PFAPA (주기적발열, 아프타구내염, 인두염, 경부 림프절염) 증후군 1례 (A case of PFAPA (periodic fever, aphthous stomatitis, pharyngitis, cervical adenitis) syndrome)

  • PFAPA 증후군은 주기적 발열과 아프타구내염, 인두염, 경부림프절염 등의 증상을 특징으로 하는 중후군으로 소아에서 발생하는 주기적 발열의 원인 중 하나이다. PFAPA 증후군은 원인과 병태생리가 아직 알려져 있지 않다. 그리고, 검사상 다른 특이적 이상이 없기 때문에 임상증상에 기초하여 다른 질환들을 감별함으로써 진단할 수 있다. 현재까지 알려진 가장 효과적인 치료 방법은 프레드니솔론을 경구로 투여하는 것이다. 프레드니솔론을 경구 투여한 후 증상은 즉각적인 호전을 보인다. 대부분 장기적 합병증 없이 회복되고, 성장과 발달 역시 정상적인 경과를 밟는다. 저자들은 3 개월간 주기적 발열을 보였던 3세 남아에서 다른 질환들을 감별한 후 PFAPA 증후군의 진단 기준을 만족하여 프레드니솔론을 경구 투여 한 후 증세 호전을 보였던 PFAPA 증후군 1례를 보고한다.

Vesicular Stomatitis Virus G Glycoprotein and ATRA Enhanced Bystander Killing of Chemoresistant Leukemic Cells by Herpes Simplex Virus Thymidine Kinase/Ganciclovir

  • Refractoriness of acute myeloid leukemia (AML) cells to chemotherapeutics represents a major clinical barrier. Suicide gene therapy for cancer has been attractive but with limited clinical efficacy. In this study, we investigated the potential application of herpes simplex virus thymidine kinase/ganciclovir (HSV-TK/GCV) based system to inhibit chemoresistant AML cells. We first generated Ara-C resistant K562 cells and doxorubicin-resistant THP-1 cells. We found that the HSV-TK/GCV anticancer system suppressed drug resistant leukemic cells in culture. Chemoresistant AML cell lines displayed similar sensitivity to HSV-TK/GCV. Moreover, HSV-TK/GCV killing of leukemic cells was augmented to a mild but significant extent by all-trans retinoic acid (ATRA) with concomitant upregulation of Connexin 43, a major component of gap junctions. Interestingly, HSV-TK/GCV killing was enhanced by expression of vesicular stomatitis virus G glycoprotein (VSV-G), a fusogenic membrane protein, which also increased leukemic cell fusion. Co-culture resistant cells expressing HSV-TK and cells stably transduced with VSV-G showed that expression of VSV-G could promote the bystander killing effect of HSV-TK/GCV. Furthermore, combination of HSV-TK/GCV with VSV-G plus ATRA produced more pronounced antileukemia effect. These results suggest that the HSV-TK/GCV system in combination with fusogenic membrane proteins and/or ATRA could provide a strategy to mitigate the chemoresistance of AML.

VSV-G Viral Envelope Glycoprotein Prepared from Pichia pastoris Enhances Transfection of DNA into Animal Cells

  • Vesicular stomatitis virus G glycoprotein (VSV-G) has been widely used for pseudotyping retroviral, lentiviral, and artificial viral vectors. The objective of this study was to establish a potential approach for large-scale production of VSV-G. To this end, VSV-G was cloned with an N-terminal His-tag into Pichia pastoris expression vector pPIC3.5K. Three clones ($Mut^s$) containing the VSV-G expression cassette were identified by PCR. All clones proliferated normally in expansion medium, whereas the proliferation was reduced significantly under induction conditions. VSV-G protein was detected in cell lysates by western blot analysis, and the highest expression level was observed at 96 h post induction. VSV-G could also be obtained from the condition medium of yeast protoplasts. Furthermore, VSV-G could be incorporated into Ad293 cells and was able to induce cell fusion, leading to the transfer of cytoplasmic protein. Finally, VSV-G-mediated DNA transfection was assayed by flow cytometry and luciferase measurement. Incubation of VSV-G lysate with the pGL3-control DNA complex increased the luciferase activity in Ad293 and HeLa cells by about 3-fold. Likewise, incubation of VSV-G lysate with the pCMV-DsRed DNA complex improved the transfection efficiency into Ad293 by 10% and into HeLa cells by about 1-fold. In conclusion, these results demonstrate that VSV-G could be produced from P. pastoris with biofunctionalities, demonstrating that large-scale production of the viral glycoprotein is feasible.

