전인적 호스피스 간호중재 프로그램이 호스피스완화의료병동 입원 환자의 자아존중감과 영적안녕에 미치는 효과 (Effects of Holistic Hospice Nursing Intervention Program on Self Esteem and Spiritual Well-being for Inpatients of Hospice Palliative Care Unit)

  • 목적: 본 연구는 단일군 사전 사후 원시실험설계 연구로서 호스피스완화의료병동 입원 환자를 위한 전인적 호스피스간호중재 프로그램("무지개 프로그램")의 자아존중감과 영적안녕에 대한 효과를 검증하고자 하였다. 방법: 2004년 4월 6일부터 2005년 4월 20일까지 경북포항시 소재 선린병원의 호스피스완화의료병동에 입원한 만18세 이상의 성인 환자로서 의사소통이 가능하여 연구참여에 서면 동의한 27명을 대상으로 사전조사 후 2주간, 총 10회(회당 120분)로 구성된 전인적 호스피스 간호중재 프로그램 제공 후, 사후 조사를 실시하였다. 효과 검정을 위해 자아존중감 측정 도구로는 성인용으로 수정보완된 Self Esteem Questionnaire (SEQ), 영적안녕 측정도구로는 Spiritual Well-being Questionnaire를 사용하였으며, 자료분석은 SPSS/WIN 12.0 프로그램을 이용하여 Paired t-test로 분석하였다. 결과: 1. 가설 1 '전인적 호스피스 간호중재 프로그램을 제공받은 호스피스완화의료병동 입원환자(이하 실험군)는 실험 전보다 실험 후의 자아존중감 정도가 높을 것이다'는 지지되었다(t=11.554, P<0.000) 2. 가설 2 '전인적 호스피스 간호중재 프로그램을 제공받은 실험군은 실험 전보다 실험 후의 영적안녕 정도가 높을 것이다'는 지지되었다(t=6.387, P<0.000). 결론: 전인적 호스피스간호중재 프로그램은 호스피스완화의료병동 입원환자의 자아존중감과 영적안녕을 증진시키는 데 효과적이므로, 호스피스완화의료 병동에 입원한 말기환자를 위해 임상 실무에서 적용 가능하며, 호스피스 전문직의 다학제적 팀 접근 모델로 연구, 교육 측면에서도 유용하리라 생각한다.

  • 본 연구는 간호대학생의 호스피스인식이 임종간호태도에 미치는 영향을 확인하고 이들의 관계에서 영적안녕의 매개효과를 알아보는 것이 목적이다. 연구 목적을 달성하기위해 344명의 간호대학생을 대상으로 설문 조사하였으며 SPSS 21과 AMOS 21로 결과를 분석하였다. 연구결과로는 첫째, 간호대학생의 임종태도는 호스피스인식 및 영적안녕과 모두 정의 상관관계를 가지는 것으로 나타났다. 둘째, 임종간호태도에 영향을 미치는 변인을 모두 고려하여 분석한 결과, 연구모형과 구조모형의 적합도는 모두 매우 양호한 수준으로 나타났다. 마지막으로 간호대학생의 임종간호태도에 호스피스인식은 긍정적인 영향을 미치며 이들의 관계에서 영적태도는 매개효과를 가지는 것으로 나타났다. 이와 같은 연구결과를 토대로 간호대학생의 호스피스에 대한 효과적인 간호에 대한 함의를 제시하고자 한다.

한국인과 미국이민 한국인의 영적안녕과 지각된 건강상태 비교 (A Comparative Study on Spiritual Well-being and Perceived Health Status between Korean and Korean-American)

  • Purpose: The purpose of this study is to compare spiritual well-being and perceived health status between Korean and Korean-American and to provide basic data that might develop comprehensive health care program including spiritual dimension. Method: The subject of the study were 411 adults chosen from religious organization located in Gyeongsang Province, Korea and Chicago, U.S.A.. The instruments used in the study were Spiritual Well-being Scale by Paloutizian & Ellison and Health Self Rating Scale by North Illinois University. Analysis of data was done by using descriptive statistics, Pearson correlation coefficient, ANCOVA, ANOVA and Duncan test with SPSS program. Result: 1) The mean score of spiritual well-being of Korean was 3.17 and Korean-American was 2.63, there was significant difference between two groups. 2) Perceived health status of Korean was 2.37 and perceived health status of Korean-American was 2.54, there was significant difference between two groups. Conclusion: According to this study, it is important to take into considerations spiritual aspects and cultural and environmental elements in developing the comprehensive health care program.

