본 연구의 목적은 말소리장애 아동이 산출하는 이중모음의 특성을 파악하여 평가 및 중재에 활용할 수 있는 기초 자료를 마련하는 것이다. 현재까지 말소리장애 아동의 이중모음 산출 특성에 관한 음향학적 연구는 미비하였다. 이에 말소리장애 아동과 일반 아동을 대상으로 집단 간 이중모음 산출 특성의 차이를 파악하고자 하였다. 이를 위해 각 10명의 만 4-5세 말소리장애와 일반 아동을 대상으로, 무의미 2음절 '이중모음+다'를 모방하도록 하였다. 산출된 이중모음의 활음 구간 내 제1, 2 포먼트 기울기, 포먼트 변화량, 활음 지속시간을 Praat(version 6.1.16)을 이용해 분석하였다. 연구 결과, 두 집단 간 /유/의 F1 기울기에 집단 간 유의한 차이가 있었다. 또한, 말소리장애 아동이 일반 아동에 비해 전반적으로 작은 포먼트 변화량과 더 짧은 활음 지속시간을 보였다. 유의한 포먼트 변화량의 집단 간 차이는 /유, 예/의 F1과 /야, 예/의 F2에서 나타났으며, 유의한 활음 지속시간의 차이는 /유, 예/에서 나타났다. 본 연구의 결과는 말소리장애 아동이 이중모음을 조음하는 범위가 일반 아동보다 상대적으로 작아 그만큼 조음하는데 걸리는 시간이 줄었음을 보여준다. 이러한 점은 말소리장애 아동의 이중모음에 관한 평가와 중재를 할 때 말소리장애 아동의 조음 범위를 고려해야 하며, 이에 음향학적 도구를 활용하는 것이 필요함을 뒷받침한다.
본 연구는 1984년부터 2020년까지 국내·외 프롬프트(Prompts for Restructuring Oral Muscular Phonetic Targets, PROMPT) 관련 연구를 분석하여 우리말에 적합한 프롬프트 프로그램 개발에 필요한 기초자료를 마련하고자 한다. 연구 대상은 1984년부터 2020년까지의 프롬프트 치료기법과 관련된 국외 연구 27편과 국내 연구 1편을 분석하였다. 분석한 결과, 프롬프트 연구가 처음 시작된 1984년부터 지속적으로 연구가 이루어지고 있으며, 연구방법은 중재연구가 16편으로, 말소리장애가 가장 많았으며 대상 연령은 3-5세로 유아기를 대상으로 가장 많이 실시되었다. 중재프로그램은 16회기가 가장 많았으며, 중재프로그램 활동은 무발화 자폐스펙트럼장애 대상을 제외하고 운동구어계층(Motor Speech Hierarchy, MSH)을 토대로 이루어졌다. 종속변인을 분석한 결과, '구어산출'이 가장 많았으며, '말 운동제어', '조음', '구어명료도' 순으로 높게 나타났다. 지금까지의 연구를 종합해보면 국외에서는 구어산출에 직접적인 도움을 주는 운동 구어 치료법인 프롬프트가 효과적으로 쓰이고 있다. 그러나 국내에서는 현재까지 우리말에 알맞은 프롬프트 프로그램 개발 및 연구가 부족한 실정이다. 그러므로 본 연구를 통해서 국내 말·언어장애 아동에게 프롬프트를 적용하여 구어산출 및 조음에 도움을 줄 수 있는 우리말 프롬프트 프로그램 개발이 필요하다는 것을 시사한다고 볼 수 있다.
Purpose: Kabuki syndrome is a multiple malformation syndrome that was first reported in Japan. It is characterized by distinctive Kabuki-like facial features, skeletal anomalies, dermatoglyphic abnormalities, short stature, and mental retardation. We report two cases of Kabuki syndrome with the surgical intervention and speech evaluation. Methods: Both patients had velopharyngeal insufficiency and had a superior based pharyngeal flap operation. The preoperative and postoperative speech evaluations were performed by a speech language pathologist. Results: In case 1, hypernasality was reduced in spontaneous speech, and the nasalance scores in syllable repetitions were reduced to be within normal ranges. In case 2, hypernasality in spontaneous speech was reduced from severe level to moderate level and the nasalance scores in syllable repetitions were also reduced to be within normal ranges. Conclusion: The goal of this article is to raise awareness among plastic surgeons who may encounter such patients with unique facial features. This study shows that pharyngeal flap operation can successfully correct the velopharyngeal insufficiency in Kabuki syndrome and post operative speech therapy plays a role in reinforcing surgical result.
