Anxiety and Symptom Assessment in Turkish Gynecologic Cancer Patients Receiving Chemotherapy

  • Diagnosis and treatment procedures in cancers and resulting anxiety negatively affect the individual and the family. Particularly treatment methods may generate psychological symptoms. The aim of this study was to determine the level of such symptoms in Turkish gynecologic cancer patients receiving chemotherapy. A total of 41 patients who were referred to our gynecologic oncology research clinic between January-March 2012, receiving 3 months or more chemotherapy and who agreed to participate were enrolled in study. All the data were collected using a personal information form, Edmonton Symptom Assesment System and State-Trait Anxiety Inventory. Patients received highest point average from fatigue symptom ($6.53{\pm}2.67$) and lowest point average from dyspnea ($1.53{\pm}3.03$) according to Edmonton Symptom Assesment System. The mean State Anxiety score of patients was $43.1{\pm}9.77$ and mean Trait Anxiety score was $46.7{\pm}7.01$. Comparing symptoms of patients and mean State Anxiety score it was found that there was a statistically significant corelation with symptoms like pain (p<0.05), sadness (p<0.001), insomnia (p<0.05), state of well being (p<0.001) and dyspnea (p<0.05). Similarly comparing symptoms of patients and mean Trait Anxiety score demonstrated significant correlations for fatigue (p<0.05), sadness (p<0.01), insomnia (p<0.01) and state of well-being (p<0.01). As a result, patients with gynecological cancers experienced symptoms related to chemotherapy and a moderate level of anxiety. In accordance, appropriate interventions should recommended for the evaluation and improvement of anxiety and symptoms related to treatment in cancer patients.

초등학교 특수학급아동의 임상적 진단 및 감정 행동특성 연구 (Clinical Diagnosis and Emotional Behavioral Characteristics Study of Children in a Special Education Class in Korean Elementary School)

  • Objectives : The special class has been made, bringing rapid increase quantitatively. The authors carried out the child psychiatric interview and evaluation for 9 special-classed children in Asan city to find out clinical diagnosis and emotional/behavioral characteristics. Methods : The child psychiatrists evaluated special class children by DSM-IV and K-SADS-PL. Tools for the evaluation were Child Behavior Checklist- Korean version, Korean Personality Inventory for Children, Children's Depression Inventory, Abbreviated Conners Parent-Teacher Rating Scale-Revised, State-Trait Anxiety Inventory for Children, Vineland Social Maturity Scale, Wechsler Intelligence Scale for Children-III, and Childhood Autism Rating Scale. Results : Ultimately 53 children, consisting of 35 boys(67.9%) and 18 girls(32.1%), participated, and the average age was $10.5{\pm}1.3$ years old. Their measure of Vineland Social Maturity Scale was $78.7{\pm}20.0$, Childhood Autism Rating Scales was $25.4{\pm}9.0$, Child Depression Inventory was $22.2{\pm}5.2$, State-Trait Anxiety Inventory for Children was $35.2{\pm}8.2/36.5{\pm}6.2$, and Abbreviated Conners Parent-Teacher Rating Scale was $11.0{\pm}4.6$. In the clinical diagnosis evaluation, the prevalence rate of learning disorder was decreased compared to early research, ADHD had been newly appeared and depression disorder and anxiety disorder had been increased. Conclusion : This result suggests that a lot of children in a special class have complex emotional and behavioral problems in addition to educational problems.

한국어판 매일 스트레스 평가서 개발을 위한 예비연구 (A Preliminary Study for the Development of the Korean Version of the Daily Stress Inventory(DSI))

