The Case Report of Chronic Ankle Sprain Improved with Hominis Placenta Pharmacopuncture Treatment (만성 족관절 염좌에 자하거 약침을 병행한 한방 치료로 호전된 2례 보고)

  • Objectives The purpose of this study is to investigate and report the effectiveness of Hominis Placenta pharmacopuncture treatment for chronic ankle sprain. Methods 2 patients are treated at Dept. of Korean Medicine, the Armed Forces Busan Hospital, diagnosed as thinning of anterior talofibular ligament (ATFL), suggestive of partial tear and anterior tibiofibular ligament partial tear. They are treated with Hominis Placenta pharmacopuncture. Each cases are measured and assessed by ankle hindfoot scale (AHS), visual analgue scale (VAS), cumberland ankle instability tool (CAIT) scores. Results 2 patients of chronic disease of ankle sprain have a different kind of diagnoses they have. After treatment of Hominis placenta pharmacopuncture, spontaneous pain is decreased and ankle instability, ankle functions are increased significantly. Conclusions Hominis placenta Pharmacopuncture has a effect on chronic ankle sprain.

The Clinical Effects of Heating-Conduction Acupuncture Therapy(tentatively called) for Anterior Talofibular Ligament Injury Induced by Acute Ankle Sprain (전열침(가칭)이 급성 족관절 염좌로 인한 전거비 인대 손상에 미치는 임상적 효과)

  • Objectives : The purpose of this study is to investigate the clinical effects of heating-conduction acupuncture therapy on anterior talofibular ligament injury induced by acute ankle sprain. Methods : From October 31, 2008 to July 21, 2009, the 11 outpatients who had admitted to department of oriental rehabilitation medicine, Dae-Jeon oriental medicine hospital, Dae-Jeon university with acute ankle sprain were performed heating-conduction acupuncture therapy on anterior talofibular ligament injury. To evaluate the efficiency of heating-conduction acupuncture therapy, pain threshold with Pressure algometer and visual analogue scale(VAS) were applied before treatment and after 1st, 2nd treatment. Results : 1. The pain threshold score and the VAS score showed statistical significant improvement after 1st treatment. 2. The pain threshold score and the VAS score showed improvement but had no statistical significance after 2nd treatment. 3. The difference between left and right were not statistical significant in pain threshold score and VAS score. Conclusions : Heating-conduction acupuncture therapy has clinical effects of pain reduction on patient with anterior talofibular ligament injury induced by acute ankle sprain.

A Clinical Study on Effectiveness of Acupuncture Treatment of Acute Ankle Sprain Combined with Positional Release Therapy (자세이완치료를 병용한 침구치료가 급성기 족관절 염좌의 치료에 미치는 영향에 대한 임상적 연구)

  • Objectives: The purpose of this study is to evaluate the therapeutic effect of acupuncture treatment of acute ankle sprain combined with positional release therapy. Methods: A prospective randomized single blind study between positional release group and knee flexed supine position group was conducted. Patients with ankle sprain within 48 hours were evaluated by Ankle injury grade chart(AIGC). In group A, positional release therapy was combined with acupuncture, whereas in group B, acupuncture was conducted in knee flexed supine position. The treatment was planned for a duration of 1 week, 3times a week. In AIGC scores, VAS, weight bearing time and weight bearing time in blind were followed up and compared. Results and Conclusion: The VAS score decreased in both group. Weight bearing time increased in group B, weight bearing time in blind increased in group A. Comparing the therapeutic effect of each group, group A had significant effectiveness in weight bearing time in blind. So we may conclude that Acupuncture treatment combined with positional release therapy is effective and recommandable at early stage of ankle sprain.

