A Study on Status of Utilization and The Related Factors of Primary Medical Care in a Rural Area (일부 농촌지역의 일차의료이용실태와 그 관련요인에 관한 연구)

  • This study was carried out, through analyzing the annual reports(year of 1973-1993) on health status of Su Dong-Myun, and specific survey data of 332 households(Su Dong-Myun 209, Byul Nae-Myun 123), located in Nam Yang Ju-Si, Kyung Gi-Do, from July 20 to July 31, 1995, to find out more effective means for primary medical care in a rural area. The results were as fellows : 1. Number of population in Su Dong-Myun was 5,419 in 1973, 4,591(the lowest) in 1987 and 5,707 in 1995. In the composition rate of population, "0-14" of age group showed markedly decreasing tendency from 43.1% in 1975, to 19.1% in 1995, however "65 and over" markedly in creasing tendency form 5.3% in 1975 to 9.8% in 1995. 2. Annual utilization rate per 1,000 inhabitants in Su Dong-Myun showed markedly increasing tendency from 1973 to 1977 such as 343 in 1973, 540 in 1975, 900 in 1977. However, since 1979, the rate showed rapidly decreasing tendency, such as 846 in 1979, 519 in 1985, 190 in 1991 and 1993. 3. The morbid household rate per year was 53.6% of respondents and the rate per 15 days was 48.2%. In disease classification rate of morbid household per year, Arthralgia & Neuralgia was the highest rate(33.9%) and gastro-intestinal disorder(19.3%), Cough(11,9%), Hypertension(7.8%), Accident(3.2%) in next order. 4. In the utilizing facilities for Primary Medical Care, Medical facilities was showed the highest rate(58.1% of respondents) and Pharmacy and Drug Shp(33.1%), Tradition Method(4.0%) in next order. In the Medical facilities, General private clinic was showed the highest rate(34.3%) and specific private Clinic(22.3%), Hospital(19.0%), Health (Sub)center(16.3%), Nurse practitioner (3.3%), Oriental hospital and clinic(2.7%) in next order. 5. Experience rate, utilizing health subcenter was 51.8% of the respondents, and it was 55.0% in Su Dong-Myun and 46.3% in Byul Nae-Myun. In utilization times of health subcenter, times-rate showed next orders such as 1-2 times/6months(31.6%), 1-2 times/year (22.1%), 1-2 times/months(19.2%), 1-2 times/3months(15.6%). 6. In objectives, visiting Health Subcenter, Medical Care was the highest rate(59.8% of the respondents) and health control(23.3%) was in next order. In Medical Care, Primary Care by general physician was higher rate(51.1%) almost all. In the Health control, Immunization too was high rate(18.0%) in health control activities. 7. The reasons rate, utilizing health subcenter showed next order, such as distance to Medical facilities(33.0% of the respondents), Medical Cost(28.1%), Simple process of consultation (10.8%), Effectiveness of cure(7.6%), Function of primary medical care(7.0%) and Attitude of physician(6.5%). 8. In the affecting factors to utilization of primary medical facilities, medical needs was showed the highest rate(29.5% of the respondents) and medical cost(15.4%), distance to medical facilities(14.2%), traffic vehicle(14.2%) and farm work(6.9%) in next order. 9. In the priority between 'daily farm work,' and 'primary medical care', only 46.4% of respondents answered that primary health care is more important than the daily farm work The 22.6% of respondents answered 'daily farm work', and the 12.3% answered 'the equal of the both'. 10. In the criterion of medical facilities choice, medical knowledge and technical quality was showed the highest rate(56.3%), distance or time to medical facilities(10.9%), sincerity and kindness of physician(9.4%), medical cost(8.7%) and traffic vehicle(6.5%) in next order 11. In the advise for improvement of health subcenter function, the 36.1% of respondents answered that 'enforcement of medical personnel and equipment' was required, and then 'improved medical technology'(25.5%), 'good attitude of physician'(14.9%), 'improved medical system'(13.3%), 'enforced drug'(6.7%) in next order. 12. The study on affecting factors to utilization of primary medical facilities was very difficult subject to systematize the analyzed results, due to a prejudice of protocol planner, surveyer and respondent, and variety and overlapping of subject matter.

