The Case Report of Chuna Treatment on Supraspinatus Tendinitis with Kyphotic Cervical Curvature (경추후만을 동반한 극상근건염 환자의 추나치험1례)

  • Objective : The purpose of this study is to evaluate the conservative treatment for supraspinatus tendinitis with kyphotic cervical curvature patient. Methods : We used chuna, Herbal medication, Acupuncture, Bee Venom Acupuncture, Physical Therapy for this patient. And we measured of VAS score, ROM and Extent of the subjective symptoms of shoulder pain to evaluate treatment effects. Result : Patient's shoulder pain and cervical curvature has improved. VAS score and Extent of the subjective symptoms of shoulder pain score were also decreased.

A Study on the Relationship between Musculoskeletal Symptoms and Health Promoting Life Style among Some Workers (일부 직업인들의 근골격계 자각증상과 강증진생활양식간의 연관성에 관한 연구)

  • In this study, grade of subjective symptom appealed by laborer of Jeollabuk-do was evaluated using questionary regarding factor made effect on musculoskeletal disease and in addition, studied relationship with health promotion life style of them. Based on the result, relationship of general characteristics of musculoskeletal subjective symptom and life-style of the subjects was concluded as below. 1. General characteristics of study subjects were as following. Ratio of male was higher as 57.7% of male and 42.2% female and age distribution was 5.1% of 20s, 34.99% of 30s, 36.3% of 40s and 23.7% of 50s and therefore, $30{\sim}40$ aged groups showed highest ratio. Most subjects (74.9%) was married status and in case of education level, high-school graduate and dropout (23.3%) and over-college graduate (46.8%) showed highest distribution. $1{\sim}2$ Mil. KRW (29.5%) and $2{\sim}2.99$ Mil. KRW (21.2%) is the main income distribution and however there was high ratio of non-reply (29.0%). In case of employment period, $10{\sim}14$ years (15.3%) and over 15 years (29.6%) showed highest ratio and there were many non-reply (39.4%) and in addition, 67.6% replied as own house and 14.3% as lease on deposit base in question of residence type. 2. Subjects showed high ratio of subjective symptom appeal of 62.79% and many cases (50.23%) appealed 1 or 2 symptoms. Symptom by body region was 29.8% (waist), 27% (shoulder), 21.2% (knee), 15.5% (neck), 9.5% (ankle), 8.1% (wrist) and 5.0% (elbow) in order. In case of relationship with general characteristics, female comparing with male, non-residence of own house, subjects with lower education level and employment period of $10{\sim}14$ years showed higher appeal rate and kind of symptoms than others. Therefore, it was concluded that rate of musculoskeletal symptom appeal have close relationship with gender, level of living, education level, age and employment period. 3. In case of severe pain of upper body except waist and ankle, it was appealed in both or right side and it means that upper body pain is originated from right side and right region pain is transited to both region pain. In addition, there was 39.41% of non-reply to existence of right-left region pain and therefore, it was evaluated that, in may cases, there was no awareness of their own symptom condition even on subjective symptom. 4. Degree of pain was, as pain over middle level, evaluated as 2.79 on full mark of 4.0 and in order of waist (2.97), ankle (2.83), knee (2.82), wrist (2.82), neck (2.79), shoulder (2.70) and elbow (2.62). In addition, 71.97% appealed $2{\sim}3$ cases for the latest 1 week. Owing to subjective symptom, 54.95% drop into hospital or pharmacy, 10.32% made temporary retirement or absence, 7.99% transferred into more comfortable duty and $39.4{\sim}54%$ experienced one or more managing mentioned above. 5. Fulfillment of health promotion life style of subjects was evaluated on full mark of 4.0 and total score was 2.63. Average mark of each area was personal relationship (3.05), self-realization (2.92), stress management (2.63), health control (2.48), physical exercise (2.19) and nutrition management (2.19) and personal relationship was highest and physical exercise and nutrition management were lowest. As general characteristics influencing health promotion life style, gender, residence style and employment period showed significant difference. Male showed higher mark than female and showed higher mark in order of own house, others, lease on deposit base, monthly rent. Subjects with longer employment period showed higher mark with significant difference. 6. Accounting of factor influencing each area of health promotion life style, self-realization showed significance in marriage status, income, residence style and education level and health control in age, residence style and employment period. Physical exercise showed significant difference in gender, age, residence style and employment period and nutrition in gender, age, residence style and employment period. Stress management showed significant difference in residence style and employment period and however not in personal relationship. 7. Health promotion life style relating with existence and kind of pain showed significant difference in all area except personal relationship area. In absence of pain, there was statistically significant high score in all area even in total health promotion life style and all area. Accounting of kind of pain, cases of $1{\sim}2$ kinds of pain and $5{\sim}6$ kinds of pain showed relatively high score and it was lower than mark of subject stated absence of pain. 8. Subjects appeal symptom were classified by symptom region and difference of total and each areas were evaluated. General area (p=0.002), self-realization (p=0.012), health management (p=0.023), physical exercise (p=0.028), nutrition management (p=0.028) and stress control (p=0.001) showed statistically significant difference and not in personal relationship area. Especially, elbow, shoulder and neck area marked high and group appealed pain of knee, arm and elbow, foot and ankle marked low. Based on those results, subjective symptom should be accounted seriously in diagnosis of occupational musculoskeletal disease of laborer and among subjective symptom, general characteristics of gender, age, condition of living, education level and employment period make effect. Generally subject appeal symptom marked lower than subject without symptom appeal and it means that life management of subject appealing musculoskeletal pain make important role in management and treatment of occupational musculoskeletal disease.

