Clinical Analysis of Acute Radiculopathy after Osteoporotic Lumbar Compression Fracture

  • Objective : The purpose of this study was to analyze the relationship between fracture pattern and the development of acute radiculopathy after osteoporotic lumbar compression fracture. Methods : This study included 59 patients who underwent bone cement augmentation for osteoporotic compression fracture below the L2 level, which can lead to radiculopathic radiating pain. The patients were divided into two groups according to the presence of radiculopathy (group A : back pain only; group B : back pain with newly developed radiating pain). We categorized compression fractures into three types by the position of the fracture line. The incidence of newly developed radiculopathy was examined retrospectively for each compression fracture type. Results : The overall incidence of newly developed leg pain (group B) was 25%, and the frequency increased with descending spinal levels (L2 : 0%, L3 : 22%, L4 : 43%, and L5 : 63%). The back pain-only group (group A) had mostly superior-type fractures. On the other hand, the back pain with radiculopathy group (group B) had mostly inferior-type fractures. Most patients in group B showed significant relief of leg pain as well as back pain after bone cement augmentation. Conclusion : The incidence of a newly developed, radiating pain after osteoporotic compression fractures increased gradually from the L3 to L5 levels. Most of these fractures were of the inferior type, and the bone cement augmentation procedures seemed to be sufficient for relief of both back and radiating pain.

Bone Cement Dislodgement : One of Complications Following Bone Cement Augmentation Procedures for Osteoporotic Spinal Fracture

  • Bone cement augmentation procedures have been getting more position as a minimally invasive surgical option for osteoporotic spinal fractures. However, complications related to these procedures have been increasingly reported. We describe a case of bone cement dislodgement following cement augmentation procedure for osteoporotic spinal fracture by reviewing the patient's medical records, imaging results and related literatures. A 73-year-old woman suffering back and buttock pain following a fall from level ground was diagnosed as an osteoporotic fracture of the 11th thoracic spine. Percutaneous kyphoplasty was performed for this lesion. Six weeks later, the patient complained of a recurrence of back and buttock pain. Radiologic images revealed superior dislodgement of bone cement through the 11th thoracic superior endplate with destruction of the lower part of the 10th thoracic spine. Staged anterior and posterior fusion was performed. Two years postoperatively, the patient carries on with her daily living without any significant disability. Delayed bone cement dislodgement can occur as one of complications following bone cement augmentation procedure for osteoporotic spinal fracture. It might be related to the presence of intravertebral cleft, lack of interdigitation of bone cement with the surrounding trabeculae, and possible damage of endplate during ballooning procedure.

Minimally Invasive Option Using Percutaneous Pedicle Screw for Instability of Metastasis Involving Thoracolumbar and Lumbar Spine : A Case Series in a Single Center

  • Objective : To report a minimally invasive treatment option using percutaneous pedicle screw fixation with adjuvant treatment for metastatic thoraco-lumbar and lumbar spinal tumors. Methods : This is a retrospective study of charts of patients with spinal metastases. All were older than 18 years of age and were considered to have more than 3 months of life expectancy. The patients had single or two level lesions, and compression fracture or impending fracture. Exclusion criterion was metastasis showing severe epidural compression with definite neurological symptoms. Usually spinal segments from one level above to below pathology were stabilized. Visual analog scale (VAS) score for pain assessment and Frankel scale for neurological deficit were used, while pre- and post-operative performance status was evaluated using the Eastern Cooperative Oncology Group (ECOG). Results : Twelve patients (nine men, three women; median age 54.29 years) underwent surgery. All patients presented with back pain with/without radicular pain. There were no early complications and perioperative mortalities. Following surgery, a significant difference between average pre- and post-operative VAS scores was found (p=0.003). Overall, 91.8% of patients (11/12) experienced improvement in their ECOG score post-operatively. The mean ambulation time was 196.9 days [95% confidence interval (CI), 86.2-307.6 days; median, 97 days]. During follow-up, nine patients died and the mean overall survival time in enrolled twelve patients was 249.9 days (95% CI, 145.3-354.4 days; median, 176 days). Conclusion : Minimally invasive treatment using percutaneous pedicle screw fixation with adjuvant treatment is a good alternative treatment option for potential instability of the thoraco-lumbar and lumbar spinal metastasis.

