Trend of Intensive Care Unit Admission in Neurology-Neurosurgery Adult Patients in South Korea : A Nationwide Population-Based Cohort Study

  • Objective : We aimed to examine trends in critically ill neurology-neurosurgery (NNS) patients who were admitted to the intensive care unit (ICU) in South Korea and identify risk factors for in-hospital mortality after ICU admission in NNS patients. Methods : This nationwide population-based retrospective cohort study enrolled adult NNS adult patients admitted to the ICU from 2010 to 2019 extracted from the National Health Insurance Service in South Korea. The critically ill NNS patients were defined as those whose main admission departments were neurology or neurosurgery at ICU admission. The number of ICU admission, age, and total cost for hospitalization from 2010 to 2019 in critically ill NNS patients were examined as trend information. Moreover, multivariable logistic regression modeling was used to identify risk factors for in-hospital mortality among critically ill NNS patients. Results : We included 845474 ICU admission cases for 679376 critically ill NNS patients in South Korea between January 1, 2010 to December 31, 2019. The total number of ICU admissions among NNS patients was 79522 in 2010, which increased to 91502 in 2019. The mean age rose from 62.8 years (standard deviation [SD], 15.6) in 2010 to 66.6 years (SD, 15.2) in 2019, and the average total cost for hospitalization per each patient consistently increased from 6206.1 USD (SD, 5218.5) in 2010 to 10745.4 USD (SD, 10917.4) in 2019. In-hospital mortality occurred in 75455 patients (8.9%). Risk factors strongly associated with increased in-hospital mortality were the usage of mechanical ventilator (adjusted odds ratio [aOR], 19.83; 95% confidence interval [CI], 19.42-20.26; p<0.001), extracorporeal membrane oxygenation (aOR, 3.49; 95% CI, 2.42-5.02; p<0.001), and continuous renal replacement therapy (aOR, 6.47; 95% CI, 6.02-6.96; p<0.001). In addition, direct admission to ICU from the emergency room (aOR, 1.38; 95% CI, 1.36-1.41; p<0.001) and brain cancer as the main diagnosis (aOR, 1.30; 95% CI, 1.22-1.39; p<0.001) are also potential risk factors for increased in-hospital mortality. Conclusion : In South Korea, the number of ICU admissions increased among critically ill NNS patients from 2010 to 2019. The average age and total costs for hospitalization also increased. Some potential risk factors are found to increase in-hospital mortality among critically ill NNS patients.

  • 최근 ITU-T/VCEG과 ISO/IEC MPEG은 공동으로 차세대 비디오 압축 표준 기술인 High efficient video coding (HEVC)를 제정하고 있다. HEVC는 H.264/AVC 표준 기술과 비교해 보면 매우 향상된 압축 효율을 보여 주고 있으며, 특히 Full HD 이상의 매우 큰 사이즈의 동영상 및 그에 따른 고화질 비디오 서비스를 주요 목표로 하고 있다. 그러나 품질과 압축 효율 향상을 위해 새로운 구조와 압축 도구들이 추가됨으로써 인코더의 연산 복잡도가 매우 증가되어 있으며, 이에 화질의 열화를 최소한으로 유지하면서 계산량을 감소시키는 연구가 필요해 지고 있는 상황이다. 본 연구에서는 화면 간 예측 시에 부호화 단위의 분할을 효율적으로 조기 종료하는 방법을 제안한다. 제안된 방법은 현재 코딩 단위 (Coding unit)의 움직임 벡터 정보, 율-왜곡 비용, 그리고 상위 코딩 단위에서의 개별 사이즈에 대한 평균 율-왜곡 비용값을 활용하여 분할을 조기에 중단한다. 실험 결과를 통하여 제안된 방법이 기존의 ECU 방법보다 약 10% 정도로 속도 향상이 있으며, HM 참조 소프트웨어와 비교 시 BD-rate 증가는 1.975% 정도로 매우 적게 나타남을 보인다.

건설기계 시간당작업량(Q) 산정 개선방안 -펌프준설선 작업효율(E)을 중심으로- (The estimating method of construction workable-quantity per unit time - Focused on Pump-Dredge -)