  • As a preliminary step to develop the Daily stress Inventory(DSI) for assessing minor stressors frequently experienced in everyday lift. We examined the reliability and validity of the DSI. The Daily Stress Inventory(DSI), the Beck Depression Inventory(BDI), the State-Trait Anxiety Inventory(STAI), the Social Readjustment Rating Scale(SRRS) and the Minnesota Multiphasic Personality Inventory(MMPI) were administered to 111 college students. Compared with the weekly DSI Event, Impact and I/E ratio scores of the American college students, the weekly event and I/E ratio scores of the Korean college students were significantly higher but weekly Impact score was lower. The internal consistency reliability of the DSI as computed by Cronbach alpha was .98. The DSI I/E ratio scores were significantly correlated with the scores of State Anxiety$({\gamma}=.27)$ and Trait Anxiety${\gamma}=.24)$, but not with BDI scores. And the SSRS scores were not significantly correlated to the DSI Event or Impact and In ratio scores. Significant correlations were revealed between the DSI Impact scores and the Sc and Ma scales of the MMPI. And the correlation between the DSI I/E ratio scores and the L, D, Hy, Pd and Pa scales of the MMPI was also significant. But no significant correlation was found between the DSI Event scores and all the scales of the MMPI. This study suggests that the DSI can be a reliable and valid tool to assess the minor stress frequently experienced in everyday life future study should be directed to obtain normative data based on more varied and larger population. Especially gathering additional evidences for the validity of the DSI using other minor stressor scale will be required.

  • Objectives: To build basic clinical data for developing an artificial intelligence algorithm for Korean herbal prescriptions for anxiety, depression, anger, and insomnia. Methods: Subjects were recruited among those who reported mild or more severe symptoms of anxiety, depression, anger, and insomnia (Anxiety: State-Trait Anxiety Inventory≥40, Depression: Beck Depression Inventory≥14, Anger: State-Trait Anxiety Inventory≥16, Insomnia: Insomnia Severity Index≥8). Clinical observation items including basic medical information and symptoms were collected from them. These data were then analyzed by experts in Hyungsang medicine, Sasang constitutional medicine, and Sanghan-Geumgwe medicine. Results and Conclusions: Experts of the three societies presented key clinical data and recommended prescriptions. Results of this study can be used as basic data for developing an artificial intelligence algorithm for Korean herbal prescriptions in the future.

농부중의 정신적 원인에 대한 연구 (Psychiatric Factors Associated with Farmers' Syndrome)

  • Backgrounds : There has been many studies investigating the causes of farmers' syndrome. In some studies, psychologic stress is related to farmer's syndrome. And the diagnostic criteria of farmer's syndrome is similar to those of generalized anxiety disorder. So we carried out this study to investigate the psychiatric causes of farmers' syndrome. Methods : This study was done in some rural and urban areas of Kyoungsangnam Province, July, August, October and November of 1996. Those who came to free medical service and completed interview, medical examination and laboratory examination and 20-59 years old were 150 persons. And those who came to health center for health examination and completed only interview and 20-59 years old were 94 persons. The questionnaire was composed of sociodemographic factors, health risk factors, farmer's syndrome, work load, BEPSI(inventory to measure stress), Spielberger's state-trait anxiety inventory, self-rating depression scale. To examine statistical significance, we used X2-test, Mantel-Haenszel test for linear association, t-test, ANCOVA, correlation, multiple regression, logistic regression. Results : The prevalence of farmer's syndrome, adjusted for age and sex with population of Kyungsangnam Province of 1993 was 208 per 1,000(90 per 1,000 in men and 329 per 1,000 in women). In bivariate analysis, farmer's syndrome was significantly related to age, sex, job, income, smoking, alcohol drinking, work load, BEPSI, state anxiety, trait anxiety, depression, body mass index, Hwa-Byung, hypertension, anemia. However, when age and sex were adjusted, job was not significantly related to farmer's syndrome. The score of farmer's syndrome was significantly related to age, sex, work load, BEPSI, trait anxiety by multiple regression. Farmer's syndrome was significantly related to increasing age(odd ratio 1.079, 95% C.I. 1.060 - 1.099), sex(odds ratio of male 0.434, 95% C.I. 0.349 - 0.540), and BEPSI(odds ratio 1.231, 95% C.I. 1.148 - 1.320) by logistic regression. Results of logistic regression analysis of the component symptoms of farmer's syndrome were as follows. Shoulder stiffness was significantly related to increasing age, female sex and BEPSI. Lumbago was significantly related to increasing age, female sex and trait anxiety. Numb limbs and nocturia was significantly related to increasing age and female sex. Breathlessness was significantly related to work load, sleeplessness was significantly related to depression, dizziness was significantly related to job and state anxiety, and abdominal fullness was significantly related to female sex. Conclusion : Farmers' syndrome was related to work load, but was more related to psychiatric factors such as BEPSI and trait anxiety. And the occupation was not risk factor of farmers' syndrome in this study, so further study is needed to investigate the cause of farmers' syndrome.

