  • Objectives : The purpose of this study was to understanding the constraint induced movement and Self-efficacy with arm training on upper motor function in Stroke Patients. Methods : Stroke, the leading cause of functional disability, causes a variety of impairments that compromise quality of life. Upper limb hemiparesis, a commonly seen impairment, is particularly problematic given its impact on activities of daily living. Because stroke was a disease to correspond to the first during domestic cause of death, and was accompanied by a lot of side aftereffects after a survival, stroke rehabilitation bought a patient and a family and a physical therapist, and it was main concern of. Results : Looks into upper extremity excrise of a subacute stroke patient estranged a acute convalescence later by a rehabilitation treatment in this consideration, and evaluates an effect to wind up constraint induced movement for an early treatment of stroke and Self efficacy, and help is one to an early rehabilitation of an stroke patient. Conclusions : Overuse sound tends after the stroke occurrence in the early stage in order to recompense for stroke, and at the time of a new aspect called learned nonuse syndrome by a movement of a paralysis part dusting off wealth with this step thing later. Constraint induced movement using self efficacy could be an effective for improving function of stroke.

Three-dimensional kinematic motion analysis of door handling task in people with mild and moderate stroke

  • Objective: This study aimed to quantify one of the useful upper extremity movements to evaluate motor control abilities between the groups of people with mild and moderate arm impairments performing a door handling task. Design: Cross-sectional study. Methods: Twenty-one healthy participants and twenty-one persons with chronic stroke (9 mild stroke and 12 moderate stroke) were recruited for this study. Stroke participants were divided into 2 groups based on Fugle-Meyer Assessment scores of 58-65 (mild arm) and 38-57 (moderate arm). All they performed door handling task including the pronation and supination phases 3 times. We measured some movement factors which were reaction time, movement time, hand of peak velocity, hand of movement units to perform door handling task using the three-dimensional motion analysis. Results: The majority of kinematic variables showed significant differences among study groups (p<0.05). The reaction time, total and phase of movement time, hand of peak velocity, the number of movement units discriminated between healthy participants and persons with moderate upper limb stroke (p<0.05). In addition, reaction time, total and phase of movement time, the number of movement units discriminated between those with moderate and mild upper limbs of stroke patients (p<0.05). Conclusions: Three-dimensional kinematic motion analysis in this study was a useful tool for assessing the upper extremity function in different subgroups of people with stroke during the door handling task. These kinematic variables may help clinicians understand the arm movements in door handling task and consist of discriminative therapeutic interventions for stroke patients on upper extremity rehabilitation.

양손 운동이 만성 뇌졸중 환자의 뇌활성도와 근활성도에 미치는 영향 (Effect of Bilateral Arm Movement on Brain and Muscle Activity in Chronic Stroke Patients)

  • PURPOSE: This study investigated the neurophysiological and behavioral adaptation during one or both hands movement in chronic stroke patients. METHODS: The study included sixteen hemiplegic stroke patients. Neurophysiological data (brain activation and muscle activation) were examined by electroencephalography (EEG) and electromyography (EMG), and behavioral adaptation was examined by wrist extension angle during wrist extension with one hand or both hands. Outcome variables of one hand or both hands were; mu rhythm of the EEG, EMG amplitude of wrist extensor and flexor muscles, and wrist angle of Myomotion 3D motion analysis. RESULTS: Our results revealed that wrist extension angle was significant increased during both hands movement compared to one hand movement (p<.05). Furthermore, in affected sensorimotor area, there was significant increase in the brain activation during both hands movement compared to one hand movement (p<.05). However, there was no significant different between one hand and both hands movement in muscle activation (p>.05). CONCLUSION: According to the findings of this experiment, bilateral arm movement improved brain activity on affected sensorimotor area and wrist extension angle. Therefore, we suggest that bilateral arm movement would positive effect on stroke rehabilitation in terms of increase in brain activation on affected motor area and wrist extension during bilateral arm movement.