  • Objectives : The purpose of this study is to investigate the clinical effects of heating-conduction acupuncture therapy on lumbago induced by iliolumbar ligament sprain. Methods : From September 25 to December 30, 2009, the 8 outpatients who had admitted to department of oriental rehabilitation medicine, Dae-Jeon oriental medical hospital, Dae-Jeon university, with acute lumbago were performed heating-conduction acupuncture therapy on iliolumbar ligament sprain. To evaluate the efficiency of heating-conduction acupuncture therapy, visual analogue scale(VAS) and short-form McGill pain questionnaire(SF-MPQ) were applied before treatment and after 1st, 2nd treatment. Results : 1.The VAS and SF-MPQ scores showed statistically significant improvement after 1st treatment. 2. The VAS and SF-MPQ scores after 2nd treatment showed better improvement than those of before and 1st treatment, but had no statistical significance compared with those of before and 1st treatment. Conclusions : Heating-conduction acupuncture therapy has clinical effects of pain reduction on patient with acute lumbago induced by iliolumbar ligament sprain.

The Effects of Alphabet Exercise with SSP Electrotherapy on Lateral Ankle Sprain (족관절 염좌에 SSP 전자침 요법을 병행한 동기침법의 효과)

  • The multiple acupuncture techniques have been used to relieve the pain of lateral ankle sprain. Recently, early functional rehabilitation is emphasized in rehabilitative concepts using active ROM, facilitated proprioception, alphabet exercise and so forth. Similarly, in oriental rehabilition medicine, traditional movement technique called 'Dong-Ki' has been used to relieve pain of ankle sprain. So we used 'Dong-Ki' technique with SSP(silver spike point) electrotherapy transformed from alphabet exercise and evaluated the effects in method of randomized controlled trials. 27 outpatients with lateral ankle sprains were randomized into two groups : 15 samples and 12 controls. Sample group was treated with 'Dong-Ki' technique(writing his name with foot) with SSP electrotherapy to relieve pain during additionally. 'Dong-Ki'. Outcomes were measured by 10cm VAS and ankle circumference(cm). In terms of VAS, Sample group had inclination of pain relieving but not significantly meaningful. In terms of ankle circumference measurement, Sample group showed significantly improved.

  • Despite applying recent advances in research and technology and taking into acount proper histories and performing thorough physical examinations, a high percentage of patients with low back pain have no identifiable pathology. Problems especially of the sacroiliac joint are commonly missed. The purpose of this article is to describe the effects of sacroiliac injection of local anesthetics and corticosteroids on patients with acute sacroiliac sprain. The sacroiliac joints are essentially nonweight-bearing joints that allows a small amount of anteroposterior rotatory movement around transverse axis usually about 5 to 10 cm below the promontory of the sacrum vertically. The ligamentous portion of sacroiliac joint is vulnerable to rotatory movement particularly when the individual is in an awkward position. Injections of 3 ml of 2% lidocaine and 10 mg of methylprednisolone were given twice at two day intervals to 26 patients with acute sacroiliac sprain. Most of those patients obtained excellent results without any adverse effects.

  • Objectives : This study was to evaluate the effectiveness of Bee Venom acupuncture therapy on sprain of C-spine by randomized controlled double blinding method. Methods : A randomized double blinding study of bee venom acupuncture therapy was conducted on sprain of C-spine. We divided sprain of C-spine patient into 2 groups: one group with both acupuncture and saline acupuncture therapy, and another group with both acupuncture and bee venom acupuncture therapy. To estimate the efficacy of treatment that applied for two groups, we used visual analog scale(VAS) and neck disability index(NDI). We compared the VAS and NDI score of two groups statistically. Results : STZ induced increase of serum creatinine, BUN and albumin secretion were lowered by AD-1 treatment.

