The hemodynamic characteristics were compared using commercial CFD code for the stenosed coronary and abdominal arteries. Numerical calculations were carried out in the axisymmetric arteries over the stenotic diameter ratios ranging from 0.25 to 0.875 (6 cases) employing the typical physiological flow conditions. In case of the coronary artery, there was only one recirculation zone observed distal to the stenosis throat during the major portion of the period. However, in case of the abdominal aorta, there were complex recirculation regions found proximal and distal to stenosis throat. For both models, the wall shear stresses(WSS) increased sharply in the converging stenosis, reaching a peak just upstream of the throat, and became negative or low values in the post-stenotic recirculation region. As the results, the oscillatory shear index(OSI) was abruptly increased at the stenosis throat. For the coronary stenosis model, the second peak in the OSI was observed distal to the stenosis. The distance between the first peak and the second peak was increased as the degree of the stenosis was raised. On the orther hand, the abdominal stenosis model showed a complex oscillatory behavior in the OSI index and did not showed such a strong second peak. As the degree of stenosis was increased, recirculation regions of the both arteries were extended much longer and flow pattern became more complex.

The characteristics of blood flow and the interaction between the blood vessel and blood flow play important roles in plaque cap rupture and the growth of atherosclerosis which may lead directly to a heart attack or a stroke. In this study, carotid arteries with different stenoses have been numerically simulated to investigate the wall shear stress(WSS) and the elastic motion of the vessel. Blood flow has been treated as physiological, laminar and incompressible flow. To model the shear thining behavior of the blood, the Carreau-Yasuda model has been employed but the viscoelasticity of blood has not been considered. The results show that the WSS of $severe(75\%)$ stenosis is much higher than those of $25\%\;and\;50\%$ stenosis in the region of stenosis. With the increase in the stenosis thickness, the expansion ratio of the center of the stenosis decreases while the expansion ratio of the upstream region of the stenosis increases.
The hemodynamics behavior of the blood flow is influenced by the presence of the arterial stenosis. If the stenosis is present in an artery, normal blood flow is disturbed. In the present study, the characteristics of pulsatile flow in the blood vessel with stenosis are investigated by the finite volume method. For the validation of numerical model, the computation results are compared with the experimental ones of Ojha et al. in the case of 45% stenosis with a trapezoidal profile. Comparisons between the measured and the computed velocity profiles are favorable to our solutions. Finally, the effects of stenosis severity and wall shear stress are discussed in the present computational analysis. It can be seen, where the non-dimensional peak velocity is displayed for all the stenosis models at a given severity of stenosis, that it is exponentially increased. Although the stenosis and the boundary conditions are all symmetric, the asymmetric flow can be detected in the more than 57% stenosis. The instability by a three-dimensional symmetry-breaking leads to the asymmetric separation and the intense swirling motion downstream of the stenosis.

The main objective of the present study is to predict characteristics of three dimensional pulstitile flow by location of stenosis in blood vessel with the second order bifurcation. The present study simulates the incompressible non-Newtonian laminar blood flows using a Fluent V. 6.0. The Carreau model is employed as the constitutive equation for blood. The numerical simulation carried out at five cases without and with symmetry or asymmetry stenosis. It is found that the no stenosis and stenosis before first bifurcation do not have influence on flow at second bifurcated blood vessel. However, the stenosis after first biburcation has effect on flow at second bifurcated blood vessel.
Coronary artery disease (CAD) is a major cause of death in the world. As a non-invasive imaging modality, computed tomography angiography (CTA) is now usually used in clinical practice for CAD diagnosis. Precise quantification of coronary stenosis is of great interest for diagnosis and treatment planning. In this paper, a novel cluster method based on a Modified Student's t-Mixture Model is applied to separate the region of vessel lumen from other tissues. Then, the area of the vessel lumen in each slice is computed and the estimated value of it is fitted with a curve. Finally, the location and the level of the most stenoses are captured by comparing the calculated and fitted areas of the vessel. The proposed method has been applied to 17 clinical CTA datasets and the results have been compared with reference standard degrees of stenosis defined by an expert. The results of the experiment indicate that the proposed method can accurately quantify the stenosis of the coronary artery in CTA.

Backgrounds: The present study in angulated coronary stenosis was to evaluate the influence of velocity and wall shear stress (WSS) on coronary atherosclerosis, the changes of hemodynamic indices following coronary stenting, as well as their effect of evolving in-stent restenosis using human in vivo hemodynamic parameters and computed simulation quantitatively and qualitatively. Methods: Initial and follow-up coronary angiographies in the patients with angulated coronary stenosis were performed (n=80). Optimal coronary stenting in angulated coronary stenosis had two models: < 50 % angle changed(model 1, n=43), > 50% angle changed group (model 2, n=37) according to percent change of vascular angle between pre- and post-intracoronary stenting. Flow-velocity wave obtained from in vivo intracoronary Doppler study data was used for in vitro numerical simulation. Spatial and temporal patterns of velocity vector and recirculation area were drawn throughout the selected segment of coronary models. WSS of pre/post-intracoronary stenting were calculated from three-dimensional computer simulation. Results: Follow-up coronary angiogram demonstrated significant difference in the percent of diameter stenosis between two groups (group 1: $40.3{\pm}30.2$ vs. group 2: $25.5{\pm}22.5%$, p<0.05). Negative WSS area on 3D simulation, which is consistent with re-circulation area of velocity vector, was noted on the inner wall of post-stenotic area before stenting. The negative WSS was disappeared after stenting. High spatial and temporal WSS before stenting fell into within physiologic WSS after stenting. This finding was prominent in Model 2 (p<0.01) Conclusions: The present study suggests that hemodynamic forces exerted by pulsatile coronary circulation termed as WSS might affect on the evolution of atherosclerosis within the angulated vascular curvature. Moreover, geometric change, such as angular difference between pre / post-intracoronary stenting might give proper information of optimal hemodynamic charateristics for vascular repair after stenting.