뇌졸중 환자 및 보호자의 건강신념이 재활치료 이행 및 일상생활동작 수행능력에 미치는 영향 (Health beliefs of patients and caregivers of stroke rehabilitation in the implementation and impact the ability to perform everyday actions)

  • Purpose : This study attempts to assist stroke patients lead independent daily life movements by providing basic data for stroke patients' successful rehabilitation program and understanding the relationship between health beliefs of stroke patients and their guardians, and patients' rehabilitation performance level and daily life movement fulfillment ability. Methods : 23 stroke patients receiving treatments at rehabilitation specializing hospital after getting diagnosed with a stroke, along with 23 guardians were selected as research subjects, and health belief was measured by modifying supplementing Byun Young-Hee(2002)'s health belief measurement tool for leg movement disorder patients. Research findings revealed the following relationship between health beliefs of stroke patients and their guardians, and patients' rehabilitation performance and daily life movement fulfillment ability. Results : The patients group's initial MBI score was $33.13{\pm}3.46$, and measurement after four weeks was $38.43{\pm}3.47$. As health beliefs got stronger, MBI score increased significantly(p<.05). Correlation analyses of factors that affect rehabilitation attendance rate showed that perceived susceptibility, seriousness and usefulness were statistically significant, and perceived disability was not statistically significant(p<.05). Guardians' health belief index(susceptibility, seriousness, usefulness, disability) had no correlation with rehabilitation attendance rate. Conclusion : In order to increase daily life movement fulfillment ability of patients who are being treated with stroke, health beliefs must be raised to increase changes in daily life movement fulfillment ability. Therefore, it is believed that managing programs that can increase health beliefs of stroke patients can allow patients obtain positive health beliefs, further increase rehabilitation performance rate of stroke patients as well as independent daily life ability.

Development of EMG-Triggered Functional Electrical Stimulation Device for Upper Extremity Bilateral Movement Training in Stroke Patients: Feasibility and Pilot study

  • Objective: Bilateral movement training is an effective method for upper extremity rehabilitation of stroke. An approach to induce bilateral movement through functional electrical stimulation is attempted. The purpose of this study is to develop an EMG-triggered functional electrical stimulation device for upper extremity bilateral movement training in stroke patients and test its feasibility. Design: Feasibility and Pilot study design. Methods: We assessed muscle activation and kinematic data of the affected and unaffected upper extremities of a stroke patient during wrist flexion and extension with and without the device. Wireless EMG was used to evaluate muscle activity, and 12 3D infrared cameras were used to evaluate kinematic data. Results: We developed an EMG-triggered functional electrical stimulation device to enable bilateral arm training in stroke patients. A system for controlling functional electrical stimulation with signals received through a 2-channel EMG sensor was developed. The device consists of an EMG sensing unit, a functional electrical stimulation unit, and a control unit. There was asymmetry of movement between the two sides during wrist flexion and extension. With the device, the asymmetry was lowest at 60% of the threshold of the unaffected side. Conclusions: In this study, we developed an EMG-triggered FES device, and the pilot study result showed that the device reduces asymmetry.

  • The aim of this study was performed to determine the inter-rater reliability of the Stroke Rehabilitation Assessment of Movement (STREAM) translated in Korean. This was a new clinical measurement tool for evaluating the recovery of voluntary movement and basic mobility following stroke. A direct-observation reliability study was conducted on 20 patients who had strokes and were in a rehabilitation setting. Subjects were assessed by two physical therapists. The reliability of the STREAM scores was demonstrated by weighted kappa statistics for inter-rater agreement on scores for individual items ranged from .83 to 1.0, intraclass correlation coefficients for total score was .99, and for subscale scores was ranged from .96 to .99. The internal consistency of the STREAM scores was demonstrated by Cronbach alphas of greater than .99 on the subscales and overall. These high levels of reliability support the use of the STREAM translated in Korean instrument for the measurement of motor recovery following stroke.

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Traditional Korean Medicine(TKM) on a patient with abnormal eye movement. ■ Methods A patient with abnormal eye movement of limbs diagnosed with midbrain infarction was treated with herbal medication, acupuncture, moxa, and herbal medical injection. Then we evaluated the improvement by measuring range of eye movement. ■ Results Increase of range of eye movement and improvement of symptom of diplopia were observed after the TKM treatment. ■ Conclusion This study proved the effect of TKM treatment on abnormal eye movement due to midbrain infarction.