Anti-Melanogenic, Anti-Wrinkle, Anti-Inflammatory and Anti-Oxidant Effects of Xylosma congesta leaf Ethanol Extract (산유자 잎 에탄올 추출물의 미백, 주름억제, 항염증 및 항산화 효능)

  • In the present study, we investigated the biological activities of Xylosma congesta leaf ethanol extract (XCO) using a variety of in vitro and cell culture model systems for anti-melanogenic, anti-wrinkle, anti-inflammatory and anti-oxidant activities. First, XCO markedly inhibited ${\alpha}$-melanocyte stimulating hormone-stimulated melanin synthesis in B16F10 cells. Secondly, XCO marginally induced procollagen synthesis in CCD-986SK cells. Thirdly, XCO dose-dependently suppressed lipopolysaccharide-induced nitric oxide (NO) production in RAW 264.7 cells. XCO did not affect cell viability at different concentrations used in this study, indicating that XCO-mediated inhibition of melanin, procollagen and NO synthesis is not mediated by cytotoxicity. Finally, XCO was found to exert anti-oxidant effect. Taken together, these findings demonstrate for the first time that XCO possesses anti-melanogenic, anti-wrinkle, anti-inflammatory and anti-oxidant activities, and suggest further evaluation and development of XCO as a functional supplement or cosmetic that may be useful for whitening skin, reducing wrinkles and treating inflammatory responses.

A Study on the Research Trend and Future Development Direction of Mt. Baekdu in Korea (국내 백두산 분화 관련 연구 동향 분석 및 향후 발전방향 제시)

  • The purpose of this paper is to figure out the research direction and information regarding Mt. Baekdu in Korea through analyses of the research field and trends. Firstly, we made inventory of journal papers, conference proceedings, and research reports published related to Mt. Baekdu. A total of 255 data, spanning the 34 years from the 1980s to the middle of 2014, were acquired and classified into categories according to the year, field, contents and study area. Results show that research on Mt. Baekdu has been performed more than twice since 2010 and study regarding prediction has been carried out in 54.7% cases. In addition, the importance of geo-spatial information is expected to increase in order to study Mt. Baekdu. Secondly, we made and analyzed a geospatial information using inventory of 234 detailed research contents in research reports by Korea Meteorological Administration (KMA) and National Institute of Meteorological Research (NIMR). Statistics on categories show that research regarding prediction accounted for 81.6% of cases and the study of geo-spatial information utilization accounted for 54.7% of cases. However, the focus on studying the Mt. Baekdu region accounted for only 20.1% of cases. Thus, this indicates that it is necessary to do research at Mt. Bakdu itself. If the directly available geo-spatial information system is developed related to Mt. Baekdu, it will save research costs and analysis time. This study can be used to manage information about the research field of Mt. Baekdu by analysing inventory of research references and geospatial information using inventories when the Mt. Baekdu area is the focus of future research.

Strength Evaluation of Sin91e-Radius Total Knee Replacement (TKR) (인공무릎관절의 단축법위 회전시 근력정가)

