Customer Retention Model in the Medical Service Organization: Focusing on Specialized Hospital Services

  • Purpose: The purpose of this study is to investigate the theoretical basis for assessing the strategic increase in customer perception of service quality, justice, and relationship benefits. Especially in terms of increasing relationship commitment and customer loyalty in the medical service sector. Research design: Data were collected by questionnaires through specialized hospital services. Survey was conducted on patients who have been treated at a spine specialized hospital. Results: Research shows that service quality, justice, and relationship benefits have the greatest direct impact on relationship commitment, and relationship commitment has a strong direct impact on customer loyalty in the medical service organization. Conclusions: The effect of combination of core and voluntary service behavioral attributes such as service quality, justice and relationship benefits have the most positve impact on relationship commitment and customer loyalty. Administratively, this study contributes to understanding the role of service quality, justice, and relationship benefits in the medical service sector. The results showed that in order to induce service quality, justice and relationship benefits should be facilitated relationship commitment and customer loyalty enlarged.

A Study on the Spatial Characteristics of the Outpatient Department in the Women′s Hospital - Focused on the Outpatient - (여성 전문병원 외래부의 공간특성에 관한 연구 - 환자관점을 중심으로 -)

  • Recently, the Korean medical system changed with diversification & specialization and the field of medical service is also specialized in these days. And healing environment is proposed as a general concept in the hospital architecture. There is a trend that a establishment of women's hospital is increasing as specialized type. In difference from general hospital, they are providing specialized and integrated services in response to the women's disease and demand. Particularly, Outpatient department space in women's hospital has been changed in terms of area, shape and program. However, there used to be not enough research efforts of responding these changes. This study includes the Outpatient department of s women's hospital in the form of small and medium sized building over a certain level in Korea. And the methods of the study adopted the theoric examination based concept and features of women's hospital, the abstraction of distinctions in the Outpatient department through the analysis of the drawing, and the analysis of distinctions in the Outpatient department of women's hospital with the structural space classified and the analysis of space relation. Therefore, the purpose of this is to study investigates and analyzes the spatial characteristics of the Outpatient department space in women's hospital through a analysis of case study. To conclude, the space composition can be classified into the entry space, the clinic space for patients, the support space of patients' check-up, the reception and waiting space, the rest and convenience space, and the link space in the Outpatient department of women's hospital. The level space can account for the waiting space type, the clinic office type, the type based on the relation between the waiting room and the clinic office with its features. As a result, the results will be applied for the planning & design for the future women's hospital.

A Comparative Study on the Financial Index of Hospital in accordance with the Weight of Medical Treatment Fee for the Rehabilitative Medicine Department (재활의학과 진료비 비중에 따른 병원 재무지표 비교연구)

  • The current hospital industry is showing relatively low profitability in comparison with other industries due to the low medical fees and high costs. Therefore, our government presented the direction of improvement through model execution and support of the specialized hospitals. However, it is estimated that the specialized hospitals also will show differentiated management performances in accordance with their specialized fields due to the characteristics of medical treatment. Therefore, this study had attempted an analysis on the financial index in accordance with the weight of medical treatment items for the rehabilitative medicine department among the whole hospital groups through getting out of analyzing financial indices of individual hospitals centered at their costs. For this p개pose, this study had carried out its research by partially reciting the study on the plan for utilizing participations of private health resources to expand rehabilitative medicine services into the private people of the Korea Health Industry Development Institute(KHIDI). As its results, it was shown that the stability, profitability, activity and productivity of hospitals with high weight of medical treatment for the rehabilitative medicine department were lower than those for the general hospitals. To support smooth operations of these hospitals 'with high weight of medical treatment for the rehabilitative medicine department or of specialized rehabilitation hospitals, it is judged that the plan such as the support for hospital management fund and the additional recognition on the rehabilitative fees, etc. together with the actualization of medical fees must be provided for the institutions which are providing rehabilitative medical services more than the fixed percentage and being equipped with the sufficient medical equipment and personnel to do them.

  • Much has changed in the healthcare field since the beginning of the industrial age. In the healthcare field changes are occurring so rapidly and dramatically that yesterday's paradigm will not be tomorrow's paradigm, creating the need above all else to stay fluid and flexible as strategies(included healthcare architecture planning) for the future are developed. The purpose of this study is to analyze the latest architectural trends of general hospital outpatient department based on the healthcare environment changes in Korea. The major healthcare environment change is to change the object of hospital's healthcare services from inpatient to outpatient. In conclusion, the first, medical faculties of outpatient department are subdivided specialized small faculty. The second, clinic systems for medical examination and treatment of specific disease are activated in the most outpatient department. The third, specialized medical centers for chronical disease(Cancer, Cardiac etc.) control are arranged in existed outpatient department or freestanding facility. Specialized medical center for preventive medicine is regionally decentralized for corresponding with the healthcare paradigm shifts.

