아동의 말초정맥관 삽입 부위 관찰창 확보가 침윤조기감지에 미치는 효과 (Effect of Observation Window at Peripheral Intravenous Catheter Site on Early Recognition of Infiltration among Hospitalized Children)

  • Purpose: The aim of this study was to identify the effect of an observation window (OW) at peripheral intravenous (IV) catheter sites on early detection of IV infiltration among hospitalized children. Methods: This was a retrospective observational study with history control group design. Participants were children who had IV infiltration after peripheral catheterization when hospitalized from January to May, 2014 and January to May, 2015 at a children's hospital located in Yangsan city, Korea. The 193 patients, who were hospitalized from January to May, 2014 formed the control group and did not have OW, and the 167 patients, who were hospitalized from January to May, 2015 formed the window group and had OW. Data were analyzed using ${\chi}^2$-test, independent samples t-test and multiple logistic regression. Results: First stage IV infiltration was 39.5% for the window group and 25.9% for the control group, which was significantly different (p=.007). The likelihood of $2^{nd}$ stage and above IV infiltration decreased by 44% in the window group, which was significantly different (p=.014). Conclusion: OW at the peripheral IV catheter site was found to be an effective measure in early recognition of IV infiltration. Considering the effect of OW, we recommend that nurses should make an OW with transparent dressing during stabilization of the IV catheter site in hospitalized children in clinical settings.

  • Power grid techniques are distributed over general power systems ranging from power stations to power transmission, power distribution, and users. To monitor and control the elements and performance of a power system in real time in the extensive area of power generation, power transmission, wide-area monitoring (WAM) and control techniques are required (Sattinger et al. 2007). Also, to efficiently operate a power grid, integrated techniques of information and communication technology are required for the application of communication network and relevant equipment, computing, and system control software. WAM should make a precise power grid measurement of more than once per cycle by time synchronization using GPS. By collecting the measurement values of a power grid from substations located at faraway regions through remote communication, the current status of the entire power grid system can be examined. However, for GPS that is used in general national industries, unexpected dangerous situations have occurred due to its deterioration and jamming. Currently, the power grid is based on a synchronization system using GPS. Thus, interruption of the time synchronization system of the power system due to the failure or abnormal condition of GPS would have enormous effects on each field such as economy, security, and the lives of the public due to the destruction of the synchronization system of the national power grid. Developed countries have an emergency substitute system in preparation for this abnormal situation of GPS. Therefore, in Korea, a system that is used to prepare for the interruption of GPS reception should also be established on a long-term basis; but prior to this, it is required that an evaluation technique for the time synchronization performance of a GPS receiver using an atomic clock within the power grid. In this study, a monitoring system of time synchronization based on GPS at a power grid was implemented, and the results were presented.

주간 절단시기 및 생장조절제를 이용한 '후지'/M9 사과나무 수관 상단부 생장조절 (Growth Control of Upper Part in 'Fuji'/M.9 Apple Tree Canopy by Cutting Time of Trunk and Plant Growth Regulators)

  • BACKGROUND: The vigorous shoot growth in upper part of apple tree canopy leads to poor fruit quality and flower bud formation in lower part of canopy. So, this study was conducted to develop the proper control method about the shoot growth in upper part of apple tree canopy. METHODS AND RESULTS: Trunks of 'Fuji'/M9 apple trees were cut (back pruned) to 2.5 m in tree height on 11 February (dormant) or 12 April (full bloom). Naphthalene acetic acid (NAA) was applied at 2.0% to cut surface when trunk was pruned. Prohexadione-calcium (Pro-Ca) was sprayed at 250 mg/L above 2.0 m in tree height at 23 April (petal fall). The NAA or Pro-Ca application after trunk was pruned at dormant (TR-2 and TR-3) significantly reduced shoot growth in upper part of canopy compared with the control (tree was only pruned at dormant, TR-1), but the percent of shoots showing the secondary growth of TR-3 was higher over 2 times than that of TR-2. The reduction of shoot growth in upper part of canopy by TR-2 and TR-3 increased the fruit red color from the lower part in the treating year and blooming of the lower part in the following year. CONCLUSION: Applying 2.0% NAA to cut surface of pruned apple trunk at dormant was the most effective way for stabilization of the tree vigor in upper part of the canopy in a high density apple orchard.