Clinical Study of the Effects of Juglandis Semen Pharmacopuncture Therapy on Chronic Shoulder Pain (호도약침이 만성견비통에 미치는 임상적 연구)

  • Objectives : The purpose of this study is to examine the effect of Juglandis Semen Herbal Acupuncture Therapy on Chronic Shoulder Pain. Methods : Clinical studies on shoulder pain were carried out on 34 patients who were treated at Department of Acupuncture & Moxibusition, Samse Oriental Hospital from June 1, 2009 to October 15, 2009. Subjects were divided into two groups: Experimental group(Group A, n=17) and Control group(Group B, n=17). Group A were treated by common acupuncture therapy, cupping therapy and Juglandis Semen Pharmacopuncture Therapy and Group B were treated by common acupuncture therapy & cupping therapy. All patients in both groups were treated three times a week over 3 weeks. For evaluating change of pain, we used Shoulder Pain and Disability Index(SPADI), Visual Analogue Scale(VAS), Degree of feeling inconvenient in everyday life and Degree of subjective symptoms of shoulder pain. Evaluations were made at baseline and after one week, two weeks and three weeks of treatment. Results : Both group showed chronic shoulder pain decrease after 3 weeks of treatment. But Group A showed better effect on decreasing SPADI, VAS, Degree of feeling inconvenient in everyday life and Degree of subjective symptoms of chronic shoulder pain than Group B. Conclusions : According to the above-mentioned results, it seems that Juglandis Semen Pharmacopuncture Therapy could be applied as the effective method for reducing shoulder pain.

Comparison of Obstetric Pain, Anxiety, and Cervical Dilatation between Epidural Analgesia and No Analgesia group during Labor Stage I (경막외 마취제 투여 유무에 따른 분만 1기 산부의 분만 통증, 불안, 자궁경관 개대 정도 비교)

  • Purpose: This research was done to compare obstetric pain, anxiety and cervical dilatation between an epidural analgesia group and a control group. Methods: Participants were assigned to the experimental or control group depending on their decisions for pain relief. Subjective / objective obstetric pain, anxiety level and cervical dilatation were measured and ANOVA was used for comparison of groups and paired t-test to make pre-post comparisons. Results: Homogeneity of pain, anxiety and cervical dilatation were assessed at the latent phase. Cervical dilatation was larger in the control group than the experimental group, at both the active and the transitional phase (F=22.9, p<.001; F=39.9, p<.001 respectively). The degree of pain and anxiety were not significantly different between the groups. Within the experimental group, subjective / objective pain and anxiety level were significantly lower post-analgesia compared to pre-analgesia in the active phase. All variables, except for sweating in the objective pain measurement, changed significantly at the transient phase. Conclusion: The results of this evidence-based research indicate that epidural analgesia while effective in relieving pain and anxiety may have an adverse effect on the cervix during labor stage I. Epidural analgesia should be used carefully during cervical dilatation in labor stage I.