호스피스 환자와 비호스피스 환자의 영적 안녕과 삶의 질 (Comparison of Spiritual Well-being and Quality of Life between Hospice Patients and Nonhospice Patients)

  • Purpose: This study was conducted to find out the effects of hospice care by evaluating the spiritual well-being and quality of life in the hospice and nonhospice patients. Method: The research design was composed of descriptive study. The data were collected using the questionnaire with interview from 30 hospice patients at three hospice institutes and 30 nonhospice patients at two general hospitals. The tools used for this study were 14-item questionnaire regarding general characteristics, a revised Spiritual Well-being Survey(Paloutzion and Ellision, 1982) and 22-item of revised Mcgill Quality of Life questionnaire. Result: The spiritual well-being of the hospice patients was higher than that of nonhospice patients(F=5.52, p=0.023). The global quality of life of the hospice patients was higher than that of nonhospice patients(F=8.84, p=0.004). There was a significant positive correlation between spiritual well-being and quality of life of the hospice patients and non hospice patients. Conclusion: The hospice care effects on spiritual well-being and quality of life of the terminal cancer patients.

중고령자의 정서적 행복감과 영적요구가 건강한 노후에 미치는 영향 (Impacts of Emotional Happiness and Spiritual Needs on Healthy Aging of Middle-aged and Elderly Population)

  • Purpose: This study aimed to identify the relationship among emotional happiness, spiritual need, and healthy aging and to identify the factors affecting healthy aging in middle-aged and elderly population. Method: The participants were 100 middle-aged and elderly individuals. Data were collected using self-report questionnaires from March 9 to May 27, 2020. Collected data were analyzed using the SPSS/WIN 26.0 program. Results: There was a positive correlation between healthy aging and emotional happiness (r=.70, p<.001) and spiritual need (r=.52, p<.001). The factors influencing healthy aging were gender (β=.13, p=.026), subjective health status (β=.19, p=.002), emotional happiness (β=.60, p<.001), and spiritual need (β=.34, p<.001). These variables explained 67% of healthy aging. Conclusion: Healthy aging had a significant impact on women than on men when subjective health status was good and when emotional happiness and spiritual need were high. Healthy aging of the middle-aged and elderly population has confirmed the importance of physical, emotional, and spiritual health. Therefore, development and operation of programs that include various aspects of physical, emotional, and spiritual for healthy aging should be considered to confirm their effectiveness.

  • Altruistic virtuous caring, possibly originated from religion and/or spirituality, is indispensable for holistic health through channeling vital energy with diet, exercise and meditation. This is a participant-observed medical anthropological research of a first generation Korean elderly immigrant health professional woman living in a four generational family. She had hypertension and was concerned about possible attack of stroke. Multi-religious, spiritual, and cosmological vital energy based on holistic Nature-oriented health beliefs and practices influenced by psychosocial, cultural and economic background, education, self- discipline and self-cultivation of individual, and group or family may create health. Self-care beliefs based on confidence in self-control of one's life style for oneself and others influence individual and group health practice. The holistic alternative health beliefs and practices were proved to be efficacious and beneficial by her self-evaluation, evaluation of significant others, biomedical professionals, and laboratory tests. That may have potential application for global health.

간호사를 위한 영적간호교육프로그램의 개발 및 효과 (Development and Effectiveness of a Spiritual Care Education Program for Nurses)

  • 본 연구는 영적간호교육프로그램이 간호사의 자아존중감, 의사소통능력, 실존적 안녕 및 영적간호역량에 미치는 효과를 확인하기 위한 비동등성 대조군 전후시차 유사실험 연구이다. 연구 참여자는 G 광역시 C대학의 간호학 학사학위과정 중에 있는 간호사 63명(실험군 30명, 대조군 33명)이었다. 실험군은 영적간호교육프로그램에 7회기 동안 참석하였으며, 회기는 회당 90분, 주 1회, 7주에 걸쳐 실시되었다. 실험군의 자료 수집과 실험 처치는 2017년 10월 10일부터 12월 5일까지 시행되었고 대조군은 2018년 4월 24일에 사전조사, 6월 5일에 사후조사가 실시되었다. 자료분석은 SPSS 21.0 windows program을 이용하여 ${\chi}^2-test$, Fisher's exact test, 독립표본 t-test로 분석하였다. 연구결과, 실험군이 대조군에 비해 실존적 안녕(p<.025)과 영적간호역량(p<.001)이 유의하게 향상되었다. 따라서 본 영적간호교육프로그램이 간호사의 실존적 안녕과 영적간호역량을 향상시키는데 효과적임을 확인할 수 있었다.