의료영상 분석과 유한요소법을 통한 추체 성형술의 다양한 인자들에 대한 생체 역학적 효과 분석 (A Biomechanical Study on the Various Factors of Vertebroplasty Using Image Analysis and Finite Element Analysis)

  • 본 연구에서는 의료 영상과 유한요소 해석을 이용하여 골절된 척추체의 치료를 위해 골 시멘트를 주입하는 추체 성형술(vertebroplasty)에 영향을 미치는 여러 인자들에 대해 생체 역학적으로 분석하여 평가하고자 하였다. 의료 영상을 통해 구현된 모델들은 분석하고자 하는 인자에 대하여 크게 단분절과 다분절 유한요소 모델의 2 가지 형태로 구분하였다. 단분절 유한요소 모델을 통해 골다공증의 정도, 주입된 골 시멘트의 양, 골 시멘트의 주입량 및 환자에게 주입된 골 시멘트 양의 적절성을 평가하였으며, 골 시멘트의 주입이 척추체의 구조적 안정성에 미치는 효과를 분석하기 위하여 척추체 상종판에 수직 압축하중을 가하여 이때의 최대 수진 변위량을 분석하였다. 또한 단분절 척추체의 강성도(stiffness) 값을 산출한 뒤, 정상 상태의 척추체의 강성도 값으로 정규화 하였다. 다분절 유한요소 모델의 경우, 압박 골절 상태를 적용하여 척추 만곡의 변화가 인체에 미치는 영향을 분석하기 위하여 골절의 정도를 3 가지로 구현하였으며, 모델의 최상부에 압축 하중을 가한 후의 척추 만곡의 변화 정도를 분석하였다. 추체 성형술에 대한 다양한 인자들에 대한 분석 결과, 추체 성형술을 통해 환자의 골밀도가 상대적으로 높은 경우(BMD 50mg/$m\ell$이상)와 환자의 척추체 망상골 부피의 약 15%에 해당하는 골 시멘트 주입 시 가장 큰 척추체의 구조적 안정성의 증가 효과를 가져올 것으로 예측되었다. 또한 척추 골절이 미치는 영향에 대한 분석 결과, -2.8$^{\circ}$의 정상적인 후만곡을 가지는 T12∼L2 운동 분절이 골절의 모델링 후 약 9∼17$^{\circ}$의 큰 전만곡을 가지게 되었으며 이러한 척추 만곡의 변화는 동일한 하중 하에서 급격한 척추 만곡의 변화를 야기하는 것으로 나타났다. 또한 골절의 정도가 심할수록 척추 변형은 더욱 크게 증가하는 것으로 나타났다. 이러한 연구 결과들로부터, 전반적으로 추체 성형술은 골 시멘트의 주입량 또는 골밀도에 상관없이 약화된 척추체의 자체의 구조적 안정성의 증가 효과를 가져왔으며 골절로 인해 추체의 높이가 손상되었을 경우 이를 회복하는 것이 중요한 인자의 하나임을 나타내는 결과로 사료되어진다.

Chronic Spinal Epidural Hematoma Related to Kummell's Disease

  • Chronic spinal epidural hematoma related to Kummell's disease is extremely rare. An 82-year-old woman who had been managed conservatively for seven weeks with the diagnosis of a multi-level osteoporotic compression fracture was transferred to our institute. Lumbar spine magnetic resonance images revealed vertebral body collapse with the formation of a cavitary lesion at L1, and a chronic spinal epidural hematoma extending from L1 to L3. Because of intractable back pain, a percutaneous vertebroplasty was performed. The pain improved dramatically and follow-up magnetic resonance imaging obtained three days after the procedure showed a nearly complete resolution of the hematoma. Here, we present the rare case of a chronic spinal epidural hematoma associated with Kummell's disease and discuss the possible mechanism.

A Retrospective Clinical Survey of Vertebral Compression Fractures

  • Background: The purpose of this study was to review whether Korean medicine treatment was effective to maintain the spinal stability and to relieve spinal symptom by reviewing medical records of 18 vertebral compression fracture patients. Methods: The subjects were 18 thoracolumbar compression fracture patients who were taken more than two lateral view x-rays of fractured vertebra from Jan, 2010 to June, 2018. They were divided into two groups: the time admitted into Department of Acupuncture & Moxibustion Medicine, Pohang Korean Medical Hospital of Daegu Hanny University from onset (Research 1) and whether or not they have a past history of osteoporosis (Research 2). Then, they were follow-up observed and compared by compression ratio and numerical rating scale (NRS) and so on. Results: The amount of weekly compression rate increase of 18 patients was 1.76% per week. According to the result of Research 1, the patients group (14 patients) who were admitted into the hospital within 1-2 weeks from getting injury was 1.88% per week whereas the patients group(4 patients) who were admitted into the hospital within 3-4 weeks from getting injury was 1.22% per week. By Research 2 result, the patients group (7 patients) who had been diagnosed as an osteoporosis patient was 2.19% per week, and those (11 patients) who had not was 1.49% per week. There was a statistical significance in the change of the amount of compression rate increase in the case of Research 1, but there was not in the case of Research 2. NRS was decreased without significant difference regardless of the time admitted into the hospital and whether or not the patients had the past history of osteoporosis. Conclusion: Considering that there were more amount of compression rate increase at 1-4 weeks from onset, it is necessary to offer treatment more actively on early time from onset. Further research is needed into the increase of compression rate by the patients' having a past history of osteoporosis or not. Given that NRS decreased without significant difference, it is assumed that Korean medicine treatment has a meaningful effect on relieving subjective symptom of patients who are diagnosed as vertebral compression fractures.