뇌졸중 환자의 앉은 자세에서 일어서기 동작 수행에 영향을 주는 다리 근 활성 비교 (A Comparison of Lower Limb Muscle Activity Affecting Sit-to-Stand Movement of Stroke Patients)

  • Purpose: This study aimed (1) to identify asymmetry observed in electromyography (EMG) activity patterns of lower limb muscles between affected and unaffected sides of stroke patients during sit to stand (STS) movement and (2) to identify the muscles of the paretic lower limb that affected STS performance in stroke patients. Methods: This study consisted of 27 patients (15 males and 12 females) with poststroke hemiparesis. The activation of the rectus femoris (RF), tibialis anterior (TA), and gastrocnemius (GCM) of the affected side and unaffected side during STS movement were recorded using surface EMG. In addition, EMG lower limb muscle activation patterns were measured in the all section, pre-thigh off (pre-TO), and post-thigh off (post-TO) section. All measurements were obtained during five repetitions of the STS (FTSTS) movement. An independent t-test was performed to compare the affected side and unaffected side of the lower limb during the STS movement. A stepwise linear regression analysis was conducted to identify the muscles that affected STS performance. Results: Activation of both the RF and TA significantly affected the lower limb during the STS movement (p<0.05). Two muscles showed a difference on the post-TO section. The results of the stepwise multiple regression analysis showed that the RF and TA muscles of the affected lower limb were affected in FTSTS. Conclusion: These results provide a basis for reinforcement training, focusing on the RF and TA muscles of the affected side of stroke patients to improve asymmetric movement and performance in STS movement.

뇌졸중환자의 동적 균형 평가를 위한 sit-to-walk의 신뢰도 연구 (A Reliability Study of Sit-to-walk for Dynamic Balance Assessment in Stroke Patient)

  • Purpose: The purposes of this study were to evaluate the correlation of clinical tools for assessment of balance and ability of gait, in order to discriminate the phases of sit-to-walk movement of patients with stroke using the motion analysis system, and to investigate the reliability of the phase of sit-to-walk movement according to functional ability of patients with stroke. Methods: Twenty -one patients participated (men 17, women 4) in this study. Sit-to-walk movement of all patients was recorded by the motion analysis system. Berg Balance Scale, Timed Up and Go test, Functional Reach Test, 10 meter Walk Timed Test, and Performance-Oriented Mobility Assessment were used as functional assessment tools. Results: The results of this study showed significant correlation between the phase I, II, IV and total phase duration of sit-to-walk movement and functional assessment tools. In addition, the intraclass correlation coefficient (ICC) showed high reliability in accordance with the functional ability of patients with stroke (Pearson's r 0.93 to 1.00). Conclusion: In conclusion, there is high reliability between measures of the phase of sit-to-walk movement of chronic stroke patients and the clinical assessment tool. Results of this study suggest that measurement of the phase of sit-to-walk movement can be used significantly as an intervention and a clinical tool for patients with stroke.

뇌졸중 환자에서 양손 보완운동의 운동형상학 (Kinematics of Bimanual Complementary Movement in Stroke Patients)

  • 본 연구는 뇌졸중 환자를 대상으로 양손 보완운동 및 한손운동을 수행하는 동안 환측의 움직임을 측정하여, 양손 보완운동의 효과를 검증하고자 수행되었다. 대상자는 부산지역에 거주하는 뇌졸중 후 편마비 환자 중 12가지 기준을 적용하여 30명을 선정하였다. 과제 수행 동안 환측의 움직임 특성을 측정하기 위해 피트메터(Fitmeter) 가속도계를 부착하여 신호벡터크기(Signal Vector Magnitude: SVM), 최고 가속도(peak acceleration), 최고 감속도(peak deceleration)를 측정하였다. 연구 결과 양손 보완운동은 한손운동보다 수행시간이 짧았고(p<0.05), 신호벡터크기가 감소하였다(p<0.05). 따라서 뇌졸중 환자가 일상생활동작을 수행할 때 환측만 사용하는 것보다 건측을 보완적으로 사용하는 것이 운동형상학적으로 움직임 향상에 도움이 될 것으로 생각된다.