Background: Velopharyngeal insufficiency that accompanies speech resonance and articulation disorders can be managed through several intervention methods such as speech-language therapy, prosthetic aids, and surgery. However, for patients with severe hypernasality, surgical interventions are highly recommended. Among available surgical techniques, the posterior pharyngeal flap is most common. Case presentation: Two adult males with high nasalance scores underwent superiorly based posterior pharyngeal flap surgery, followed by speech testing by an expert speech-language therapist. Nasalance scores and articulation accuracy were assessed up until 1 year after the surgery. Nasalance scores were measured five times using a nasometer, after which the average value was calculated. Conclusions: Consistent declines in hypernasality over time are not easy to explain since the pedicled pharyngeal flap narrowed over time, secondary to cicatrization. However, scar tethering of the soft palate in a posterior direction could reduce the velopharyngeal port size over time. Therefore, long-term follow-up with intensive speech therapy is suggested for patients with severe hypernasality.
The current study examines PCC (percentage of correct consonant), speech intelligibility, and oral motor function between the group of typically developing children and the group of children with Down's syndrome. To 15 children with Down's syndrome (mean CA: 9;7) and 15 typically developing children on receptive language age, the following tests were administered: K-WPPSI (2001), Picture Vocabulary Test (Kim et al., 1995), Oral and Speech Motor Control Protocol for total oral functional score (Robbins et al., 1987), DDK and Assessment of Phonology and Articulation for Children (APAC, Kim et al., 2007) for PCC and speech intelligibility. Pearson correlation coefficients were computed for the total oral functional score, PCC and DDK of each group. The statistical analysis showed that there is no significant difference in total functional score and DDK when IQ was controlled. There was a significant correlation between total oral functional score and PCC in the Down's syndrome group and a significant correlation between total oral functional score and intelligibility in the Down's syndrome group whether IQ was controlled or not. The findings suggest that both cognitive ability and overall oral motor function need to be considered for the intervention to enhance PCC or speech intelligibility of children with Down's syndrome.
This study investigated longitudinally early vocalization and phonological developments of typically developing children. Ten typically developing children participated in the study from 9 months to 18 months of age. Spontaneous utterance samples were collected at 9, 12, 15, 18 months of age and phonetically transcribed and analyzed. Utterance samples were classified into 5 levels using Stark Assessment of Early Vocal Development-Revised(SAEVD-R). The data analysis focused on 4 and 5 levels of vocalizations classified by SAEVD-R and word productions. The percentage of each vocalization level, vocalization length, syllable structures, and consonant inventory were obtained. The results showed that the percentages of level 4 and 5 vocalizations and word significantly increased with age and the production of syllable structures containing consonants significantly increased around 12 and 15 months of age. On average, the children produced 4 types of syllable structure and 5.4 consonants at 9 months and they produced 5 types of syllable structure and 9.8 consonants at 18 months. The phonological development patterns in this study were consistent with those analyzed from children's meaningful utterances in previous studies. The results support the perspective on the continuity between babbling and early speech. This study has clinical implications in early identification and speech-language intervention for young children with speech delays or at risk.
The purpose of this study was to investigate the syllable and phoneme frequencies in the spontaneous speech of some Korean children. Sixty four normally developing children aged from 2 to 5 were involved (male: female=1 : 1, 16 children in each age group). Fifty connected utterances were analyzed using the KCLA (Korean Computerized Language Analysis) 2.0 and Exel. The findings were as follows: 1) /i/ was the most frequently used syllable and was followed by /yo/, /k/, /s'/, /nen/ and so on. 2) The most frequently used Korean phonemes were syllable-initial consonant /k/, syllable- medial vowel /a/ and syllable-final consonant /n/. 3) All seven syllable final consonants (/p,t,k,m,n,n,l/) were used more frequently in the word-medial position than in the word-final position. Three syllable initial consonants(/k, I, s'/) were used more frequently in the word-medial position than in the word-initial position. The syllable and phoneme frequencies in the Korean children's spontaneous speech will provide valuable information in interpreting the severity of phonological disorder and in developing tools for the Korean phonological assessment and intervention.