암면배지내 인공 zeolite 형 각종 이온교환제 처리가 온실멜론의 이온교환능에 미치는 영향 (Effects of Artificial Zeolite and Various ion Exchangers Supplemented in Rockwool on the Ion Exchangability of Muskmelon)

  • 본 실험은 인공 zeolite 및 각종 이온교환제를 암면슬라브내에 첨가하므로서 완형능력을 부여하여 온실멜론의 암면재배에 대한 이온 교환능 및 흡착능의 작용기작을 비교검사하고자 수행하였다. 1. 이온교환제의 처리에 따른 멜론의 생육특성중 초장은 대조구인 원시 1/2단위에서 124cm인데 대해 Ca형 인공zeolite에서는 131,2cm로 켰으며 경 및 엽의 생체중에서도 같은 경향이 보였다. 2. 과실중은 인공 zeolite를 첨가한 경우에 무거워지는 경향을 나타냈으며, 당도 및 외관은 처리에 따른 차이가 거의 보이지 않았다. 3. Zeolite의 첨가에 의해 배양액의 pH나 이온교환능을 안정시키는 것이 가능하고, 양분흡수를 순조롭게 하며, 양분흡수의 증가를 촉진하고 생육촉진도 기대할 수 있었다. 이상의 결과는 인공 zeolite를 암면배지내에 첨가하므로서 이온 보유능, 흡착능을 갖게하여 배지내의 생육환경이 안정되고 암면배지는 인공 zeolite 첨가로 배지의 CEC를 대폭적으로 개선시키는 효과가 컸다.

내시경과 선택적 도뇨관 풍선을 이용한 안와하벽복원술 (Orbital Floor Reconstruction Using Endoscope and Selected Urethral Balloon Catheter)

  • Purpose: Blow-out fractures can be reduced using various methods. The orbital reconstruction technique using a balloon under endoscopic control has advantages over other methods. However, this method has some problems too, such as postoperative follow-up, management of the balloon catheter, and reduction of the posterior orbital floor. Thus, we developed a simple, effective method for orbital floor reduction that involves molding and shaping the antral balloon catheter. Methods: A 0, 30, or $70^{\circ}$, 4-mm endoscope was placed though a two-point, 5-mm maxillary antrostomy. The balloon catheter is placed directly at the orbital apex to reconstruct the anterior shelf (spherical shape), while it is turned in a U-shape towards the anterior maxilla for the posterior shelf (elliptical shape). Orbital floor defects, compound or comminuted fractures are reconstructed with alloplastic materials through an open lid incision under the endoscopic control. Results: This technique was applied to ten patients with orbital floor fractures: five anterior shelf and five posterior shelf fracture, respectively. Four of the patients had zygomatico-orbital fractures, while the rest had isolated orbital floor fractures. Two patients were given porous polyethylene implants Synpor$^{(R)}$) and three underwent reconstruction with a resorbable mesh plate. No complication associated with this technique was identified. Conclusion: The freestyle placement and selection of a urinary balloon catheter under endoscopic control and the preoperative estimation of the volume enhanced the stabilization of the orbital contour. This method improves the adaptation of the orbital floor without the risk of injuring the surrounding orbital contents, dissecting blindly, or using sharp traction. One drawback of this method is the patient's discomfort from the catheter during treatment.

Brachytherapy of Uveal Melanomas with Ruthenium-106 Plaques

  • Background: Brachytherapy is the most commonly used conservative treatment for the uveal melanoma. The aim of this study was to evaluate therapeutic results of Ruthenium-106 plaque brachytherapy in the management of localized uveal melanoma cases. Methods: We reviewed retrospectively the clinical records of all patients treated in our department for an uveal melanoma, undergoing Ruthenium-106 plaque brachytherapy, from January 1996 to December 2015. We focused on clinical features, therapeutic characteristics, local and distant tumor control and side effects. Results: Nineteen patients were enrolled in our study. Mean age was 56.2 years (28-79) and the sex ratio was 1.37:1 males to females. Diagnosis was made on the basis of ophthalmological clinical examination, angiography, ultrasound and/or magnetic resonance. Median tumor diameter was 9.7 mm (6-13) and median thickness 4.4 mm (2.5-8). The dose of Ruthenium-106 plaque brachytherapy prescribed to the apex of each tumor was 70 Gy in all cases. The median radiation dose to the sclera surface was 226.4 Gy (range: 179.6-342.3) and the median total application time 115.2 hours (range: 27 to 237). After a median follow-up of 61.5 months, local control was achieved in 17 patients (89%): 16 demonstrated a partial tumor response and 1 tumor stabilization. Two patients suffered local progression leading to enucleation, one dying of hepatic metastasis. Radiation-induced complications were cataracts in 3 cases and vitreal hemorrhage in 2. Conclusion: Ruthenium-106 plaque brachytherapy is an efficient treatment for localized uveal melanoma, offering good local control with low toxicity.

