고혈압 의료비 지역 간 변이 및 변이 요인 분석 (Analysis on geographic variations and variational factors in expenditures for hypertension)

  • 본 연구는 우리나라의 고혈압 의료비의 지역별 변이와 지역의 사회학적, 의료공급, 건강행태 등의 변이 요인을 규명함으로써 이를 기반으로 지역별 고혈압 관리 사업 방안 마련에 기초자료를 제시하는 것이다. 이를 위해 전국 시 군 구 247개 지역을 대상으로, 2012년 지역별 의료이용 통계자료(국민건강보험공단), 지역사회건강조사(질병관리본부) 및 국토해양부 자료를 이용 분석하였다. 연구결과 전국 247개 시 군 구의 고혈압 건강보험진료비는 평균 6만2천원이었고 변동계수는 30.0으로 지역 간 높은 고혈압 의료비 변이를 보였다. 주요 변이 요인으로는 인구밀도, 유배우자율, 평균가구원수입, 인구십만명당병원수, 관외의료비비율, 월간음주율, 중증도이상신체활동실천율, 평생의사진단율 등으로 나타났다. 의사결정나무를 이용한 분석 결과 평생의사진단율, 평균가구원수입, 유배우자율, 인구십만명당병원수, 비만율, 월간음주율에서 유의한 차이가 있었다. 본 연구의 결과 고혈압 의료비의 지역 간 변이요인으로는 의료공급이나 인구사회학적 특성뿐만 아니라 건강행태에 있음을 확인하였고 이는 고혈압의료비 절감을 위한 지역보건사업 정책 결정에 참고가 될 수 있을 것이다.

독도의 국가경관이미지 특성에 관한 연구 (Characteristics of National Landscape Image in Dokdo Island)

  • 본 연구는 독도가 대한민국 국민에게 특별한 의미를 갖는 지역이기 때문에, "국가경관이미지"를 갖고 있을 것이라는 가설을 증명하기 진행되었다. 이를 위해 내국인 대학생 51명과 외국인 대학생 44명을 대상으로 어의척도 설문조사를 실시하고 통계분석을 실시하였다. 그 결과 내국인 남녀간에는 큰 차이는 없는 것으로 나타났다. 그렇지만 [독도전체 경관사진]에 대해서는 "상당히 혹은 매우 귀중하고 인상적인 동시에 신비롭고 상징적이다"라는 이미지를 갖고 있었다. 이것은 여타 국립공원의 자연경관에 대한 반응들에서는 찾아 볼 수 없는 독도 특유의 이미지들이다. 한편, 내국인과 외국인의 응답 결과는 모든 변수에서 큰 차이를 보였다. 특히 [독도전체 경관사진]에 대해서 몇몇 경관형용사들 [인상적이다(1.41), 신비롭다(1,25), 상징적이다(1.00), 귀중하다(0.98), 친근하다(0.95), 아름답다(0.93), 흥미롭다(0.85)]은 내외국인 응답자들간 뚜렷한 차를 보였다. 바위 이름 자체에 국가경관이미지와 관련된 이름을 가진 바위들[한반도바위(0.57)]와 [독립문바위(0.51)]에 대해서도 내외국인 응답에 상당한 차이를 발견할 수 있었다. 요인분석 결과 도출된 세 가지 주요 요인 가운데 첫 번째 요인은 국가경관이미지, 두 번째 요인은 독도의 자연성, 세 번째 요인은 독도의 공간감으로 나타났다. 본 조사 연구는 제한된 시간과 여건 속에서 한정적 조사연구가 수행되었지만 추후 좀 더 전 국민을 대상으로 한 확대 연구가 필요하다. 또한 CVM(조건부 가치측정법)에 의한 독도의 경제적 가치 재평가는 조속한 시일 내에 다시 수행될 필요성이 있다.

경기도 지방하천에서의 보, 낙차공 및 교량 시설물의 실태 및 개선 방안 제시 (Improvement plan and factual survey for weirs, drop structures and bridges in medium scale streams of Kyonggi province)