구중질환(口中疾患)의 침구치료(鍼灸治療)에 관한 고찰(考察) (A study of Literature Review on the acupuncture and moxibution treatments for stomatopathy)

  • Objectives : To study acupuncture and moxibution treatments for stomatopathy( aphthae, oral ulceration, mycolic stomatitis, halitosis, thirst, bitter) the ancient and the present literatures were reviewed. Methods : We've got compared and analyzed 55 kinds of literatures. Results and Conclusions : 1. The acupuncture meridians used frequently for stomatopathy were $Su-yangmy\bar{o}ng-Taejang-ky\bar{o}ng(LI),\;Chok-yangmy\bar{o}ng-Wi-Ky\bar{o}ng(S),\;Immaek-Ky\bar{o}ng(CV)$. 2. The acupoints used frequently for aphthae were $Sungjang(CV_{24}),\;Yomchon(CV_{23}),\;Hapkok(LI_4),\;Nogung(P_8),\;Chok-samni(S_{36}),\;Hyopko(S_6),\;Sugu(GV_{26})$. 3. The acupoints used frequently for oral ulceration were $Hapkok(LI_4),\;Nogung(P_8),\;Chok-samni(S_{36}),\;Kokchi(LI_{11}),\;Sotaek(SI_1),\;Pisu(B_{20}),\;Wisu(B_{21}),\;Samgan(LI_3),\;Yomchon(CV_{23}),\;Chichang(S_4)$. 4. The acupoints used frequently for mycolic stomatitis were $Hapkok(L_4),\;Chichang(S_4),\;Hyopko\;(LI_4),\;Sungjang(CV_{24}),\;Samumgyo(SP_6)$. 5. The acupoints used frequently for halitosis were $Naejong(S_{44}),\;Chok-samni(S_{36}),\;Chung-wan\;(CV_{12}),\;Sang-wan(CV_{13}),\;Hawan(CV_{10}),\;Kongson(SP_4),\;Wisu(B_{21}),\;Nogung(P_8),\;Sugu(GV_{26}),\;Sungjang(CV_{24})$. 6. The acupoints used frequently for thirst were $Sosang(L_{11}),\;Sangyang(LI_1),\;Sotaek(SI_1),\;Kwanch'ung(TE_1),\;Ch'\bar{o}kt'aek(L_5),\;T'ae-gye(K_3),\;Kokt'aek(P_3),\;Sugu(GV_{26}),\;Samgan(LI_3),\;Igan(LI_2),\;T'aech'ung(Liv_3),\;Sojangsu(B_{27})$. 7. The acupoints used frequently for bitter were $Yangn\bar{u}ngch'\bar{o}n(G_{34}),\;Hy\bar{o}njong(G_{39}),\;Kwanch'ung(TE_1),\;Tamsu(B_{19}),\;Chokkyu\bar{u}m(G_{44}),\;Y\bar{o}n-gok(K_2),\;Shinmun(H_7),\;Chok-Samni(S_{36})$.

한양방 병용치료를 받은 110명의 암 환자에 대한 후향적 조사연구 (Study on 110 cancer patients treated by combination of Oriental and conventional treatment)

  • Objective: To investigate a clinical worth of combination therapy of Oriental and conventional cancer treatment focusing on reduction of chemotherapy-induced side effects. Methods: 110 patients treated by Oriental treatment after intravenous or oral chemotherapy were reviewed, from January, 2005 to April, 2008 at the East-West Cancer Center of Dunsan Oriental hospital. Symptoms were investigated by National Cancer Institute-Common Toxicity Criteria (NCI-CTC) Version 2.0. Results: 80% patients of 110 patients had at least one symptom among eight main side effects (neutropenia, anorexia, nausea, vomiting, diarrhea, stomatitis, constipation, headache). The presence of those was as follows: nausea 63%, anorexia 61 %, neutropenia 45%, vomiting 28%, constipation 21 %, headache 19%, diarrhea 11 %, and stomatitis 10%. Except neutropenia, above symptoms has ameliorated by Oriental treatment in seven treat days. Conclusions: This study first presented the general characteristics of cancer patients treated by Oriental and conventional therapy, and showed a clinical potential of combinational therapy aiming to chemotherapy-induced side effects.

괴사성 치은구내염환아의 증례보고 (NECROTIZING GINGIVOSTOMATITIS : CASE REPORT)

  • 괴사성 치은염 (Necrotizing ulcerative gingivitis, NUG), 괴사성 치주염 (necrotizing ulcerative periodontitis, NUP), 괴사성 구내염(necrotizing stomatits, NS), 그리고 수암(noma, cancrum oris)은 급속도로 파괴적이며 쇠약하게 하는 잠재성이 큰 구강감염으로 질환의 같은 진행과정에 대해 다른 임상단계로 간주되기도 하나 집합적으로 Vincent's infection, infectious oral necrosis 또는 괴사성 치은구내염(Necrotizing gingivostomati-tis, NG)라고 한다. 이러한 necrotizing gingivostomatitis(NG)의 발생율은 $0.19{\sim}0.5%$이며 $2{\sim}6$세에 가장 높은 발병율을 보인다. Necrotizing gingivostomati-tis는 치명적인 바이러스 감염과 면역체계가 약화된 경우 발생할 수 있으며 주 진단학적 증상으로 동통, 치간유두 부위의 궤양이나 괴사, 치은출혈이 있으며 이차적인 증상은 위막 등이 있다. 이에 본원 소아과에서 의뢰되어 necrotizing gingivostomatitis로 진단받은 환아의 구강내 소견 및 치료경과에 대해 보고하고자 한다.

