수중재활운동이 뇌졸중 환자의 신체기능과 폐기능에 미치는 영향 (The Effect of Aquatic Exercise on the Improvement of Physical and Pulmonary Function After Stroke)

  • Purpose: This study examined the effect of aquatic exercise on the improvement in physical and pulmonary function after stroke. Methods: Fourteen candidates, who had experienced stroke, were enrolled in this study. The program was carried out three times weekly, 1 hour per session and for 10 consecutive weeks. At pre-treatment and post-treatment, the subjects were tested with a 10 m and 100 m timed gait test, a timed get up and go test, a functional reach test, the difference in thoracic girth at inspiration and expiration, and breaths per minute. The forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) were measured using a spirometer. Results: After ten weekends of an aquatic exercise program, the subjects showed a significant difference in all the test results except for the FEV1 (p<0.05). Conclusion: Intervention with this aquatic exercise program can improve the physical and pulmonary function in people who have had a stroke.

뇌졸중 후 수부 부종에 대한 아시혈 침치료의 효과 (The Effect of A-Shi Point Acupuncture on the Post-stroke Hand Edema)

  • Objectives : The purpose of this study is to evaluate the effect of A-Shi point acupuncture on the post-stroke hand edema. Methods : The subjects in this study were 8 patiens with post-stroke hand edema. The patients were treated with acupuncture on A-Shi point in hand. We treated them once a day, 7 days. The effectiveness was assessed by hand volumeter and mesuring tape. And Wilcoxon signed rank test was performed to evaluate the effect. Results : A significant change was observed after 7 times acupuncture treatment. Both volume and circumference of hand were decreased after treatment. Conclusions : This study suggests that A-Shi point acupuncture is effective for reducing post-stroke hand edema, although further study would be necessary.

아급성과 만성 뇌졸중 환자 및 정상인의 심폐와 보행 기능 비교 (Comparison of Pulmonary and Gait Function in Subacute or Chronic Stroke Patients and Healthy Subjects)

  • Purpose: The aim of this study was to evaluate functional gait and pulmonary function of patients with subacute or chronic stroke for aperiod after the onset of stroke. Healthy people of similar ages served as a control group. The study focused on the clinical importance of intervention with cardiopulmonary rehabilitation treatment in patients with stroke. Methods: The standard time period used to differentiate the subacute and the chronic stroke groups was six months. Each group, including the control, was allocated 11 subjects. Vital capacity (VC), forced vital capacity (FVC), and forced expiratory volume at one second ($FEV_1$) were measured with a spirometer for each subject. Walk tests (10 m and 6 min) and functional gait tests were then conducted. Results: Significant differences were noted for VC, FVC, and $FEV_1$ between the subacute stroke group and the normal group and between the chronic stroke group and the normal group (p<0.05), but no significant difference was evident between the subacute stroke group and the chronic stroke group (p>0.05). No significant difference was seen between the subacute stroke group and the chronic stroke group in the 6min walk test (p>0.05), whereas a significant difference was observed between the subacute stroke group and the chronic stroke group in the 10 m walk test (p<0.05). Conclusion: The general rehabilitation treatment is effective with respect to functional aspects, but it has limited effect in improving pulmonary function and muscular endurance. Therefore, additional intervention of cardiopulmonary rehabilitation is necessary in the rehabilitation treatment process of patients with stroke.

목 안정화와 호흡 재교육 운동이 만성 뇌졸중 환자의 목 깊은 굽힘근육의 두께, 노력성 폐활량과 최대 기침 유량에 미치는 효과 (Effects of the Neck Stabilizing Exercise Combined With the Respiratory Reeducation Exercise on Deep Neck Flexor Thickness, Forced Vital Capacity and Peak Cough Flow in Patients With Stroke)

