to review the literature on the understanding, reliability and effectiveness of SFMA, and to consider its role in the future of rehabilitation. Methods : In this review, a total of nine articles were selected according to the eligibility criteria of three major thesis topics. The main topics covered in the literature are reliability and effectiveness of clinical use of SFMA. To gather research articles, we searched official term 'Selective Functional Movement Assessment'. Among the searched 60 studies, nine were mentioned in this study that contained overlapping information and matched our desired topic. We reviewed four reliability analyses, four case reports, and one experimental study. Results : Reliability has an intermediate degree between high raters and within raters. The validity of the SFMA system is influenced by a combination of experience and logic; hence, further improvement is needed. Therefore, if the intervention was effective based on the diagnosis result, then biomechanical evidence is necessary to further support the claim. Conclusion : In future research, to use SFMA as a diagnostic tool with high accuracy, it is necessary to improve the reliability of the main problem through breakout, support for guidelines and validity and efficiency.

철도차량의 이동 특성을 고려한 네트워크 이동성 지원을 위한 효율적인 Cave of Address 구성 방안 (An Efficient Care of Address Configuration Scheme for Network Mobility Support based on Movement Pattern of a Train)

  • In this paper, we propose an efficient Care of Address (CoA) configuration scheme for network mobility support in the railroad environment. Mobile routers in a train visit each access router in the fixed order. Such regular movement characteristic of a train enables a current subnet access router to inform the next subnet access router of the mobile routers' interface identifiers before the mobile routers move to the next subnet access router. So, the next subnet access router can make care of addresses for the mobile routers in advance. The numerical results show that the proposed scheme outperforms the stateless scheme with respect to CoA configuration time.

Effect of Pelvic Compression Belt on Abdominal Muscle Activity, Pelvic Rotation and Pelvic Tilt During Active Straight Leg Raise

  • Background: Uncontrolled lumbopelvic movement leads to asymmetric symptoms and causes pain in the lumbar and pelvic regions. So many patients have uncontrolled lumbopelvic movement. Passive support devices are used for unstable lumbopelvic patient. So, we need to understand that influence of passive support on lumbopelvic stability. It is important to examine that using the pelvic belt on abdominal muscle activity, pelvic rotation and pelvic tilt. Objects: This study observed abdominal muscle activity, pelvic rotation and tilt angles were compared during active straight leg raise (ASLR) with and without pelvic compression belt. Methods: Sixteen healthy women were participated in this study. ASRL with and without pelvic compression belt was performed for 5 sec, until their leg touched the target bar that was set 20 cm above the base. Surface electromyography was recorded from rectus abdominis (RA), internal oblique abdominis (IO), and external oblique abdominis (EO) bilaterally. And pelvic rotation and tilt angles were measured by motion capture system. Results: There were significantly less activities of left EO (p=.042), right EO (p=.031), left IO (p=.039), right IO (p=.019), left RA (p=.044), and right RA (p=.042) and a greater right pelvic rotation angle (p=.008) and anterior pelvic tilt angle (p<.001) during ASLR with pelvic compression belt. Conclusion: These results showed that abdominal activity was reduced while the right pelvic rotation angle and anterior pelvic tilt angle were increased during ASLR with a pelvic compression belt. In other words, although pelvic compression belt could support abdominal muscle activity, it would be difficult to control pelvic movement. So pelvic belt would not be useful for controlled ASLR.

지역사회 시민운동단체(NGO)의 실태와 발전방안 (The Actual Situation and Development Strategies for Community Based NGO Movement)

  • The purposes of this study were to draw out the situational problems of the community based NGO (Non Governmental Organization) movement and to suggest development strategies for NGO movement in Korea. The study was conducted through extensive review of literature and analysis of the related data. The major findings of this study were as follows; As the development of democracy and localization process were in progress, the community based NGO movement was also developing quantitatively, however not yet qualitatively. The NGO Times Net, a internet newspaper for NGOs revealed that the number of community based NGO was 3,397 as in 2004, which was 5 times larger than 597 central based or Seoul located NGOs. However, some of the problems identified were lower participation of local people, quality of the movement compare to the original purposes of the movement, and the problems in local leadership, etc. In order to overcome various problems, this study suggested some strategies for development such as enhancement of civil education, expansion of voluntary spirits, financial support for NGOs, better mutual understanding among the GOs (Government Organizations), POs(Profit Organizations) and NGOs. Self evaluation and innovative strategies will be needed for further development of NGO movement in Korea.

영화 <카트>가 수용자의 정치적, 사회적 인식에 미치는 영향 (Effect of movie on viewer's political and social recognition)

  • 본 연구는 비뉴스 미디어로 분류할 수 있는 영화가 수용자의 정치적, 사회적 태도를 점화할 수 있는지 살펴보기 위해서, 대학생 피험자 집단을 대상으로 비정규직 근로자들의 노동운동을 소재로 한 영화 '카터'의 관람 유무가 수용자의 인식에 미치는 영향을 살펴본 실험연구이다. 본 연구에서는 98명의 대학생 피험자들을 처치 집단과 비처치집단 등 2개의 집단으로 나누어, 종속변인인 비정규직 노동자 문제의 심각성, 노동 운동의 필요성, 야당 지지도와 현 정부의 지지도 등을 설문조사로 측정하여 변량 분석하였다. 분석결과 영화 '카트'를 관람한 피험자들은 영화 '카트'를 관람하지 않은 피험자들보다 비정규직 문제를 더 심각하게 인식하는 사회적 성향이 있고, 야당을 더 적극적으로 지지하는 정치적 성향이 있었다. 그러나 '카트'를 관람한 피험자들이 이 영화를 관람하지 않은 피험자들에 비해 노동운동을 더 적극 지지할 것으로 예측한 연구가설과 정부에 대한 지지도가 낮을 것으로 예측한 연구가설은 지지되지 못했다.