  • 보는 하천수량을 확보하고 낙차공은 하상안정을 위한, 그리고 교량은 차량 등의 이동을 위한 하천횡단시설물이다. 그러한 시설물의 안정성을 확보하기 위하여 하천설계기준에서는 시설물의 최소 규모에 관한 지침을 제시하고 있으나 실태 조사 결과에 의하면, 기존 보 및 낙차공의 물받이 및 바닥보호공의 규모는 설계기준을 만족시키지 못하는 경우가 대부분이고 교량의 교각은 일부만 만족시키는 것으로 나타났다. 기존 보 및 낙차공의 수리적 안정성을 제고하기 위해서는 설계기준에서 제시한 바닥보호공에 대한 물받이의 비율인 3.3을 최소값으로 확보해줄 필요가 있다. 표본하천 시설물 실태조사에 의하면 기존 보 및 낙차공을 자연형 여울공으로 개량할 경우 적용하는 종단경사는 1:20이 가장 적정한 것으로 나타났다. 또한, 교량의 철거 또는 재가설에 관한 결정에서 교각의 여유고 및 경간장 위배사항만 고려하고 있지만, 향후 교량 노후도 및 경간장 완화 규정 등을 종합적으로 검토하여 결정하는 것이 바람직하다. 본 연구에서는 설계기준, 하천실태 조사결과, 일본사례 등을 통하여 기존 보, 낙차공 및 교각의 수리적 안정성을 제고하는 종합적인 방안을 제시하였다. 하천횡단시설물의 안정화 노력은 정부 차원의 예산 지원과 적극적인 하천관리를 통하여 이룰 수 있으며, 이를 통하여 하천재해를 예방하고 건전한 하천 환경을 유지할 수 있을 것이다.

한약을 사용하는 아시아권 국가의 유해사례 보고 양식에 관한 비교 연구 (Comparison about adverse drug reaction report forms among Asian's countries using herbal medicine)

  • Objective : The purpose of this study is to find out the possibility of application to herbal medicine's report form for adverse drug reaction (ADR) by reviewing and analyzing Asian countries's ADR report forms. Method : We investigated, compared, and analyzed ADR report forms (ADR-RF) of Asian countries's ADR institutions (ACAI), such as, Korea institute of drug safety & risk management and Dongguk university Ilsan oriental hospital (DUIOH) in Korea, national center for ADR monintoring (NCAM) in China, pharmaceuticals and medical devices agency (PMDA) in Japan, Ministry of Health and Welfare (MOHW) in Taiwan, and drug office, department of health, the government of the Hong Kong special administrative region (GHKSAR) in Hong Kong. Results : ADR-RF for ACAI included common contents, such as, patients information (name(initial), gender, age, weight), adverse event (AE)'s report information (Recognition and report for AE occurrence, first or follow up report, Severe AE), the detailed information of AE (the title of AE, onset & closing date of AE symptoms, the progress & results detailed test of AE), the information of AE's medicine (the types of medicine, product name, ingredient name, suspected or combination drug, single dose & frequency, dosage form, administration route, dealing for AE-suspected medicine), and AE reporter's information (reporter's information, institution's information). Taiwan had ADR-RF and the department exclusively for herbal medicine (HM), but others (except DUIOH) had not only no ADR report form but also contents for HM. Conclusion : ADR-RF for HM have to include the common contents of ACAI at least, as well as HM information related to ADR, such as the title, composition and types of HM, history related to HM's ADR, and the contents of drug-induced liver injury and so on. In addition, the main department of government for HM's ADR will be needed.

울산 지역에서 2001년 이후 이산화황(SO2)의 고농도 사례 분석과 저감 정책 방안의 검토 (Case Analysis on High Concentration of SO2 and Review on Its Reduction Policy in the Ulsan Metropolitan Area since 2001)

  • Until comparatively lately, the annual time series of the $SO_2$ concentration had been shown in a decreasing trend in Ulsan as well as other Korean cities. However, the high concentration of $SO_2$ was frequently found in the specific countermeasure region including the national industrial complex such as Mipo and Onsan in the Ulsan city for the period of $2001{\sim}2004$. There are many conditions that can influence the high concentration of $SO_2$ at monitoring sites in Ulsan, such as: First, annual usage of the fuel including sulfur increased in comparison with the year before in spite of the fuel conversion policy which wants to use low sulfur oil less than 3% and LNG. Second, point source, such as the power plants and the petroleum and chemistry stacks, was the biggest contributor in $SO_2$ emission, as a analyzed result of both the air quality modeling and the stack tole-monitoring system (TMS) data. And third, the air pollutants that occurred in processes of homing and manufacturing of the fuel including sulfur were transported slow into a special monitoring site by accumulating along the frontal area of see-breeze. It was concluded that Ulsan's current environmental policy together with control methods should be changed into the regulation on total amount of emission, including a market-based emission trading with calculating of atmospheric environmental critical loads, for the $SO_2$ reduction like the specific countermeasure for the $O_3$ and PM10 reduction in the Seoul metropolitan area. And this change should be started in the big point sources of $1{\sim}3$ species because they are big contributors of Ulsan's $SO_2$ pollution. Especially it is necessary to revitalize of the self-regulation environmental management. Other control methods for sustaining the $SO_2$ reduction are as follows: maintenance of the fuel conversion policy, reinforcement of the regional stationary source emission standard, and enlargement of the stack TMS.