  • 국내 공공건설공사 예정가격작성기준에서 제시하는 건설기계의 시간당 작업량(Q)의 중요한 부분을 차지하는 작업효율(E)은 현장여건 등에 따라 차이가 발생하기 때문에 적용기준이 명확해야 한다. 그러나 국내 기준은 정성적인 표현이 사용되며, 이를 판단할 수 있는 기준 또한 포괄적이고 모호하여 설계자의 주관적이고 자의적인 판단에 따라 설계가 이루어지고 있는 실정이다. 본 연구에서는 펌프준설선을 대상으로 국내 외 관련 문헌조사 및 현장실사를 통하여 합리적인 작업효율(E) 제시방법을 제안하고 이로 인해 건설기계의 시간당 작업량 산정방법을 개선하고자 하였다. 문헌조사를 통해 국내기준 또한 일본기준의 형태로 작업효율(E)을 공사구분별, 흙두께, 평면형상, 단면형상, 해상조건 등으로 세분화 시킬 것을 제시하였으며, 이를 판단할 수 있는 기준도 객관화 시킬 수 있는 형태로 개선되어야 할 것을 제시하였다. 또한, 현장조사를 통해 얻어진 산출량을 일본의 예정가격작성기준을 적용하여 계산된 산출량과 비교하여 일본기준의 적정성을 검토하였다. 펌프준설선 (4,476kW)은 배송거리 3,600~6,000m의 시간당 준설량(Q)을 실측하여 표준값인 전동환산(q)을 비교하여, 일본대비 102.3% 수준으로 거의 유사한 결과값을 얻었다. 본 연구를 통해 제시된 각종 자료 및 수치들은 건설기계와 기술의 급속한 발전을 예정가격작성기준에 적용할 수 있는 기초가 될 수 있을 뿐 아니라, 관련 연구의 토대가 될 수 있으리라 기대한다.

Protection Strategies Against False Data Injection Attacks with Uncertain Information on Electric Power Grids

  • False data injection attacks have recently been introduced as one of important issues related to cyber-attacks on electric power grids. These attacks aim to compromise the readings of multiple power meters in order to mislead the operation and control centers. Recent studies have shown that if a malicious attacker has complete knowledge of the power grid topology and branch admittances, s/he can adjust the false data injection attack such that the attack remains undetected and successfully passes the bad data detection tests that are used in power system state estimation. In this paper, we investigate that a practical false data injection attack is essentially a cyber-attack with uncertain information due to the attackers lack of knowledge with respect to the power grid parameters because the attacker has limited physical access to electric facilities and limited resources to compromise meters. We mathematically formulated a method of identifying the most vulnerable locations to false data injection attack. Furthermore, we suggest minimum topology changes or phasor measurement units (PMUs) installation in the given power grids for mitigating such attacks and indicate a new security metrics that can compare different power grid topologies. The proposed metrics for performance is verified in standard IEEE 30-bus system. We show that the robustness of grids can be improved dramatically with minimum topology changes and low cost.

계란의 유통에 대한 조사연구 (Studies on the Marketing of Eggs)

  • 본 연구는 우리나라의 계란 유통현황을 조사하기 위하여 서울근교, 대전근교, 광주근교, 부산근교에 소재하고 있는 수집상 8개소, 도배상 16개소, 소매상 12개소 및 슈퍼마켓 6개소를 대상으로 설문서를 작성하여 설문조사하였고 그 결과를 계란의 유통규격 설정을 위한 기초자료를 얻고자 실시하였다. 1. 계란의 중량별 구분은 생산자가 주로 실시하며 이 계란의 유통가격은 유통단계별로 2-3원 차가 있었으며 왕란 및 특란은 가정소비가 많았고 대란은 음식점과 가정집에서 그리고 중란과 소란은 음식점에서 주로 소비하고 있다. 2. 소비자 측면에서 볼 때 갈색란을 개당 1.5원씩 더 주고 구입하는 경향이 있었고 중량별 구분은 대체로 믿고 있었다. 3. 소비자의 계란 1회구입량은 30개 주였으며 포장할 경우 10개 단위의 포장을 희망하고 있으며 10개의 포장비용은 10-20원이었고 포장판매를 원하고 있는 비율이 55-64% 정도나 되었다. 4. 유통상인이 구입한 계란은 7일이내에 대부분 판매하고 있으며 계란이 가장 많이 판매되는 계절은 가을이며 G. P센타가 필요하다고 생각하는 비율이 36.4%나 되었다.

  • 정부는 2005년 1월 '사회기반시설에 대한 민간투자법'이 개정되면서 적극적으로 교육시설 BTL사업을 추진하고 있으나 많은 시행착오와 문제점들도 발생되고 있다. 특히, 주무관청이 BTL 사업의 적격성 여부를 판단하기 위해 실시하는 투자가치 평가에서 산정된 비용이 실제 협약된 협약안의 비용과 현격한 차이가 나고 있는 것으로 조사되었다. 따라서, 본 연구에서는 효율적인 교육시설 BTL 사업의 추진을 위해 현행 투자가치 평가의 문제점을 파악하고 이를 개선하기 위해 실제협약안과의 상대비교를 통해 투자가치 평가항목에 대한 명확한 기준을 제시하여 협상지연이 발생하지 않도록 투자가치 산정에 정확성을 제시할 수 있는 모형을 구축하였다. 본 연구에서 구축한 투자가치 평가를 위한 운영비 모형을 교육시설 BTL 사업의 적격성 평가 단가에서 활용하면 사업자 선정 및 협상 뿐 아니라, 교육시설 유지관리와 사용 후 평가(POE)에서도 객관적이고 합리적 인 결과를 도출할 수 있을 것으로 기대된다.