  • Impaired respiratory function is common in patients with stroke. The purpose of this study were to investigate the effectiveness of exercises and to assess forced vital capacity and peak cough flow after completion of neck stabilizing and respiratory reeducation exercises (combining diaphragmatic breathing and pursed-lip breathing exercises). The 45 participants were randomly assigned to an experimental group 1 ($n_1=15$), experimental group 2 ($n_2=15$), and a control group ($n_3=15$). All subjects performed conservative physical therapy for 30 minutes. Experimental group 1 undertook the neck stabilizing exercise and the respiratory reeducation exercise. Experimental group 2 undertook the respiratory reeducation exercise. Additional exercise did not exceed 30 minutes, five times a week for six weeks. The subjects were assessed for deep neck flexor thickness and breathing function (forced vital capacity, forced expiratory volume at one second, forced expiratory volume at one second/forced vital capacity, peak expiratory flow, and manual assisted peak cough flow) at pre-post value. The results of this study were as follows. Experimental group 1 showed a significant increase only in deep neck flexor thickness change rate (p<.05). All groups showed significant increases in forced vital capacity, forced expiratory volume at one second, and peak expiratory flow in pre-post measurement (p<.05). Experimental groups 1 and 2 showed an increase in manual assisted peak cough flow in pre-post measurement (p<.05). There was no significant difference between experimental group 1 and experimental group 2, but experimental group 1 improved more than experimental group 2 in respiratory function as a whole. In conclusion, these findings suggest that the neck stabilizing exercise in combination with the respiratory reeducation exercise can improve forced vital capacity and peak cough flow in patients with stroke.

  • Objective : The purpose of this study was to investigate the neuroprotective effect of Jukryuk on 4-vessel occlusion(4-VO) and middle cerebral artery (MCA) ischemia. Method : After administration of Jukryuk, we compared the Jukryuk-treated group, the control, and the sham groups, in view of several points as follows 1) We evaluated the damage characterized by coagulative cell change of pyramidal neurons and pronounced gliosis in each group 2) We counted the number of normal pyramidal shapes after ischemia in each group 3) Immunohistochemistry (cyclooxygenase-2) 4) In focal ischemic injury model, we measured the volume of ischemic area Results : In this experiment, the effect of Jukryuk was determined to be protecting neuron cell shape, reducing the number of neuron cells damaged by ischemia and the volume of the ischemic area. In immunohistochemistry, Jukryuk reduced cyclooxygenase-2 expression Conclusions : According to this study, Jukryuk can protect neuron cells from injury by cerebrovascular ischemia.

Uni-flow 소기방식 2행정 프리피스톤 수소기관의 스트로크변화에 따른 역화 특성 (The Characteristics of Backfire for 2 stroke Free-Piston Hydrogen Fueled Engine with Uni-flow Scavenging)

  • Backfire characteristics for hydrogen fueled free piston engine with uni-flow scavenging is investigated with different stroke, exhaust vlave openning timing and fuel-air equivalence ratio by using RICEM (Rapid Intake Compression Expansion Machine) for combustion research of free piston engine. As results, it is found that backfire can be occurred due to slow combustion of unhomogeneous mixture in the piston crevice volume or/and in the cylinder near piston head. And the more stroke of free piston H2 engine with uni-flow scavenging is short the more opening timing of exhaust valve have to be advanced to control backfire.

한의학적 중풍 동물 모델 설정을 위한 실험적 연구 (The Experimental Study on the Animal Stroke Model of Oriental Medicine)

  • The purpose of the present study was to explore the proper method for animal stroke model of Oriental medicine To this end, brain ischemia was induced by distal middle cerebral artery occlusion(dMCAO) and proximal middle cerebral artery occlusion(pMCAO) and evaluated with the method of Triphenyl Tetrazolium Chloride (TTC) staining and Swimming Behavior Test. Results demonstrated that first, infarct size and volume of pMCAO group were significantly bigger that those of dMCAO group. Second, analysis of swimming behavior test revealed that the percentage of left turning angles of pMCAO was significantly bigger than that of dMCAO. Third, during swimming behavior test, there were peculiar traces of small successive circles that represent motor dysfunction and conscious disturbance among dMCAO group. The results of the study thus indicate that non-invasive intraluminal method of pMCAO was the appropriate animal stroke model for Oriental medicine in the light of brain ischemia as hemiplesia and conscious disturbance.

