한의약 표준화 발전방안 연구: 한-중 전통의학 표준화 정책비교를 바탕으로 (Strategic Development Plan of Standardization on Korean Medicine : Comparison with Traditional Medicine Policies between Korea and China)

  • Objectives: This study aims to investigate the policy status of traditional medicine standardization between Korea and China as well as to derive a strategic development plan for Korean medicine standardization. Methods: The existing national plans and preceding studies on traditional medicine standardization in Korea and China were reviewed, and scope of the study was categorized as having three main aspects: infrastructure; standard development and standardization activities; and application and diffusion. Results: Nine development plans for Korean medicine standardization derived under the study were as follows: With regard to infrastructure of standardization, 1) standardization plan of Korean medicine shall be established based on involvement of multi-government ministries. 2) It shall be set with a consistent cooperation process among standardization-related organizations, as well as 3) encourage the industry to participate in standardization activities. To activate standards development, 4) launching the R&D-standardization-industrialization linked project, 5) supporting Korean medicine society and the mirror committee for motivation, and 6) planning the international joint research for development of standard and expanding standardization activities are recommended. In the aspect of application, there is a need for 7) expanding certification systems for the industry and 8) evaluation and feedback on the life-cycle of standardization. Lastly, 9) educational programs for training experts of standardization shall be developed and implemented. Conclusions: In order to invigorate standardization activities for Korean medicine, enhancement of planning and evaluation capacity, ensuring the strategic development of standards, broadening boundaries of international standardization activities, and training professionals are required.

  • Objectives: The aim of this study is to introduce the WFME Global Standards and Recognition process and to consider Improvement direction of Korean traditional medical curriculum. Methods: To Investigate the Standards and Recognition process of WFME and the traditional medical curriculum of each country(China, Taiwan, Japan, Korea). Results: The WFME Global Standards and Recognition process aims to train doctors who are educated and active in world standard medical Curriculum. The traditional medical colleges have not received recognition, but those colleges in Korea, China and Taiwan contain a lot of standards contents, and they need to be recognized if they belong to WDMS. Conclusions: Korea University of Oriental Medicine has a lot of subjects of WFME Standards and there is a medical education recognition association, which is advantageous for the standardization process of world medical education. Therefore, it is necessary to aim at world standard medicine while preserving the tradition of Oriental medicine, WFME Global Standards should be used to reorganize the curriculum and train a world-class medical professional.

  • Objective : We proposed fundamental rules of prospective Korean Standard Classification of Diseases(Oriental Medicine). Methods : We analysed Korean Standard Classification of Diseases(Oriental Medicine)(established in 1994) in comparison with ICD-10 and Chinese Standard Classification of Disease(Traditional Chinese Medicine). Secondly, we analysed the diagnostic structure of Modem oriental medicine. Results : Korean Standard Classification of Diseases has an inappropriate writing structure, logical errors of classification, confusion of symptoms, 'bing', and 'zheng', inappropriate comparison of disease designations in oriental medicine and western medicine, and the ommission of important items. Secondly, we demonstrate the relations of 'bing' and 'zheng' in modem oriental medicine and disease designations in oriental medicine and western medicine. Conclusions : We propose the separate classification of 'bing' and 'zheng', the qualification of designated names, the structure of 'bing' and 'zheng' system, and a different writing method.

지적재산권 보호를 위한 전통의학 DB 구축에 관한 고찰 (Grope for a Summary Program about a Traditional Medical Database for Intellectual Property Protection in WIPO)

  • Objectives : In the WIPO conference about the protection of intellectual property for traditional knowledge (TK) and genetic resources (GR), there was a demand concerning the necessity of recognizing the invention of TK and GR and the foundation of a related database. Methods : In the standard of the foundation of a database, the content and resource identification standards, the technological standards and the security and transmission standards were discussed. In a decision on intellectual property, it means the protection and recognition as a technology precedent of the materials of the database. Results : Massive material on traditional medical care technology in Korea has existed in many areas. However, it is hard to make a database because it is difficult to organize and find the data. In addition, a database demanded by WIPO is not only the material of images or texts of simple technology information, but also the use of patent decisions about traditional knowledge technology and the public sharing of information in related areas. A traditional medical care database would consist of the core of a subject and the related areas. The core part is the medical herb resource and Oriental clinical technology. Conclusions : These are necessary to detail the kind of medicines and prescriptions, their use, and their processing. In the related areas, it needs to detail PIAS and clinical data in regarding the industrialization. These DB have to protect the TK and the GR of Korea.