A Development of DCS Binding Delay Analysis System based on PC/Ethernet and Realtime Database

  • DCS has many processing components and various communication elements. And its communication delay characteristic is affected diverse operating situation and context. Especially, binding signal which traversed from one control-node to another control-node undergo all sort of delay conditions. So its delay value has large deviation with the lapse of time, and the measurement of delay statistics during long time is very difficult by using general oscilloscope or other normal instruments. This thesis introduces the design and implementation of PC-based BDAS(Binding Delay Analysis System) System developed to overcomes these hardships. The system has signal-generator, IO-card, data-acquisition module, delay-calculation and analyzer module, those are implemented on industrial standard PC/Ethernet hardware and Windows/Linux platforms. This system can detect accurate whole-system-wide delay time including io, control processing and network delay, in the resolution of msec unit, and can analyze each channel's delay-historic data which is maintained by realtime database. So, this system has strong points of open system architecture, for example, user-friendly environment, low cost, high compatibility, simplicity of maintenance and high extension ability. Of all things, the measuring capability of long-time delay-statistics obtained through historic-DB make the system more valuable and useful, which function is essential to analyze accurate delay performance of DCS system. Using this system, the verification of delay performance of DCS for nuclear power plants is succeeded in KNICS(Korea Nuclear Instrumentation & Control System) projects

실내평가기법에 의한 새로운 격자지보재의 하중지지력 평가 (Load Bearing Capacity Evaluation of New Lattice Girder by Laboratory Test Techniques)

  • Load bearing capacity of new lattice girder has been evaluated with optimized spider for lattice girder utilized in the construction of tunnels. This newly developed lattice girder is different from existing lattice girder as its design is associated with existing spider with newly developed 2 types of form. The spacing of lattice girder's spider is linked with the weight and it decides the unit cost and construction therefore, different spacing of the developed spider has been produced to evaluate the measurement of load bearing capacity. As the result of the tests by producing the spacing of spider as 0cm and 4cm for developed lattice girder-2, the load bearing capacity of 0cm with spacing of 21%, and 4cm with 25% of increase when they are compared with the existing lattice girder, and the weight of specimen was decreased. As the result of the tests by producing the spacing of 1cm and 6cm for developed lattice girder-3, the spacing of 1cm with 42%, and the spacing of 6cm with 11% of increase which presented higher load bearing capacity in all newly developed forms, and there was a certain degree of increase in weight in case of 1cm of spacing. The result of evaluation regarding on the displacement by applying the evaluation method suggested by the German Railroad administration, the entire specimens were found to satisfy all the evaluation standard suggested by the administration.

부유식 천연액화가스(LNG) 터미널의 설계 기술 개발

  • With the expansion of natural gas demands in many countries, the necessity of LNG receiving terminals has been increased. The offshore LNG Floating Storage and Regasification Unit (FSRU) attracts attentions not only for a land based LNG receiving terminal alternative, but also for a feasible and economic solution. Nowadays, as the reliability of offshore oil and gas floating facilities and LNG carriers gains with proven worldwide operations, the FSRU can achieve a safety level that can be comparable to an onshore terminal. The design development related with safety features of the FSRU has been extensively carried out by oil and gas companies, shipyards, engineering companies, and equipment vendors, and has been successful so far in many fields. The construction of the FSRU can be achieved by integrating various technologies and experiences from many disciplines and many participating companies and vendors. In this paper, reviews on some of the important design features and design improvements on FSRU together with the practical construction aspects in cargo containment, vaporization system, ESD system, and operation modes, have been covered in comparison with actual LNG carrier, onshore receiving terminal, and FPSO systems. In order to materialize an FSRU project, the technical and economic justification has to be preceded. It is believed that once the safety and technical soundness is convinced, the FSRU can bring a higher project feasibility by reducing the overall construction time and cost. Through this study, an FSRU design readily applicable to an actual project has been developed by incorporating experiences gained from many marine and offshore projects. The wide use of proven standard technologies adopted in the series construction of LNG carriers and offshore FPSOs will bring the project efficiency and reliability.

메시지 복잡도를 중심으로한 UGV 통신효과 분석 (Analysis of UGV Communication Effectiveness focused on Message Complexity)

  • Purpose: In the near future, it is expected that UGV(unmanned ground vehicle) will be put into battle due to IT technology and unmanned technology development. In this study, we analyze the combat effectiveness considering communication effect where complex combat information and commands are transmitted. Methods: We use ABM(agent-based modeling) and wireless channel module which provides sophisticated communication effect through geographic information and UGV performance. And UGV combat simulation using wireless channel module is used to grasp the combat effectiveness according to the number of packets, which is a unit for storing all information and commands having high complexity. Results: The result of this study is to derive the optimal number of packets which does not decrease the combat effectiveness and the number of lost tanks. The number of packet increases, the survival ratio of our tanks are decreased. Conclusion: In this study, we reveal that the communication success or failure could affect the combat effectiveness. Also, it helps develope the standard communication protocol between UGVs and could be applied to analyze the cost effectiveness analysis in UGV combat environment.