  • Baculovirus 발현 시스템은 박테리아 발현 시스템, 특히 복잡한 번역 후 변형을 필요로 하는 것과 비교하여 다량의 재조합 단백질을 생성하는 빠르고 비용 효율적인 방법을 포함하는 많은 알려진 장점을 갖는다. 특히 재조합 baculovirus는 광범위한 포유류 세포 유형에서 벡터를 전달하고 재조합 단백질을 발현 할 수 있다. 본 연구에서는 pcDNA3.1로부터 재구성된 baculovirus 벡터를 사용하였는데 이 벡터는 cytomegalovirus (CMV) 프로모터, uroplakin II promoter, polyhedron promoter, 수포 구내염 바이러스 G (VSVG), 녹색 형광 단백질 (EGFP), 단백질 전달 도메인 (PTD) 유전자 등 다양한 유전자들로 재조합 되어 개발되었다. 이러한 재구성 된 벡터를 다양한 세포 및 세포주에 감염시켰다. 이렇게 개발된 baculovirus 벡터는 재조합된 유전자들의 전이성 및 발현성을 기존의 일반적인 벡터와 비교하여 분석하였다. 본 연구결과로 이렇게 개발된 baculovirus 벡터는 기존의 대조군 벡터보다 전이성 및 발현성면에서 더 높은 효율을 갖는다는 것을 확인하였다.

한국 전통 된장의 구강미생물에 대한 항균효과 (Anti-microbial Activity of Korean Fermented Soybean Paste (Doen-jang) Against Oral Microbes)

  • This study was performed to investigate the anti-microbial activity of extract from Korean fermented soybean paste (doen-jang) against 16 types of oral microbes, and to determine the minimum inhibition concentration (MIC) of the extract for three major microbes causing human oral diseases (Streptococcus mutans, Porphyromonas gingivalis, and Candida albicans). The extract was prepared using ethyl acetate and it was treated with the oral microbes at a concentration of 5.00 mg/ml (0.5%). The anti-microbial activity and MIC were measured using broth dilution method. Significant reduction of microbial activities of 16 types of oral microbes occurred when the soybean paste extract was added to the broth compared to the control (p<0.01), and striking inhibition (more than 99%) was observed in ten types. S. mutans, which causes dental caries, showed MIC at a concentration of 1.25 mg/ml for the extract. P. gingivalis, which causes adult periodontal disease, showed MIC at a concentration of 2.50 mg/ml for the extract. C. albicans, which causes denture stomatitis and angular stomatitis, showed MIC at a concentration of 20 mg/ml for the extract. These results indicate that ethyl acetate extract of doen-jang showed strong anti-microbial effect against 16 types of oral microbes, and the anti-microbial effect of the extract against oral microbes was stronger against bacteria than against fungi. The anti-microbial effect might be possibly enhanced by the fermentation of soybeans.

The Relationship Between Symptoms of Oral Mucosal Diseases and Psychological Status

  • Purpose: The purpose of this study was to assess the relationship between psychological factors and symptoms of oral mucosal diseases by means of the revised version of the Minnesota Multiphasic Personality Inventory (MMPI-2). Methods: Three hundred and eighteen college students in Gyeonggi-do completed the MMPI-2 and a questionnaire related to symptoms of oral mucosal diseases and collected data were analyzed by Student's t-test and one way ANOVA. Results: Subjects who have experienced herpetic stomatitis revealed significantly higher mean values of T-scores than subjects who have not on hypochondriasis (Hs) and hysteria (Hy) clinical scales of the MMPI-2 (p<0.05). In comparison with subjects who have had recurrent aphthous ulceration, subjects who have not had it manifested significantly higher elevations on the schizophrenia (Sc) clinical scale (p<0.05). Subjects who have suffered oral lichen planus (OLP) showed significantly higher mean values of T-scores than subjects who have not on paranoia (Pa) (p<0.05) and hypomania (Ma) (p<0.01) clinical scales. Mean values of T-scores on Ma clinical scale were significantly higher in subjects having experienced glossodynia than in subjects having not experienced (p<0.05). Subjects having felt taste disturbance exhibited significantly higher elevations on Hs (p<0.0001) and Hy (p<0.01) clinical scales compared to subjects having not. Conclusions: Symptoms of oral mucosal diseases such as herpetic stomatitis, taste disturbance, OLP, and glossodynia were found to be related to Hs, Hy, Pa, Ma clinical scales of the MMPI-2. Therefore, comprehensive psychological evaluations of those oral mucosal diseases may be beneficial in a therapeutic approach.
