A Study on Policy Improvement for Ensuring the Effectiveness of Suicide Prevention Law (「자살예방 및 생명존중 문화 조성을 위한 법률」의 실효성 확보를 위한 정책적 개선 방안 - 「개인정보보호법」과의 충돌문제 해결을 중심으로 -)

  • The essential policy of suicide prevention is to continuously manage and treat suicide attempted people through data base related to suicide retry rate and follow-up study report. In Korea, only few people are allowed to follow-up by the Personal Information Protection Act. As a result, the research participation rate and the service participation rate are rather low, so that the research participants is limited to a part of the suicide attempted people. Therefore, the policy proposals to be improved in the Ministry of Health and Welfare Act were examined comparatively in order to increase the practical utilization of the suicide prevention about Article 14 and Article 20 of the Suicide Prevention Act. As a criterion for policy improvement, measures for non-discrimination of information to be considered in terms of technical and ethical dimensions and non-profit research and medical information for medical purposes were suggested. In addition to the severity of the suicide, the suicide risk was assessed and the criteria for the objective assessment of the follow-up observation were considered in consideration of the severity of the suicide.

A Convergent Study of Variables Influencing on Suicide Ideation of Adolescents in Multicultural Family (다문화가족 청소년의 자살생각에 영향을 미치는 요인에 대한 연구)

  • The purpose of this study was to identify socio-demographic variables, psychological variables and socio-relational variables influencing on suicide ideation of multicultural families. The participants were 186 multicultural adolescents. Data analysis was used descriptive statistics, correlation and multiple regression with the SPSS 23.0 program. The major findings were as; Self-esteem is highest in 3.40, hopelessness is 3.34, discrimination experience is 3.14, school violence victim is 2.78 and suicide ideation is 1.98. Suicide ideation was positively correlated with stress, hopelessness, depression, school violence victim and discrimination experience except self-esteem. Multicultural adolescents' suicide ideation was positively affected by significant hopelessness, depression, stress, school violence victim and discrimination experience. And the findings was provided basic report for the intervention program on suicide ideation regarding to psychological variables and socio-relational variables of multicultural adolescents.

The Correlation of Psychopathology and Suicide in North Korean Refugee Adolescents (탈북 청소년의 정신병리와 자살과의 관련성)

  • Objectives : This study was designed for the purpose of the relationship between psychopathology and suicide in North Korean Refugee Adolescents. Methods : The subjects were 104 North Korean Refugee Adolescents. We investigated the The Korean version of the Center for Epidemiologic Studies for Depression, Revised Children's Manifest Anxiety Scale and Rosenberg Self-Esteem Scale. We also investigated the suicide ideation, planning and attempt. The self-report was taken to the North Korean Refugee Adolescents. Results : The Center for Epidemiologic Studies for Depression Score(Z=-4.050, p<0.001), Revised Children's Manifest Anxiety Score(t=-2.757, p<0.01), Rosenberg Self-Seteem Score(Z=-2.328, p<0.05) were significantly associated with the suicide ideation in North Korean Refugee Adolescents. In logistic regression analysis, only The Center for Epidemiologic Studies for Depression Score(p<0.005) expected the suicidal ideation. Conclusions : The management of depression would be helpful to reduce the suicide in North Korean Refugee Adolescents.

A Case Study on Workers' Compensation Approval for a Hospital Nurse's Suicide (병원 간호사 자살에 대한 산업재해 승인 사례연구)

  • Purpose: This study aimed to examine the process from occurrence of a hospital nurse's suicide to workers' compensation approval, responses of the parties involved, issues debated during approval deliberations, and significant policy changes resulting from the incident. Methods: We conducted in-depth interviews with involved parties and collected various documents, including newspaper articles, forum proceedings, and the agency report on determination of workers' compensation. Content analysis was performed on the collected data. Results: A Joint Task Force continuously reported its progress and findings through mass media such as newspaper, radio, and TV. These activities exerted pressure on a government agency to conduct an occupational disease review and significantly impacted the workers' compensation approval. The agency recognized associations between the hospital's inadequate nurse training and the suicide but did not confirm the excessive overtime and workplace harassment experienced by the nurse as causes of the suicide. This case's media coverage and impact resulted in a law prohibiting workplace harassment and a hospital system dedicating at least one nurse to training activities. Conclusions: This incident had a significant social impact as the first case of workers' compensation approval for a hospital nurse's suicide. However, the case produced no structural changes in nurses' working conditions such as heavy workloads.

Suicide in Jeonbuk region: Psychological autopsy study (심리적 부검을 통한 전북지역 자살 실태에 관한 연구)

  • This study is aimed to analyse the causes and characteristics of suicide in jeonbuk region using psychological autopsy. Psychological autopsy is a technique to get comprehensive analysis on suicidal death by collecting data from interviewing suicide victim's family, relatives, and friends added with additional data of police report, hospital records, and suicide victim's lifetime records for objective evidence. For the data from 42 cases(male 24, female 18) of suicides that occurred between May 2013 and April 2014 with agreement for psychological autopsy, we analyzed a survey that recorded by interviewers who had a consultation based on half-structured psychological interview panel. As the result, out of 42 cases, 30(71%) had previous suicidal attempts and 21(50%) had a family history of suicides. Psychiatric disorders(38%) was the most common cause of suicide followed by interpersonal problems(31%). Among psychiatric problems, depressive disorder was the most common. many suicide victims had previously received psychiatric treatments but discontinued the therapy later on. The results showed that suicide is strongly related to mental disorders and psychiatric therapy should be continued until full recovery. Preventive efforts should focus on persons who have a history of previous suicidal attempts and have a family history of suicide. Implications of these findings are discussed and directions for future research are advanced.

