급성 심정지로 나타난 연수경색 (Medullary Infarction Presenting as Sudden Cardiac Arrest: Report of Two Cases and Review of the Literature)

  • The causes of sudden death after medullary infarction involve arrhythmia, central respiratory failure, and dysautonomia. Sudden cardiac arrest in a medullary infarction is uncommon. Most of these cases experienced sudden cardiopulmonary arrest within 2 weeks from stroke onset as the extent of lesion increased. Here, we report two cases of medullary infarction presenting as sudden cardiac arrest. These cases indicate that medullary infarction could be one of the causes of sudden cardiac arrest.

병원 밖에서 발생한 급성 심장정지 환자의 생존결과에 영향을 미치는 요인 : 다수준 분석 (Determinants on survival outcomes of sudden out-of-hospital cardiac arrest: a multilevel analysis)

  • Purpose: This study aimed to identify the factors affecting the survival outcomes of out-of-hospital cardiac arrest based on the Sudden Cardiac Arrest Survey by the Korean Centers for Disease Control and Prevention from 2012 to 2016. Methods: Out of 84,776 cases, 57,104 cases of cardiac arrest were analyzed. To identify the factors that affect survival outcomes after a sudden cardiac arrest (SCA), we performed a logistic regression using SPSS. We also performed a multilevel analysis using SAS to determine whether the survival outcomes were affected by the socioeconomic level and health index of the communities. Results: When SCA was witnessed by someone, the possibility of discharge with survival outcomes increased by a factor of 4.54. If CPR was administered immediately in emergency situations, this possibility further increased. When defibrillation was performed before hospitalization, the possibility was increased by a factor of 10.31. The multilevel analysis reflected the personal and regional factors that had an impact on the survival outcomes. Conclusion: Because the initial response in SCA is crucial, a community response system is essential before hospitalization. It is necessary to actively publicize and educate the people because the their understanding, sympathy, and cooperation in emergency situations play a role in determining the survival outcomes of the patients.

  • The From COVID-19 until recently, the demand for hiking has been rapidly increasing due to the popularization of mountain seekers.,On weekends and holidays, people who do not hike frequently while hiking in large and small mountains to see wildflowers and foliage in the mountains are more likely to be exposed to safety accidents due to an increase in sudden muscle use.,If you fall or get muscle damage during a hike, you can prevent a major accident with a simple treatment. In some cases, the error of first aid may permanently damage the important function of the body.,In particular, during a safety accident on a mountain, while climbing in light clothes, the body temperature rises due to the sweat that was shed at the beginning, and the blood pressure rises due to the contraction of the blood vessels due to the drop in body temperature from the top, resulting in an emergency such as cardiac arrest.,According to statistics from the National Park Service, nearly half (48%) of deaths in national parks are known to be sudden cardiac deaths.,There are many safety accidents that occur frequently in the mountains, but among them, we will study how to cope with acute diseases such as cardiac arrest due to increased blood pressure due to insufficient body temperature control, chest pain or dyspnea, and heart burden due to excessive hiking.

A case of rescuing a patient with acute cardiovascular instability from sudden and massive intraoperative pulmonary thromboembolism by extracorporeal membrane oxygenation

  • Intraoperative pulmonary thromboembolism is a high mortality situation. Early mortality in patients with pulmonary thromboembolism varies from 2% in normotensive patients to 30% in patients with cardiogenic shock. The use of extracorporeal cardiopulmonary resuscitation can improve survival and neurologic outcomes of cardiac arrest. We report a case of intraoperative massive pulmonary thromboembolism with circulatory collapse and cardiac arrest during anesthesia for pelvic bone fracture surgery, which were rescued by extracorporeal membrane oxygenation.