Comparison of the Degree of Pain According to Nursing Intervention Method during Arteriovenous Fistula Needle Insertion for Patients on Hemodialysis (혈액투석 환자의 동정맥루 천자 시 간호중재요법에 따른 통증정도 비교)

  • Purpose: This research was done to compare the pain relief effects of various nursing interventions, such as cold therapy, attention diversion and 10% Lidocaine spray during arteriovenous fistula needling for patients on hemodialysis, and also to identify and develop more effective nursing interventions for pain relief in these patients. Methods: This research was conducted from October 1, 2010 to January 31, 2011 with 8 hemodialysis patients, who were on regular dialysis (3 times a week) at K University Hospital in Seoul and had an arteriovenous fistula within the past 3 months. Each patient received the three nursing interventions (cold therapy, attention diversion and 10% Lidocaine spray therapy) prior to the arteriovenous fistula needling and applied in turn with the series being repeated 4 times. After each intervention, physiologic indexes, subjective and objective pain were measured at the time of needling. ANOVA was used with SPSS/WIN 12.0 to analyze pain scores and comparison of physiologic indexes (BP, pulse). Results: No significant differences were found for subjective pain (p=.574), objective pain (p=.562) and total pain (p=.506) among the 3 interventions. Systolic blood pressure (p=.689), diastolic blood pressure (p=.969) and pulse (p=.980) also showed no significant difference among the 3 interventions. Conclusion: These 3 interventions are all possible for pain relief during arteriovenous fistula needling for these patients. However, the only interventions that nurses can do independently are cold therapy and attention diversion so we recommend that these nursing interventions be used.

A study on the Influences of Working Conditions to the Subjective Symptoms in VDT Workers (VDT 작업자의 작업특성에 따른 자각증상 비교)

  • In order to investigate the subjective symptoms of VDT workers according to the working condition, this qutionnaire survey was conducted on 477 workers of 11 general hospitals, 15 banks and 9 research institutions in Taejon city and Chungnam province from June, 1993 to october 1933. The following results were obtained : 1. The main subjective symptoms answered by VDT workers were "eye strain". "eye pain". "blurred vision" for eye symptoms, "fatigue", "tendency to be fed up", "impatience" for psychoneurotic symptoms and "shoulder stiffness", "neck stiffness", "low back pain" for musculoskeletal symptoms. Most of these symptoms of VDT workers higher in female than male. 2. The number of VDT workers who complained eye and musculoskeletal symptoms were significantly increased. Smoking VDT workers showed higher complained rate of eye, psychoneurotic and musculoskeletal symptoms than nonsmoking group but alcohol drinking group showed higher rate of symptoms complaints of eye and musculoskeletal symptoms than not drinking alcohol group. 3. The VDT wokers who worked longer hours continuously and more hours of daily work, complained more frequent eye and musculoskeletal symptoms. 4. All subjective symptoms showed positive correlation to the sex and daily VDT working hours.

The relationship between workers' self-recognized oral health status and disruption of work (수도권 일부 지역 산업체 근로자들의 주관적 구강건강상태와 업무지장의 관련성)

  • Background: Workers' oral health problems result in work disruption, including absenteeism or early leave, which reduces work efficiency. This study was conducted to investigate the subjective oral health status and oral problems of workers,and to identify the factors disrupting workflow due to oral problems. Methods: A self-report questionnaire was administered to 300 industrial workers in a metropolitan area. A total of 284 individuals were finally analyzed, after excluding the data of 16 workers who had missing responses among the recovered questionnaires. Results: Subjective oral health status was average in 44.4%, healthy in 32.0%, and unhealthy in 16.9% of the study population. Subjective oral health problems were the highest in the order of food impaction(28.9%), cavity(26.8%), tooth sensitivity (22.9%), and calculus(21.1%). However, sudden and unexplained tooth pain (12.0%), gum swelling and tooth mobility (10.2%), and wisdom tooth pain (4.9%) were relatively low. The average monthly income (p<.05) and subjective oral health status (p<.01) were statistically significant factors interrupting workflow. Conclusion: To minimize workers' oral health problems and work disruption due to oral diseases, it is necessary to promote workers' oral health and oral disease prevention programs within the workplace.

