International Spine Radiosurgery Consortium Consensus Guidelines에 따른 Spine Stereotactic Radiosurgery에서 IMRT와 VMAT의 비교연구 (Comparison of IMRT and VMAT Techniques in Spine Stereotactic Radiosurgery with International Spine Radiosurgery Consortium Consensus Guidelines)

  • 정위적 체부 방사선치료(Stereotactic Body Radiation Therapy, SBRT)는 척추 전이암을 치료하는데 있어서 점점 증가하고 있다. 표적 종양의 급격한 선량 변화와 등선량 분포를 얻기 위해서, 세기조절방사선치료(Intensity-modulated radiation therapy, IMRT)와 체적변조회전치료(Volumetric-modulated arc therapy, VMAT)는 척추 방사선수술에 있어서 필수적인 치료기법이다. 이 연구의 목적은 표적 종양을 위한 International Spine Radiosurgery (ISRC) Consortium의 consensus guideline으로 그려진 표적에 있어서 IMRT와 VMAT의 치료기법을 질적으로 비교하고자 한다. 경부, 흉부, 요추 부위에 종양치료를 받은 3명의 환자를 선택 하였다. 표적 종양은 ISRC의 consensus guideline을 바탕으로 정의 하였다. $T_B$는 vertebral body만 포함하였고, $T_{BPT}$는 vertebral body, pedicle, transverse process를 포함하였다. 그리고 $T_{ST}$는 spinous process와 transverse process를 포함하여 그렸다. Maximum spinal cord선량은 $T_B$, $T_{BPT}$, $T_{ST}$에서 각각 12.46 Gy, 12.17 Gy, 11.36 Gy였고, IMRT, RA1, RA2에서 각각 11.81 Gy, 12.19 Gy, 11.99 Gy였다. 평균 감소(90%~50%) 선량 거리 (mm)는 $T_B$, $T_{BPT}$, $T_{ST}$에서 각각 3.5 mm, 3.3 mm, 3.9 mm였고, IMRT, RA1, RA2에서 각각 3.7 mm, 3.7 mm, 3.3 mm였다. 가장 복잡한 $T_{BPT}$의 경우에서 IMRT, RA1, RA2의 conformity index는 각각 0.621, 0.761, 0.817 이었고, rDHI는 0.755, 0.796, 0.824 였다. IMRT와 VMAT 모두 척추 정위적 방사선수술에서 표적 종양에 급격한 선량 변화와 등선량 분포를 전달하였다. 그러나 표적 종양이 vertebral body, pedicle, transverse process를 포함한다면, IMRT 치료기법은 VMAT 치료기법과 비교해서 conformity index 측면에서 불충분하였다. 그럼에도 불구하고, IMRT 치료기법은 RA1, RA2와 비교해서 대부분의 영역에서 maximum spinal cord 선량을 줄이는데 더 효과적이었다.

  • Granulocytic sarcomas are solid tumors resulting from the localized proliferation of myelogenous leukions cells. Epidural involvement of granulocytic sarcoma is very rare in acute myelogenous leukemia(AML). We report a patient with a thoracic epidural granulocytic sarcoma whose presentation with acute paraparesis led to the diagnosis of relapsing of alleged AML. Early recognition of the etiology of the paraparesis and treatment with emergency decompressive, laminectomy, radiation therapy and chemotherapy resulted in an excellent neurological and hematological outcome.

Clinical Applications of the Tubular Retractor on Spinal Disorders

  • Tubular retractor system as a minimally invasive surgery (MIS) technique has many advantages over other conventional MIS techniques. It offers direct visualization of the operative field, anatomical familiarity to spine surgeons, and minimizing tissue trauma. With technical advancement, many spinal pathologies are being treated using this system. Namely, herniated discs, lumbar and cervical stenosis, synovial cysts, lumbar instability, trauma, and even some intraspinal tumors have all been treated through tubular retractor system. Flexible arm and easy change of the tube direction are particularly useful in contralateral spinal decompression from an ipsilateral approach. Careful attention to surgical technique through narrow space will ensure that complications are minimized and will provide improved outcomes. However, understanding detailed anatomies and keeping precise surgical orientation are essential for this technique. Authors present the technical feasibility and initial results of use a tubular retractor system as a minimally invasive technique for variaties of spinal disorders with a review of literature.

Intramedullary Subependymoma of the Thoracic Spinal Cord

  • An Intramedullary subependymoma of the spinal cord is a rare tumor with only 43 reported cases in the literature. Most of them are reported to be localized within the cervical spinal cord. We report a rare case of a thoracic spine intramedullary subependymoma in a 37-year-old female who presented with back pain and radiating leg pain. Subtotal resection and post-operative radiotherapy were performed. Subependymomas developing in the spinal cord are benign with a low proliferative potential. Complete resection of the tumor appears to be the optimal method for a complete cure. However aggressive surgery may cause severe neurological deficit. Therefore, if severe neurological deficits are expected after complete removal, a partial removal and postoperative radiation therapy is an alternative method for treatment in selected cases. A large-scale randomized study is mandatory to clarify the effectiveness of radiotherapy and to establish the recurrence rate and prognosis with respect to the surgical removal of these tumors.