심리언어학적 접근에 기반한 말처리 모델은 다양한 경로를 통해 말소리장애아동의 구체적인 말처리 결함을 한눈에 파악할 수 있는 모델이다. 말소리장애아동이 보이는 말산출 결함은 원인을 알 수 없는 경우가 대부분이기 때문에 개별화된 중재를 위해서는 기저의 강약점을 파악하는 것이 중요하다. 또한 말소리장애 아동의 모국어 결함은 외국어 산출에도 영향을 미칠 수 있기 때문에 모국어와 외국어라는 두 언어 간 말처리 능력을 함께 살펴볼 필요가 있다. 본 연구는 한국어-영어 말처리 평가시스템 개발을 위한 예비연구로, 말소리장애아동(SSD) 10명과 일반아동(NSA) 20명을 대상으로 말산출 과제와 말처리 과제(변별, 음운표상판단, 비단어따라말하기)를 한국어와 영어로 각각 실시하여 언어종류에 따른 집단 간 비교를 시도하였다. 연구 결과, SSD 집단은 두 언어에서 모두 NSA 집단에 비해 유의하게 낮은 산출능력을 보였다. 말처리 과제 결과, 변별과제에서는 유의한 차이를 보이지 않은 반면, 음운표상판단과제의 경우 언어 종류 간, 비단어따라말하기과제의 경우 언어종류와 집단 간에서도 그 차이가 유의하였다. 본 연구의 결과를 통해 아동의 모국어와 외국어 처리능력은 상이할 수 있으며, 추후 말처리 평가 프로그램 개발을 위해 하위과제를 보다 세분화하고 난이도를 조절할 필요가 있음을 확인하였다.
In view of the hypothesis that the effects of Parkinson disease on voice production can be detected before pharmacological intervention, the voice differences of patients with Idiopathic Parkinson's disease and a healthy aging group were diagnostically analyzed with the long term object of establishing, for clinical purposes, early disease-progression biomarkers. Fifteen patients with Idopathic Parkinson's disease (prior to pharmacological intervention) and a healthy control group of 15 were selected and every voice was recorded three times using praat (ver. 5022) with a headset mic. Relevant parameters - acoustic measure of /a/ phonation, F0 related parameters, MPT related parameters, articulatory ratio, VOT - were then analyzed by MANOVA. Significant differences were found in the F0 related (low F0, high F0, F0 range) and MPT related parameters. There were also significant differences in acoustic measurements (intensity, shimmer, HNR, jitter), AMR (/$t{\Lambda}$/,/$k{\Lambda}$/) and VOT (/ta/), The findings indicated that the voice production of patients with Idiopathic Parkinson's disease have normal pitch but bad quality. In particular, with slow articulatory ratios and VOT values, the tongue tip functioning of patients was lower than for the healthy group.
설유착증(tongue-tie)은 비정상적으로 짧은 설소대로 인하여 혀의 움직임이 제한되는 것을 의미한다. 영유아에서 심한 설유착증은 수유곤란을 야기하기도 하고, 증상이 지속적으로 존재할 때는 혀의 운동범위가 제한되어 발음문제를 야기할 수 있다. 또한 비정상적인 혀의 위치로 인해 여러 가지 교정적인 문제를 초래 할 뿐만 아니라, 기능적, 사회적 장애요소로 작용할 수도 있다. 설유착증의 치료방법으로는 관찰, 언어치료, 단순절개술 그리고 절제술 등이 있는데, 수술시기에 대한 논란은 있지만 정도가 심하여 장기간 언어장애가 지속되거나 교정적인 문제를 야기 할 것으로 판단되는 경우 조기수술이 권장되고 있다. 진정요법은 조기치료가 필요한 나이가 어리고 비협조적인 환아들이나, 신체적, 정신적 이유로 통상적 인 치과진료에 어려움이 있는 환아들에게 효과적으로 사용될 수 있는 방법이다. 본 증례는 설유착증으로 수술이 필요하나 협조전단계인 환아를 대상으로 경구투여를 이용한 진정요법으로 소대절제술을 시행하여 양호한 결과를 얻었기에 이를 보고하는 바이다.
