Purpose: Pnumosinus dilatans is a rare disease that one or more of the paranasal sinuses are dilatated without functional alteration. The most frequently involved sites are frontal and sphenoid sinus. Facial asymmetric contour is the most common signs and nasal obstruction and pain may be combined. The purpose of reconstruction of pneumosinus dilatans is two-fold, to re-establish a permanent pressure equilibrium of sinus and to correct the possible facial deformities. Methods: We present a case of a 24-year-old female with a 7-year history of protrusion of right malar region. Plane radiography and computed tomography detailed an abnormal expansion of the right maxillary sinus without thinning of bony wall, leading to diagnosis of maxillary pneumosinus dilatans. Surgical decompression and maxilloplasty were achieved by ostectomy of anterior wall of maxillary sinus and repositioning of removed bony fragment with miniplate. Results: Post operative course was uneventful without complication and malar height became symmetric by physical and radiologic examination. Conclusion: We corrected successfully pneumosinus dilatans of maxillary sinus by surgical decompression and maxilloplasty. For this case, we reviewed literature related to this topic.

일반적으로 어지럼증은 여러 가지 복합적인 원인에 의해서 발생하지만 그 중에서 신경계에 속하는 전정계통의 기능 장애에 의한 증상이 가장 환자에게 심한 증상으로 다가오며, 구역질과 구토를 동반하게 된다. 이러한 어지럼증에 대한 치료는 약물요법, 수술요법, 재활치료 등이 있으며, 약물요법이나 수술요법은 대체적으로 후유증의 위험 때문에 재활치료요법인 전정재활훈련을 많이 시행하고 있다. 전정재활훈련은 안구훈련, 자세안정훈련, 보행훈련 등이 있는데 그 중 보행훈련은 의사 또는 전문치료사의 감독하에 일정한 공간에서 진행되므로 시간적, 공간적 부담이 가중되어 환자들이 훈련을 진행기 불편한 단점을 가진다. 이를 해결하고자 본 논문에서는 증강현실 기술을 활용해 사용자가 직접 보행재활훈련을 진행 할 수 있는 보행훈련 콘텐츠를 구현하였다. 추후 어지럼증 환자를 대상으로 한 임상시험을 통해 의료 환경에서 사용 가능한 어지럼증 재활 콘텐츠로 활용 가능할 것으로 기대한다.

이 글은 신상옥이 남북한에서 각각 연출한 "춘향전" 원작 영화들을 '몽룡' 캐릭터에 주목해 분석하려는 시도다. 사실상 그들 영화는 남북한 정치사의 체제 전환기에 등장했다. 대한민국에서 연출한 <성춘향>은 이승만 정권이 붕괴한 이후, 제2공화국 시기에 제작되었고, 북한에서 연출한 <사랑 사랑 내 사랑>은 김일성에서 김정일로의 권력 이양기에 제작되었다. 그러한 정치사적 변화에 주목해 보면, 몽룡의 성격화 과정에 당대 남북한 사회가 배태한 '권력자상'이 읽힌다고 할 것이다. 먼저 <성춘향> 속 몽룡은 춘향의 헌신적 포용과 희생에 낭만적 순정으로 보은하는 사적 연인의 이미지가 강하다. 몽룡에게서 권력자로서의 면모를 읽는다면, 민주적 지도자, 혹은 조정자의 이미지를 검출할 수 있겠다. 반면, <사랑 사랑 내 사랑> 속 몽룡은 계급혁명을 완수하는 영웅적 권력자의 이미지가 강하다. 민중의 열망을 수렴해 애민선처의 정신을 실천하는 전복적 개혁가로서의 성격이 두드러지는 것이다. 이 연구를 통해 고전을 원작으로 한 흥행 영화들이 공시적 조건에서 지니는 의미를 비교 분석하는 연구들이 확산되길 기대한다.

Purpose: Fingertip injuries are the most common hand injuries and may lead to significant disability. Knowledge of fingertip anatomy is mandatory to treat these injuries effectively. All surgical techniques used for coverage of fingertip injuries must be based on the nature of the injury and the patient's age. Many authors have studied the method of fingertip reconstruction because goals of these treatments should include maintaining length, sensibility, motions, and appearance. The purpose of this study is to evaluate the effect of digital artery perforator flap for fingertip reconstruction without aesthetic and functional problems. Methods: From November 2006 to March 2007, the authors performed fingertip reconstruction on 3 fingers of 3 patients, aged between 41 to 54 years (average age, 47 years) using digital artery perforator flap. Results: All fingers recovered successfully and there were no necrosis of the flap. We followed up 3 cases more than 5 months. Light touch and temperature sensation could be detected in all flaps and the static two-point discrimination test was 8 mm. Conclusion: This flap is an alternative choice for coverage of fingertip defects. This method also takes short time to procedure and to recovery. The digital artery perforator flap has never been reported in Korea, however it is considered as a useful method for treatment of fingertip injury.

In 1983, Sten H$\ddot{a}$kanson first reported the clinical safety and efficacy of retrogasserian glycerol injection as a treatment of typical trigeminal neuralgia in 96 of 100 patients during a follow-up period of 1~6 years. Since September 1987, we have injected sterile pure glycerol into the trigeminal cistern using an anterior percutaneous approach via the foramen ovale (H$\ddot{a}$rtel route) for treatment of tic douloureux in 12 patients who were suffering from attacks pain of following discontinuation of carbamazepine. The results were as follows; 1) Eight patients were completely free from pain attacks with a single dose of glycerol (0.4 ml). The remaining four patients needed a second dose (0.4 ml) several days later following the single dose. The degree of patient's subjective satisfaction by those injections was very good in 11 and fair in one. 2) During the follow-up period (1~13 months), persistent sensory deficit as determined by the pin prick test, appeared to be mild in 10 and moderate in one patient. There was no sensory deficit in one patient. further attacks of pain from those injections were still noted. 3) As a transient complication, there was headache in all patient, facial hematoma in 4, nausea and vomiting in two each, and vertigo and herpes simplex in one each. In conclusion, we confirmed that the above glycerol injections into the trigeminal cistern were clinically very effective as a treatment of tic douloureux even though the follow-up period was short.

