  • The cutaneous stimulation is an independent nursing intervention used in various painful conditions, and is explained by gate control theory. This study was aimed at identifying the effect of cutaneous stimulation on reduction of arteriovenous fistula puncture pain of hemodialysis patients. One group repeated measurement post test research was designed. Forty-five hemodialysis patients who received arteriovenous fistula puncture regulary in hemodialysis units of an attacted D hospital to K university have been studied from August 16 to 21, 1993. First the arteriovenous fistula puncture pain of control period was measured, and then the arteriovenous fistula puncture pain of experimental period(with cutaneous stimulation) was measured. The instrument used for this study were visual analogue pain scale as subjective pain measurement, objective pain behavior checklist and Spielberger's Trait Anxiety Inventory as intervening variables. Analysis of data was done by use of paired t-test, t-test, ANOVA and Perarson correlation coefficient. The results of this study were summarized as follows; 1) The first hypothesis that the subjective pain score of arteriovenous fistula puncture pain in experimental period(with cutaneous stimulation) will be lower than in control period was partly supported. The subjective pain score of arterial line was rejected(paired t=-0.28, p=0.77) and the subjective pain score of venous line was supported(paired t=2.61, p=0.01). 2) The second hypothesis that the objective pain behavior score of arteriovenous fistula pain in experimental period(with cutaneous stimulation) will be lower than in control period was rejected(arterial line paired t=-0.45, p=0.65; venous line paired t=-0.36, p=0.72). 3) The third hypothesis that the cardiopulmonary signs of arteriovenous fistula puncture pain in experimental period(with cutaneous stimulation) will be lower than in control period was rejected(pulse paired t=-0.8, p=0.42; systolic BP paired t=0.98, p=0.33; diastolic BP paired t=0.43, p=0.66). Further experimental studies with simple intravenous injection patients will be recommended in order to identify the effect of cutaneous stimulation.

  • Backgroud and Objectives: Because the pain is subjective in human, we frequently need the objective tool for estimation of pain. The Visual Analog Scale (VAS) is a method for pain grading, but it is also a subjective method. The purpose of this study is to seek an objective method for measurement of pain. Author suggests that the estimation of local body temperature may be associated with the severity of pain in tonsillar disease and the change of it measured by Digital Infrared Thermographic Imaging (DITI), may also be correlated with the change of pain. Materials and Method : Four groups were selected and measured for VAS and body temperature in DITI. Group A and B were post-tonsillectomy groups divided by age. Group C was acute tonsillitis group. Group D was peritonsillar abscess group. Results : In Group A and B, the subjective VAS was significantly correlated with objective local body temperature (correlation coefficient r=0.673, 0.647) and significant correlation was also present in acute tonsillitis and peritonsillar abscess groups(r=0.596,0.642). Conclusion : The change of pain was strongly associated with that of local body temperature in tonsillar disease. DITI is a useful method for objective grading of pain and can be used for the study of postoperatve pain and effectiveness of pain control.

  • This study was conducted to determine how precooling reduces the subjective reported pain and objective pain and to evaluate the effectiveness of precooling the injection site before administration of local anesthesia in children. Electronic databases (PubMed, Ovid SP, Cochrane Central Register of Controlled Trials) were searched for publications from 1980 to 2020. Studies were screened for titles and abstracts, followed by full-text evaluation of included reports. Six studies were included in this systematic review. The primary outcome evaluated was the pain perception or the subjective pain reported by the child receiving the injection. The secondary outcome evaluated was objective pain evaluated in each study. Among 5 studies that evaluated child reported pain scores on a visual analogue scale (VAS), 4 studies reported lower scores in the precooling group and one study reported a higher VAS score in the precooling group than in children treated with 20% benzocaine topical anesthesia. Among 6 studies that evaluated the pain reaction of children by Sound Eye Motor (SEM) score, 4 studies reported a lower SEM score in the precooling group, one study reported no significant difference between the precooling and control groups, and one study reported higher SEM scores in the precooling group than in children treated with 20% benzocaine topical anesthesia. Within the limits of this systematic review, evidence suggests that precooling the injection site with ice can be an effective adjunct to topical anesthesia in reducing both subjective and objective pain during local anesthesia administration in children.