전통침구기법(傳統鍼灸技法)의 복원을 위한 문헌조사 (Document Research for the Restoration of Traditional Acupunctural Methods)

  • 1) Objective This research is a process of examining the Traditional Acupunctural Methods left in our documents. The objectives of this paper are explaining the process of researching documents and reporting several points revealed in this process. 2) Process This study was done through the following steps: (1) We located the Acupunctural Methods in the original text database on the web. (2) We sorted them according to criterion. (3) We set up some standard clauses in order to organize the selected information. (4) We re-organized the information following the standards within the categories. (5) As for clauses that were not fully explained in the documents, we tried to restore them through other documents. 3) Results (1) 11 standard clauses were set up in order to organize the Traditional Acupunctural Methods. (2) Traditional Acupunctural Methods were extracted from 22 documents, (3) 24 Traditional Acupunctural Methods were extracted and organized by standards. (4) We learned of similar methods with resembling ways of drawing blood and methods with deep interest in the depth of needle insertion. Also, we were able to gain a clue to the exchange of Acupunctural Methods among the three Asian countries and the possibility of research using the Korean Medicine Database.

Metal concentrations of Chinese herbal medicine products in the United States

  • We determined arsenic, lead, mercury and cadmium concentrations in Chinese herbal medicines sold in the United States by medical use parts. 54 kinds of herbal products including 9 medical use parts (radix, rhizoma, cortex, pericarpium, fructus, lignum, semen, folium, and herba) were analyzed using inductively coupled plasma-mass spectrometry for arsenic, lead and cadmium, and using mercury analyzer for mercury. Arsenic (median concentration, 0.25 mg/g), mercury (0.20 mg/g), lead (3.78 mg/g) and cadmium (0.39 mg/g) were detected in 71%, 54%, 35%, and 18% of 143 herbal medicine samples, respectively. A total of 27% and 12% of 143 products analyzed contained mercury and cadmium above the regulatory standards. Herba and folium (leaves of herbal plants) were the most contaminated parts from metals, whereas pericarpium, lignum and semen (outer layers and seeds) were less contaminated. This study suggests that metal contamination is different by medical use parts. Our findings provide further evidence that efforts to protect people using traditional remedies from metal intoxication should be made to enforce the regulatory standards.

구안와사 입원환자 25명에 대한 황련해독탕 약침을 병행한 한방치료 증례 보고 (25 Cases of Facial Paralysis Inpatients by Korean Medical Treatment with Hwangryunhaedok-tang Pharmacopuncture)

  • Objectives : The purpose of this report is to show the effectiveness of traditional korean medical treatment with Hwangryunhaedok-tang pharmacopuncture. Methods : Twenty five patients were treated by korean medical treatment including Hwangryunhaedok-tang pharmacopuncture. We measured valuation standards such as House-Brackmann Grade(H-B Grade) and Numeric Rating Scale(NRS) to assess the effectiveness of treatments. These valuation standards were measured at admisson and discharge. Results : The results were obtained as below. 1. In House-Brackmann Grade, 60.0% of patients were improved and 36.0% were maintained at the same level. 4.0% of patients were worsen. 2. Average of Numeric Rating Scale was changed from 8.5 to 3.0 after whole treatment. Conclusions : Korean medical treatments including Hwangryunhaedok-tang pharmacopunture can be effective for improving symptoms of facial paralysis.