우울과 불안이 흉통 환자의 삶의 질에 미치는 영향 (Impact of Depression and Anxiety on Quality of Life in Patients with Chest Pain)

  • Objectives : There has been substantial evidence that patients with chest pain have depression and anxiety, and show impaired quality of life (QoL). This study aimed to campare the QoL according to types of chest pain and to examine the impact of depression and anxiety on QoL in patients with chest pain. Methods : Forty-seven patients with chest pain were divided into Cardiac-Typical Chest Pain (CTCP, n=22) and Non-Cardiac-Atypical Chest Pain groups (NCACP, n=25) according to the pain characteristics and cardiovascular disease. Patients were assessed for depression using the Beck Depression Inventory (BDI), for anxiety using the State-Trait Anxiety Inventory (STAI), and QoL was assessed using the Korean version of the SmithKlein Beecham 'Quality of Life' Scale (KvSBQOL). Results : Compared with the CTCP group, the NCACP group reported significantly higher anxiety, and lower QoL. There was no significant difference in QoL between the two groups after adjusting for anxiety. The QoL was associated with depression and trait-anxiety in the CTCP group, and with trait-anxiety in the NCACP group. Conclusion : The findings suggest that there are different effects of depression and anxiety on QoL in individuals with CTCP and NCACP. Understanding about these differences can be important in the treatment of patients with chest pain. A large prospective study is needed to confirm these results.

성별, 수술부위, 출산계획이 다른 수술환자의 상황-불안 비교 (Comparison of Surgical Patients’State Anxiety by Sex, Area of Operation, and Family Planning)

  • The major purpose of this study was to compare the state anxiety of surgical patients by sex (male/female), area of operation (sex-organ/non sex-organ), and family planning (having the plan of child-bearing/having no plan of child-bearing). One hundred sixty patients who were to get surgical operation were equally divided into eight groups resulted from combination of variables of sex, area of operation, and family planning, The state anxiety of surgical patients was measured in terms of the discrepancy score between the state anxiety score on the State-Trait Anxiety Inventory (STAI) administered at a day before operation and the trait anxiety score on it which was administered at a day before discharge. In order to test statistically the differences among meant scores of the state anxiety obtained by eight groups, multiple comparisons were carried out by Scheffe method. The results of this study led to the conclusions that, (1) there was no significant sex difference in the state anxiety of surgical patients, when the area of operation and the family planning variables were disregarded, (2) the state anxiety of patients who were to get operation of their sex-organ was significantly higher than that of patients who were to get operation on the parts other than their sex-organ, when variables of the sex and the family planning were not taken into account, (3) there was no significant sex difference in the state anxiety of patients who were to Bet operation on the parts other than their sex-organ, when the family planning variable was disregarded, (4) the state anxiety of female patients who were to get operation on their sex-organ was significantly higher than the stale anxiety of male patients who were to get operation on their sex-organ, when the family planning variable was not taken into account.

원형탈모증 환자의 정서상태와 성격특성에 관한 임상적 연구 (Clinical Study of Emotional State and Personality Characteristics in Patients with Alopecia Aerata)

  • Objectives: This study was aimed to investigate the emotional state and personality characteristics of alopecia aerata patient and to determine whether these characteristics are similar with those of neurotic patient. Methods : 20 alopecia aerata patients, 20 neurotic patients, and 20 normal healthy controls were studied. Beck Depression Inventory(BDI), State and Trait Anxiety Inventory(STAI) and Korean standardized edition of Catell's 16 Personality factors Questionnaire(16PF) were used for assessment. Results: 1) Total score of BDI was higher in neurosis group than alopecia aerata group and normal control group(p<0.05). Inspire of statistical non-significance, total score of in alopecia aerata group was higher than normal control group. 2) Total scores of state anxiety and trait anxiety in alopecia aerata and neurosis group were significantly higher than normal control group(p<0.001). 3) In first-stratum source traits of 16PF, normal control group was significantly higher than alopecia aerata group and neurosis group in B-factor(intelligence) and C-factor(ego-strength)(p<0.05). Alopecia aerata group and neurosis group were significantly higher than normal control group in O-factor(guilt-proneness) and $Q_4-factor(tension,\;anxiety)$(p<0.001). 4) In second-stratum source traits of 16PF, alopecia aerata group and neurosis group were significantly higher than normal control group in ANX-factor(anxiety)(p<0.05). Conclusions : These results suggest that emotional state and personality characterstics of patients with alopecia aerata are a distinguishable different to compare with normal control, and are very similar with those of neurotic patients. The authors propose that psychiatric management in patient with alopecia aerata will be more benificial.