  • This study is aimed to find out and define how the muscular-skeletal pain of the Aged, according to their residential circumstance, sex and age, can affect the subjective health index and how all these are related and associated with. For the period of June 1 to July 31, 2001, in order to study and define how the muscular-skeletal pain are related to the subjective health index of the Aged, we have conducted an enquete through a direct interview with 693 persons over age sixty-five (65) in Daejon and in other adjacent areas, divided into three different residential types "The Aged living at home", "The Aged living at welfare facilities" and "The Aged living alone". The study concludes followings : 1. Generally, muscular-skeletal pain and the subjective health index of the Aged, are a lot influenced by and related to their residential circumstance, their sex and their age. 2. With regard to the muscular-skeletal pain of the Aged by their sex, it was analyzed that, on an average, the female-Aged gains 3.0 point and the female-Aged suffers from this pain more severely. In analyzing this pain by their residential type, it was found that, on an average, the 3.0 point goes for "the Aged living alone", which explains the Aged living alone is having the most serious pain. 3. With regard to the subjective heath index of the all Aged participated in this research, the analysis indicates 8.8 point and this is considered as a general standard (7-10 point). In analyzing this index by their sex, the female-Aged gains 8.6 point only and it explains a lot of female-Aged consider they are not really healthy. In analyzing this index by their residential type, "the Aged living at welfare facilities" and "the Aged living alone" gain the comparatively lower point, - respectively 8.4 point for the Aged living at welfare facilities and 8.8 point for the Aged living alone. The Aged of these two residential types express they are obviously in a bad condition of health, which makes us think a lot.

  • This study aimed to assess the combined use of extraoral vibratory stimulation and extraoral cooling in reducing the pain (subjective and objective) of dental local anesthesia administration in children. PubMed, Cochrane Central Register of Controlled Trials, and Ovid SP databases were searched up to July 2021. Article titles were screened and full-text evaluations of the selected articles were performed. Finally, seven studies (391 children, aged 4 - 12 years) were included in this qualitative and quantitative analysis. The pooled data determined the combined effect of extraoral vibration and extraoral cooling as a single measure. Extraoral vibration or cooling alone were not compared. The measured primary and secondary outcomes were pain perception and subjective and objective pain, respectively. When compared with the control, extraoral vibration and cooling resulted in significant differences in the mean combined data for the variables, pain perception, and pain reaction. Children's subjective pain as measured by pain scores were reduced when extraoral vibration and cooling was used during local anesthesia administration (mean difference -3.52; 95% confidence interval [-5.06 - 1.98]) and objective pain (mean difference -1.46; 95% confidence interval [-2.95 - 0.02] ; mean difference -1.93; 95% confidence interval [-3.72 - 0.14]). Within the confines of this systematic review, there is low-quality evidence to support the use of combined extraoral vibration and cooling for reducing pain (subjective and objective) during intraoral local anesthesia administration in children.

  • Purpose: This study investigated factors influencing the xerostomia symptoms in the patients with temporomandibular disorders (TMDs). Methods: Eighty-six participants over the age of twenty were randomly enrolled from patients with TMDs. The patients were diagnosed by Diagnostic Criteria-TMD (DC-TMD). Intensity of the pain and level of stress of the patients were recorded using TMD pain visual analogue scale (VAS) and stress VAS, respectively. The Symptom Checklist-90-Revised (SCL-90R), dry mouth symptom questionnaire, unstimulated salivary flow rate (USFR) and oral moisture were measured. Results: The patients who had above the mean of the TMD pain VAS had significantly higher scores on the stress VAS, subjective dry mouth symptoms and T-score of somatization. The patients who suffered from pain more than three months had significantly higher TMD pain VAS, subjective dry mouth symptoms and T-score of somatization. There were no significant differences in xerostomic symptoms according to the diagnosis of TMDs. Moreover, TMD pain VAS and the stress VAS did not correlated to USFR and oral moisture. Conclusions: The level of individuals' TMD pain and stress significantly affect their subjective dry mouth symptoms, however, it did not affect USFR and oral moisture.

  • This study was conducted to identify the characteristics of fatigue and its influencing factors in patients having chronic arthritis. The subjects of this study were 120 patients who visited a Rheumatis Center of H University Hospital. Data were collected from Aug. 9, 1997 to Feb. 5, 1998. For analysing data, SPSS/Win was used for descriptive statistics & Pearson's correlation coefficient, and Lisrel 8.0 was for path analysis. The findings were as follows : 1. Mean score of subjective fatigue was 5.36 and it means moderate degree of fatigue. 2. Fatigue showed positive relationship with pain & depression, and it showed negative relationship with ADL & self efficacy. 3. Fatigue was influenced by pain and depression. and was not influenced by ADL and self efficacy directly. But ADL and self efficacy influenced on fatigue through pain and depression indirectly. According to this findings. the significant influencing factors of fatigue were pain and depression, therefore the development of nursing intervention for relieving pain and depression would be needed.