  • Artificial joint replacement is one of the major surgical advances of the 21th century. The primary purpose of a TKA (Total Knee Arthroplasty) is to restore normal knee Auction. Therefore, ideally, a TKA should: (a) maintain the natural leverage of the knee joint muscles to ensure generating adequate knee muscle moments to accomplish daily tasks such as rising from a chair or climbing stairs;(b) allow the same range of motion as an complete knee; and (c) provide adequate knee joint stability. Four individuals (2 peoples after surgery one year and 2 peoples after surgery three years) participated in this study. All they were prescreened for health and functional status by the same surgeon who performed the operations. Two days of accommodation practice occurred prior to the actual strength testing. The isometric strength (KIN-COM III) of the quadriceps and hamstring were measured at 60$^\circ$ and 30$^\circ$ of knee flexion, respectively. During isokinetic concentric testing, the range of motion was between 10$^\circ$ to 80$^\circ$ of knee flexion (stand-to-sit) and extension (sit-to-stand). for a given test, the trial exhibiting maximum torque was analyzed. A 16-channel MYOPACTM EMG system (Run Technologies, Inc.) was used to collect the differential input surface electromyographic (EMG) signals of the vastus medialis (VM), vastus lateralis(VL), rectus femoris (RF) during sit-to-stand and stand-to-sit tests. Disposable electrodes (Blue SensorTM, Medicotest, Inc.) were used to collect the EMG signals. The results were as follows; 1. Less maximum concentric (16% and 21% less for 1 yew man and 3 years mm, respectively) and isometric (12% and 29%, respectively) quadriceps torque for both participants. 2.14% less maximum hamstrings concentric torque for 1 year man but 16% greater torque for 3 years mm. However, 1 year man had similar hamstring isometric peak torque for both knees. 3. Less quadriceps co-contraction by 1 year man except for the VM at 10$^\circ$-20$^\circ$ and 30$^\circ$-50$^\circ$ range of knee flexion.

Geographic Distribution of Periphyton Diatom Species: A Case Study of Achnanthes convergens in Nakdong River Basin (낙동강 수계 청수성 부착조류의 공간분포 특성에 관한 연구: Achnanthes convergens 를 사례로)

  • This study attempts to identify the spatial distributions of Achnanthes convergens, and elucidate the environmental factors that affect the Periphyton diatom habitat. Data in 250 points of Nakdong river basin are collected between April(primary) and September(secondary) 2012, with the National Institute of Environmental Research's support. We define "clean area" over 10% of Achnanthes convergens appearance, and the others as "non-clean areas". Spatial statistics of Kriging, Hotspot, LISA are used in this study. Results show that 1) 56 points are identified as clean areas in the primary survey, while 41 points are discovered in the following survey; 2) using water quality variables, density of turbidity(clean $101.83{\mu}s/cm$; non-clean $598.48{\mu}s/cm$) and conductivity(clean 1.95 NTU; nonclear 5.58 NTU) are five-fold lower in clean-areas; 3) Habitat and Riparian Factors in Nakdong basin illustrate that natural sand bar, diversity of velocity, sediment condition, levee material, riverside land affect Achnanthes convergens; 4) Hotspots of Achnanthes convergens are located in watersheds, including upper Andong Dam, upper Imha dam, Wi-cheon, Miryang river, Nam river and Hwang river whereas mainstream/downstream of Nakdong river and Keumho river watershed are shown as coldspots.

The Comparison of Susceptibility Changes in 1.5T and3.0T MRIs due to TE Change in Functional MRI (뇌 기능영상에서의 TE값의 변화에 따른 1.5T와 3.0T MRI의 자화율 변화 비교)

  • Purpose : The purpose of this study was to find the optimum TE value for enhancing $T_2^{*}$ weighting effect and minimizing the SNR degradation and to compare the BOLD effects according to the changes of TE in 1.5T and 3.0T MRI systems. Materials and Methods : Healthy normal volunteers (eight males and two females with 24-38 years old) participated in this study. Each volunteer was asked to perform a simple finger-tapping task (sequential opposition of thumb to each of the other four fingers) with right hand with a mean frequency of about 2Hz. The stimulus was initially off for 3 images and was then alternatively switched on and off for 2 cycles of 6 images. Images were acquired on the 1.5T and 3.0T MRI with the FLASH (fast low angle shot) pulse sequence (TR : 100ms, FA : $20^{\circ}$, FOV : 230mm) that was used with 26, 36, 46, 56, 66, 76ms of TE times in 1.5T and 16, 26, 36, 46, 56, 66ms of TE in 3.0T MRI system. After the completion of scan, MR images were transferred into a PC and processed with a home-made analysis program based on the correlation coefficient method with the threshold value of 0.45. To search for the optimum TE value in fMRI, the difference between the activation and the rest by the susceptibility change for each TE was used in 1.5T and 3.0T respectively. In addition, the functional $T_2^{*}$ map was calculated to quantify susceptibility change. Results : The calculated optimum TE for fMRI was $61.89{\pm}2.68$ at 1.5T and $47.64{\pm}13.34$ at 3.0T. The maximum percentage of signal intensity change due to the susceptibility effect inactivation region was 3.36% at TE 66ms in 1.5T 10.05% at TE 46ms in 3.0T, respectively. The signal intensity change of 3.0T was about 3 times bigger than of 1.5T. The calculated optimum TE value was consistent with TE values which were obtained from the maximum signal change for each TE. Conclusion : In this study, the 3.0T MRI was clearly more sensitive, about three times bigger than the 1.5T in detecting the susceptibility due to the deoxyhemoglobin level change in the functional MR imaging. So the 3.0T fMRI I ore useful than 1.5T.