Effects of Sequential Application of Superficial Cold and Heat on Pain, Patient Satisfaction with Pain Control, Comfort Level and Subjective Response after Spine Surgery (순차적 냉·온 요법이 척추 수술 후 통증과 통증 조절 만족도, 안위, 주관적 반응에 미치는 효과)

  • Purpose: The aims of the current study were to evaluate the effects of superficial cold and heat after spine surgery on pain, satisfaction with pain control and comfort level, and to identify subjective responses and adverse effects. Methods: A prospective, single-blind, randomized controlled trial was utilized. The intervention group (n=36) received superficial cooling until the wound drain was removed and thereafter followed by superficial heating until discharge, while the control group (n=34) received only superficial cooling until wound drain was removed. Data were collected from August 4 to November 11 2014. Results: There was significant difference in pain according to time within groups (F=71.87, p<.001). However, we found no difference in pain between groups. The intervention group reported higher patient satisfaction with pain control (4 vs 3, z=-2.83, p=.005) and higher comfort level (5 vs 4, z=-4.12, p<.001) than the control group. Conclusion: Results indicate that sequential application of superficial cold and heat is a useful method in clinical practice for management of pain after spine surgery.

Association between Oral Health Behavior and Oral Health Status in Adolescent (청소년의 구강건강행태와 구강보건상태와의 관련요인)

  • Purpose : This study was conducted to improve cognitive oral healthcare care by analyzing the correlation between oral health behavior and oral health status Korean adolescents. The subjects were 65,528 youths in 798 schools monitored in the Twelfth Korea Youth Risk Behavior Web-based Survey in 2016. Methods : The general characteristics consisted of 9 items including sex, grade, and academic performance. The academic performance and economic status were restructured into "upper, middle, lower". Subjective health perception and subjective oral health perception were reorganized as "health", "normal", "no health". The sub-areas of oral health behavior include five times of daily brushing during the day yesterday. The number of brushing times was "0", "1-2", "3", "more than 4" After lunch at school, brushing was restructured as "always," "sometimes," "not." The oral health status subscale consisted of 6 items including tooth breakage and toothache at the time of eating. Result : Tooth fracture(43.7 %), pain in mastication(43.7 %), tooth tingling(47.4 %), gum pain and bleeding(45 %), pain in buccal mucosa(43.8 %) and halitosis(46.6 %) were rare in adolescent having healthy oral health status cognitively(P<.001). Nosymptom adolescents without dental sealant showed low tooth fracture(0.64 times), pain in mastication(0.67 times), tooth tingling(0.59 times), gum pain and bleeding(0.84 times), pain in buccal mucosa(0.76 times), and halitosis(0.90 times). Nosymptom adolescents without scaling showed low tooth fracture(0.88 times), pain in mastication(1.03 times), tooth tingling(0.82 times), gum pain and bleeding(0.64 times), pain in buccal mucosa(0.70 times) and halitosis(0.82 times). Conclusion : This study revealed that oral health status is correlated with oral health behavior. Development of educational media and program, and aggressive promotion required to establish oral health Korean adolescent.

The Effect of the Combined Analgesia of Epidural and Cryoanalgesia after Thoracotomy Surgery (개흉술 후 냉각요법과 경막외 Fentanyl 병용요법의 비교)

  • Background: Thoracotomy is the operation that produces the most postoperative pain, necessitating the highest requirements for postoperative analgesics. The common methods of treating postthoracotomy pain are the use of thoracic epidural analgesia, intemittent or continuous intercostal nerve blocks, intravenous narcotics and cryoanalgesia. We designed to assess the analgesic effect of epidural analgesia, cryoanalgesia and the combined analgesia in thoracic surgery. Methods: A prospective study was carried out in 59 patients undergoing elective thoracotomy for parenchymal disease. Patients were randomized into three groups: C (cryoanalgesia), CE (cryoanalgesia and thoracic epidural analgesia), E (epidural analgesia). All patients had standard anesthesia with endotracheal intubation using a double lumen endotracheal tube, and one-lung ventilation. Subjective pain relief was assessed on a visual analog scale. Analgesic requirements, complications and the degree of satisfaction were evaluated during the 7 days following surgery. Results: Subjective pain relief was significantly better in Group CE and Group E in comparison with Group C (P < 0.05). Cryoanalgesia provided a better pain score on the 6th and 7th POD than the early postoperative periods. Analgesic requirements were higher in Group C than in the Group CE and Group E during the first POD. The incidence of side effects was similar in Group CE and Group E. Conclusions: If we can reduce the concentration of fentanyl and local anesthetics in combined analgesia of epidural and cryoanalgesia, the disadvantages of each method would be overcome and would be a better method of postthoracotomy pain control.