Sudden unexpected cardio-respiratory arrest after venipuncture in children

  • Purpose: This study aimed to investigate the clinical and socioenvironmental characteristics of sudden cardiorespiratory arrest after venipuncture in children. Methods: We conducted a retrospective email-based survey of all members of the Korean Pediatric Society. The questionnaire included items on patient demographics, socioenvironmental circumstances of the venipuncture, type of cardiorespiratory arrest, symptoms and signs, treatment, prognosis, and presumed cause of the arrest. Results: Fourteen patients were identified. Of these, 13 were young children (<2 years old), and 1 was 14 years old. All patients had been previously healthy and had no specific risk factors for sudden cardiorespiratory arrest. Most cases (n=11, 79%) were defined as cardiac or cardiorespiratory arrest, while the remaining cases (n=3, 21%) were defined as respiratory arrest. Aspiration (n=3), acute myocarditis (n=2), and laryngeal chemoreflex (n=1) were presumed as the causes; however, the exact causes were unclear. The overall prognosis was poor (death, n=7; morbidity, n=5; full recovery, n=2). The medical institutions faced severe backlash because of these incidents (out-of-court settlement, n=5; medical lawsuit, n=5; continuous harassment, n=3). Conclusion: Cardiorespiratory arrest after venipuncture is unpredictable and the probable cause of most cases is a vasovagal reaction. Medical personnel must be aware of the risk of unexpected cardiorespiratory arrest during routine intravenous procedures.

병원외 심정지 환자의 자발적 순환 회복에 영향을 미치는 요인 (Factors influencing the return of spontaneous circulation of patients with out-of-hospital cardiac arrest)

  • 병원외 심정지는 오늘날 우리나라의 중대한 보건문제로서, 환자의 퇴원 시 생존율은 3.5%이며, 이 중 1%만이 신경학적 기능을 회복하는 것으로 나타났다. 이처럼 낮은 병원외 심정지 환자의 생존율을 높이기 위해서는 병원 도착 전 환자의 자발적 순환을 회복시키는 것이 매우 중요하다. 따라서 본 연구에서는 질병관리본부의 2009년도 심정지 의무기록 조사 자료를 활용하여 병원외 심정지 환자의 자발적 순환 회복률을 향상시킬 수 있는 요인들에 대한 심층 분석을 수행하였다. 심정지 환자의 자발적 순환 회복에 영향을 미치는 요인은 의사결정나무기법을 적용하여 분석하였으며, 그 결과 도착전 CPR여부, 병원 도착전 심정지 목격여부, 심정지시 활동, 과거력(암/심장질환/뇌졸중), 심정지 발생 장소, 병원전 일반인 CPR여부, 신고~현장 도착까지 걸린 시간, 연령 등이 중요한 요인으로 밝혀졌다. 이 요인들의 조합을 통해 의사결정나무모형으로 분류된 심정지 환자는 총 16개 유형이었으며, 그 중 유형 1의 특징을 갖는 집단의 자발적 순환 회복률(29.6%)이 가장 높게 나타났다. 더불어 비공공장소에서 심정지가 발생한 환자에게 일반인이 CPR을 시행하였을 경우, 심정지 환자의 자발적 순환 회복률이 향상된 것으로 보아 지역주민들에 대한 CPR교육이 중요함을 파악할 수 있었다.

  • Purpose: The first responder's role during a cardiac arrest scene is to initiate CPR. The AHA has recognized and included the first responder's role for improving the survival rate of cardiac arrest patients. Health personnel working in nursing, emergency care, dental hygiene, radiology, and ocular optics frequently confront sudden cardiac arrest while working. This study was to identify the relationship between the educational experience and recognition with the level of knowledge about CPR for college students. Method: Five hundred forty college students enrolled in the department of nursing science, radiological technology, ocular optics, emergency medical technician, or dental hygiene in Daejeon city were surveyed. The tool used was CPR knowledge developed by the authors based on a literature review including 2005 AHA's CPR guideline. Result: The higher educational experience of CPR was, the higher the level of knowledge. The knowledge of the students in nursing or emergency medical technician was higher than students in dental hygiene, radiology, and ocular optics. Conclusion: CPR class should be included in the curriculum for college students in order to improve their accuracy as a first responder to cardiac arresting patients.