Effect of Mekenzie Lumbar Support on Pulmonary Function for Wheelchair Patients with Stroke

  • This study aimed to determine the effect of McKenzie lumbar support on pulmonary function in Stroke patients. Twenty subjects (n=20) were divided into two groups: a McKenzie lumbar support group (MLS group=10), a control group (n=10). Pulmonary function was performed to assess its effectiveness. A spirometer was used to measure the forced vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow (PEF). The intervention was conducted for four weeks. In the MLS group, FEV1, FVC, and PEF were increased after McKenzie lumbar support. (p<0.05), while no significant differences in the variables were found in the control group (p>0.05). There were no significant differences in variables between the MLS group and the control group (p>0.05). Our findings suggest that applying Mckenzie lumbar support may be an alternative maneuver to improve pulmonary function in stroke patients.

Pulmonary Function Index Comparisons Depending on Various Postures of Stroke Patients

  • PURPOSE: To prevent secondary complications from decreased pulmonary functions and promote neurological recovery, identification of respiratory capacity change patterns depending on different postures of stroke patients and investigation of their properties are needed for active rehabilitation. Therefore, this study was conducted to investigate the changes in vital capacity in response to different positions and to implement the results as clinical data. METHODS: A respiratory function test was administered to 52 patients with stroke in the sitting, supine, paretic side lying, and non-paretic side lying positions. Pulmonary function indexes used for comparison were forced vital capacity (FVC), forced expiratory volume at 1 second (FEV1), forced expiratory flow 25-75% (FEF 25-75%), and maximum voluntary ventilation (MVV). One-way repeated ANOVA was used for analysis, and post hoc analysis was conducted using least significant difference (LSD). RESULTS: All pulmonary function indexes were measured in the order of sitting, paretic side lying, supine, and non-paretic side lying positions. Excluding the FEF25-75% and MVV of the supine compared with the paretic side lying position, all other pulmonary function indexes differed significantly (p<.05). CONCLUSION: There are differences in pulmonary function indexes depending on different postures of stroke patients, and the study showed that the non-paretic side lying position yielded the greatest effect on lung ventilation mechanisms. Based on these results, appropriate postures need to be considered during physical therapy interventions for stroke patients.

허혈성 자극에 의한 심방이뇨 호르몬 분비 반응의 특성 (Characteristics of hypoxia-induced ANP Secretion in Perfused Beating Atria)

  • Background: Cardiac atrium is an endocrine gland secreting a family of natriuretic peptides. The secretion of atrial natriuretic peptide(ANP) had been shown to be controlled by variable factors. The change in atrial dynamics have been considered as one of the most prominent stimuli for the stimulation of ANP secretion. Hypoxic stress has been shown to increase cardiac ANP secretion. However, the mechanism by which hypoxia increases ANP secretion cardiac ANP secretions. However, the mechanism by which hypoxia increases ANP secretion has not to be defined. Therefore, the purpose of the present study was tow-fold: to develop a protocol to defined the effect of hypoxia on ANP secretion in perfused beating rabbit atria and to clarify the mechanism responsible for the accentuation by hypoxia of ANP secretion. Material and Method: Experiments have been done in perfused beating rabbit atria. ANP was measured by radioimmunoassay. Result: Hypoxic stimulus with nitrogen decreased atrial stroke volume. The decrease in atrial stroke volume recovered basal level during the period of recovery with oxygen. ANP secretion and the concentration of perfusate ANP in terms of extracellular fluid(ECF) translocation which reflects the rate of myocytic release of ANP were increased by hypoxia and returned to basal levels during the recovery. Changes in ECF translocation paralleled by hypoxia and returned to basal levels during the recovery. Changes in ECF translocation paralleled to that of atrial stroke volume. At the start of recovery in atrial storke volume, ECF tranalocation incrased for several minutes. The above responses were stable and reproducible. Glibenclamide treatment prevented the recovery in atrial stroke volume. Increments by hypoxia of ANP secretion and ANP concentration were suppressed by glibenclamide. Conclusion: These results indicate that hypoxia incrased atrial myocytic ANP release and that the mechanism responsible for the accentuation is partially related to the change in K+ATP channel activity.