중국(中國) 소수민족(少數民族)인 묘족(苗族)의 민족복식(民族服飾)에 관(關)한 연구(硏究) (A Study on Traditional Costume of the Miaos, one of China's Minorities)

  • The Miaos who is the minority people mainly living in the southwestern part of China, expressed their indicator and solidarity through the costume in order to maintain their racial character while experiencing numerous adversities over thousands of years, where the costume has served as a source of cohesion as well as a primitive religious thought, and also showed their faith, desire, longing and aspiration. This study examined the Miao's traditional costume by classifying it into the following; hair style, headdress, upper and lower garments, and other costume. And the silver ornaments used for attire and their symbolic meaning were examined. The result of the study is summarized as follows. 1. The reason that types of the costume has been diversified is because there was promise of ancestors who intended to differently express the type of a kind as symbol of the racial branch that is the Miao's special type of society. Thus, the costume type could tell where a tribe live. Another reason is because only marriage between families with different surname but the same type of costume was accepted. 2. As women made and wore the costume themselves, it also served as a means of being proud of their skill or wealth, they tried to make it more beautiful and it was also used as a token of marriage or love between relatively enlightened men and women. 3. The design used on the costume was expressed as a symbolic meaning of indicator to strengthen the racial solidarity because it connoted worship to ancestors who had experienced lots of adversities. 4. The hair style was expressed in various styles by using Kache such as Chukye, Byunbal and Kokye. It is likely that ornaments used on the head of women in the form of cow's horn or silver crown were used as one of the methods to stress the valuableness of the cattle that were essential to agricultural life. In addition, various styles of turbans were used to indicate the respective regions. 5. Cock's feather ornaments or silver ornaments in the form of pheasant's feather on the edge of women's skirts, peasant's feathers that men wore on their head, or Baekjoui and men wore resulted from the Miaos' thought of adoration for birds, which implied a primitive religious meaning. 6. As the region where the Miaos live yields much silver, the silver ornaments were mostly used to be proud of wealth, which symbolized light and pureness.

Spatiotemporal Trends of Malaria in Relation to Economic Development and Cross-Border Movement along the China-Myanmar Border in Yunnan Province

  • The heterogeneity and complexity of malaria involves political and natural environments, socioeconomic development, cross-border movement, and vector biology; factors that cannot be changed in a short time. This study aimed to assess the impact of economic growth and cross-border movement, toward elimination of malaria in Yunnan Province during its pre-elimination phase. Malaria data during 2011-2016 were extracted from 18 counties of Yunnan and from 7 villages, 11 displaced person camps of the Kachin Special Region II of Myanmar. Data of per-capita gross domestic product (GDP) were obtained from Yunnan Bureau of Statistics. Data were analyzed and mapped to determine spatiotemporal heterogeneity at county and village levels. There were a total 2,117 malaria cases with 85.2% imported cases; most imported cases came from Myanmar (78.5%). Along the demarcation line, malaria incidence rates in villages/camps in Myanmar were significantly higher than those of the neighboring villages in China. The spatial and temporal trends suggested that increasing per-capita GDP may have an indirect effect on the reduction of malaria cases when observed at macro level; however, malaria persists owing to complex, multi-faceted factors including poverty at individual level and cross-border movement of the workforce. In moving toward malaria elimination, despite economic growth, cooperative efforts with neighboring countries are critical to interrupt local transmission and prevent reintroduction of malaria via imported cases. Cross-border workers should be educated in preventive measures through effective behavior change communication, and investment is needed in active surveillance systems and novel diagnostic and treatment services during the elimination phase.

합곡(合谷)$(LI_4)$ 자침(刺鍼)이 구순부(口脣部)의 온도변화(溫度變化)에 미치는 영향(影響) (Effects of Acupuncture at Hap-Kok$(LI_4)$ on the Thermal Changes of Labia Oris Surface in Man)

  • We reported that acupuncture on $LI_4$ effected thermal change on $LI_4$ and ST25 area, And this study was performed on 100 healthy students to observe the effects of acupuncture at Hap-kok ($LI_4$) according to the meridian and qi-xue(氣血) phenomenon of oriental medicine's theory, The results was 1. In healthy man, mean skin temperatures of $LI_4$, ST25, CV12 areas(Control) were decreased, 2. In left $LI_4$ acupuncture group, the temperature of abdominal skin and $LI_4$ area had the tendency of increase, both right and left $LI_4$ acupuncture group, the temperature of abdominal skin and $LI_4$ area had not significant tendency. 3. In the research of thermal difference which eliminate a special region temperature, in the control group, the thermal changes had not significant tendency, in the left $LI_4$ acupuncture group and both right and left $LI_4$ acupuncture group, the thermal changes had tendency to be increased, 4. In the deviation analysis which mean of each group is excluded, in the control group, it turned out that deviation is regular for 10 minutes; in the left $LI_4$ acupuncture group and both right and left $LI_4$ acupuncture group, it turned out that deviation is more larger than the control group for 10 minutes. The above results indicates that Digital Thermography is a useful method to observe effects and changes by acupuncture stimulation on objective evaluation of phenomenon for the meridian system. Thus, acupuncture on $LI_4$ affects to thermal changes of the abdominal skin and $LI_4$ area, but exact examination of thermal changes on abdominal skin will have to be done.