A Study on Development Strategy of Green Health Industry Specialized fields in Gangwon-do Privince (강원도형 녹색건강산업 특화분야의 발전 방안)

  • As Gangwon-do Province needs an environmental-friendly alternative tourism according to low carbon green growth, this study analyzes specialized fields in green health industry, selects seven specialized fields in green health industry that is fit to Gangwon-do Province, and suggests a development strategy by conducting recognition surveys on preference and impotence of the specialized fields. Surveys and interviews are conducted of 106 students at Green Health Industry Education Center which is operating by Kwandong University and 84 persons who work at medical institutions and tourism related companies in Ganwondo. The surveys and interviews were completed from November 5 to November 19 in 2010, determined factors through a factor analysis when they have many questions, and analyzed by using SPSS 14.0. Out of the seven specialized fields, rest & recuperation field is highly recognized. In order to have competitive edge compared to other self-governing provinces, Ganwon-do Province should prioritize rest and recuperation field. A healthcare program in the green health industry specific to Ganwon-do Province includes hot springs and spa programs as a priority. Gangwon-do's hot springs are deemed to be competitive resources as global medical tourists prefer spa and sauna healthcare programs, while the province promotes its spring resources and builds medical tourism infrastructures. Gangwon-do Province can promote a medical tourism industry that is well suitable for the strength and characteristics of the province. It might pursue oriental medicine therapy tourism, which is related to a recuperating medical service that uses both oriental and western medicine. It can also run such programs as forest bathing (oxygen road), spring and spa, and sea water treatment, which are the specialized fields in green health industry with respect to recreation and healthcare.

The Relationship between Hospital Specialization and Operational Performance: Focusing on Diseases of the Musculoskeletal System and Connective Tissue (병원의 전문화 전략과 운영성과 간의 관계: 근골격계 및 결합조직 질환을 중심으로)

  • This study is aimed at investigated and compared the differences in the affect of hospital specialization according to hospital size using claims data of the Health Insurance and Review Assessment National Inpatient Sample in 2018 for diseases of the musculoskeletal system and connective tissue. To this end, we used multivariate hierarchical linear models(a.k.a., multi-level models) using two-tier data from 106,599 patients discharged after diseases of the musculoskeletal system and connective tissue from 734 hospitals. Multivariate results indicate that patients who were discharged with diseases of the musculoskeletal system and connective tissue from specialized hospitals with 200 beds or less stayed shorter and paid less inpatient charge than those who were discharged from less specialized hospitals. But for hospitals with 201-300 beds, no positive impact relationship was found between hospital specialization and operational performance. This finding may be limited evidence that the affect of a hospital's specialization strategy may vary depending on the size of the hospital. We discussed several managerial and health policy implications below.

A Study on the Space Composition and Area Planning of the Cardiovascular Center in General Hospital (종합병원 심혈관센터의 공간구성 및 면적계획에 관한 연구)

  • Purpose: As the nation is becoming an aging society, there is an increasing number of the patient who has Diseases of cardiovascular disease. So it is necessarily to build a facility which provides specialized health care. Methods: Field surveys to facilities and analysis to the floor plan have been conducted in order to suggest adequate architectural composition to the specialized cardiovascular center and the dimensions of rooms. Results: The result of this study can be summarized into six points which contain guide to how to organize the specialized cardiovascular center. One of the most important things in this study is to suggest the architectural type which provides efficient collaborating system between the outpatient and angiography unit. Implications: It can be said that this paper can be useful research material when the architect tries to plan the specialized cardiovascular center.

The influences of Medical Service Quality on Customer Trust, Customer Value, and Customer Loyalty in Specialized Hospitals (전문병원의 서비스품질이 고객신뢰와 고객가치 및 고객충성도에 미치는 영향)

  • Objectives: The purpose of this study was to explore how service quality in specialized hospitals influences customer loyalty under the mediating effects of customer trust and customer value. Methods: A statistical review was done based on the survey results of patients who had used a specialized hospital in Gyeonggi Province, South Korea. A total of 209 questionnaires were collected. After data collection, an empirical analysis was performed using SPSS 21.0 and AMOS 21.0 software. Results: Interaction quality (β = 0.254, p < 0.05) and result quality (β = 0.179, p < 0.05) significantly influenced customer trust, while physical environmental quality (β = 0.121, p > 0.05) did not. Customer trust (β = 0.571, p < 0.01) influenced customer value (β = 0.136, p < 0.05) and customer value influenced customer loyalty. In addition, customer confidence (β = 0.668, p < 0.05) was shown to have directly impacted customer loyalty without the mediating effect of customer value (0.078). Conclusions: The findings showed that interaction quality and resulting quality had important implications for customer loyalty. We therefore suggest compliance with appointment times, training to improve work-related skills for employees, improving customer satisfaction, and enhancing communication skills.

Acute pain management in the trauma patient population: are we doing enough? A prospective observational study

  • Purpose: There is a strong correlation between trauma and pain. Pain increases the rate of depression, posttraumatic stress disorder, and even mortality in trauma patients. Methods: This institution-based, provider-blinded and patient-blinded, observational study was conducted among trauma patients treated at Tikur Anbessa Specialized Hospital. Over the course of 3 months, this study included patients who had no prior pain management at other hospitals before presentation, and who presented within 24 hours of the traumatic event. Results: Of the 74 patients evaluated, none of the patients had their pain level scored. The researcher-provided pain scale showed a severe subjective pain score for 79.7% of the patients and a severe functional activity score for 59.5% of the patients. Analgesia was provided at an average of 55.4 minutes after presentation and all patients received either diclofenac or tramadol. Satisfactory pain reduction after analgesia was 28.8% for patients initially complaining of severe pain, 54.6% for moderate pain, and 66.7% for mild pain, with the difference being statistically significant (P<0.05). Forty percent of patients discharged home received no analgesia after the first dose provided upon presentation. Conclusions: Pain scoring was nonexistent during the course of the study. The poor utilization rate of analgesia combination and opioids led to unsatisfactory pain outcomes in patients evaluated and followed for 24 hours after presentation.