Background: A lipo-prostaglandin E1 agonist is effective for the treatment of neurological symptoms of spinal stenosis when administered by an oral or intravenous route. we would like to reveal the therapeutic effect of an epidural injection of lipo-prostaglandin E1 on hyperalgesia in foraminal stenosis. Methods: A total of 40 male Sprague-Dawley rats were included. A small stainless steel rod was inserted into the L5/L6 intervertebral foramen to produce intervertebral foraminal stenosis and chronic compression of the dorsal root ganglia (DRG). The rats were divided into three groups: epidural PGE1 (EP) (n = 15), saline (n = 15), and control (n = 10). In the EP group, $0.15{\mu}g{\cdot}kg-1$ of a lipo-PGE1 agonist was injected daily via an epidural catheter for 10 days from postoperative day 3. In the saline group, saline was injected. Behavioral tests for mechanical hyperalgesia were performed for 3 weeks. Then, the target DRG was analyzed for the degree of chromatolysis, chronic inflammation, and fibrosis in light microscopic images. Results: From the fifth day after lipo-PGE1 agonist injection, the EP group showed significant recovery from mechanical hyperalgesia, which was maintained for 3 weeks (P < 0.05). Microscopic analysis showed much less chromatolysis in the EP group than in the saline or control groups. Conclusions: An epidurally administered lipo-PGE1 agonist relieved neuropathic pain, such as mechanical hyperalgesia, in a rat foraminal stenosis model, with decreasing chromatolysis in target DRG. We suggest that epidurally administered lipo-PGE1 may be a useful therapeutic candidate for patients with spinal stenosis.
The objective of the present study is to evaluate the effects of the stenting shapes on flow velocity and wall shear stress in angulated coronary stenosis by computer simulation. Coronary angiogram and Doppler ultrasound measurement in the patients with angulated coronary stenosis were obtained. Inlet wave velocity distribution obtained from in vivo intracoronary Doppler data was used for the numerical simulation. Spatial pattern of blood flow velocity and recirculation area were drawn through out the selected segment of coronary models. Wall shear stresses in the intracoronary stent models were calculated from three-dimensional computer simulation. A negative shear stress region, which is consistent with re-circulation area on flow pattern, was noted on the inner wall of post-stenotic area of pre-stenting model. The negative shear stress was disappeared after stenting. Shear stress in the post-stenting model was markedly reduced up to about two orders of magnitude compared to that of the pre-stenting model.

Background: Tracheal stenosis is an urgent but uncommon disease. Therefore, primary care clinicians have limited clinical experience. Animal models of a tracheal stenosis can be used conveniently for the learning, teaching, and developing new diagnostic and therapeutic modalities for tracheal stenosis. Recently, a canine model of a tracheal stenosis was developed using a Nd-YAG laser. To describe the methods and results of developed animal model, we performed this study. Methods : Six Mongrel dogs were generally anesthetized and the anterior 180 degree of tracheal cartilage of the animal was photo-coagulated using a Nd-YAG laser. The animals were bronchoscopically evaluated every week for 4 weeks and a pathologic evaluation was also made. Results : Two weeks after the laser coagulation, the trachea began to stenose and the stenosis progressed through 4 weeks. All animals suffered from shortness of breath, wheezing, and weight loss in the 3 weeks after the laser treatment, and two died of respiratory failure just before the fourth week. The gross pathologic findings showed the loss of cartilage and a dense fibrosis, which resulted in a fibrous stricture of the trachea. Microscopy also showed that the fibrous granulation tissue replaced destroyed cartilage. Conclusion : The canine model can assist in the understanding and development of new diagnostic and therapeutic modalities for tracheal stenosis.

Swirl flow is often found in proximal coronary arteries, because the aortic valves can induce swirl flows in the coronary artery due to vortex formation. In addition, the curvature and tortuosity of arterial configurations can also produce swirl flows. The present study was performed to investigate fractional flow reserve alterations in a post-stenotic distal part due to the presence of pre-stenotic swirl flow by computational fluid dynamics analysis for virtual stenotic models by quantifying fractional flow reserve(FFR). Simplified stenotic coronary models were divided into those with and without pre-stenotic swirl flow. Various degrees of virtual stenosis were grouped into three grades: mild, moderate, and severe, with degree of stenosis of 0 ~ 40%, 50 ~ 60%, and 70 ~ 90%, respectively. In this study, three-dimensional computational hemodynamic simulations were performed under hyperemic conditions in virtual stenotic coronary models by coupling with a zero-dimensional lumped parameter model. The results showed that the influence of pre-stenotic swirl inflow is dominant on FFR alteration in mild stenosis, whereas stenosis is dominant on FFR alteration in moderate/severe stenosis. The decrease in FFR caused by swirl flow is more significant in mild stenosis than moderate/severe stenosis. Biomechanical modeling is useful for clinicians to provide insight for medical intervention strategies. This hemodynamic-based parameter study could play a critical role in the development of a non-invasive imaging-based strategy-support system for percutaneous transluminal angioplasty in cases of mild/moderate stenosis.