Analysis on Growth and Yield of Cherry Tomato Grown in a Two-Story Bed System Adapted to Strawberry Cultivation as Affected by the Planting Time during the Uncultivated Period (딸기 재배용 2단 베드 시스템에서 휴작기 이용 방울토마토 재배 시 정식 시기에 따른 생육과 생산성 분석)

  • This study was conducted to determine the yield of cherry tomato (Solanum lycopersicum Mill.) grown at three planting times during the uncultivated period of strawberry. Cherry tomato was planted under condition filled with strawberry dedicated culture medium on a two-story bed with April 20, April 30, and May 10 at 2015. Fruit harvest was completed on July 31. The supply concentration of nutrient solution at the time of transplanting was started as EC $1.2dS{\cdot}m^{-1}$ and it was gradually increased to EC $2.5dS{\cdot}m^{-1}$ after blooming of the first flower cluster. Netherlands PBG solution was supplied for one minute six times per day. The heights of cherry tomato plants planted at earlier were significantly greater than those of tomato plants planted later. The heights of cherry tomato plants grown at the bottom of the bed were greater than those grown in the upper bed. The yield of cherry tomatoes planted on April 20 at the bottom of the bed was greatest with an average of 2,954 g of tomatoes per plant. There were no significant differences in the average weight and sugar content of fruit according to planting times and bed position. The yield of cherry tomato plants planted on April 20 was 18% and 34% higher than that of plants planted on April 30 and May 10. We confirmed to increase the yield of the cherry tomato when early plants planted on two-story bed. These results indicate that farmers can choose the best period of producing cherry tomato during the un-cultivated period of strawberry under two-story bed conditions.

The Effects of Plant Growth Regulators on Plant Regeneration and Direct Shoots Formation of Petal Explants of Chrysanthemum Flower Color Mutants Varieties, 'ARTI-purple' and 'ARTI-queen' (국화 화색 돌연변이 품종 'ARTI-purple' 및 'ARTI-queen' 꽃잎 조직의 재분화와 신초형성에 미치는 식물생장호르몬의 영향)

  • Chrysanthemum is one of the most popular ornamental plants worldwide. Recently, lots of new and novel chrysanthemum varieties have been developed using mutagenesis. However, there was no study for comparison of tissue culture condition among the mutant varieties derived from one original variety, until now. This study was conducted to compare the efficient regeneration condition of the two chrysanthemum mutant varieties, 'ARTI-purple' and 'ARTI-queen'. Two different flower parts (disk and ray florets) at the unopened and early blooming stages were used for comparison of regeneration condition on MS medium supplemented with combinations of three growth regulators (BA, NAA, and IAA). The highest regeneration rate was identified on the NAA and BA combination when the disk florets at unopened blooming stage are used. The best optimum combinations of growth regulators were identified as NAA $1.0mg{\cdot}L^{-1}$ and BA $0.5mg{\cdot}L^{-1}$ at 'ARTI-purple', which displayed 47.9% regeneration. However, regeneration of 'ARTI-queen' was the highest as 25.6% at NAA $2.0mg{\cdot}L^{-1}$ and BA $1.0mg{\cdot}L^{-1}$. There results indicate that there is a difference for the optimum regeneration condition between the mutant varieties derived from one original variety. These results will be useful for construction of efficient regeneration system of diverse chrysanthemum mutants developed by mutation breeding.