고객센터 상담원의 이직의도 철회에 영향을 미치는 요인 연구 (A Study on Factors Affecting Retractation of Turnover Intention of Customer Center Counselor)

  • 본 연구는 이직의도 철회 경험이 있는 상담원이 이직의도 철회시 어떠한 요인들이 중요한 역할을 했는지 알아보기 위해 모 통신사 고객센터 상담원을 대상으로 3개월에 걸쳐 인터뷰 설문조사 연구를 실시하였다. 연구결과 업무이동, 동기부여, 자기효능감, 정서조절이 상담원 이직의도 철회에 주요 영향 요인으로 나타났으며, 상사지원, 동료지원은 이직 의도 철회에 영향을 미치지 못하는 것으로 나타났다. 그중, 정서조절과 업무이동은 이직의도 철회에 있어 다른 요인보다 많은 영향을 미치는 것으로 나타났다. 본 연구의 결과는 기업에서 중요성이 점점 커지고 있는 고객센터 부서의 상담원 이직률을 낮춰 조직의 안정화를 극대화하고, 또한 고객센터 상담원의 이직의도 철회에 관련된 연구에 이론적 기반을 제공할 수 있을 것으로 생각된다.

태양광발전시스템 독립계통 안정화 기술개발 (Technology Development for Stabilization of PV Independent System)

  • 도서 지역과 같은 환경에서 기존 상용 독립계통 디젤 발전기에서 소모되는 화석연료를 저감하기 위해 독립계통 용량의 40% 이상을 부담하는 태양광발전시스템의 개발이 필요하다. 이러한 개발을 위해 우선적으로 도서 지역에 설치된 독립계통의 안정성을 향상시키고 고품질의 전력을 부하에 공급해줄 수 있는 대용량 인버터 개발이 필요하다. 또한, 독립계통 태양광 발전시스템을 위한 EMS용 발전제어 시스템은 태양광 발전원 계통과 연계된 디젤 발전기의 병렬운전 전력제어시스템으로서 부하율과 태양광발전에 따른 디젤 발전기 출력을 제어하여 해당 부하에 전력을 안정적으로 공급하는 시스템이 필요하다. 이러한 독립계통형 태양광 발전시스템으로 인해 독립계통 지역에 대한 태양광 보급 확대 및 독립계통의 전력안정도를 증가시킬 수 있으며 독립계통의 유 무효 전력 제어에 따른 화석연료 사용 저감이 추가적으로 기대되며, PCS 사용률의 증가로 경제성 조기 확보를 기대할 수 있다.

화력발전소의 회처리장 내 비산먼지 저감을 위한 화학적 먼지억제제 적용 연구 (Application of Chemical Dust Suppressants for Control of Fugitive Dust in Ash pond of Thermal Power Plant)

  • The objective of this study is to evaluate efficiencies of chemical suppressants for control of fugitive dust in ash pond of thermal power plant. In this study, $MgCl_2$, PAM (polyacrylamide), and PVA (poly vinyl alcohol) that are generally applied to suppression of fugitive dust generated from unpaved road, coal mining, storage piles and etc, were employed as chemical dust suppressants. The coal ash (coal combustion residuals) were sampled from the ash pond of Yeongheung power division in Incheon, South Korea. The characterization of the sample including particle size distribution, pH, $pH_{PZC}$ and pore volume as well as XRF analysis were carried out. The suppressant treated-samples were investigated with the wind tunnel experiments to estimate and compare the effect of suppressants on stabilization of the surface of coal ash samples. According to the results, the stability of suppressant-treated samples were significantly improved compared to water-treated samples. Among the three kinds of suppressants, PAM and PVA showed higher efficiencies and cost saving than $MgCl_2$.

Massive Hemorrhage Facial Fracture Patient Treated by Embolization

  • Major maxillofacial bone injury itself can be life threatening from both cardiovascular point of view, as well as airway obstruction. Significant hemorrhage from facial fracture is an uncommon occurrence, and there is little in the literature to guide the management of these patients. We report a 73-year-old male driver who was transported to our hospital after a motor vehicle collision. The patient was hypotensive and tachycardic at presentation and required active fluid resuscitation and transfusion. The patient was intubated to protect the airway. All external attempts to control the bleeding, from packing to fracture reduction, were unsuccessful. Emergency angiogram revealed the bleeding to originate from terminal branches of the sphenopalatine artery, which were embolized. This was associated with cessation of bleeding and stabilization of vital signs. Despite the age and severity of injury, the patient recovered well and was discharged home at 3 months with full employment. In facial trauma patients with intractable bleeding, transcatheter arterial embolization should be considered early in the course of management to decrease mortality rate.