A Retrospective Study of Acute Lumbar Sprain Patients on the Effect of Korean Medicine Treatment with Dokhwal-tang (Duhuo-tang) (급성 요추부 염좌로 입원했던 환자군의 독활탕 치료 효과에 대한 후향적 고찰)

  • Objectives This study investigated the effect of korean medicine treatment with Dokhwal-tang (Duhuo-tang) on Lumbal sprain patients by comparing with other herbal medicines. Methods 17 lumbar sprain patients of group treated with Dokhwal-tang (Duhuo-tang) were treated with Dokhwal-tang (Duhuo-tang), acupunture, cupping, oriental physical therapy. 13 lumbar sprain patients of group treated with other herbal medicines were treated with 9 prescriptions of herbal medicine, acupunture, cupping, oriental physical therapy. ODI (Oswestry diability index), VAS (Visual Analogue Scale) were checked to evaluate the effectiveness of the treatment. Results 1) ODI, VAS of patients treated with Dokhwal-tang (Duhuo-tang) were all improved and there were statistical significance (p<0.05). 2) VAS of group treated with Dokhwal-tang (Duhuo-tang) and that of group treated with other herbal medicines were all improved significantly. And treatment period of group treated with Dokhwal-tang (Duhuo-tang) was shorter than that of group treated with other herbal medicines. But there was no statistical significance between VAS of group treated with Dokhwal-tang (Duhuo-tang) and that of group treated with other herbal medicines. 3) In comparing medical history period on group treated with Dokhwal-tang (Duhuo-tang), ODI, VAS of hyper-acute phase group and acute phase group were improved but them of sub-acute phase group were less improved. Conclusions According to the study, korean complex therapy with Dokhwal-tang (Duhuo-tang) might improve acute lumbar sprain.

Case Series Study on the Use of BU Pharmacopuncture Treatment in Patients with Acute Lumbar Sprain

  • Objective: The purpose of this study was to investigate the clinical effects of BU pharmacopuncture therapy consisting of bear's gall(fel ursi) and ox bezoar(bovis calculus) on acute lumbar sprain. Methods: 12 patients diagnosed as acute lumbar sprain in 6 designated local Korean medicine clinics from October 2017 to February 2018 were treated by BU pharmacopuncture. Several acupoints in abdomen and lumbar region were selected by clinicians at their own discretion. The effectiveness of the therapy was evaluated using VAS and ODI. After that we reviewed the medical records of all these patients to evaluate the effectiveness and safety of the therapy. Results: The VAS and ODI scales were significantly decreased after BU pharmacopuncture therapy. And no major complications and adverse effects were reported. Conclusion: BU pharmacopuncture showed rapid pain relief in patients with acute lumbar sprain. It is possible to shorten the treatment period of acute lumbar sprain and prevent progressing to chronic back pain in advance. To establish the effects of BU pharmacopuncture therapy, more succeeding clinical and laboratory studies are needed.

Comparison Study for Analgesic Effects of Electroacupuncture on Surgical Ankle Sprain Model Classified as Grade 3 in Rats (수술적 방법으로 유도된 3단계 고도(高度) 발목염좌 모델에서 혈위(穴位)에 따른 전침효과의 비교)

  • Objectives : The purpose of this study was to determine whether electroacupuncture(EA) is effective in reducing pain on the severe ankle sprain classified as grade 3 in rats. Methods : The severe(grade 3) ankle sprain model was induced surgically by ankle ligament injury(the anterior talofibular, the calcaneofibular and the posterior talofibular) in the Sprague-Dawley rats(180~250 g). The effects of EA on weight bearing forces(WBR) of the affected foot were examined in a rat model of ankle sprain. EA was applied to either SI6, ST37, GB34, GB39 or GB42 acupoints by trains of electrical pulses(2 Hz, 1 ms pulse width, 2 mA intensity) for 15 min. Results : Cutting of the lateral ankle ligament complex produced the severe ankle sprain symptoms as grade 3. EA of the contralateral SI6 resulted in more analgesic effect than one of ipsilateral SI6 even though there was significant effect. EA of the ipsilateral GB34 and GB39 produced potent analgesic effects on the surgical ankle sprained pain behaviors. However, there were no significant analgesic effects in the contralateral GB34 and GB39 EA groups. In addition, both side of ST37 and GB42 did not result in analgesic effect on the surgical ankle sprained rat. Conclusions : The data suggest that EA induced analgesia shows point specificity on the severe ankle sprained pain model classified as grade 3.