Authentication and classification of strawberry varieties by analysis of their leaves using near infrared spectroscopy.

  • It is well known now that near infrared spectroscopy (NIRS) is a fast, no destructive, and inexpensive analytical technique that could be used to classify, identify, and authenticate a wide range of foods and food items. Therefore, the main aims of this study were to provide a new insight into the authentication of five strawberry (Fragaria x ananassa) varieties and to correlate them with geographical zones and the propagating methods used. Three weeks plants of five different strawberry varieties (F. x ananassa Duch. cv Camarosa, Seascape, Chandler, F. Chiloensis, and F. Virginiana) were cultivated in vitro first and then transferred to pots with special soil, and grown in a greenhouse at CINVESTAV, all varieties were acquired from California (USA). After 18 months, ten leaves from each variety were collected. Transmission spectra from each leave were recorded over a range of 10, 000-4, 000 cm$-^{1}$, 32 scans of each strawberry leave were collected using a resolution of 4 cm$-^{1}$ with a Paragon IdentiCheck FT-NIR System Spectrometer. Triplicates of each strawberry leave were used. All spectra were analyzed using principal component analysis (PCA) and soft independent modeling class analogy (SIMCA). The optimum number of components to be used in the regression was automatically determined by the software. Camarosa was the only variety grown from the same shoot but propagated by a different method (direct or in vitro). Five different classes (varieties) or clusters were observed among samples, however, larger inter class distances were presented by the two wildtype samples (F. Chiloensis and F. Virginiana). Camarosa direct and Camarosa in vitro displayed a small overlapping region between them. On the other hand, Seascape variety presented the smallest rejection percentage among all varieties (more similarities with the rest of the samples). Therefore, it can be concluded that the application of NIRS technique allowed the authentication of all strawberry varieties and geographical origin as well. It was also possible to form subclasses of the same materials. The results presented here demonstrate that NIRS is a very powerful and promising analytical tool since all materials were authenticated and classified based on their variety, origin, and treatment. This is of a tremendous relevance since the variety and origin of a plant material can be established even before it gives its typical fruit or flower.

나주 오량동 가마와 운곡동 분묘 유적 출토 대형전용옹관의 재료학적 동질성과 제작기법 해석 (Interpretation of Material Homogenity and Making Techniques of the Jar Coffins from the Oryangdong Kiln Site and the Ungokdong Tomb Site in Naju, Korea)

  • 이 연구는 나주 오량동 가마 유적과 운곡동 고분 유적에서 출토된 대형전용옹관의 재료학적 동질성과 소성환경 및 제작기법적 특성을 비교검토 한 것이다. 모든 옹관편은 회색계열로 물리적, 광물학적 및 지구화학적 특성이 상당히 유사하다. 옹관 기벽의 두께는 약 2.3~5.3㎝로 미정질의 기질 사이로 분급이 다양한 다량의 석영을 포함하며 부분적으로 정장석과 흑운모가 관찰된다. 이는 대형의 옹관 제작시 건조 및 수축률을 조절하고 두꺼운 기벽을 유지하기 위해 첨가된 것으로 사료되며 약 11.4~19.8%의 면적비를 가진다. 옹관편의 X-선 회절분석 결과, 뮬라이트와 헤르시나이트 등이 검출 되었으며, 이를 열적특성 분석 결과와 함께 종합할 때 약 $1,000{\sim}1,150^{\circ}C$의 소성온도를 경험한 것으로 추정된다. 또한 옹관의 태토로 추정되는 오량동 태토저장공 토양과 옹관편과의 동질성이 입증되어 의미가 있다. 그러나 점토질의 오량동 가마 소성토와 원료산지로 추정되는 유적 남사면 토양의 경우, 미량 및 희토류원소의 거동특성은 일치하나 주성분원소에서 다소 차이를 보인다. 이들은 옹관 태토와 동일 기반암의 풍화에 의해 생성된 토양으로 간단한 정선이나 비짐첨가등 별도의 과정을 거친다면 옹관태토로의 이용 가능성이 충분하다.