  • The elderly people was over 8.3% in 2003. This seament is projected to grow to 14.4% by 2019(Aged society) and over 20% by 2026(Super aged society). As the elderly population is increasing and young population is declined social issues around elderly care are emerged in Korea. As a result of that, they are in the condition of poor system of the welfare of the old and it is the worst one in Korea. And because of the worst financial independence and difficulty in Korea, welfare facilities and medical instruments are so insufficient and deteriorated, so the quality of service is getting low. Furthermore the mortality of cancer is rapidely increased in recent years. So it is expected that the number of families who are caring for terminal cancer patient will be increased. We can not solve those problems only with government's policy such as to secure ample budget and to enlarge the aged welfare institutions. Definitely, to acheve the suggetions persistence concern about old people is most important, and family, community and national government should be linked to accomplish to goal. Accordingly, for this study attempts to provide conceptual framework of the respite care, spiritual care and home hospice, nursing home for the elderly. And this study is to discuss the necessity and effect about the construction of housing welfare infrastructure and to make a cooperation and linkage system among the aged welfare institutions, government and the aged welfare programs. As is well known, the issue of the increasing aged people is neither a matter of individuals nor of the family, but rather a complex matter of whol society. Therefore it can be resolved by the active participation of government. Conclusively this study tries to provide an direction of the improvement with regard to the welfare policy for the elderly. The study is as follwos: Chapters 1: The theological base, conception, essentiality, respite care, spiritual care and home hospice, nursing home for the elderly. Chapters 2: Definitions of respite care, spiritual care, hospice, patient of termina cancer and quality of life. And relations between respite care and nursing home. Chapters 3: The necessity of long-term care insurance and the perspectives of spiritual care. Chapters 4: Conclusions and summarizing(The directions of improvement of welfare policy for the elderly). To sum up, the problems of the aged people has not effects only to the aged people. This means that the problems of aged is the problems of the young generation today. Because young generation will be the old generation in the near future.

Comparison of Spiritual Needs between Patients with Progressive Terminal Kidney Disease and Their Family Caregivers

  • Purpose: The purpose of this study was to compare differences in spiritual needs (SNs) and factors influencing SNs between patients with progressive terminal kidney disease and their family caregivers. Methods: An explorative comparative survey was used to identify the SNs of patients (N=102) with progressive terminal kidney disease undergoing hemodialysis and their family caregivers (N=88) at a general hospital located in Seoul, South Korea. The data were analyzed using descriptive statistics, the chi-square test, the independent t-test, one way analysis of variance, the Scheffe test, and multiple regression with dummy variables. Results: The SNs among family caregivers were higher than in the patient group. SNs were higher among those who were religious in both groups. Loving others was the highest-ranked subdimension in the patient group, followed in descending order by maintaining positive perspective, finding meaning, Reevaluating beliefs and life, asking "why?", receiving love and spiritual support, preparing for death, and relating to God. In the family group, the corresponding order was maintaining positive perspective, loving others, finding meaning, receiving love and spiritual support, preparing for death, relating to God, and asking "why?". The factors that had a negative influence on the level of SNs were not being religious in the patient group and having only a middle school level of education in the family group. Conclusion: The results of this study may serve as evidence that spiritual care for non-cancer patients' family caregivers should be considered as an important part of hospice and palliative care.

호스피스 완화의료에서의 초기평가와 돌봄 계획의 수립: 평가도구를 중심으로 (Initial Assessment and Care Planning in Palliative Hospice Care: Focus on Assessment Tools)

  • 포괄적이고 총체적 돌봄을 제공하는 호스피스 완화의료의 경우 평가도구를 사용하여 문제목록의 객관화하고 돌봄 계획의 구체화하는 것이 필요하다. 초기평가는 입원 당일 혹은 1일 이내에 환자의 증상과 사회적, 영적 문제, 완화의료에 대한 요구가 무엇인지 파악하며 개괄적인 돌봄의 방향을 설정하는 과정이다. 현재 병에 대한 인식과 예후를 파악하는 것부터 연명의료계획서 작성까지 함께 검토한다. 초기평가도구로는 간단하지만 포괄적으로 신체적, 정신적, 사회적, 영적 영역을 아우르는 13개의 간단한 질문으로 구성된 NEST를 추천한다. 초기평가에서 파악된 문제에 대하여 구체적인 평가도구들을 활용하여 돌봄 계획을 수립하며 입원 후 3일이내 시행하는 것이 좋다. 신체적 영역의 경우 기능상태를 포함하여 통증과 비 통증 증상, 삶의 질을 함께 파악할 수 있는 다증상 평가도구가 도움이 될 수 있다. 환자의 증상은 단독으로 의미를 가지는 것이 아니라 복합적인 상호작용으로 발생하며, 그에 따라 포괄적으로 접근해야 하기 때문이다. 정신심리적 영역은 심리적 고통과 불안, 우울을 평가한다. 사회적 영역의 평가는 의사결정, 사회경제적 환경 파악, 가족 평가 및 임종 준비의 단계로 이루어진다. 영적 평가 역시 중요한데 FACIT-Sp나 SHI를 사용할 수 있다. 평가도구를 활용하는 것은 환자의 고통을 경감시키고 삶의 질을 향상시킬 뿐 아니라 의료진이 체계적으로 훈련되는 방안이 될 수 있으며 그 과정 자체가 더 나은 돌봄 제공을 위한 발판이 될 수 있다.