시력장애와 사지마비를 호소하는 시신경척수염 환자의 한방 증례 보고 1례 (A Case Report of Treatment of a Patient with Neuromyelitis Optica and Suffering from Vision Disorder and Quadriplegia with Korean Traditional Medicine)

  • Objectives: This is a case report regarding the effect of Korean traditional medicine on vision disorder and quadriplegia in a patient with neuromyelitis optica. Methods: We treated a patient who was diagnosed with neuromyelitis optica with Korean traditional medicine, including acupuncture, moxibustion, and herbal medicine (Gigugyanghyeol-tang gamibang) for 106 days. We evaluated the patient with the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI), Modified Barthel Index (MBI), Modified Ashworth Scale (MAS) Grade, and Numeric Rating Scale-11 (NRS-11). Results: After treatment, the patient's symptoms were improved. The ISNCSCI scores increased from 42 to 66 in motor score, from 152 to 196 in sensory score, and from A to D in the ASIA impairment scale; the MBI score increased from 9 to 33, while the score of the MAS Grade decreased from I+ to I, and the NRS-11 scores of vision disorder, spasticity, and tingling decreased from 10 to 7, 3, and 2-3, respectively. Conclusions: Korean traditional medicine may be effective for treatment of vision disorder and quadriplegia in patients with neuromyelitis optica.

스위스에서의 국민투표에 의한 보완의학 건강보험 급여화 사례 연구 (A case study on benefit coverage of complementary medicine in public health insurance by the referendum in Switzerland)

  • Background : Efforts towards increasing insurance coverage for traditional Korean medicine (TKM) are being continued. However, various difficulties are faced in generating evidence for TKM due to limited financial support and the low quality of research methodology. Objectives : The objectives of this study were to review the Swiss evaluation program for complementary and alternative medicine (CAM) and assess the expansion in public health insurance coverage of complementary medicine as approved by referendum in Switzerland. Methods : The regulations of CAM in the European Union were assessed. Research articles, reports, government publications and websites which deal with the 'Programm Evaluation $Komplement{\ddot{a}}rmedizin$ (PEK)' and the referendum in Switzerland were searched for and analyzed. Results : The PEK was conducted from 1998 to 2005. The PEK evaluated the efficacy, utilization and cost-effectiveness of anthroposophical medicine, homeopathy, neural therapy, phytotherapy and traditional Chinese medicine. However, clear conclusions could not be drawn from the evaluation according to the PEK Report. Later, a referendum was implemented in which 5 therapies would be added to the Switzerland Constitution with the support of the public. The coverage of CAM was approved by Swiss a plebiscite with an approval rate of 67.0%. Conclusions : The reason for the successful referendum is suggested to be public support and the solidarity with CAM experts and politicians. It may be surmised that recognition of the political efforts and scientific aspects required to expand insurance coverage of TKM, and towards obtaining public support, is necessary.

임상 한의사의 이침 사용 실태 조사 보고 (Survey on Ear Acupuncture Usage of Clinical Traditional Korean Medicine Doctor)

  • Objectives : The survey was conducted to find out ear acupuncture usage of clinical traditional Korean medicine doctor and lay a foundation for the standards for its clinical application. Methods : An e-mail was sent to Korean medicine doctors whose email addresses are registered at the Association of Korea Oriental Medicine twice over two weeks. 280 completed questionnaires were used for the analysis. Results : 66.07% of respondents answered in use, whereas the answer that "less than 10 per 100 patient applied" was more than majority of 54.29%. The major field of treatment is addictive disorders such as smoking and the 80.11% patients got less than 3 days of treatment. Ear region was the most commonly treated area with 66.67%. Since the self removal frequency was high as 65.05% in compared to the case of visiting clinic. The rate of side effects was less than 10% (96.70%), and these side effects showed light symptoms such as "pain" (46.95%), itchiness (29.27%), flare (19.51%). The Acquired complements for improving ear acupuncture treatment was "the development of effective treatment manual" (28.57%) and "increasing insurance costs" (27.86%). Conclusions : Based on the survey, the vast majority of the TKM doctor used ear acupuncture but the frequency of use was low. We hope this study can be the basis for development of treatment manuals for various indications and safe treatment guidelines.