A Case of Successful Recovery from High Dose Intravenous Nicorandil Infusion in Refractory Coronary Vasospasm with Hemodynamic Collapse

  • A 70-year-old male came to the emergency room of the authors' hospital because of sudden cardiac arrest due to inferior wall ST elevation myocardial infarction. His coronary angiography revealed multiple severe coronary spasms in his very long left anterior descending artery. After an injection of intracoronary nitroglycerine, his stenosis improved. The cardiac arrest relapsed, however, accompanied by ST elevation of the inferior leads, while the patient was on diltiazem and nitrate medication to prevent coronary spasm. Recovery was not achieved even with cardiac massage, intravenous injection of epinephrine and atropine, and intravenous infusion of nitroglycerine. The patient eventually recovered through high-dose nicorandil intravenous infusion without ST elevation of his inferior leads. Therefore, intravenous infusion of a high dose of nicorandil must be considered a treatment option for cardiac arrest caused by refractory coronary vasospasm.

심장관통자상의 응급수술 치험 3례 (Penetrating Wound of the Heart: A Report of Three Cases)

  • Three patients who sustained penetrating stab wound of the heart have been treated successfully by emergency thoracotomy in the Department of Thoracic Surgery, Chonnam University Hospital. There were two knife and one glass wound. The location of the injury were all on the right ventricle, but in one patient, it was penetrated to ventricular septum. All patients were in shock with a systolic pressure under 60 mmHg when admitted to the emergency room. In one of the three patients, blood pressure was not detectable and subsequently cardiac arrest. Two patients required immediate thoracotomy because of intrathoracic hemorrhage and increased pericardial tamponade and the other one required prompt thoracotomy because of sudden onset of cardiac arrest. There were no death postoperatively. Two patients are living without any complication in 4 years and 4 weeks after operation. One who had penetrating wound to ventricular septum, turned to cardiac decompensation, but he is living now in 4$\frac{1}{2}$ years after operation. Exploratory thoracotomy should be performed immediately in all the patients in whom a penetrating wound of the heart or pericardial tamponade following a penetrating wound of the chest wall is suspected.

최근 17년간(1988년-2004년) 학교내 학생 돌연사 빈도 및 추이 분석 (A study of incidence and trend of unexpected sudden death of studentsin school during 17 years from 1988 to 2004 in seoul)

  • Purpose : This study was to identify the incidence, trend, and situation of the unexpected sudden death of elementary, middle, and high school students in school for 17 years from 1988 to 2004 in seoul. Methods : We used the data of The Seoul Safety Mutual aid Association in seoul. Subjects were 53 unexpected sudden death in school from elementary, middle, and high school students, and we explored the gender, month, a day of the week, year, contents of situation, and medical certificate of death. Statistic analysis were chi-square test, Cochran-Armitage trend test, and the tool was SAS 9.1.Results : There were 53 unexpected sudden death students(male 42 and female 11). The incidence of male students was higher than that of female during 16 years except in 1995. The incidence occurred in middle school students were 31 (58.5%) of 53 and were also most frequent in middle school students in both of male and female students. In 1990, the incidence was 8(15.1%) students, 6(11.3%) in 1992 , and 5(9,4%) in 2000 respectively. The frequency of unsuspected sudden death in March was 11(20.8%) of 53 students, 11 (20.8%) in September, 7(13.2%) in October. In Thursday, it was 12(22.6%) of 53 students. Only 14(26.4%) students of 53 died during general life, but 39(73.6%) were related to excercise. The diagnosis of 14 students died during general life were cardiac arrest 7(50.5%), brain disorder 3(21.4%) which were based on medical certificate. But the diagnosis of 39 students died during or after excercise were brain disorder including cerebral hemorrhage 9(23.1%), heart disease 9(23.1%), cardiac arrest 8(20.5%), and unknown 6(15.4%), respectively. Conclusion : The incidence of unexpected sudden death were more frequent in male students, in middle school, and in excercise-related situation and the trend was similar for 17 years. Therefore, to prevent the unexpected sudden death, it needs to further study substantially the risk factors of unexpected sudden death including past history, life-style, nutrition and development, family history, and learning environment.