  • Cancer is a life crisis which inflicts major psychological and physical trauma upon the victim. Most of the cancer patients suffer from major depression, profound frustration, and impaired social adjustment. Therefore suicidal ideation and suicidal attempt are also becoming a serious threat to cancer patients and their families. Hospice is patient-centered, and accepts the inevitability of 'death' while simultaneously being life-affirming. Even though there is no chance of physical cure, there is much scope for psychosocial and spiritual healing. Most of cancer patients who commit suicide suffer hem many mental problems. Hospice specialists must play an important role in evaluating and managing emotional or behavioral problems associated with suicidal ideatior and are also are expected to serve as informed commentators regarding suicide. It is crucial that hospice specialists define their role and develop clinical skills to intervene in suicidal event effectively. A systematic approach to suicidal cancer patients is a essential, and there is need for specific training for all hospice professionals. In this case report, the author introduced knowledge and clinical guidelines for a desirable approach to suicidal cancer patient.

Progressive nicotine poisoning by multiple transdermal nicotine patches

  • The pharmacokinetic properties of transdermal nicotine patches (TNPs) are different from those of other routes of nicotine administration; further, acute nicotine poisoning by TNPs may present with different clinical features. In the present report, we describe the case of a 23-year-old woman who was admitted to emergency department (ED) at Jeju National University Hospital with loss of consciousness. Five hours before the ED visit, she used multiple TNPs to attempt suicide. Initially, nausea and vomiting occurred, and the symptoms worsened over time. We immediately removed the TNPs, and the application sites were gently washed with sterile water. The patient's level of consciousness gradually improved, and she fully recovered an altered mental status 5 hours later. Her initial urinary cotinine level was 324 ng/mL. Physicians should be aware that acute nicotine poisoning by TNPs can cause various toxic symptoms.

  • Background : Euthanasia is defined as "a deliberate intervention undertaken with express intention of ending a life so as to relieve intractable suffering". There have been keen debates in the medical literatures on the questions relating to the legalisation and the provision of euthanasia and physician-assisted suicide. Methods : Literatures on the debate of euthanasia published during the last several years were reviewed, and the indications used in performing euthanasia were listed. And the results of a more liberal policy on euthanasia in Netherland were introduced. Results : Other clinical practices are sometimes described as euthanasia but can be distinguished by examination of ethical principles involved. The guidelines for the practice of euthanasia and physician-assisted suicide in the Netherlands were inadquate and were abused, while the Remmelink Committee Report said that euthanasia in the Netherland had been adequate. Conclusions : There are no clinical situations necessitating the legalisation of euthanasia or physician assisted suicide. Comprehensive and mutidisciplinary palliative care can effectively relieve much of the suffering of the terminally ill that is presently cited as justification for euthanasia or physician-assisted suicide.

Prevalence and Related Risk Factors of Suicidal Ideation in Urban Adolescents (일 도시지역 청소년의 자살사고 유병률과 위험인자에 대한 연구)

  • Objective: The aim of the present study is to assess the prevalence and related risk factors of suicide ideation among middle and high school students in an urban area. Methods: We surveyed 3,691 middle and high school students (2,159 male, 1,532 female, 11-19 years of age) using a self-report questionnaire that covered basic socio-demographic data, academic achievements, presence of physical or psychiatric illness, sleep duration on weekdays, time spent at private academies on weekdays and weekends, and subjective needs for counseling. The Beck Depression Inventory (BDI) and Reynolds' Suicidal Ideation Questionnaire (SIQ) were included in the survey. Results: The prevalence of students with high suicide ideation ($SIQ{\geq}62$) was 4.6%. In a logistic regression model, female sex (p = 0.002), younger age (p < 0.001), poor academic achievement (p = 0.043) and higher score of BDI (p < 0.001) were associated with a higher SIQ score. In addition, younger age (p = 0.045) and a higher BDI score (p < 0.001) were associated with a higher SIQ score adolescents having high suicide ideation ($SIQ{\geq}62$). Conclusion: Related risk factors of suicide ideation in adolescents were female sex, younger age, poor academic achievement, and a depressive mood. It would be especially helpful to pay more attention to younger adolescents and a depressive mood as a high-risk group. The understanding of these factors will be helpful for providing an effective suicide screening and prevention program for adolescents.

  • A 72-year-old women whose symptom was severe dysphagia and x-ray film revealed esophageal stricture and dilatation. She had attempted suicide by swallowing lye liquids 50 years ago. A conclusive clinical and histological diagnosis of esophageal carcinoma after corrosive stricture was made following a esophagectomy with esophagogastrostomy, I experienced one case of esophageal carcinoma after corrosive stricture and reviewed it with references.
