A Novel Technique of Hand-Sewn Purse-String Suturing by Double Ligation Method (DLM) for Intracorporeal Circular Esophagojejunostomy

  • Purpose: The optimal method for intracorporeal esophagojejunostomy remains unclear because a purse-string suture for fixing the anvil into the esophagus is difficult to perform with a laparoscopic approach. Therefore, this study aimed to evaluate our novel technique to fix the anvil into the esophagus. Materials and Methods: This retrospective study included 202 patients who were treated at our institution with an intracorporeal circular esophagojejunostomy in a laparoscopy-assisted total gastrectomy with a Roux-en-Y reconstruction (166 cases) or a laparoscopy-assisted proximal gastrectomy with jejunal interposition (36 cases). After incising 3/4 of the esophageal wall, a hand-sewn purse-string suture was placed on the esophagus. Next, the anvil head of a circular stapler was introduced into the esophagus. Finally, the circular esophagojejunostomy was performed laparoscopically. The clinical characteristics and surgical outcomes were evaluated and compared with those of other methods. Results: The average operation time was 200.3 minutes. The average hand-sewn purse-string suturing time was 6.4 minutes. The overall incidence of postoperative complications (Clavien-Dindo classification grade ${\geq}II$) was 26%. The number of patients with an anastomotic leakage and stenosis at the esophagojejunostomy site were 4 (2.0%) and 12 (6.0%), respectively. All patients with stenosis were successfully treated by endoscopic balloon dilatation. There was no mortality. Regarding the materials and devices for anvil fixation, only 1 absorbable thread was needed. Conclusions: Our procedure for hand-sewn purse-string suturing with the double ligation method is simple and safe.

  • The equation of motion of the drill string along the excavation trajectory was analyzed using the Lagrangian approach together with the finite element method (FEM). A drill string of circular cross section is constructed by combining a plurality of circular axes each having 12 degrees of freedom (DOF). FEM analysis can observe the vibration and dynamic changes of the entire drill string, and it is easy to apply comprehensive boundary conditions to reproduce the simulation of a realistic drill string. In this study, the constructed FEM motel was simulated. In order to apply the FEM program to the actual drill trajectory, the dynamic analysis of the curved beam was verified by comparison with the actual values. The dynamic change over time was observed.

Boundary Control of Axially Moving Continua: Application to a Zinc Galvanizing Line

  • In this paper, an active vibration control of a tensioned, elastic, axially moving string is investigated. The dynamics of the translating string are described with a non-linear partial differential equation coupled with an ordinary differential equation. A right boundary control to suppress the transverse vibrations of the translating continuum is proposed. The control law is derived via the Lyapunov second method. The exponential stability of the closed-loop system is verified. The effectiveness of the proposed control law is simulated.

  • In this paper, an active vibration control of a translating tensioned string with the use of an electro-hydraulic servo mechanism at the right boundary is investigated. The dynamics of the moving strip is modeled as a string with tension by using Hamilton’s principle for the systems with changing mass. The control objective is to suppress the transverse vibrations of the strip via boundary control. A right boundary control law in the form of current input to the servo valve based upon the Lyapunov’s second method is derived. It is revealed that a time-varying boundary force and a suitable passive damping at the right boundary can successfully suppress the transverse vibrations. The exponential stability of the closed loop system is proved. The effectiveness of the control laws proposed is demonstrated via simulations.

  • To prevent an insulator failure, a new inspection system is presented for 154kV insulator strings. The inspection system detects fault insulators by measuring resistance and voltage of each insulators in the 154kV insulator string. The inspection instrument obtains autonomously insulation resistance and assigned voltage along the insulator string. The insulator inspection instrument is manipulated by two operators. In addition, a set of mechanism is applied to prevent the system from being reset under live-line condition. We confirmed its effectiveness through experiments.

  • Parsing 작업의 결과인 Parse Tree 정보는 문장에 관한 구조적 정보를 가지고 있는 Tree 정보로 이 정보를 이용하여 정보 검색에 활용하는 알고리즘을 제안한다. 제안하는 알고리즘은 XML 기술과 스트링 매칭 기법을 이용하였으며, 사용한 스트링 매칭 기법은 Approximate String Matching 기법이다. Query 정보와 문서 정보를 Parsing하여 얻은 Parse Tree를 XML 형태의 정보로 변환한 후, 두 정보를 가지고 Approximate String Matching 기법을 적용하여 Query 정보와 문서 정보 간의 유사도를 계산한다. 제안하는 알고리즘의 장점은 구조 기반의 정보 검색 기능이 가능하고 비슷한 정보에 대한 검색 기능이 가능하며 비슷한 구조에 대한 검색 기능이 가능하다는 것이다.

Developing JSequitur to Study the Hierarchical Structure of Biological Sequences in a Grammatical Inference Framework of String Compression Algorithms

  • Grammatical inference methods are expected to find grammatical structures hidden in biological sequences. One hopes that studies of grammar serve as an appropriate tool for theory formation. Thus, we have developed JSequitur for automatically generating the grammatical structure of biological sequences in an inference framework of string compression algorithms. Our original motivation was to find any grammatical traits of several cancer genes that can be detected by string compression algorithms. Through this research, we could not find any meaningful unique traits of the cancer genes yet, but we could observe some interesting traits in regards to the relationship among gene length, similarity of sequences, the patterns of the generated grammar, and compression rate.

  • This paper presents the segmentation of sentence area and diagram area from docwnent image. For extracting the sentence area, we perform the Dilation, basic operation of Morphology, to the document image and obtain the smeared document image. After the smeared docwnent image is blocked, we determine the writing form by the vertical and horizontal characteristics of the document image and calculate the skew from it. And then, we relocate the document image and extract the chatacter string from the relocated docwnent. 11 document images of three classes are considered and the character string has been well extracting from 11 document images.

  • A new method that improves the performance of large scale voice dialling system is presented using speaker adaptation. Since SI (Speaker Independent) based speech recognition system with phoneme HMM uses only the phoneme string of the input sentence, the storage space could be reduced greatly. However, the performance of the system is worse than that of the speaker dependent system due to the mismatch between the input utterance and the SI models. A new method that estimates the phonetic string and adaptation vectors iteratively is presented to reduce the mismatch between the training utterances and a set of SI models using speaker adaptation techniques. For speaker adaptation the stochastic matching methods are used to estimate the adaptation vectors. The experiments performed over actual telephone line shows that proposed method shows better performance as compared to the conventional method. with the SI phonetic recognizer.

  • Time-dependent frequency and energy of free vibration of the Spagetti problem, that is the axially moving continuum with time-varying length, are investigated. Exact expressions for the natural frequency and time-varying vibration energy are derived by dealing with traveling waves. When the string length is increased, the vibration period increases, but the free vibration energy varies as a function of both translating velocity and boundary velocity of the continuum. However, when the string undergoes retraction, the vibration energy increases with time, String tension together with non-zero instantaneous velocity at the moving boundary results in energy variation.