불면장애 환자의 한의치료 효과 및 심리척도 변화간 상관성 분석: 후향적 관찰 연구 (Effect of Korean Medical Treatment on Patients with Insomnia and Analysis of Correlation among Psychological Scale Changes: A Retrospective Chart Review)

  • Objectives: To examine effect of Korean medical treatment on patients with insomnia and correlations among changes in psychological scales before and after treatment. Methods: Medical records of 38 patients diagnosed with insomnia based on DSM-V who received Korean medical treatment (herbal-medication, acupuncture, Korean psychotherapy) for at least 8 weeks were retrospectively reviewed. Psychological scales including Insomnia Severity Scale (ISI), Beck Depression Inventory-II (BDI-II), State-Trait Anxiety Inventory (STAI), Beck Anxiety Inventory (BAI), and State-Trait Anger Expression Inventory (STAXI) were measured every four weeks and analyzed. Results: After eight weeks of treatment, ISI, BDI-II, BAI, STAI-X-1/2, and STAXI-S/T showed statistically significant decreases. Psychological scale pairs that showed a statistically significant correlation were ISI and BDI-IIㆍSTAI-X-1ㆍSTAI-X-2ㆍTA, BDI-II and BAIㆍSTAI-X-1ㆍSTAI-X-2ㆍAX-I, BAI and STAI-X-1ㆍSTAI-X-2ㆍSAㆍTA, STAI-X-1 and STAI-X-2, SA and TA, and TA and AX-I. In women, the improvement of AX-C was high. The improvement in AX-I score was significant when the disease duration was more than one year. A lower pretreatment BAI value predicted a greater decrease in ISI score after treatment. Conclusions: Korean medical treatment including herbal medicine, acupuncture, and Korean psychotherapy was effective in improving insomnia symptoms and accompanying symptoms such as depression, anxiety, and anger. In the future, more in-depth follow-up research is needed on the mechanisms by which various psychological problems (depression, anxiety, anger, etc) cause and worsen insomnia and the psychological symptoms secondary to insomnia.

심인성 발기부전 환자에서 Norepinephrine과 Serotonin에 관한 연구 (Norepinephrine and Serotonin in the Patients with Psychogenic Impotence)

  • Various neurotransmitters have been proposed as possible mediators of penile erection. Especially, norepinephrine and serotonin might have a important role in sexual arousal and penile erection. And it could be hypothesized that the psychogenic impotence is associated with the depletion or imbalance of norepinephrine and serotonin from evidences, such as the symptomatic manifestation of depression and the antidepressantinduced sexual dysfunction. The authors investigates the association of norepienphrine and serotonin with psychogenic impotence. The psychogenic impotent group(PIG) consisted of twenty-three patients with psychogenic impotence and the controlled group(CG) consisted of twenty-seven patients without psychogenic impotence. PIG had no organic cause accounting for their erectile dysfunction. The Beck Depression Inventory(BDI) and the State-Trait Anxiety Inventory(STAI) were applied to each subject to assess mood, state anxiety(SA) and trait anxiety(TA). Plasma norepinephrine level from systemic blood and 5-hydroxyindoleacetic acid(HIAA) levels from 24-hours urine were measured in each subject. The mean score of BDI of PIG was significantly higher than that of CG(p=0.015). PIG had a tendency of higher TA compared with CG(p=0.054). And also SA was higher in PIG, but did not show significant difference(p=0.193). The level of norepinephrine was significantly lower in patient with psychogenic impotence(p=0.000). And the level of 24-hours urine 5-HIAA was lower in PIG but did not show significant difference(p=0.494). Although the authors did not exclude depressive disorders in PIG, the present findings suggest that psychogenic impotence might have higher depressive mood and trait anxiety, and be associated with the depletion of norepinephrine in systemic blood.