Capillary Hemangioma of the Thoracic Spinal Cord

  • Capillary hemangiomas are common soft tissue tumors on the skin or mucosa of the head and neck in the early childhood, but very rare in the neuraxis. A 47-year-old man presented with one month history of back pain on the lower thoracic area, radiating pain to both legs, and hypesthesia below 17 dermatome. Thoracic spine MRI showed $1{\times}1.3{\times}1.5\;cm$, well-defined intradural mass at T6-7 disc space level, which showed isointensity to spinal cord on T1, heterogeneous isointensity on T2-weighted images, and homogeneous strong enhancement. The patient underwent T6-7 total laminotomy, complete tumor removal and laminoplasty. Histologically, the mass showed a capsulated nodular lesion composed of capillary-sized vascular channels, which were tightly packed into nodules separated by fibrous septa. These features were consistent with capillary hemangioma.

  • Neuroendoscopic surgery is performed because it causes minimal damage to normal structures, carries a lower rate of complications, and achieves excellent outcomes. Surgeons using an endoscope and related instruments can perform complex operations through very small incisions, which is especially useful for minimally invasive procedures for the brain and spine. Neuroendoscopic surgery is now performed in cases of obstructive hydrocephalus, various intraventricular lesions, hypothalamic hamartomas, craniosynostosis, skull base tumors, and spinal lesions. This review discusses the brief history of neuroendoscopy and the current state and future perspectives of endoscopic surgery.

Mammalian target of rapamycin inhibitors for treatment in tuberous sclerosis

  • Tuberous sclerosis complex (TSC) is a genetic multisystem disorder that results from mutations in the TSC1 or TSC2 genes, and is associated with hamartomas in several organs, including subependymal giant cell tumors. The neurological manifestations of TSC are particularly challenging and include infantile spasms, intractable epilepsy, cognitive disabilities, and autism. The TSC1- and TSC2-encoded proteins modulate cell function via the mammalian target of rapamycin (mTOR) signaling cascade, and are key factors in the regulation of cell growth and proliferation. The mTOR pathway provides an intersection for an intricate network of protein cascades that respond to cellular nutrition, energy levels, and growth factor stimulation. In the brain, TSC1 and TSC2 have been implicated in cell body size, dendritic arborization, axonal outgrowth and targeting, neuronal migration, cortical lamination, and spine formation. The mTOR pathway represents a logical candidate for drug targeting, because mTOR regulates multiple cellular functions that may contribute to epileptogenesis, including protein synthesis, cell growth and proliferation, and synaptic plasticity. Antagonism of the mTOR pathway with rapamycin and related compounds may provide new therapeutic options for TSC patients.

Bronchioloalveolar Carcinoma in a Juvenile Rhadomyosarcoma Patient

  • Primary tumors of the lung are uncommon in pediatric patients, particularly bronchioloalveolar carcinoma (BAC). An 11-year-old female suffering from back pain for 1 month was referred to Seoul St. Mary's Hospital for treatment of a pathologic fracture of the lumbar spine. Comprehensive evaluation disclosed numerous pulmonary metastases of rhabdomyosarcoma (stage IV). During chemotherapy, most of the lung lesions regressed, with the exception of two nodules. Wedge resections, intended for diagnosis and cure, yielded a histologic diagnosis of BAC.

  • Lesions occurring simultaneously in the somatosensory or motor cortex of the brain and the cervical spine are rare. Brain tumors can cause similar symptoms to cervical lesions which can lead to confusion in treatment priorities. Moreover, if cervical disease is noticeably observed in radiologic findings of a patient complaining of cervical radiculopathy with non-specific electromyography results, it is common to no longer perform further evaluation. Here we introduce two cases where the cause of cervical radiculopathy was first considered to be the result of a degenerative cervical disease but was later discovered to be a result of a brain tumor.

흉요추부에서 발견된 경막외 해면상 혈관종 - 증례 보고 - (A Thoracolumbar Pure Spinal Epidural Cavernous Hemangioma - A Case Report -)

  • 연구계획: 증례 보고 목적: 신경인성 종양과 유사한 흉요추부의 경막외 해면상 혈관종 환자를 보고하고자 한다. 선행 연구문헌의 요약: 뼈에 침습이 없는 경막외 해면상 혈관종은 매우 드문 질환으로 제한된 문헌 보고만이 존재한다. 대상 및 방법: 53세 여자 환자가 내원 한달여 전부터 시작된 좌측 하지의 감각저하를 동반한 저린 느낌을 주소로 내원하였다. 내원시 시행한 신경학적 검진에서 기타 다른 신경학적 이상 소견은 관찰되지 않았다. 자기공명영상에서는 흉추 11-12번간 좌측 추간공에 $2{\times}0.5cm$ 크기의 종괴가 관찰되었다. 수술을 시행하여 이를 완전 적출하였다. 결과: 병리 검사에서 해면상 혈관종이 확진되었다. 수술 부위의 약한 통증을 제외하고, 추적 관찰 2년동안 추가적인 신경학적인 증상 발생 없이 좋은 결과를 보였다. 수술 후 2년째에 촬영한 MRI에서 종양의 재발 소견은 관찰되지 않았다. 결론: 척추의 경막외 해면상 혈관종은 매우 드문 질환이며, 다른 경막외 병변들과 감별하기는 쉽지 않다. 그러나, 수술적 치료의 좋은 예후를 보이는 점을 감안하면, 경막외 병변의 감별진단시 이를 반드시 고려해야 할 것으로 생각한다.