The Patient Specific QA of IMRT and VMAT Through the AAPM Task Group Report 119 (AAPM TG-119 보고서를 통한 세기조절방사선치료(IMRT)와 부피적세기조절회전치료(VMAT)의 치료 전 환자별 정도관리)

  • The aim of this study was to evaluate the patient specific quality assurance (QA) results of intensity modulated radiation therapy (IMRT) and volumetric modulated arc therapy (VMAT) through the AAPM Task Group Report 119. Using the treatment planning system, both IMRT and VMAT treatment plans were established. The absolute dose and relative dose for the target and OAR were measured by using an ion chamber and the bi-planar diode array, respectively. The plan evaluation was used by the Dose volume histogram (DVH) and the dose verification was implemented by compare the measured value with the calculated value. For the evaluation of plan, in case of prostate, both IMRT and VMAT were closed the goal of target and OARs. In case of H&N and Multi-target, IMRT was not reached the goal of target, but VMAT was reached the goal of target and OARs. In case of C-shape(easy), both were reached the goal of target and OARs. In case of C-shape(hard), both were reached the goal of target but not reached the goal of OARs. For the evaluation of absolute dose, in case of IMRT, the mean of relative error (%) between measured and calculated value was $1.24{\pm}2.06%$ and $1.4{\pm}2.9%$ for target and OAR, respectively. The confidence limits were 3.65% and 4.39% for target and OAR, respectively. In case of VMAT the mean of relative error was $2.06{\pm}0.64%$ and $2.21{\pm}0.74%$ for target and OAR, respectively. The confidence limits were 4.09% and 3.04% for target and OAR, respectively. For the evaluation of relative dose, in case of IMRT, the average percentage of passing gamma criteria (3mm/3%) were $98.3{\pm}1.5%$ and the confidence limits were 3.78%. In case of VMAT, the average percentage were $98.2{\pm}1.1%$ and the confidence limits were 3.95%. We performed IMRT and VMAT patient specific QA using TG-119 based procedure, all analyzed results were satisfied with acceptance criteria based on TG-119. So, the IMRT and VMAT of our institution was confirmed the accuracy.

Estimate and Analysis of Planetary Boundary Layer Height (PBLH) using a Mobile Lidar Vehicle system (이동형 차량탑재 라이다 시스템을 활용한 경계층고도 산출 및 분석)

  • Planetary Boundary Layer Height (PBLH) is a major input parameter for weather forecasting and atmosphere diffusion models. In order to estimate the sub-grid scale variability of PBLH, we need to monitor PBLH data with high spatio-temporal resolution. Accordingly, we introduce a LIdar observation VEhicle (LIVE), and analyze PBLH derived from the lidar loaded in LIVE. PBLH estimated from LIVE shows high correlations with those estimated from both WRF model ($R^2=0.68$) and radiosonde ($R^2=0.72$). However, PBLH from lidar tend to be overestimated in comparison with those from both WRF and radiosonde because lidar appears to detect height of Residual Layer (RL) as PBLH which is overall below near the overlap height (< 300 m). PBLH from lidar with 10 min time resolution shows typical diurnal variation since it grows up after sunrise and reaches the maximum after 2 hours of sun culmination. The average growth rate of PBLH during the analysis period (2014/06/26 ~ 30) is 1.79 (-2.9 ~ 5.7) m $min^{-1}$. In addition, the lidar signal measured from moving LIVE shows that there is very low noise in comparison with that from the stationary observation. The PBLH from LIVE is 1065 m, similar to the value (1150 m) derived from the radiosonde launched at Sokcho. This study suggests that LIVE can observe continuous and reliable PBLH with high resolution in both stationary and mobile systems.