Subcutaneous cervicofacial, mediastinal emphysemas are complications associated with head and neck surgery, trauma, infectious processes, tooth extraction. Drill cooling stream and dental syringe air ject are the sources of high pressure air that may enter exposed soft tissue. Since the introduction of the high-speed air turbine drill in the 1960s, The incidence of iatrogenic subcutaneous emphysema has increased. Most cases begin to resolve after 2 to 3 days and residual swelling is usually minimal at the end of 7 to 10 days. Surgical approach is not advised because it is likely to be ineffective. The differential diagnosis of neck swelling after dental procedure includes hematoma, cellulitis, angioedema, allergic reaction, subcutaneous emphysema. We report a rare case of patient with subcutaneous cervicofacial emphysema and mediastinal emphysema secondary to third molar extraction.

Idiopathic scrotal calcinosis is rare entity, characterized by the presence of multiple, firm nodules within the dermis of scrotal skin. The lesions are evident as asymptomatic, round, firm, yellowish papules, from several millimeters up to 1 cm in diameter, with a tendency to occur symmetrically. Because polypoid formation of the calcified cysts is unusual in scrotal calcinosis, we report our experience of a case of scrotal calcinosis with cysts having polypoid appearence. A 34-year-old man was admitted to our department with painless, firm, widespread nodules within the scrotum. Patient started to have these lesions at puberty and progressed to a polypoid form over time. There was neither surgical nor medical treatment had been attempted before our treatment. Physical examination revealed localized polypoid cysts in addition to multiple small, firm nodules within the scrotal skin. No underlying anomalies were detected in the physical examination and laboratory analysis. We experienced the treatment of polypoid idiopathic scrotal calcinosis and a case is presented with the review of literatures.

최근 스마트폰의 보급과 더불어 근거리 무선 통신의 발달로 인해 다양한 서비스들이 연구 개발되고 있다. 한국에 방문하는 외래 관광객 수는 꾸준히 증가하고 있는 추세이며, 이 중 중국인 관광객은 2013년 기준 전체 방한 외국인 관광객 중 약 35.5%를 차지하였다. 중국인 관광객의 82.8%가 쇼핑을 목적에 두고 있으며, 그 중 향수, 화장품, 의류 항목에 가장 많은 여행시간을 투자하고 있다. 또한 방한 중국인 관광객 중 대다수가 인터넷을 사용하여 관광 정보를 입수 하고 있지만, 이러한 정보를 편리하게 제공할 서비스는 부족한 상황이다. 본 논문에서는 현재 발달되고 있는 근거리 무선 통신 중 NFC와 Beacon을 활용하여 안드로이드 기반의 관광 큐레이션 플랫폼을 구현 하였다. NFC 태그를 활용하여 쇼핑, 관광 관련 정보 및 정확한 제품 및 서비스 정보를 제공하고, Beacon을 이용하여 매장정보 및 각종 마케팅 프로모션과 이벤트 정보를 수신하도록 하였다. 추가적으로 현재 많이 사용되고 있는 Zxing 라이브러리를 활용한 QRCode 및 Barcode 리더기를 활용하여 스캔을 통한 제품 정보를 확인 가능하도록 구현하였다.

Purpose: Despite a high frequency of acquired nail disease, congenital absence of the nail, also called as anonychia, is a rare anomaly. It may be seen as an isolated of phalangeal bone(ectrodactyly), nail-patella syndrome, birth trauma, impaired peripheral circulation, alopecia areata, and pemphigus, idiopathic atrophy of the nail, bullous drug eruptions, periodic shedding, lichen planus, Stenvens-Johnson syndrome and so forth. Methods: We have experienced a rare case of 40-day-old neonate, suffering from intrauterine growth retardation, but without familial history, chromosomal anomalies or any other diseases. Results: There was no nail on left 5th finger and distal phalangeal bone of same finger. So, We diagnosed as Congenital Anonychia with ectrodactyly of 5th Finger. Conclusion: We report this case as congenital anonychia of 5th finger which have developed from underlying distal phalangeal ectrodactyly. We also review other reported cased in the literatures.

Toll-like receptors (TLRs) are one of the families of pattern recognition receptors (PRR) operating in the innate immunity. TLRs have the ability to recognize relatively conserved microbial components, which are generally referred to as pathogen-associated molecular patterns (PAMPs). The activation of TLRs signaling leads to the activation of $NF-{\kappa}B$ and the expression of pro-inflammatory gene products such as cytokines and inducible nitric oxide synthase (iNOS). To evaluate the therapeutic potential of pristimerin, which is a naturally occurring triterpenoid compound from Celastraceae plants, iNOS expression induced by MALP-2 (TLR2 and TLR6 agonist), Poly[I:C] (TLR3 agonist), or LPS (TLR4 agonist) were examined. Pristimerin suppressed the iNOS expression induced by MALP-2, Poly[I:C], or LPS. These results suggest that pristimerin can modulate TLRs signaling pathways leading to decreased inflammatory gene expression.