Osteoplasty in Acute Vertebral Burst Fractures

  • Objective : Acute vertebral burst fractures warrant extensive fixation and fusion on the spine. Osteoplasty [vertebroplasty with high density resin without vertebral expansion] has been used to treat osteoporotic vertebral compression fractures. We report our experiences with osteoplasty in acute vertebral burst fractures. Methods : Twenty-eight cases of acute vertebral burst fracture were operated with osteoplasty. Eighteen patients had osteoporosis concurrently. Preoperative MRI was performed in all cases to find fracture level and to evaluate the severity of injury. Preoperative CT revealed burst fracture in the series. The patients with severe ligament injury or spinal canal compromise were excluded from indication. Osteoplasty was performed under local anesthesia and high density polymethylmethacrylate[PMMA] was injected carefully avoiding cement leakage into spinal canal. The procedure was performed unilaterally in 21 cases and bilaterally in 7 cases. The patients were allowed to ambulate right after surgery. Most patients discharged within 5 days and followed up at least 6 months. Results : There were 12 men and 16 women with average age of 45.3[28-82]. Five patients had 2 level fractures and 2 patients had 3 level fractures. The average injection volume was 5.6cc per level Average VAS [Visual Analogue Scale] improved 26mm after surgery. The immediate postoperative X-ray showed 2 cases of filler spillage into spinal canal and 4 cases of leakage into the retroperitoneal space. One patient with intraspinal leakage was underwent the laminectomy to remove the resin. Conclusion : Osteoplasty is a safe and new treatment option in the burst fractures. Osteoplasty with minimally invasive technique reduced the hospital stay and recovery time in vertebral fracture patients.

  • 주5일제 근무의 도입으로 인하여 스포츠 활동에 참여하는 인구가 많이 증가하였으며 그 결과로 스포츠 손상의 빈도가 비례하여 증가하는 추세이다. 겨울 스포츠 중 최근 청소년과 젊은이들 사이에 인기를 얻고 있는 스노우 보드로 인한 척추 관련 손상은 그 문헌 보고가 많지 않은 실정으로 저자들이 스노우 보드 손상에 따른 요추의 압박 골절 환자를 경험하여 이를 보고하고자한다.

Clinical outcomes of stereotactic body radiotherapy for spinal metastases from hepatocellular carcinoma

  • Purpose: To investigate the outcomes of patients with spinal metastases from hepatocellular carcinoma (HCC), who were treated by stereotactic body radiotherapy (SBRT). Materials and Methods: This retrospective study evaluated 23 patients who underwent SBRT from October 2008 to August 2012 for 36 spinal metastases from HCC. SBRT consisted of approximately 2 fractionation schedules, which were 18 to 40 Gy in 1 to 4 fractions for group A lesions (n = 15) and 50 Gy in 10 fractions for group B lesions (n = 21). Results: The median follow-up period was 7 months (range, 2 to 16 months). Seven patients developed grade 1 or 2 gastrointestinal toxicity, and one developed grade 2 leucopenia. Compression fractures occurred in association with 25% of the lesions, with a median time to fracture of 2 months. Pain relief occurred in 92.3% and 68.4% of group A and B lesions, respectively. Radiologic response (complete and partial response) occurred in 80.0% and 61.9% of group A and B lesions, respectively. The estimated 1-year spinal-tumor progression-free survival rate was 78.5%. The median overall survival period and 1-year overall survival rate were 9 months (range, 2 to 16 months) and 25.7%, respectively. Conclusion: SBRT for spinal metastases from HCC is well tolerated and effective at providing pain relief and radiologic response. Because compression fractures develop at a high rate following SBRT for spinal metastases from primary HCC, careful follow up of the patient is required.

병적 척추체 압박 골절 환자에서 경피적 풍선 척추 성형술 시행 후 발생한 마미 증후군 (Cauda Equina Syndrome after Percutaneous Balloon Kyphoplasty for Pathologic Compression Fracture)

  • 경피적 풍선 척추 성형술은 병적 척추체 압박 골절에서 통증 호전을 위한 시술로 사용되고 있다. 시술 후 합병증으로 시멘트가 피질 결손 부위를 통해 누출되면서 발생한 신경 압박 사례가 종종 보고되고 있다. 하지만 시멘트 누출이 아닌 종양성 병변이 후방 전위되면서 발생한 신경 압박에 대한 보고는 거의 없었다. 이에 저자들은 병적 척추체 압박 골절로 수술 전에 신경 압박이 없던 65세 여자 환자에서 경피적 풍선 척추 성형술 시행 후 종양성 병변이 후방 전위되면서 발생한 마미 증후군으로 추가 수술을 시행한 증례를 보고한다. 시술 후 촬영한 자기공명영상에서 피질골을 뚫고 척수강 내로 후방 전위된 종양성 병변이 확인되었다
